How medical student placements at a psychiatry hospital can be utilised to make psychiatry a popular career choice

It is perceived that negative attitudes towards mental illness in undergraduate medical students can impact student's decision in choosing psychiatry as a medical career. Improvement in psychiatry placements for undergraduate medical students can result in changing student's attitude towards psychiatry as a career choice. We demonstrate how students’ placements from various medical schools at a major psychiatric hospital contributed towards enhancing student's interest towards psychiatry. Medical students who had their placement over the last one year were contacted for an anonymised student perspective survey.While majority of students did not have psychiatry as their potential career choice before they started their placements more than two third rated psychiatry as a potential career choice based on their experience from the placements. This encouraged us to improve the placement standards based on student's perspective. Students suggested that more use of medical training in psychiatry, improvement in teaching and placement standards and more psychiatry placements before specialised training can contribute towards making psychiatry as one of the popular career choices. Student's preferred interactive teaching sessions including case based discussions and informal teaching sessions during ward rounds and clinics. Overall students found their placements helpful but more so to perform well in their examinations as compared to coverage of full psychiatry curriculum.It is planned to conduct the survey again after necessary changes based on student's perspective to evaluate whether further improvement in placements can continue in contributing towards increasing medical recruitment in psychiatry.

AbstractSome decades ago being a medical doctor was characterized unambiguously as a profession that offers help and serves the patients’ needs during medical treatment. In today’s society, this image of the medical profession has been substantially changed. The present paper aims to examine medical career choice motivations and preferences of choosing speciality, in the light of current social and economic changes in Hungary. The study was carried out by using a voluntary, self-administrated, questionnaire among first-year medical students and resident doctors in four medical faculties in Hungary. The career choice motivations of the first-year medical students and resident doctors are similar and match to the traditional health profession career choice motivations. Nevertheless the first-year students consider high income as one of the most important factors. They appear more conscious and more ambitious regarding their future speciality choice. The Hungarian health care system and medical education must be prepared for the presence of students that are aware of the high market value of a medical diploma, have excellent language skills, and consider migration as one main factor in their motivation when choosing a medical profession.

Background: The ability of communicating with people is of great importance for physicians. Predicting unknown variables like personality traits is one of the important impressive issues in choosing future medical experts in medical training. Regarding this, medical colleges have training programs all around the world. The present study aimed to investigate the attitude toward learning communication skills based on the personality traits of medical students. Materials and Methods: This correlational study was done in 2019 and the participants were medical students who were selected using a convenience sampling method. Zuckerman-Kuhlman personality questionnaire and communication skills attitude scale (CSAS) were used for data collection. CSAS was used to assess positive and negative attitudes. Results: The results showed that only the sociability dimension among the five dimensions of personality had a significant relationship with attitude toward learning communication skills (P=0.018). Moreover, significant relationships of gender (P=0.022), mother’s education level (P=0.049), and residence (P=0.036) with attitude to learning communication skills were found. Conclusion: According to the results, the dimension of demographic characteristics is effective in improving communication skills of medical students. Therefore, it is recommended that it should be considered in medical curriculum development and revision and modifications.

Background Career choice of medical students plays a vital role for health care system of a country. It also plays a crucial role to bring medical specialties into mainstream focus. Materials and Method A cross-sectional, questionnaire-based study has been performed at Manipal College of Medical Sciences, during the period of 1st May to 31st August–2011. A self reported questionnaire was given to the undergraduate medical students of Basic sciences. Questionnaire included career choices, nature of career, reason for choice, and education level and occupation in their parents. Results Data was collected from 269 medical students, among them 145 males and 124 female students participated in the study. The mean age of Indian (85, 31.6%), Nepalese (110, 40.9%) and Sri Lankan (74, 27.5%) students were 18.3 ± SD 0.7, 18.6 ± SD 0.8 and 18.0 ± SD 1.5 years respectively. Education and Occupation of parents seems to influence the determination of career choice in case of Medical students (p<0.01). Females have 2.19 times preference of choosing Medicine and 0.22 times Surgery compared to males (p<0.01). In case of Nepali male students, Surgery (60.3%) was the most preferred Postgraduate subject followed by medicine (20.6%) and in female students, medicine (44.7%) and OBG (31.9%). But in case of Indian male students 58.0% & 31.4% female students want to choose surgery as their career. 53.1% of the Sri-Lankan male students want to choose medicine as their post graduation and apart from these 50% of the female students also, which is followed by surgery 46.9% (male) and 21.4% (female) respectively. Conclusion Our study supports that Post-graduation in clinical specialties is always preferred over Basic sciences due to more interaction with patients and the practical aspects of medical profession. The relatively less popularity of some streams may show long-term impact on the health of health system of Nepal. Abstract Background Assessing competence of advanced undergraduate medical students based on performance in the clinical context is the ultimate, yet challenging goal for medical educators to provide constructive alignment between undergraduate medical training and professional work of physicians. Therefore, we designed and validated a performance-based 360-degree assessment for competences of advanced undergraduate medical students. Methods This study was conducted in three steps: 1) Ten facets of competence considered to be most important for beginning residents were determined by a ranking study with 102 internists and 100 surgeons. 2) Based on these facets of competence we developed a 360-degree assessment simulating a first day of residency. Advanced undergraduate medical students (year 5 and 6) participated in the physician’s role. Additionally knowledge was assessed by a multiple-choice test. The assessment was performed twice (t1 and t2) and included three phases: a consultation hour, a patient management phase, and a patient handover. Sixty-seven (t1) and eighty-nine (t2) undergraduate medical students participated. 3) The participants completed the Group Assessment of Performance (GAP)-test for flight school applicants to assess medical students‘ facets of competence in a non-medical context for validation purposes. We aimed to provide a validity argument for our newly designed assessment based on Messick’s six aspects of validation: (1) content validity, (2) substantive/cognitive validity, (3) structural validity, (4) generalizability, (5) external validity, and (6) consequential validity. Results Our assessment proved to be well operationalised to enable undergraduate medical students to show their competences in performance on the higher levels of Bloom’s taxonomy. Its generalisability was underscored by its authenticity in respect of workplace reality and its underlying facets of competence relevant for beginning residents. The moderate concordance with facets of competence of the validated GAP-test provides arguments of convergent validity for our assessment. Since five aspects of Messick’s validation approach could be defended, our competence-based 360-degree assessment format shows good arguments for its validity. Conclusion According to these validation arguments, our assessment instrument seems to be a good option to assess competence in advanced undergraduate medical students in a summative or formative way. Developments towards assessment of postgraduate medical trainees should be explored.

ABSTRACT: Introduction: The objective of the study was to learn about the concepts of violence among medical undergraduate students in the state of Bahia, their personal experiences with the phenomenon and advice regarding case referral. Method: a qualitative research was carried out with 20 undergraduate medical students from public institutions in the state of Bahia. The data were collected via the web through an electronic file made available by Google Forms. The students were informed about the page address through an e-mail. Results: Most of the students said that the topic of “Violence against Children” was addressed during their undergraduate years. Shared conceptions by most of the students on the subject are related to the definitions of violence as physical injuries inflicted on the victims, but broader definitions of social and subjective perception, encompassing different dimensions of the phenomenon were also identified. The most frequently cited feelings experienced in situations of violence were the following: helplessness, fear, sadness, unpreparedness, compassion, empathy, anger and rage. The difficulties that the students encountered in approaching the victims of violence stem from the lack of preparation in the training and from the positions related to the physicians themselves, such as fear of involvement and accountability. The inherent characteristics of children and distrust in protective services were also mentioned. Conclusion: Although the students reported having contact with the topic during graduation, most of them evaluated the training as insufficient. The lack of professional preparation to approach the medical-social issues, such as violence, has been partially attributed to the biologicist bias of the medical training. In this sense, we highlight the understanding of violence as an essentially social and historical phenomenon, to the detriment of the different dimensions of the illness that imply in the health-disease process. From this perspective, this bias obscure the recognition of the different manifestations of violence as objects of healthcare work, suggesting a need for a broader approach in medical education, which can help to contemplate the complexity of the subject.

Student selected components (SSCs) are increasingly described elements of medical undergraduate education, training and curricula. SSCs offer the potential for integration into both traditional (‘pre-clinical’ versus ‘clinical’) medical curricula, as well as into other innovative or evolving medical training curricula. This article employs a structured and descriptive approach to exemplify the process by which year 1 medical students were supported in a practical manner to undertake a distinct small group SSC project. In this illustration, the SSC was focused on a quality improvement (QI) topic of relevance to clinical pharmacology and therapeutics (CPT), and involved a review of the anticholinergic burden of inpatient prescriptions for a defined cohort. The SSC was completed in the context of a teaching hospital’s medicine of the elderly (MoE) clinical service. In a sequential manner, the paper describes experiential learning points from the perspective of a supervisor of an SSC project. The paper offers educational value with a potential for generalisable application to non-clinical and clinical educationalists. Furthermore, the paper offers guidance to supervisors, teachers, tutors and facilitators, with encouragement to consider how they may design similar projects for the training of undergraduate medical students in centres that they are affiliated with. The paper also highlights another key driver for productive SSCs i.e. the central principle of striving to promote projects and activities that support active student engagement, rather than merely passive inclusion.

Background: In recent years, there has been an epidemic of violence against health professional in many nations including India. The Indian Medical Association (IMA) has reported that 75% of doctors have faced physical or verbal violence during their lifetime. The objective of present study was to evaluate the medical student perspective on rising violence against doctors.Methods: This is a cross sectional questionnaire based study carried out at Sri Guru Ram Das Institute of medical Sciences and Research Amritsar. Undergraduate medical students from second year onwards, interns and post- graduate students of the institution were included in the study. Data was compiled and statistically analysed.Results: Total 497 medical students participated in the study. Among participants 327 (65.8%) were undergraduates, 106 (21.3%) were interns, 64 (12.9%) were postgraduate students 97.18% of students said that they were aware of rising incidents of violence against doctors. while 96% of participants said that they were concerned about the problem. For 86.1% students source of information of these incidents was social media. 82.5% participants said that doctors are at higher risk of being victim of violence than other profession. 89.1% of participants who had said that doctors are at higher risk of being victims of violence than other professions have said yes to the question that doctors need to be trained in martial arts. 70.2% said that these incidences would affect their future carrier choices. 60.8% Students said that certain specialties are more prone to receive violence than others. In response to an open-ended question which specialties are more prone, 83.6% participants had written surgical branches and obstetrics and gynecology.Conclusions: The study indicates that they find certain specialties more-risky and their inclination towards non- surgical branches for post-graduation.

SynopsisThe significantly favourable changes in medical students' general attitudes to psychiatry which we found after their 8-week clerkship in psychiatry and at the end of their clinical curriculum were not maintained at the end of their first post-graduate year. Three of their specific attitudes to psychiatry changed significantly in an unfavourable direction over this 2-year period. Our findings suggest that, while favourable changes in students' specific attitudes to psychiatry can be found following a clerkship, these attitudes do not seem to endure and, in some cases, they later become less favourable. The results are discussed with reference to the relationships between undergraduate medical training in psychiatry, attitudes to psychiatry, and subsequent career choice, particularly general practice.
