Children in hoarded homes: A call for protection, prevention, intervention and compassionate care

Abstract This is a call for action to protect and assist children of hoarding parents. Action that minimises harm to children living in hoarded homes also promotes family safety and health. Optimal care involves the whole family system, both children and parents. Prevention of harm through early identification and intervention for hoarding can reduce the burden of a disorder that often increases in severity over a lifetime and deprives humans of a full and safe life. Helping children and their parents severely affected by hoarding disorder is for the public good. Public policy and funded programmes can reduce long-term and immense costs to children, families and the many systems hoarding affects. Specifically, public policy can facilitate and fund outreach, education, coordination of providers of health, social and public services, and research-driven methods for assessment and intervention on behalf of children, individuals and families. Releasing people from the grips of hoarding disorder can enable them to devote generative resources to themselves, their families and their communities. Ultimately, prevention of hoarding through early assessment and intervention for minors and young adults is the most efficient, long-term and cost-effective method for minimising harm. When unidentified, hoarding disorder intensifies, people go into hiding, risk increases and opportunity for detection and intervention decreases. Informing the public about hoarding disorder must be followed with sufficient resources to address it, otherwise, helplessness ensues, and people will likely remain in the hidden world it governs.

A main avenue for influencing public policy available to unions and business is public opinion campaigning. As groups with substantial credibility in the minds of the public, unions and employers have the potential to move immigration attitudes and, thereby, have a long-term indirect influence on immigration policy. The article asks, first, who is (not) convinced by arguments from business or labour leaders and second, what messages are most convincing. We present the results of a survey experiment in three very different immigration regimes and interest group environments (Canada, the UK and Germany). The results suggest that the net effects of public arguments are small, but vary widely across demographic groups.

Context. Relative density indices assuming uniform distribution of the target species are often the only cost-effective method for monitoring a population over the long term and at landscape scale, and the only source of valuable historical data. Yet, theoretical models emphasise the dangers of ignoring spatial heterogeneity, especially in short-term field data. Aims. To test whether Brown’s index of patchiness (BIP) can offer a simple means of checking rodent and mustelid survey data for violations of the assumption of uniform distribution. Methods. We use BIP to interrogate long-term legacy data collected by index trapping of mice (Mus musculus), rats (Rattus rattus and R. norvegicus) and stoats (Mustela erminea) in New Zealand forests. Key results. We found evidence of moderately patchy distributions that were independent of abundance in all three species. In two South Island beech (Nothofagus) forest valleys, 19% (6 of 31) of mouse samples and 8% (3 of 36) of stoat samples were significantly patchy, correlated with a seedfall event; in mixed forest at Pureora in the North Island, significant patchiness in distribution of ship rats was recorded in 19% (16 of 84) of Fenn trap samples and 5% (2 of 42) of rodent trap samples. Conclusions. Moderate patchiness is common. The consequences for any given study depend on the purpose of the work, but may be more important for practical management than for population modeling.

Pipe support functionality is critical to the long term life of piping system. Spring supports degrade with time due to flexing in the spring and wear in constant support bearings. It is not unusual to measure constant support hangers with resistance 25% to 50% different than the design load. This leads to excessive sustained pipe stress, pipe sag (or uplift), and in high temperature systems, accelerated creep damage. Supports may also not move properly from shut down to full operation, which can create excessive fatigue stress, failed hanger components, and other piping system damage. In-situ hanger testing has been found to be a reliable and cost effective method to determine the functionality of pipe supports. Results are used as input to set revised recommended loads, and to determine if any hangers need to be replaced. This paper provides examples of the types of problems that are often found in the field, the method to test, and typical resolutions to maximize the pipe life and minimize the risk of failure.

A majority of patients with tuberculosis (TB) in India are diagnosed and treated in the private sector. Yet, most private providers do not use most recent WHO-endorsed microbiological tests such as liquid cultures, line probe assays and Xpert MTB/RIF due to a combination of factors such as lack of awareness, misaligned incentives and high prices that are unaffordable for patients. We designed a market-based approach to transform a high-price, low-volume market equilibrium into a low-price, high-volume equilibrium to improve the uptake of these tests. Toward this end, a non-profit consortium of private laboratories, called Initiative for Promoting Affordable and Quality Tuberculosis Tests (IPAQT) was formed in India in March 2013. It negotiated lower pricing on equipment and reagents with manufacturers, closer to that offered to the public sector. In return, IPAQT assured that this discount was passed on to patients, who typically paid for these tests out of their pockets, through an informally agreed on retail ceiling price. IPAQT also invested in demand generation activities that complemented the supply-side effort. IPAQT membership grew from 56 laboratories in 2013 to 211 in 2018. During this period, the initiative resulted in a 10-fold increase in the uptake of Xpert and a 30%–50% reduction in price. This initiative is planned to be expanded to other South Asian countries with similar TB epidemic and private market structure and dynamics. However, long-term sustainability of the initiative would require developing more cost-effective marketing activities and integration with broader private sector engagement agenda of the national TB programme.

The long-term economics of mitigating climate change over the long run has played a high profile role in the most important analyses of climate change in the last decade, namely the Stern Report and the IPCC’s Fourth Assessment. However, the various kinds of uncertainties that affect these economic results raise serious questions about whether or not the net costs and benefits of mitigating climate change over periods as long as 50 to 100 years can be known to such a level of accuracy that they should be reported to policymakers and the public. This paper provides a detailed analysis of the derivation of these estimates of the long-term economic costs and benefits of mitigation. It particularly focuses on the role of technological change, especially for energy efficiency technologies, in making the net economic results of mitigating climate change unknowable over the long run. Because of these serious technical problems, policymakers should not base climate change mitigation policy on the estimated net economic impacts computed by integrated assessment models. Rather, mitigation policies must be forcefully implemented anyway given the actual physical climate change crisis, in spite of the many uncertainties involved in trying to predict the net economics of doing so.

Purpose The purpose of this paper is to argue the many ways in which the conservation and revitalisation processes in Novara carried out over several decades are representative not only of the Italian approach to urban conservation but also of the fruitful relationship between institutional and social bodies. Design/methodology/approach Through an exploratory study approach, this paper illustrates how social actors contributed to the conservation process of Novara Old Town, and documents the regeneration of two historic complexes, the Castle of Novara and Casa Bossi. To do so, it drew upon content analyses of the official reports and planning tools and a set of semi-structured interviews conducted with the representatives of the community organisations and the Municipal Planning Board. Findings The study demonstrates the effectiveness of a third actor whose double role entails cooperation and conflict. The participatory approach applied in these two heritage complexes has proved to be cost-effective because it is inherently able to effect “planning in the public domain” and address socially sustainable outcomes. Originality/value The Italian approach to the conservation of historic cities has been widely acknowledged as a “good recipe” in the international context. However, little attention has been paid to how this success story depends on a unique relationship between public bodies and local communities. To fill this gap, the paper shed light on the historic centre of Novara and two particular heritage complexes in terms of the relations of different actors in conservation and regeneration processes.

In developing countries, road maintenance is a major problem. The country's economy relies significantly on public transit. Driving a vehicle on a badly maintained road is very dangerous. Rain causes the road's oil-spill quality to degrade. Car accidents may occur as a result of such obstacles. To solve this problem, we have developed a project that we believe would be of the most use to the public. The project's main aim is to automatically detect and report potholes. This article discusses a cost-effective method for detecting potholes and speed bumps on roadways, as well as giving timely warnings to drivers to assist them avoid accidents or vehicle damage. Potholes and humps are detected using ultrasonic sensors, which may also be utilised to assess their depth and height. A global positioning system receiver is used in the proposed system to record the geographic locations of potholes and speed bumps. Some of the discovered data in the database include pothole depth, hump height, and geographic location. This is a valuable resource for both government officials and motorists. As an alert, flash messages with an audible buzzer are used.

This article is a product of the research project "Public policies, citizenship and the human rights approach in the Norte de Santander Department" carried out as a public management project - social extension of the Comfanorte Higher Studies Foundation - FESC in the Public Management Specialization Program. The aim was to describe conceptual and methodological elements that would allow the proposal of foundations for the formulation of public policies with a long-term vision and a human rights-based approach for the Norte de Santander Department, with a scope that includes the identification of the problem, the definition of the population, the diagnosis and the action plan. To this end, from the qualitative approach, theoretical foundations were adopted to support the proposal under the protection of interdisciplinary information sources through interviews with public policy experts in the region. Finally, the importance of differential approaches in the formulation of public policy was recognized, since their specificity guarantees in a differential manner the satisfaction of the rights of the different population groups, as well as the transverse and incidental participation of the citizenry as necessary in all phases of the formulation, which is fundamental to the success of public policy.
