Tetratrichomonads from the oral cavity and respiratory tract of humans

To clarify the taxonomy of trichomonads associated with human respiratory diseases, we examined a collection of axenic trichomonad strains isolated from the oral cavity and bronchi of patients from pulmonary diseases clinics in Tallin, Estonia. The oral and bronchial strains were compared mutually as well as with a reference strain of Trichomonas tenax, a common inhabitant of the human oral cavity, and other trichomonad species from humans and animals. Unexpectedly, the morphological studies, as well as DNA sequencing of ITS1-5.8S rRNA-ITS2 regions revealed that the Estonian strains belong to the genus Tetratrichomonas, with a high similarity to the avian species Tetratrichomonas gallinarum. None of the strains belonged to Trichomonas tenax. DNA fingerprinting using the RAPD method separated Estonian strains into 2 distinct groups: ‘bronchial’ consisting of 5 and 2 strains isolated from bronchi and ‘oral’ cavity, respectively, and oral consisting of 3 oral strains. Consistent differences between ‘bronchial’ and ‘oral’ groups were confirmed by analysis of ITS1-5.8S rRNA-ITS2 sequences. Our results have revealed novel trichomonad species of the human oral cavity and bronchi.

Aim. To study the number of neutrophils in the oral cavity, their bactericidal potential, to assess as an indicator for predicting the course of recurrent bronchitis (J40) and community-acquired focal pneumonia in children. Methods. 87 children between 5 and 10 years old, including 52 children with recurrent bronchitis and 35 with focal community-acquired pneumonia were observed. The control group consisted of 37 conditionally healthy children of a similar age. Viral antigens were studied by chemiluminescence immunoassay. Oral neutrophil counts and functional activity were determined. Antibacterial antibodies were measured by an enzyme-linked immunosorbent assay (ELISA). Results. 70.11% of patients had a viral antigen, and 57.47% had immunoglobulins M and G against bacterial pathogens. Oral neutrophil counts increased in the main group compared to the control group: up to 163.826.5 cells (p 0.001) in recurrent bronchitis, to 110.925.5 (p 0.05) in community-acquired pneumonia. By the recovery period, the number of oral neutrophils counts decreased in recurrent bronchitis (1.7 times higher compared to the control group, p 0.01) and remained practically unchanged in community-acquired pneumonia (115.026.9, p 0.05). Myeloperoxidase level had opposite changes for the groups compared to the control group: with recurrent bronchitis, it was 1.610.09 to the level in the control group (p 0.05), with community-acquired pneumonia 0.730.09 to the level in the control group (p 0.001). The level of lysosomal cationic proteins decreased to 0.770.09 to the level in the control group (p 0.05) in recurrent bronchitis, and to 0.800.09 (p 0.05) in pneumonia. Conclusion. In inflammation of the respiratory tract, neutrophil migration to the oral cavity, as well as myeloperoxidase level, increases, indicators of spontaneous luminol-dependent chemiluminescence are activated, and a deficiency of lysosomal cationic proteins occurs; this prevents the penetration of the pathogen into the lower respiratory tract.

Exosomes are nanosized vesicles released from every cell in the body including those in the respiratory tract and lungs. They are found in most body fluids and contain a number of different biomolecules including proteins, lipids, and both mRNA and noncoding RNAs. Since they can release their contents, particularly miRNAs, to both neighboring and distal cells, they are considered important in cell-cell communication. Recent evidence has shown their possible importance in the pathogenesis of several pulmonary diseases. The differential expression of exosomes and of exosomal miRNAs in disease has driven their promise as biomarkers of disease enabling noninvasive clinical diagnosis in addition to their use as therapeutic tools. In this review, we summarize recent advances in this area as applicable to pulmonary diseases.

Alumina refineries are among the leading sources of atmospheric air pollution with a wide range of pollutants hazardous to human respiratory organs. It is relevant to study and evaluate the occurrence of the risks for development of respiratory diseases in children living in the area affected by the emission components of an alumina refinery. We assessed air quality of the area under observation and comparison according to monitoring observations, risk of non-carcinogenic effects from the respiratory organs. The content of chemicals in the blood and urine adequate to risk factors was quantified. The structure of individual groups of respiratory diseases was analyzed. The causal relationships of violations of laboratory parameters with an increased content of chemicals in biological media were evaluated. It was found that an aerogenic exposure of chemical pollutants is formed on the territory with the production of metallurgical alumina. It determines the risk for development of respiratory diseases, exceeding an acceptable level up to 49.9 times. In the exposed children, the content of manganese, chromium, nickel, copper, xylenes, formaldehyde and aluminum, fluoride ion in the urine was increased to 4.2 times in relation to the indices in the comparison group. A high level of additional respiratory morbidity(1.8 times) was revealed. Chronic lymphoproliferative diseases of the nasopharynx and inflammatory diseases of the upper respiratory tract (up to 6.6 times more often), inflammatory diseases with a predominance of the mechanism of allergic inflammation ( up to 2.1 times more often)are more often detected in the framework of the respiratory diseases. Negative effects on the part of the respiratory system in the form of activation of antioxidant processes, the development of an inflammatory reaction, local, general and specific sensitization of the respiratory tract were established. It confirms the occurrence of the risks for the development of respiratory diseases in children in the exposure area of the chemical factors of alumina refinery-associated economic activity.

ABSTRACT Members of the phylum “Synergistetes” have frequently been detected in the human oral cavity at sites of dental disease, but they have rarely been detected in studies of oral health. Only two oral “Synergistetes” taxa are cultivable. The aims of this study were to investigate the diversity of “Synergistetes” in the oral cavity, to establish whether “Synergistetes” taxa are more strongly associated with periodontitis than with oral health, and to visualize unculturable “Synergistetes” in situ. Sixty samples (saliva, dental plaque, and mucosal swabs) were collected from five subjects with periodontitis and five periodontally healthy controls. Using phylum-specific 16S rRNA gene primers, “Synergistetes” were identified by PCR, cloning, and sequencing of 48 clones per PCR-positive sample. Subgingival plaque samples were labeled with probes targeting rRNA of unculturable oral “Synergistetes” using fluorescent in situ hybridization (FISH). Analysis of 1,664 clones revealed 12 “Synergistetes” operational taxonomic units (OTUs) at the 99% sequence identity level, 5 of which were novel. “Synergistetes” OTU 4.2 was found in significantly more subjects with periodontitis than controls (P = 0.048) and was more abundant in subgingival plaque at diseased sites than at healthy sites in subjects with periodontitis (P = 0.019) or controls (P = 0.019). FISH analysis revealed that unculturable oral “Synergistetes” cells were large curved bacilli. The human oral cavity harbors a diverse population of “Synergistetes.” “Synergistetes” OTU 4.2 is associated with periodontitis and may have a pathogenic role.

The activation of the kinin system of the blood in diseases of the respiratory system leads to an increase in the inflammatory response in the respiratory tract, causes bronchospasm to a large extent, and has an inhibitory effect on the function of the ciliated epithelium. There are no unequivocal judgments about the therapeutic effect of prednisolone in obstructive bronchitis and bronchiolitis in the literature. There is a negative answer to this question. At the same time, the beneficial effect of corticosteroids on pulmonary mechanics in bronchiolitis was noted.

Introduction. The actuality of children allergic respiratory diseases problem is due to the steady increase oftheir occurrence all over the world. Computerized capillaroscopy (ССS) of the nail bed is a non-invasive and highly informative method for assessing the structural and functional parameters of capillaries in real time and physiological conditions, and so it is effectively applied in pediatric practice. However, studies in this promising direction are rare. Aim. Was to determine the microcirculation peculiarities in children suffering from respiratory allergic diseases with prolonged cough, using the method of computerized capillaroscopy of the nail bed. Materials and methods. 238 children aged from 2 to 17 years with acute and chronic respiratory diseases, accompanied by prolonged (more than 4 weeks) cough, were examined and divided into 4 groups (Gr): Gr1 (n=68) – patients with acute or exacerbation of the chronic upper respiratory tract diseases of the infectious genesis; Gr2 (n=53) – patients with lower respiratory tract infection; Gr3 (n=39) – patients with allergic rhinitis; Gr4 (n=78) – patients with bronchial asthma (BA). All patients underwent history, examination and ССS of the nail bed. Results. Patients with allergic diseases of the respiratory tract, especially with BA, were found to have structural and functional disorders of the capillary bed and a pronounced increase in the length of the perivascular zone indicating an increase in the hydration degree of the interstitial space due to chronic allergic inflammation, in contrast to children with respiratory tract infection getting changed microcirculation parameters mainly in the venous part of capillaries. Conclusion. CСS of the nail bed is a highly informative method to identify functional features of the capillaries in allergic and infectious pathologies of the respiratory system, which can be successfully used as additional criteria for their differential diagnosis.
