AbstractThe general condition, clinical and pathological characteristics, and treatment regimens of patients prenatally and postnatally diagnosed with congenital choledochal malformation (CM) were analyzed in order to investigate the clinical significance of early diagnosis, treatment, and intervention in CM. We retrospectively analyzed 33 children who were admitted to the Children’s Hospital of Soochow University between 1 March 2010 and 31 May 2019, and their diagnosis of CM was confirmed by radiological, surgical and pathological findings. All the patients were under 36 months of age. The patients were divided into prenatally diagnosed and postnatally diagnosed groups. There were 16 and 17 CM patients in the prenatally and postnatally diagnosed groups, respectively, with a preponderance of females in both groups. Compared with the prenatally diagnosed group, the postnatally diagnosed group had a higher incidence of abdominal pain and vomiting (p < 0.05) and higher AST, GGT, and TB levels (p < 0.05). Although postoperative histopathological examination showed inflammation in both groups, congestion in the cyst walls and fibrous tissue hyperplasia were more significant in the postnatally diagnosed group (p < 0.05). In addition, operation time, length of time required to resume a normal diet after surgery, and total length of hospitalization differed between the 2 groups (p < 0.05), with the prenatally diagnosed group having a relatively longer operation time and taking longer to resume a normal diet after surgery. However, the total length of hospitalization in the prenatally diagnosed group was shorter than that in the postnatally diagnosed group. Compared with prenatally diagnosed CM patients, more symptoms, greater severity of symptoms, and more time to recovery after surgery were observed in postnatally diagnosed CM patients.

Bizzare parosteal osteochondromatous proliferation, or Nora‘s lesion is a unique bone lesion that most often arises in the small bones of hands and feet.  It is characterised by proliferation of chondroid, bony and fibrous tissue, and is occasionally misdiagnosed as a malignant process.  Our case was a 31 yr old lady, who presented with a painless swelling near the 5th metacarpal bone of right hand.  X-ray showed well marginated mineralised mass arising from the cortical surface of the metacarpal bone.  Histopathological examination revealed bizarre parosteal osteochondromatous proliferation composed of varying amounts of cartilage, bone and spindle cells. Cartilage was hypercellular and chondrocytes were enlarged. Ossification was irregular and had a peculiar blue tinctorial quality.

Background: Acetaminophen is a medication used to treat pain and fever. It is typically used for mild to moderate pain relief. Spirulina is a biomass of cyanobacteria that cultivated worldwide and used as a dietary supplement or whole food. It is also used as a feed supplement in the aquaculture, aquarium, and poultry industries. The present study was undertaken to determine the effect of Spirulina on blood and kidney in mice. Methods: Twenty Swiss Albino mice (Mus musculus) were randomly divided into 4 equal groups’ viz., A, B, C, and D. Group A was kept as control and fed only normal diet. Group B was supplemented with acetaminophen where as group C was supplemented with acetaminophen and Spirulina, and group D was treated with Spirulina. Results: The application of acetaminophen did not have significant effect on hemoglobin and uric acid content. But the addition of Spirulina to the diet increased significantly (P<0.05) hemoglobin and uric acid level. Histopathological examination revealed that no significant changes were found in the kidney of all of treated groups in comparison with the mice of control group. Conclusions: The research work suggests that long-term ingestion of acetaminophen does not exert a significant change in hemoglobin or uric acid level but spirulina supplemented diet may be associated with the rise in hemoglobin and uric acid level without affecting renal tissue texture in a significant manner.

Background. Fibrous hamartoma of infancy is a rare soft tissue lesion of infants and young children with characteristic triphasic morphology. Case Description. An 18-month-old female child was presented with complaints of swelling over right leg shin since birth. On examination, a lump of size 7x3 cm was identified which was mobile and nontender. Local excision was performed and tissue sent for histopathological examination. On gross examination, a globular, capsulated, firm to hard tissue had cut section revealing solid grey-white to grey-brown lesion with myxoid areas identified. Microscopic examination revealed a poorly circumscribed lesion comprising intersecting trabeculae of fibrous tissue, areas of immature oval and stellate cell within myxoid matrix, and varying amounts of interspersed mature fat cells. Conclusion. Even though fibrous hamartoma of infancy is a rare benign entity with limited clinical knowledge, proper diagnosis is mandatory as its prognosis is excellent.

This paper presents rotordynamic performance measurements of multilayer gas foil journal bearings (GFJBs) supporting the rotor of oil-free microturbomachinery, and a comparison with the model predictions. A series of rotor coast-down tests from 60 krpm were conducted to compare the rotordynamic performances of three previously developed multilayer GFJBs: types A, B, and C. During the tests, two sets of orthogonally positioned displacement sensors recorded the horizontal and vertical rotor motions, and an axially positioned displacement sensor measured the thrust of the runner axial motion. The test results revealed that the type C GFJBs have a superior rotordynamic capability over the other types. The additional coast-down tests from 100 krpm for the type C showed that the synchronous motions of the rotor are dominant at up to ∼50 krpm, but that large amplitudes of subsynchronous motion associated with the natural frequency of a rotor-GFJB system occur above this speed. Thermal transient response measurements were conducted using four k-type thermocouples at increasing rotor speeds of 20 to 100 krpm with increments of 10 krpm. The operation time required to establish steady-state temperatures was approximately 25 min for each speed. For most of the speeds tested, the front GFJB near the rotor impeller end showed the lowest temperatures, and both the rear GFJB near the thrust runner end and the permanent magnet (PM) motor showed the highest temperatures. The GFTB showed the lowest temperature at low speeds of below 50 krpm, and the highest temperature at the top speed of 100 krpm owing to the increasing axial load caused by the impeller force. The measured impeller pressure and motor output power increased nonlinearly with the increasing rotor speed and fits best with the second-order and third-order polynomial equations, respectively. The measured axial displacement revealed that the rotor moved axially up to ∼ 270 μm toward the impeller side as the speed increased to 100 krpm. Further experiments using a decrease in radial clearance of 30 μm demonstrated a suppression of the large amplitude of the subsynchronous rotor motion to a certain degree. In addition, the onset speed of the subsynchronous motions increased to 80 krpm for the type C GFJBs with the decrease in the radial clearance. Rotordynamic model predictions with the predicted GFJB stiffness and damping coefficients were benchmarked against the test data. The predicted natural frequencies, onset speed of instability (OSI) where the damping ratio became negative, and synchronous rotor response versus speed agreed reasonably with the measured whirl frequencies of the subsynchronous motions, the onset speed of subsynchronous motions (OSS), and the filtered synchronous rotor motion versus speed, respectively. The predictions also showed that the OSI increased from 50 krpm to 80 krpm with a decrease in the radial clearance, thus validating the present rotordynamic model.

Abstract Schistosomiasis is a serious parasitic infection affecting millions worldwide. This study aimed to explore the anti-schistosomal activity of curcumin and curcumin loaded gold-nanoparticles (Cur-GNPs) with or without praziquantel (PZQ). We used six groups of the C57BL/6 mice in which five groups were infected with Schistosoma Mansoni (S. mansoni) cercariae and exhibited, separately, to different treatment regimens of curcumin, curcumin loaded nanoparticle, and PZQ, in addition to one untreated group which acts as a control. Mice were sacrificed at the 8th week where both worms and eggs were counted in the hepatic and porto-mesenteric vessels in the liver and intestine, respectively, in addition to a histopathological examination of the liver granuloma. Curcumin caused a significant reduction in the worms and egg count (45.45%) at the 3rd week. A significant schistosomicidal effect of PZQ was found in all groups. Cur-GNPs combined with PZQ 97.4% reduction of worm burden in the 3rd week and the highest reduction in the intestinal and hepatic egg content, as well, besides 70.1% reduction of the granuloma size. The results suggested the curcumin in combination with PZQ as a strong schistosomicidal regimen against S. mansoni as it alters the hematological, biochemical, and immunological changes induced.

Background. Angiofibroma is a benign tumor, consisting of fibrous tissue with varying degrees of vascularity, characterized by proliferation of stellate and spindle cells around the blood vessels. It most commonly arises from the nasopharynx, although it may rarely arise in extranasopharyngeal sites. Case Report. A 46-year-old male presented with left side nasal obstruction and epistaxis for one month. Clinical nasal examination revealed left sided polypoidal mass arising from the vestibular region of the lateral nasal wall. Results. CT scan and MRI showed highly vascular soft tissue mass occupying the anterior part of the left nostril. Preoperative selective embolization followed by transnasal excision was performed. Histopathological examination confirmed the diagnoses of nasal vestibular angiofibroma. Conclusion. Extranasopharyngeal angiofibroma is a very rare pathology. It should be kept in mind as a differential diagnosis with any unilateral nasal vestibular mass causing nasal obstruction and epistaxis. A biopsy without further investigation can cause life threatening bleeding in the patient.

Proliferative-inflammatory pathologies may occupy the temporal bone, resulting in: hearing loss, vestibular dysfunction, and neuropathies from cranial nerve compression. Although their occurrence is episodic, the appropriate diagnostic procedure is extremely important to achieve expected therapeutic effect. The aim of study was characterization of selected proliferative-inflammatory pathologies that may occupy the temporal bone: fibrous dysplasia, inflammatory pseudotumor, osteoradionecrosis, and presentation of diagnostic methods for the differentiation of these diseases as well as discussion on appropriate therapeutic options. Fibrous dysplasia (fibrous dysplasia) is a slowly progressive, benign bone disorder of unknown etiology characterized by abnormal proliferation of fibrous tissue. IPT (inflammatory pseudotumor) is a rare, non-malignant inflammatory process of unknown etiology, characterized by connective tissue proliferation and infiltration of inflammatory cells. Osteoradionecrosis of the temporal bone (TB-ORN) is a rare but potentially fatal complication of radiotherapy for head and neck cancer. Due to the similarity of symptoms with typical inflammatory conditions of middle ear (pain, otorrhea, hearing loss), selected disorders may be a dilemma regarding the diagnosis and proper further treatment. The clinical examination is mandatory, however radiological imaging may demonstrate the existence of specific changes and direct the diagnosis. The computed tomography of fibrous dysplasia shows the abnormal organization of the bone structure. Magnetic resonance, as the most sensitive for inflammatory pseudotumors, visualizes inflammatory infiltrates in soft tissues. The spiral tomography of temporal identifies the erosion in the course of osteoradionecrosis. However the final diagnosis may be establish post the histopathological examination and exclusion of the neoplastic process.

SummaryCathaemasia hians is an obligate trematode parasite of Black storks that are on the List of protected birds in Europe. In the present study, adult trematodes were isolated from the Black stork post mortem and morphological study revealed C. hians species. In total, 10 worms were found in the oesophagus and the ventriculus of the bird. Histopathological examination of the tissue samples of oesophagus, proventriculus and ventriculus was performed on paraffin sections using a set of staining procedures. The sporadic lesions were seen in the tela submucosa of oesophagus containing connective tissue mast cells, eosinophils and heterophils and some foci were surrounded by the fibrous tissue. In addition, a few inflammatory nodules had larval-like material inside, probably being of the same species. There were no visible morphological alterations in the epithelial layer of lamina propria mucosae of proventriculus, rich in goblet cells as well as in the tela submucosa. Majority of trematodes were localized in the ventriculus, where the lamina propria mucosae was damaged or disrupted sporadically. In these sites, in the tela submucosa, a various food-originated inorganic/organic material and eggs of C. hians were deposited, stimulating a weak inflammatory response. Nodules containing larvae were not observed in any of ventriculus tissue layers. This study demonstrated, for the first time, infection with adults of C. hians trematode in the Black stork nesting in Slovakia. The presence of larvae and eggs in the tissues of the upper gastrointestinal tract of bird was associated with mild inflammatory response but feeding behaviour of adult worms in the ventriculus probably contributed to the enhanced susceptibility of the lamina propria mucosae to mechanical damage by inorganic material. Although larval stages have not yet been documented in the intermediate hosts in Slovakia, our report indicates that the life cycle of C. hians might occur in Central Europe.

ABSTRACT CONTEXT: Lymphedema consists of extracellular fluid retention caused by lymphatic obstruction. In chronic forms, fat and fibrous tissue accumulation is observed. Genital lymphedema is a rare condition in developed countries and may have primary or acquired etiology. It generally leads to urinary, sexual and social impairment. Clinical treatment usually has low effectiveness, and surgical resection is frequently indicated. CASE REPORT: We report a case of a male-to-female transgender patient who was referred for treatment of chronic genital lymphedema. She had a history of pelvic radiotherapy to treat anal cancer and of liquid silicone injections to the buttock and thigh regions for esthetic purposes. Radiological examinations showed signs both of tissue infiltration by liquid silicone and of granulomas, lymphadenopathy and lymphedema. Surgical treatment was performed on the area affected, in which lymphedematous tissue was excised from the scrotum while preserving the penis and testicles, with satisfactory results. Histopathological examination showed alterations compatible with tissue infiltration by exogenous material, along with chronic lymphedema. CONCLUSION: Genital lymphedema may be caused by an association of lesions due to liquid silicone injections and radiotherapy in the pelvic region. Cancer treatment decisions for patients who previously underwent liquid silicone injection should take this information into account, since it may represent a risk factor for radiotherapy complications.
