AbstractThe incidence of dry eye disease is increasing worldwide because of the aging population and increasing use of information technology. Dry eye disease manifests as tear-layer instability and inflammation caused by osmotic hypersensitization in tear fluids; however, to our knowledge, no agent that treats both pathologies simultaneously is available. Molecular hydrogen (H2) is known to be effective against various diseases; therefore, we aimed to elucidate the effects of H2on tear dynamics and the treatment of dry eye disease. We revealed that administering a persistent H2-generating supplement increased the human exhaled H2concentration (p < 0.01) and improved tear stability (p < 0.01) and dry eye symptoms (p < 0.05) significantly. Furthermore, H2significantly increased tear secretion in healthy mice (p < 0.05) and significantly suppressed tear reduction in a murine dry eye model (p = 0.007). H2significantly and safely improved tear stability and dry eye symptoms in a small exploratory group of 10 human subjects, a subset of whom reported dry eye symptoms prior to treatment. Furthermore, it increased tear secretion rapidly in normal mice. Therefore, H2may be a safe and effective new treatment for dry eye disease and thus larger trials are warranted.

The high and increasing prevalence of Dry Eye Disease (DED) highlights the need for new treatment treatments and more effective management strategies for this chronic disease. After training, lid grooming, and various ocular lubricants, the Tear Film & Ocular Surface Society Dry Eye Workshop II (TFOS DEWS II) Management and Therapy Subcommittee recently proposed Intense Pulsed Light (IPL) as the second phase of therapy. Brief flashes of non-coherent light (400–1,200 nm) are delivered to the skin’s surface using IPL technology. Toyos et al. found in 2005 that rosacea sufferers who were treated with IPL in the periocular region had a significant increase in their dry eye symptoms.

Abstract To assess the impact of COVID-19 lockdown measures on dry-eye symptoms in a community-based population, a cross-sectional study was conducted during the first wave of the COVID-19 outbreak in Thailand. An online survey was distributed via social media between June and July 2020. The questionnaire elicited information on demographics, dry-eye symptoms, viewing habits, and mental health status. There were 535 respondents. Thirty-seven percent reported having been diagnosed with dry-eye disease (DED). During the lockdown, the mean dry-eye symptom score (DESS) of overall participants dropped significantly from 81.6±15.9 to 79.8±17.4 (P<0.001). The mean, daily, visual display terminal (VDT) usage increased from 10.55±5.16 hours to 13.08±5.65 hours (P<0.001). A negative correlation between age and VDT usage was observed in both the normal and lockdown situations. One-quarter of all participants had an abnormal mental health status. The female gender (OR 1.86; 95% CI 1.14–3.04) and increased VDT usage during the lockdown (OR 5.68; 95% CI 3.49–9.23) were independently associated with worsening dry-eye symptoms. The lockdown measures abruptly altered the behaviors and lifestyles of the overall population. Excessive exposure to VDTs were associated with deteriorated dry-eye symptoms, and it possibly contributed to the increased DED incidence in the general population.

Intense pulsed light (IPL) are medical-esthetical procedures that emit light at a wavelength of 500 – 1200 nm, interacting with epidermal and dermal tissues. IPL is a relatively new treatment of growing popularity thanks to its versatility and efficacy, mainly in dermatology and recently also in ophthalmology. These devices are used to treat dry eye disease, meibomian gland dysfunction, rosacea, and periocular lesions with outstanding results.

Purpose. To investigate tear function, ocular manifestations, and squamous metaplasia of the conjunctival epithelium (SMCE) in children with end stage renal failure (ESRF) on dialysis.Methods. Thirty children with ESRF and 20 age and sex matched controls during the period from January 2014 to May 2015 underwent full ophthalmologic examination and the tear function was investigated by the Schirmer and tear film break-up time (TBUT) tests. SMCE was evaluated by impression cytology and immunocytochemistry. The correlations of tear function status with ESRF-related clinical and biochemical variables were measured statistically.Results. Dry eye symptoms were detected in 26% of children with ESRF, compared with none of the controls (P=0.05) and SMCE was almost absent. Values of the Schirmer and TBUT tests were significantly lower in children with ESRF for right eye (t=24.63,P=0.01, andt=11.9,P=0.002, resp.) and left eye (t=24.7,P=0.02, andt=11.4,P=0.0004, resp.). TBUT and the Schirmer test values were correlated inversely with the duration of ESRF (R=-0.45,P=0.01, andR=-0.46,P=0.01, resp.) and with the duration of dialysis (R=-0.39,P=0.03, andR=-0.45,P=0.01, resp.). None of the following parameters was associated with distorted tear function including serum creatinine, electrolytes, parathyroid hormone, total protein, albumin, CBC parameters, and systolic or diastolic blood pressure.Conclusion. The basal tear secretion and tear film stability were lower while the dry eye symptoms such as itching and redness were more common among children with ESRF. The duration of ESRF and dialysis duration seem to be related to the disturbances in tear secretion and tear film stability. However, SMCE is very rare.

Abstract Background While aging is a potent risk factor of dry eye disease, age-related gut dysbiosis is associated with inflammation and chronic geriatric diseases. Emerging evidence have demonstrated that gut dysbiosis contributes to the pathophysiology or exacerbation of ocular diseases including dry eye disease. However, the relationship between aging-related changes in gut microbiota and dry eye disease has not been elucidated. In this pilot study, we investigated the association between aging-dependent microbiome changes and dry eye severity in C57BL/6 male mice. Results Eight-week-old (8 W, n = 15), one-year-old (1Y, n = 10), and two-year-old (2Y, n = 8) C57BL/6 male mice were used. Dry eye severity was assessed by corneal staining scores and tear secretion. Bacterial genomic 16 s rRNA from feces was analyzed. Main outcomes were microbiome compositional differences among the groups and their correlation to dry eye severity. In aged mice (1Y and 2Y), corneal staining increased and tear secretion decreased with statistical significance. Gut microbiome α-diversity was not different among the groups. However, β-diversity was significantly different among the groups. In univariate analysis, phylum Firmicutes, Proteobacteria, and Cyanobacteria, Firmicutes/Bacteroidetes ratio, and genus Alistipes, Bacteroides, Prevotella, Paraprevotella, and Helicobacter were significantly related to dry eye severity. After adjustment of age, multivariate analysis revealed phylum Proteobacteria, Firmicutes/Bacteroidetes ratio, and genus Lactobacillus, Alistipes, Prevotella, Paraprevotella, and Helicobacter to be significantly associated with dry eye severity. Conclusions Our pilot study suggests that aging-dependent changes in microbiome composition are related to severity of dry eye signs in C57BL/6 male mice.

The study is aimed at investigating the effect of dry eye disease on the level of protein and lipids in tears of postmenopausal women. The study was carried out on 33 postmenopausal women with dry eye symptoms and 22 postmenopausal women that were asymptomatic. The ocular surface disease index (OSDI) questionnaire classified participants as being symptomatic or asymptomatic of dry eye. Tear stability and tear volume were assessed with Fluorescein Tear Break up Time (FBUT) and Schirmer’s test respectively. Tears were analysed for protein (total protein, albumin, and globulin) and lipid (cholesterol and triglycerides). The mean total protein for Dry Eye (DE) was higher than that for Non-dry eye (NDE). Albumin and globulin were also higher for the DE group when compared to the NDE group. These were however, not statistically significant (p > 0.05). There was also no significant difference in triglycerides and cholesterol between DE and NDE. There was no significant correlation between clinical signs (tear volume and tear breakup time) and total protein, albumin, globulin cholesterol and triglycerides in the NDE and DE eye groups. The study shows that proteins and lipids do not change in postmenopausal women irrespective of the presences of dry eye disease.
