Squat Muscle Activation Patterns with Hip Rotations in Subjects with Genu Varum Deformity

AbstractThe purpose of this study was to compare muscle activation during the squat with different hip rotations (neutral, 15, 30, and 45° of internal and external hip rotation) in subjects with and without Genu Varum deformity deformity. Surface electromyography were recorded from 32 men with (n=16) and without (n=16) Genu Varum deformity. In the Genu Varum deformity group, the squats with 30, 45 and 15° of internal rotations of the hip showed significantly greater gluteus medius activation as compared to other positions. Moreover, the tensor fascia lata activity increased with greater external rotation of the hip, and significantly more than hip internal rotations (p<0.05). For vastus medialis and vastus lateralis, both hip internal and external rotation showed a significantly greater activation compared to the neutral hip positions (p<0.05). There were significant differences in the gluteus medius:tensor fascia lata activity ratio (p=0.001) and the vastus medialis: vastus lateralis activity ratio (p=0.001) between the different hip positions in the Genu Varum deformity and healthy groups. These results demonstrate that muscle activation patterns varied significantly with the position of different hip rotation in both groups. Those with Genu Varum deformity may use this information to aid in an injury prevention strategy by choosing squat positioning that favorably alters muscle activation patterns.

Purpose:Manufacturers of uncoupled cycling cranks claim that their use will increase economy of motion and gross efficiency. Purportedly, this occurs by altering the muscle-recruitment patterns contributing to the resistive forces occurring during the recovery phase of the pedal stroke. Uncoupled cranks use an independent-clutch design by which each leg cycles independently of the other (ie, the cranks are not fixed together). However, research examining the efficacy of training with uncoupled cranks is equivocal. The purpose of this study was to determine the effect of short-term training with uncoupled cranks on the performance-related variables economy of motion, gross efficiency, maximal oxygen uptake (VO2max), and muscle-activation patterns.Methods:Sixteen trained cyclists were matched-paired into either an uncoupled-crank or a normal-crank training group. Both groups performed 5 wk of training on their assigned cranks. Before and after training, participants completed a graded exercise test using normal cranks. Expired gases were collected to determine economy of motion, gross efficiency, and VO2max, while integrated electromyography (iEMG) was used to examine muscle-activation patterns of the vastus lateralis, biceps femoris, and gastrocnemius.Results:No significant changes between groups were observed for economy of motion, gross efficiency, VO2max, or iEMG in the uncoupled- or normal-crank group.Conclusions:Five weeks of training with uncoupled cycling cranks had no effect on economy of motion, gross efficiency, muscle recruitment, or VO2max compared with training on normal cranks.

ACL reconstruction (ACLr) has been found to restore the functionality and mobility of those afflicted with ACL tears. In the process of this restoration, previous work has shown alterations in the activity of the musculature surrounding the reconstructed knee [1]. Specifically, at 5 months post-op, the gastrocnemius, vastus lateralis, and rectus femoris demonstrate significantly different activation patterns. These differences raise the question if the reconstructed leg is the only limb affected by ACLr.

Although it is known that the muscle activation patterns used to produce even simple movements can vary between individuals, these differences have not been considered to prove the existence of individual muscle activation strategies (or signatures). We used a machine learning approach (support vector machine) to test the hypothesis that each individual has unique muscle activation signatures. Eighty participants performed a series of pedaling and gait tasks, and 53 of these participants performed a second experimental session on a subsequent day. Myoelectrical activity was measured from eight muscles: vastus lateralis and medialis, rectus femoris, gastrocnemius lateralis and medialis, soleus, tibialis anterior, and biceps femoris -long head. The classification task involved separating data into training and testing sets. For the within-day classification, each pedaling/gait cycle was tested using the classifier, which had been trained on the remaining cycles. For the between-day classification, each cycle from day 2 was tested using the classifier, which had been trained on the cycles from day 1. When considering all eight muscles, the activation profiles were assigned to the corresponding individuals with a classification rate of up to 99.28% (2,353/2,370 cycles) and 91.22% (1,341/1,470 cycles) for the within-day and between-day classification, respectively. When considering the within-day classification, a combination of two muscles was sufficient to obtain a classification rate >80% for both pedaling and gait. When considering between-day classification, a combination of four to five muscles was sufficient to obtain a classification rate >80% for pedaling and gait. These results demonstrate that strategies not only vary between individuals, as is often assumed, but are unique to each individual. NEW & NOTEWORTHY We used a machine learning approach to test the uniqueness and robustness of muscle activation patterns. We considered that, if an algorithm can accurately identify participants, one can conclude that these participants exhibit discernible differences and thus have unique muscle activation signatures. Our results show that activation patterns not only vary between individuals, but are unique to each individual. Individual differences should, therefore, be considered relevant information for addressing fundamental questions about the control of movement.

Background: The leg press is one of the most typical exercises for strengthening the lower limbs. The objectives of this study were to compare 5 inclined leg press exercise conditions, varying the feet width stance (100% or 150% hip width), the feet rotation (0° or 45° external rotation) on the footplate and using 2 different movement velocities (MVs; maximum intended, and 2:2 seconds steady-paced velocities) to determine their effect on muscle activation as well as on the kinematic parameters between trained men and trained women. Hypotheses: There will be no significant differences in muscle activation with regard to the feet position. The higher the MV, the greater the muscle activation. Study Design: A cross-sectional cohort study. Level of Evidence: Level 3. Methods: A repeated-measures between-group design was performed to examine muscle activation and kinematic parameters for the different conditions between gender groups. The level of significance was set at alpha = 0.05 for all statistical analyses. Results: Muscle activation presented no differences between conditions regarding feet width stance or feet rotation. Furthermore, muscle activation was greater during positive phases than negative phases of the exercise for all conditions and was also greater under maximum intended velocity conditions compared with steady-paced conditions. Otherwise, the muscle activation pattern presented slight differences by gender. In men, the greatest muscle activation was for the vastus medialis, followed by the vastus lateralis (VL), rectus femoris (RF), and gluteus medialis (GMED), while in women, the greatest muscle activation was for the vastus medialis, followed by the RF, VL, and GMED. Finally, greater mean propulsive velocity, maximum velocity, maximum power, and footplate displacement values were reported for men than for women under all the conditions. Conclusion: The inclined leg press exercise produces the highest muscle activation in the vastus medialis, regardless of the velocity, feet stance, or gender. Clinical Relevance: Given that there are no differences in muscle activation regarding the feet stance, a participant’s preferred feet stance should be encouraged during the inclined leg press exercise. Furthermore, the MV would preferably depend on the session objective (a training or a rehabilitation program), being aware that there is greater muscle activation at higher speeds. The inclined leg press exercise could be performed as a closed kinetic chain exercise when the main objective is to activate the vastus medialis.

Knee joint muscle activation imbalances, especially weakness in the vastus medialis oblique, are related to patellofemoral pain within the female population. The available literature presents the leg press as an exercise which potentially targets vastus medialis oblique activation, thus reducing imbalances in the quadriceps muscles. The main aim of the present study was to compare thigh muscle activation and kinematic parameters under different conditions during the inclined leg press exercise in a young female population. A cross-sectional study was conducted on 10 young, trained females. Muscle activation of the vastus medialis oblique, vastus lateralis, rectus femoris and gluteus medialis was analyzed under five different inclined leg press conditions, modifying the feet rotation (0–45° external rotation) and the stance width (100–150% hip width) on the footplate. All the conditions were performed at two different movement velocities: controlled velocity (2″ eccentric–2″ concentric) and maximal intended velocity. Mean propulsive velocity, maximum velocity and maximum power were also assessed. The results show that both controlled velocity conditions and maximal intended velocity conditions elicited a similar muscle activation pattern with greater activation during the concentric phase (p < 0.001, ηp2 = 0.96). The maximal intended velocity conditions showed greater overall muscle activation (p < 0.001, ηp2 = 0.91). The vastus medialis oblique presented the greatest muscle activation, followed by the rectus femoris, vastus lateralis and, the gluteus medialis. Furthermore, the inclined leg press condition with 0º feet rotation, 100% hip width distance and the maximal intended velocity generated the greatest kinematic parameter outputs. In conclusion, the inclined leg press exercise might be an optimal exercise to target vastus medialis activation regardless of the feet rotation and stance width conditions.

Background The phases of the football throw need definition so that muscle activation patterns during the overhead football throw can be fully described. Hypothesis Electromyographic analysis of shoulder musculature can better define muscle activation patterns during the football throw. Study Design Descriptive anatomic study. Methods Videos of 20 elite-level quarterbacks were reviewed to define phases of the overhead football throw; 14 recreational male athletes underwent electromyography and motion analysis testing. Results Four sequential phases of the football throw were consistently observed. Early cocking (49% ± 11% of throw) was initiated at rear foot plant and continued to maximal shoulder abduction and internal rotation. Late cocking (20% ± 6%) started at maximal shoulder abduction and internal rotation and ended with maximal shoulder external rotation. The acceleration phase (15% ± 4%) began with maximal shoulder external rotation and ended with ball release. Follow-through (16% ± 5%) was defined as the phase from ball release to maximal horizontal adduction (across the body). Conclusion The four phases demonstrated little variation in motion analysis and electromyographic activation between subjects and were associated with muscle activation patterns consistent with upper extremity movements. Clinical Relevance A clearer understanding of muscle activation patterns may help to explain patterns of muscle injury and improve rehabilitation protocols in football-throwing athletes.

This study compared the kinetics, barbell, and joint kinematics and muscle activation patterns between a one-repetition maximum (1-RM) Smith machine squat and isometric squats performed at 10 different heights from the lowest barbell height. The aim was to investigate if force output is lowest in the sticking region, indicating that this is a poor biomechanical region. Twelve resistance trained males (age: 22 ± 5 years, mass: 83.5 ± 39 kg, height: 1.81 ± 0.20 m) were tested. A repeated two-way analysis of variance showed that Force output decreased in the sticking region for the 1-RM trial, while for the isometric trials, force output was lowest between 0–15 cm from the lowest barbell height, data that support the sticking region is a poor biomechanical region. Almost all muscles showed higher activity at 1-RM compared with isometric attempts (p < 0.05). The quadriceps activity decreased, and the gluteus maximus and shank muscle activity increased with increasing height (p ≤ 0.024). Moreover, the vastus muscles decreased only for the 1-RM trial while remaining stable at the same positions in the isometric trials (p = 0.04), indicating that potentiation occurs. Our findings suggest that a co-contraction between the hip and knee extensors, together with potentiation from the vastus muscles during ascent, creates a poor biomechanical region for force output, and thereby the sticking region among recreationally resistance trained males during 1-RM Smith machine squats.

AbstractAge-related weakness due to atrophy and fatty infiltration in oropharyngeal muscles may be related to dysphagia in older adults. However, little is known about changes in the oropharyngeal muscle activation pattern in older adults. This was a prospective and experimental study. Forty healthy participants (20 older [> 60 years] and 20 young [< 60 years] adults) were enrolled. Six channel surface electrodes were placed over the bilateral suprahyoid (SH), bilateral retrohyoid (RH), thyrohyoid (TH), and sternothyroid (StH) muscles. Electromyography signals were then recorded twice for each patient during swallowing of 2 cc of water, 5 cc of water, and 5 cc of a highly viscous fluid. Latency, duration, and peak amplitude were measured. The activation patterns were the same, in the order of SH, TH, and StH, in both groups. The muscle activation patterns were classified as type I and II; the type I pattern was characterized by a monophasic shape, and the type II comprised a pre-reflex phase and a main phase. The oropharyngeal muscles and SH muscles were found to develop a pre-reflex phase specifically with increasing volume and viscosity of the swallowed fluid. Type I showed a different response to the highly viscous fluid in the older group compared to that in the younger group. However, type II showed concordant changes in the groups. Therefore, healthy older people were found to compensate for swallowing with a pre-reflex phase of muscle activation in response to increased liquid volume and viscosity, to adjust for age-related muscle weakness.
