Postnatal Paraclinical Parameters Associated to Occurrence of Intracerebral Hemorrhage in Preterm Infants

AbstractIntracerebral hemorrhage (ICH) is the most frequent complication in postnatal development of preterm infants. The purpose of the present work is the statistical evaluation of seven standard paraclinical parameters and their association to the development of ICH. Clinical records of 265 preterm infants with gestational age (GA) 23 to 30 weeks were analyzed. According to ICH status, patients were divided into control (without ICH) and affected (with ICH) groups. Mean values of paraclinical parameters at each week of gestation were compared. Different ICH grades, periods before and after ICH were considered separately. Lower hematocrit, SaO2, and pH were statistically significant for preterm infants with 23 to 30 weeks GA and diagnosis of ICH relative to infants without ICH. Additionally, for preterm infants with 27 to 30 weeks GA, higher C-reactive protein, as well as lower values of thrombocytes were associated with the occurrence of ICH. Preterm infants with 23 to 26 weeks GA showed C-reactive protein values similar to those in the group without ICH and lower levels of thrombocytes after bleeding. Significant differences in paraclinical parameters between preterm infants with and without ICH may constitute useful indicators for closer clinical observation of preterm infants at risk of ICH.

Abstract Short-term intensive exercise may be associated with many short-lasting metabolic changes. These changes depend on the duration and intensity of the exercise. The aim of our study was to determine potential changes in C-reactive protein (CRP) and selected haematological and biochemical parameters in clinically healthy German shepherd dogs before short duration high-intensity exercise and 6 hours after the exercise. During the study, the dogs were subjected to the following defensive training: detaining a figurant running away from a dog (2×) and detaining with a counterattack (2×). The running distance was 200 m at a mean speed of 28 km.h-1. The investigation of haematological parameters revealed a significant decrease in the mean values of platelet haematocrit (PCT) 6 hours after the training. Significant differences in the levels determined before and after exercise were observed also for phosphorus (P) and for iron (Fe). At the same time, we observed a significant increase in the mean calcium (Ca) level and a significant decrease in sodium (Na) and chlorides (Cl). The activity of aspartate aminotransferase (AST) significantly increased after exercise. Concentrations of acute phase C-reactive proteins were increased 6 hours after exercise in comparison to those before exercise but the difference was insignificant. Although the concentration of CRP was increased insignificantly after the exercise, when interpreting concentrations of this nonspecific indicator of inflammation, one should keep in mind that intensive exercise may also affect its concentrations.

Objective Evaluation of the inflammatory activity in patients on chronic peritoneal dialysis (PD) and patients on chronic hemodialysis (HD) in comparison to patients with chronic renal insufficiency without dialysis treatment and healthy volunteers. Design Open, non randomized prospective study. Setting Nephrology Department, including HD and PD therapy in a university hospital. Patients Twenty -four patients on chronic PD, 21 patients on chronic HD therapy using a cuprophan dialyzer, 16 patients with chronic renal insufficiency without dialysis treatment, and 33 healthy volunteers; 8 additional patients before and after initiation of chronic HD therapy. All patients and controls were without infection or immunosuppressive therapy. Main Outcome Measures As a marker of the inflammatory activity in the different patient groups, C-reactive protein (CAP) was measured serially using a sensitive, enzyme-Iinked, immunosorbent assay in order to detect values below the detection limit of standard assays. Results All patient groups had CAP levels higher than the normal controls (p < 0.01). Patients on HD had CAP levels significantly higher than PD patients (p < 0.01) whose levels were comparable to patients without dialysis therapy. Accordingly, longitudinal measurements before and after initiation of chronic HD showed a significant increase in CAP levels after the beginning of HD treatment (p < 0.04). Conclusions The results suggest that induction of the inflammatory activity is lower during PD compared to HD, since stimulation by the dialyzer membrane, dialysate buffer, or bacterial fragments in the dialysate is avoided. This observation might indicate a possible lower risk of long-term complications in patients with PD.

<b><i>Objective:</i></b> Timely detection of elevated intracranial pressure (ICP) in highrisk preterm infants may be critical to avoid permanent neurologic sequelae. Size of optic nerve sheath diameter (ONSD) is highly correlated with changes in ICP. Normal ultrasonographic ONSD values for preterm infants have been published. This study sought to compare these data with MRI measured OSND and to propose suggested ultrasonographic ONSD values. <b><i>Methods:</i></b> The ONSD in preterm MRIs were retrospectively measured and related to pre-existing ultrasonographic ONSD. Data were stratified for corrected gestational age. Simple linear regression between ONSD mean values and age was modeled for both eyes, and <i>R</i><sup>2</sup> was calculated. Suggested values for ultrasonographic ONSD were ascertained through linear regression and calculated prediction intervals. <b><i>Results:</i></b> ONSD measurements demonstrated <i>R</i><sup>2</sup> values of 0.95 (right ONSD MRI), 0.95 (left ONSD MRI), 0.96 (right ONSD ultrasound), and 0.93 (left ONSD ultrasound). Suggested ONSD values were incremental with corrected gestational age. <b><i>Conclusion:</i></b> ONSD measurements with MRI and ultrasound are similar. The proposed suggested ONSD values may be helpful in clinical situations where ICPs are suspected or known.

This study compared the effectiveness of the neutrophil/lymphocyte ratio (NLR) versus C-reactive protein (CRP) for evaluating the prognosis and degree of inflammation in patients with amputation for a diabetic foot ulcer (DFU). This study enrolled 56 patients with amputations for DFU with gangrene and compared the CRP levels and NLR measured before and after surgery. Overall, 24 patients (42%) died within 2 weeks postoperatively. Mortality increased with a preoperative/postoperative CRP difference ≤1.5 ( P < .001) and age 73 years or older ( P < .001). The postoperative NLR was lower than the preoperative value but was not significant as a prognostic or inflammatory marker ( P = .945). An increasing serum CRP level is a significant predictor of mortality. CRP and old age are reliable prognostic factors in patients with DFU.

Background/Aim. Injury-induced anergy is one of the key factors contributing to trauma victims' high susceptibility to sepsis. This group of patients is mostly of young age and it is therefore essential to be able to predict as accurately as possible the development of septic complications, so appropriate treatment could be provided. The aim of this study was to assess kinetics of interleukin (IL) -6 and -10, phospholipase A2- II and C-reactive protein (CRP) in severely traumatized patients and explore the possibilities for early detection of potentially septic patients. Methods. This prospective study included 65 traumatized patients with injury severity score (ISS) > 18, requiring treatment at surgical intensive care units, divided into two groups: 24 patients without sepsis and 41 patients with sepsis. C-reactive protein, IL-6 and -10 and phospholipase A2 group II, were determined within the first 24 hours, and on the second, third and seventh day of hospitalization. Results. Mean values of IL-6 and phospholipase A2-II in the patients with and without sepsis did not show a statistically significant difference on any assessed time points. In the septic patients with ISS 29-35 and > 35 on the days two and seven a statistically significantly lower level of IL-10 was found, compared with those without sepsis and with the same ISS. C-reactive protein levels were significantly higher in septic patients with ISS 18-28 on the first day. On the second, third and seventh day CRP levels were significantly lower in the groups of septic patients with ISS 29-35 and > 35, than in those with the same ISS but without sepsis. Conclusion. Mean levels of CRP on the first day after the injury may be useful predictor of sepsis development in traumatized patients with ISS score 18-28. Mean levels of CRP on the days two, three and seven after the injury may be a useful predictor of sepsis development in traumatized patients with ISS score more than 28. Mean levels of IL-10 on the second and seventh day after the injury may be a useful predictor of sepsis development in traumatized patients with ISS score > 28.

Abstract Background Because of the immaturity and severe condition preterm infants undergoes a lot of painful and stressful procedures during their care that can cause short-term and long-term negative consequences. Maintenance of optimal comfort is essential for good clinical practice. To assess the comfort of preterm infants before and after the capillary blood sampling procedure. Methods The quantitative observational study was conducted in May 2019—February 2020 in the University hospital NICU. The research instrument was a neonatal comfort scale COMFORTneo, which total scores ranged 6–30. Higher scores indicated higher discomfort. Preterm infant comfort was assessed before the capillary blood sampling procedure and 30 min. after applying non-pharmacological pain relief and comfort assurance methods and implementing the procedure. The research was conducted in accordance with ethical principles. Results Comfort was assessed in 157 preterm newborns with a gestational age of 30 ± 3.3 weeks and a postnatal age of 6.0 ± 5.7 days. The mean value of neonatal comfort before the capillary blood sampling procedure was 10.0 ± 3.2 points, after the procedure 9.1 ± 2.4 points. A statistically significant inverse relationship was found between pre-procedure and post-procedure comfort (Z = -5.374; P &lt; 0.001). A statistically significant weak inverse relationship was found between gestational age and comfort scores before the procedure (r = -0.204; P = 0.01) and after the procedure (r = -0.321; P = 0.008). Conclusions In most cases, preterm infants were feeling moderate discomfort before and after the capillary blood sampling procedure. Newborns had more discomfort before the procedure than 30 min. after the completion of the procedure. Lower gestational age newborns were feeling greater discomfort.

Background: Sleep-disordered breathing is a common problem in children and is associated with increased cardiovascular morbidity. The circulating level of C-reactive protein, a pro-inflammatory protein, is associated with increased risk for atherosclerosis. The aim of this study was the comparison of C-reactive protein level and erythrocyte sedimentation rate in children with tonsillar hypertrophy before and after adenotonsillectomy. Methods: All patients aged between 4 to 10 years who had underwent adenotonsillectomy due to obstructive sleep apnea were enrolled. Serum C-reactive protein and erythrocyte sedimentation rate were measured before and after adenotonsillectomy. Data were analyzed with SPSS version 19. Results: In this study, 384 patients were enrolled. The average age of the patients was 6.7 years. The C-reactive protein level in 91 patients (23.7 %) was decreased and in 286 patients (74.5%) did not change and in 7 patients (1.8%) it was increased. Decreasing serum C-reactive protein level and erythrocyte sedimentation rate was statistically significant after adenotonsillectomy (P-value < 0.05). Conclusion: Obstructive sleep apnea induces a systemic inflammatory response so frequently associated with an increase in serum C-reactive protein and erythrocyte sedimentation rate that is reversible with surgery and if left untreated may lead to cardiovascular morbidity.
