Abstract Aim : The aim of the present study is to provide parameters of 2nd metacarpal bones for sex determination from Saurashtra populations. Material and method: Forty two pairs of 2nd metacarpal bones were measured and their dimensions were compared with the dimensions of Spanish and American white population. The mean, SD, t value and p value were calculated, and demarcating point method for sexual dimorphism was employed. Result : Mean interarticular length of 2nd metacarpal was higher in male as compared to female for both sides (right and left) and the difference was statistically highly significant. Mean anteroposterior width at base of 2nd metacarpal was higher in male as compared to female for both sides(right and left),but the difference was statistically insignificant for right side and statistically significant for left side. Conclusion: The present study showed that interarticular length and anteroposterior width at base of 2nd metacarpal are effective parameters for sex determination.

Background: Since a long time ago, the experts have realized that determination of cut-off point for diagnosing diabetes will be revised over time with the lower blood glucose level as the more sensitive diagnosis for detecting the occurring complication and biochemical changes.Methods: This cross sectional study was carried out in the department of medicine, M.G.M. Medical College and M.Y. Hospital Indore from July, 2016 to August, 2017 in 200 individuals and patients having euglycemic status attending General Medicine OPD.Results: In the low and high normal group 2 (2.0%) and 8 (8.0%) were having abnormal total cholesterol (TC) level respectively. The mean total cholesterol in the low normal group was 117.16±26.94mg/dl and it was 154.74±28.38mg/dl in the high normal group. The difference was found to be statistically significant (p value 0.000). In the low and high normal group, 4 (4.0%) and 17 (17.0%) were having abnormal triglyceride (TG) levels respectively. The mean TG levels in the low and high normal group were 96.93±22.64mg/dl and 110.55±32.37mg/dl respectively. The difference was found to be statistically significant (p value 0.001). In the low and high normal group, 6 (6.0%) and 14 (14.0%) patient was having abnormal uric acid levels respectively. The mean uric acid levels in the low and high normal group was 4.88±1.10mg/dl and 5.31±1.31mg/dl respectively. The difference was found to be statistically significant (p value 0.013).Conclusions: Higher levels of Cholesterol and Triglycerides were found more commonly in high normal euglycemic group compared to low normal euglycemic group. Mean cholesterol and mean triglyceride levels were higher in high normal euglycemic group.

Abstract Context Computerized tomography (CT) is an invaluable imaging investigation for evaluating COVID-19 disease. CT detects early changes of COVID-19 pneumonia and predicts the disease prognosis based on a semiquantitative 25-point CT severity score (CT-SS). India launched its vaccination drive in January 2021 with two different vaccines being approved by the government. These vaccines are believed to prevent the disease itself, in majority of the cases and at least decrease disease severity, in the rest. Aim This study aims to evaluate the CT-SS in vaccinated and non-vaccinated subjects who have been diagnosed with COVID-pneumonia or are COVID suspects. Subjects and Methods A total of 3,235 patients with typical COVID-19 related imaging findings on HRCT thorax were included in the study. These subjects were divided into three age categories, 18–44, 45–59 and ≥60 years. The CT severity scores were allotted by experienced radiologists. Medians of the scores in different age groups were compared amongst vaccinated and non-vaccinated individuals using the Kruskal–Wallis H test. A p-value < 0.05 was considered significant. All results were shown with 95% confidence interval. Results The difference in the medians amongst the vaccinated and non-vaccinated groups was significant, p-values being < 0.001 in all age categories. Conclusion The mean CT-SS was less in vaccinated subjects and the difference in median CT-SS amongst vaccinated and non-vaccinated individuals was statistically significant, thus sending an important message that it is mandatory for the population at large to get vaccinated to reduce infection rate/disease severity.

Background: Moderate anaemia seen in about 15-20% of pregnant women. Iron sucrose complex which is used intravenously for the correction of Iron deficiency anaemia. The drug has been able to raise the haemoglobin to satisfactory level when used in moderately anaemic iron deficient pregnant women. The objective of this study was to study the improvement of Hb% after treatment with intravenous Iron sucrose complex in moderately anaemic pregnant women belonging to 24-32 weeks of gestational age.Methods: 50 antenatal patients between gestational age 24-32 weeks with hemoglobin between 8-9.5g/dl were selected and included in this study. They were subjected to blood hemoglobin estimation, hematocrit and peripheral smear study. In each infusion, the maximum total dose administered was 200 mg iron sucrose in 100 ml of normal saline, slow IV infused over 30 minutes. Monitoring was done throughout the infusion to observe for any side effects.Results: Mean hemoglobin among the 50 patients before starting the therapy was 8.172g/dl and the mean hemoglobin at the end of one month of completing the therapy was 11.066g/dl. The rise in mean hemoglobin i.e. the difference in the mean hemoglobin before and after treatment was 2.894g/dl. The p value is 0.0001 which is statistically significant. The mean hematocrit of the 50 patients studied before starting the treatment was 26.772% with a standard deviation of 1.914. The mean hematocrit after completing the therapy was 33.872% with a standard deviation of 1.321. The difference in the mean hematocrit was 7.100% with a p value of 0.0001 which is statistically significant.Conclusions: Intravenous iron sucrose complex is well tolerated and highly efficacious in improving hemoglobin, hematocrit in the treatment of iron deficiency anaemia in antenatal women.

Farmers Perception Of Options To Sell Rice Penebas Or Perpadi(especially in Subak Benel, Kaliakah Village, Negara districts,Jembrana Regency) Various efforts have been implemented by the Provincial Food Crops Agency of Balito stabilize the price of grain, this is done by giving Funds of Rural EconomicEmpowerment Capital to the government for the purchase of rice. Although it hasbeen done from 2003, farmers are still selling it to Penebas with a bondage system.The purpose of this study to determine the perception of farmers to the choice ofselling rice to Perpadi or Penebas seen and differences in farmers' perceptions of thechoice of selling rice to Penebas or Perpadi. The research location is located in SubakBenel, Kaliakah Village, State District, Jembrana District. The analytical methodused is qualitative descriptive that aided with score, to answer the purpose ofresearch by using questionnaire. The results of this study indicate that the perceptionof farmers to the choice of selling rice to Perpadi better than to Penebas. This is seenin the achievement of the farmers' choice of selling paddy to Perpadi is very goodand to Penebas is good. The difference of Perpadi with Penebas is measured throughDifferent Test with SPSS Independent Sample T test program which the result of Sigvalue. Or p value of 0.004 where <0.05 then there is a statistically significantdifference in probability 0.05, the mean difference or mean of both groups is shownin Mean Difference ie .29559.

Abstract: Special need children is children with disability mental, physical, and emotion which different with the normal children, thus their more needed parents help in keeping hygene especially oral hygene. Every disability they have influenced the behaviour of special need children in keeping their oral hygiene. This study aimed to obtain the difference of oral hygiene between special needs children in SLB-B and SLB-C in Tomohon.This was a descriptive analytical study. Samples were obtained by total sampling method. This study was conducted at SLB-B GMIM Damai Tomohon and SLB-C Katolik Santa Anna Tomohon. There were 101 children in this study. The results of independent t test showed that there were significant differences between the mean value of OHI-S status at SLB-B (1.86) and the mean value OHI-S status at SLB-B ( 2.50) with a P value of <0,05. Conclusion: Oral hygiene of SLB-B children was significantly better than of SLB-C children.Keywords: oral hygiene, special need childrenAbstrak: Anak berkebutuhan khusus merupakan anak yang memiliki keterbatasan mental, fisik dan emosi yang berbeda dengan anak normal, sehingga mereka memerlukan bantuan dalam menjaga kebersihan diri khusunya kebersihan gigi dan mulut. Perbedaan keterbatasan yang mereka miliki, memengaruhi perilaku anak berkebutuhan khusus dalam menjaga kebersihan gigi dan mulut. Penelitian ini bertujuan untuk melihat bagaimana perbedaan status kebersihan gigi dan mulut pada anak berkebutuhan khusus di SLB-B dan SLB-C kota Tomohon, Jenis penelitian yang digunakan adalah deskriptif analitik. Pengambilan sampel penelitian ini secara total sampling. Pengambilan data dilaksanakan di SLB-B GMIM Damai Tomohon dan SLB-C Katolik Santa Anna Tomohon. Jumlah anak dalam penelitian sebanyak 101 anak. Hasil penelitian diolah dengan uji statistik t tidak berpasangan (independent t test).Dari uji statistik diperoleh bahwa terdapat perbedaan rerata yang bermakna, antara status OHI-S SLB-B dengan nilai rata-rata 1,86 dibandingkan status OHI-S SLB-C dengan nilai rata-rata 2,50 dan nilai p<0,05. Simpulan: Rerata status kebersihan gigi dan mulut SLB-B lebih baik secara bermakna dibandingkan dengan SLB-C.Kata kunci: kebersihan gigi dan mulut, anak berkebutuhan khusus

Hepatic encephalopathy (HE), a syndrome observed in some patients with cirrhosis, with depressed level of consciousness. Lactulose as well as lactitol has been used in the treatment of HE. Lactitol is comparable to lactulose in the treatment of HE with fewer side effects and better tolerated. However, literature showed equal efficacy of both drugs. So we conducted this trial to find better drug to implement its use in future. Objective: To compare the effectiveness of lactulose and lactitol in patients with acute hepatic encephalopathy. Study Design: Randomized Controlled Trial. Setting: Department of Medicine OPD and Emergency (East, West, North, South), Mayo Hospital, Lahore. Period: 6 Months June 2017 to Dec 2017. Material & Methods: 570 patients were included through non-probability, consecutive sampling after informed consent. Initial grade of HE was assessed and patients were randomly divided in two groups by using lottery method i.e. lactulose or lactitol. Patients were admitted to ward for management and kept under observation for 5 days. After 5 days, HE grades was measured again, then improvement in grade of HE (effectiveness) was measured. All data was entered and analyzed by using SPSS version 21.0. Chi-square was applied to compare both groups for effectiveness taking p-value≤0.05 as significant. Results: In this study the mean age of the patients was 44.22 ±11.81 years, the male to female ratio of the patients was 2.4:1. The mean duration of the cirrhosis of the patients was 3.73±1.61 months. In our study the effectiveness was achieved in 538 (94.39%) patients, out of which 263 cases were from lactulose group and 275 were from lactitol group and the difference was significant (p<0.0.5). Conclusion: Our study results concluded that Lactitol is better choice for the treatment of patients with acute hepatic encephalopathy as compared to lactulose. More efficacy was achieved in lactitol group patients than in lactulose group patients.

Objective: To compare the effect of 0.5% and 0.75% hyperbaric Bupivacaine on haemodynamic stability in terms of mean systolic blood pressure and heart rate recorded at 4 min in patients undergoing caesarian section in spinal anesthesia. Study Design: Quasi experimental study. Place and Duration of Study: Department of Anaesthesiology, Combined Military Hospital, Malir, from Jul to Dec 2018. Methodology: The patients were assigned in two groups (A and B) using lottery method. Group A received 0.5% hyperbaric Bupivacaine solution. Group B received 0.5% hyperbaric Bupivacaine solution. Spinal anaesthesia was given, blood pressure and heart rate were recorded. Data were analyzed in SPSS version 23. Both groups were compared for mean systolic blood pressure and heart rate by using independent sample t-test. Results: The mean age of patients was 29.62 ± 6.21 years in 0.75% Bupivacaine group while 29.31 ± 6.20 years in 0.5% Bupivacaine group. The mean systolic blood pressure of patients was 111.63 ± 5.96 mmHg in 0.75% Bupivacaine group while 117.16 ± 7.12 mmHg in 0.5% Bupivacaine group. The difference was significant in both groups (p-value <0.05). The mean heart rate of patients was 92.27 ± 4.71 beats per min (bpm) in 0.75% Bupivacaine group while 97.68 ± 4.58 bpm in 0.5% Bupivacaine group. The difference was significant in both groups (p-value <0.05). Conclusion: 0.5% hyperbaric Bupivacaine was better than 0.75% hyperbaric Bupivacaine solution in spinal anaesthesia during caesarean section.

Introduction: All surgical procedures on nose affect the olfactory function to some extent. There are different viewpoints about olfactory function after septorhinoplasty. We aim to investigate smell perception changes in patients following septorhinoplasty. In this study the use of Rapid Smell Test (RST) as a fast and reliable clinical tool was investigated. Methods: A prospective cohort study was done in Rasool Akram teaching hospital in 2018 and 2019. Group of 74 septorhinoplasty candidates aged over 18 was selected by convenience. Persian Smell Identification Test (PSIT) and rapid smell test (RST) was obtained from patients before surgery and one month after, if needed three months after and six months after. In addition for patients with dysfunction in any of follow up periods (one month, 3 months and 6 months after surgery) smell magnitude test (SMT) was used. All data were analyzed using Statistical Package for Social Sciences, version 24.0 (IBM SPSS Statistics Inc., Chicago, IL, USA). Results: Out of 74 patients participating in this study 67 were female. Mean age of 25.68±5.18 years was recorded for participants. All patients before surgery had PSIT score more than 19 and the mean PSIT score before operation was 22.63±1.39. Also all patients scored 5 out of 5 in RST before surgery. Mean PSIT score one month after surgery decreased to 22.21±2.20 but still 93 percent (69 patients) of participants were in range for normal olfactory function. We recorded this to be significant (p value = 0.044). RST after one month showed the same results. The difference between RST before and after surgery was also significant (p value = 0.024). For those 5 patients with abnormal olfactory function PSIT and RST were done 3 months after operation which determines normal olfactory function for all of them. As well as PSIT, RST also shows the same results and five participants recorded 5 out of 5 three months after surgery. Conclusion: This study showed changes in olfactory function after septorhinoplasty will resolve at most 3 months after surgery and it is safe to say there is no effect on olfactory function three months after operation. Also we demonstrated that RST can be a fast and reliable tool for assessing patients’ olfactory function in clinical setting.

Cervical carcinoma is the most common gynecological malignancy and almost curable cancer, if identified at an early stage. Cervical biopsy remains the ‘gold standard’ for the diagnosis of precancerous cervical lesion. The objective of this study was to review the histopathological findings of cervical biopsy including cervical polypectomy and hysterectomy specimens at Dhulikhel Hospital from January 2008 to December 2018. Out of 2098 cases, almost half of the cases (49.3%) were of Brahmin/Chhetri castes. There were significant differences in mean ages among hysterectomy cases (47.55±9.26), cervical biopsy cases (43.08±11.50) and polypectomy cases (33.59±12.47) (p value < 0.005). Out of 925 cervical specimens (cervical biopsy and polypectomy), colposcopy and hysteroscopy were performed in 18.2% and 12.7%, respectively. Out of 1173 hysterectomies, there were 110 cases (9.4%) of malignancies and cervical cancer was the most common (67, 60.9%). The mean age of cervical cancer patients was 52.87±11.94 years. Cervical cancer detected in age group of 40 – 59 years was significantly high (p value < 0.005). Incidence of cervical cancer was lowest in Brahmin/Chhetris (2.4%) compared to Newars (3.4%) and other Janajatis (12.8%) and the difference was statistically significant (p value < 0.005). The findings of this study is useful in updating the histopathological pattern of cervical biopsies.

AbstractAnterior cruciate ligament (ACL) is the most frequently injured ligament in the knee and is often injured during sport-related activities. ACL injuries influence the abilities of the subjects during standing and walking. Although early surgical intervention is preferred treatment for the majority of knee surgeons, the effect of this approach on postural stability of patients is not fully understood. Therefore, the aim of this study was to determine the difference between stability of ACL-reconstructed subjects before and after surgery. A group of 15 consecutive ACL injured patients participated in this study. Postural stability of the patients was evaluated 1 week before and 6 months after surgery (ACL reconstruction with hamstring autograft). A Kistler force plate was used to evaluate center of pressure (COP) sway during quiet standing. The mean values of the COP parameters were obtained in pre and postsurgery conditions. Paired sample t-test was used to evaluate the difference between the stability parameters of the two conditions. The significant point was set at 0.05. The mean value of path length of COP velocity in mediolateral (ML) direction was 1,485.57 ± 479.42 mm and 2,641.33 ± 996.26 mm before and after surgery, respectively (p-value = 0.01). Although the mean value of COP velocity in anteroposterior and ML directions increased after surgery, the difference was only significant for velocity in ML direction (p-value = 0.049). The results of this study showed that the standing stability of those with ACL reconstruction decreased significantly after ACL reconstruction, which may be due to the effects of the surgery on sensory mechanism of ACL and inability of patients to return to their previous deep sense perception and knee proprioception.
