Multiple-regression analyses were done to evaluate the effects of aerobic fitness (maximum oxygen uptake per kg fat-free mass), fatness (body fat content, %), body size (as reflected in fat-free mass), and age on the strain (i.e. the physiological and subjective responses) and fireline productivity of four crews of firefighters (N=21) while (1) they undertook 'line rakes', in which they attempted to suppress well-developed experimental bushfires of intensities commonly faced by hand-tool crews, and also built fireline in the same way without fire, during three summers in Australian eucalypt forests; and (2) they underwent the Shvartz heat-tolerance test, which requires 15 minutes of step-climbing at a fixed pace in a room temperature of 23°C. The results showed that the expected effects of fitness and fatness, established through laboratory studies and confirmed in the Shvartz test, were attenuated or absent during the line rakes, apparently because of self pacing and the absence of rapid climbing. In the line rakes fitter men did not work harder than the less fit but were more productive and efficient and had somewhat lower heart rates; fatter men tended to be more productive than leaner men but did not experience greater strain; bigger men (as measured by fat-free mass) were more productive than smaller men; age had no effect on strain or productivity; and associations observed during work in both the line rakes and the Shvartz test were already present before the work commenced, apparently because of emotional arousal. Responses in the Shvartz test (including the heat-tolerance score) were uncorrelated with their counterparts in the line rakes, emphasising the limited application of laboratory findings to normal fire-suppression duties. Productivity and efficiency in timed trials of firefighters' line-building performance were uncorrelated with their counterparts during the line rakes. We conclude that the adverse effects of fatness or low aerobic fitness are minimized by work practices that allow firefighters to adjust their own pace, but will become apparent whenever the pace is fixed, the terrain is steep, or some emergency calls for sustained maximal effort.

Hiking with a pack is the highest-intensity task that wildland firefighters (WLFFs) perform during sustained activities related to wildland fire suppression. Firefighters perform this and other tasks together as a crew; this provides a unique model to evaluate select physical and physiological changes in members of working crews over a fire season during extended operations. The objective of this study was to evaluate changes in peak aerobic fitness (VO2peak), sustainable aerobic fitness at the ventilatory threshold (VO2vt) and body composition over a 5-month wildland fire season. WLFFs from four crews (55 males, 10 females) participated in a maximal graded exercise treadmill test and body composition evaluation pre- and post season. VO2peak values and variance did not change across the fire season (pre=3.96±0.65, post=3.96±0.69Lmin−1, not significant). VO2vt average decreased slightly while variance decreased greatly within each crew (pre=37.5±7.0, post=35.4±2.3mLkg−1 min−1, P<0.05). There was an improvement in VO2vt in initially less-fit WLFFs and a VO2vt decrease in initially more-fit WLFFs. WLFFs lost fat mass (−1.56±−1.06kg, P<0.01) and fat-free mass (−0.38±−1.24kg, P<0.05). Post-season VO2vt values were the same as the higher range of the documented metabolic cost of uphill load carriage and reveal a unique group adaptation to extended physical demands.

We hypothesized that there is an association between the angiotensin I-converting enzyme (ACE) insertion (I)/deletion (D) polymorphism with the variability in exercise heat tolerance in humans. Fifty-eight Caucasian men were exposed to a 2-h exercise heat-tolerance test. We analyzed the association between their heat-tolerance levels with the ACE DD ( n = 25) and I+ ( n = 33) genotypes and with various anthropometrical parameters and aerobic fitness. It was found that the relative changes in body core temperature, heat storage, and heart rate during the 120-min exposure to exercise heat stress was consistently lower in the I+ genotype group compared with the DD genotype group (0.8 ± 0.2 vs. 1 ± 0.1°C, P < 0.05; 17.7 ± 1.8 vs. 19.8 ± 1.3 W/M2, P < 0.05; and 33 ± 7 vs. 44 ± 5 beats/min, respectively, P = 0.06). No significant association was found between heat strain response and the anthropometrical measurements or aerobic fitness in the various genotype groups. We suggest that the ACE I+ polymorphism may be considered as a possible candidate marker for increased heat tolerance.

Hyperinsulinemia, insulin resistance, and increased adipose tissue are hallmarks of the obesity state in both humans and experimental animals. The role of hyperinsulinemia as a possible preceding event in the development of obesity has been proposed. We previously demonstrated that administration of diazoxide (DZ), an inhibitor of insulin secretion, to obese hyperinsulinemic Zucker rats resulted in less weight gain, enhanced insulin sensitivity, and improved glucose tolerance. Assuming that hyperinsulinemia plays a major role in the development of human obesity, then its reversal should have therapeutic potential. To test this hypothesis, we conducted a randomized placebo-controlled trial in 24 hyperinsulinemic adults [body mass index (BMI) &gt; 30 kg/m2]. All subjects were placed on a low-calorie (1260 for females and 1570 for males) Optifast (Sandoz, Minneapolis, MN) diet. After an initial 1-week lead-in period, 12 subjects (mean ± se for age and BMI, 31 ± 1 and 40 ± 2, respectively) received DZ (2 mg/kg BW·day; maximum, 200 mg/day, divided into 3 doses) for 8 weeks; and 12 subjects (mean± se for age and BMI, 28 ± 1 and 43 ± 1, respectively) received placebo. Compared with the placebo group, DZ subjects had greater weight loss (9.5 ± 0.69% vs. 4.6 ± 0.61%, P &lt; 0.001), greater decrease in body fat (P &lt; 0.01), greater increase in fat-free mass to body fat ratio (P &lt; 0.01), and greater attenuation of acute insulin response to glucose (P &lt; 0.01). However, there was no significant difference in insulin sensitivity and glucose effectiveness, as determined by the insulin-modified iv glucose tolerance test (Bergman’s minimal model) and no significant difference in glycohemoglobin values. Conclusion: 8 weeks treatment with DZ had a significant antiobesity effect in hyperinsulinemic obese adults without inducing hyperglycemia.

Advancing age has been found to be associated with a decline in insulin action. Nevertheless, no study has been conducted in healthy centenarians. Our study investigates glucose tolerance and insulin action in centenarians. Fifty-two subjects were enrolled. The subjects were divided in three groups as follows: 1) adults (< 50 yr; n = 20);2) aged subjects (> 75 yr; n = 22); and 3) centenarians (> 100 yr; n = 14). Body composition was studied by bioimpedance analysis. In all subjects, an oral glucose tolerance test and euglycemic glucose clamp were performed. Centenarians have a lower fat-free mass (FFM) than aged subjects and adults, whereas fasting plasma glucose, triglycerides, free fatty acids, urea, and creatinine were not different in the groups studies. Centenarians had a 2-h plasma glucose concentration (6.0 +/- 0.2 mmol/l) that was lower than that in aged subjects (6.6 +/- 0.5 mmol/l, P < 0.05) but not different from adults [6.4 +/- 0.4 mmol/l, P = not significant (NS)]. During the clamp, plasma glucose and insulin concentrations were similar in the three groups. In these conditions, centenarians had a whole body glucose disposal (34.1 +/- 0.6 mumol.kg FFM-1.min 1) that was greater than that in aged subjects (23.3 +/- 0.5 mumol.kg FFM-1.min-1 P < 0.01) but not different from adults (34.6 +/- 0.5 mumol/kg x min, P = NS). In conclusion, our study demonstrates that centenarians compared with aged subjects had a preserved glucose tolerance and insulin action.

The purpose of this study was to examine the effects of 10 weeks of aerobic endurance training (AET), resistance exercise training (RET), or a control (CON) condition on absolute and relative fat mass (FM) or fat-free mass (FFM) in the total body (TB) and regions of interest (ROIs) of sedentary overweight middle-aged males and females. Following prescreening, 102 subjects underwent anthropometric measurements, dual-energy X-ray absorptiometry, and strength and aerobic exercise testing. Randomized subjects (male RET, n = 16; female RET, n = 19; male AET, n = 16; and female AET, n = 25) completed supervised and periodized exercise programs (AET, 30–50 min cycling at 70%–75% maximal heart rate; RET, 2–4 sets × 8–10 repetitions of 5–7 exercises at 70%–75% 1 repetition maximum) or a nonexercising control condition (male CON, n = 13 and female CON, n = 13). Changes in absolute and relative TB-FM and TB-FFM and ROI-FM and ROI-FFM were determined. At baseline, and although matched for age and body mass index, males had greater strength, aerobic fitness, body mass, absolute and relative TB-FFM and ROI-FFM, but reduced absolute and relative TB-FM and ROI-FM, compared with females (p < 0.05). After training, both female exercise groups showed equivalent or greater relative improvements in strength and aerobic fitness than did the male exercise groups (p < 0.05); however, the male exercise groups increased TB-FFM and reduced TB-FM more than did the female exercise groups (p < 0.05). Male AET altered absolute FM more than male RET altered absolute FFM, thus resulting in a greater enhancement of relative FFM. Despite equivalent or greater responses to RET or AET by female subjects, the corresponding respective increases in FFM or reductions in FM were lower than those in males, indicating that a biased dose–response relationship exists between sexes following 10 weeks of exercise training.

The impact of aerobic fitness level on the production and disposal of serum free fatty acids was investigated in 26 normal young volunteers. The fitness level was ascertained by history and confirmed by determination of maximal aerobic capacity. Energy expenditure and substrate oxidation at rest were measured with indirect calorimetry. Free fatty acid turnover was measured with an infusion of [14C]palmitic acid. All tests were done > or = 48 h after the last bout of exercise. The sedentary (SED) volunteers had higher rates of systemic delivery of fatty acids than aerobically fit (FIT) individuals (532 +/- 53.4 vs. 353 +/- 62.3 mumol/min; P = 0.05). This difference was accentuated when the values were normalized to fat-free mass (9.2 +/- 0.8 and 5.9 +/- 0.98 mumol.kg-1.min-1 for SED and FIT, respectively). Fatty acid oxidation was similar between FIT and SED volunteers in absolute numbers (209 +/- 25 vs. 202 +/- 21 mumol/min, respectively; NS) as well as when normalized to fat-free mass (3.8 +/- 0.9 vs. 3.6 +/- 1.4 mumol.kg-1.min-1, respectively; NS). In contrast, the nonoxidative disposal of serum fatty acids was higher in SED (330 +/- 46.1 mumol/min) than in FIT individuals (144 +/- 52 mumol/min; P = 0.026). Thus, the ratio of nonoxidative to oxidative disposal rates of fatty acids was higher in SED than in FIT individuals (1.65 +/- 0.29 vs. 0.75 +/- 0.17; P = 0.021). The data support the hypothesis that high aerobic fitness level is associated with a low rate of systemic delivery of fatty acids at rest. Nevertheless, subjects with high aerobic fitness levels have fat oxidation at the same rate as unfit individuals.
