Infectious Disease as a Foreign Policy Threat

The last 30 years have seen the global consequences of newly emerging and re-emerging infectious diseases, starting with the international spread of HIV/AIDS, the emergence of Ebola and other hemorrhagic fevers, SARS, MERS, novel influenza viruses, and most recently, the global spread of Zika. The impact of tuberculosis, malaria, and neglected tropical diseases on society are now better understood, including how these diseases influence the social, economic, and political environment in a nation. Despite international treaties and norms, the specter of intentional use of infectious disease remains present, particularly as technological barriers to access are reduced. The reality is that infectious diseases not only impact population health, but also have clear consequences for international security and foreign policy. Foreign policy has been used to coordinate response to infectious disease events and to advance population health around the world. Conversely, collaboration on infectious disease prevention, preparedness, and response has been used strategically by nations to advance diplomacy and improve foreign relations. Both approaches have become integral to foreign policy, and this chapter provides examples to elucidate how health and foreign policy have become intertwined and used with different levels of effectiveness by governments around the world. As the scope of this topic is extensive, this article primarily draws from U.S. examples for brevity’s sake, while acknowledging the truly global nature of the dynamic between infectious diseases and foreign policy, and noting that the interplay between them will vary between countries and regions. In 2014, U.S. President Barak Obama called upon global partners to, “change our mindsets and start thinking about biological threats as the security threats that they are—in addition to being humanitarian threats and economic threats. We have to bring the same level of commitment and focus to these challenges as we do when meeting around more traditional security issues”. With world leaders increasingly identifying disease as threats to security and economic stability, we are observing infectious diseases—like no other time in history—becoming an integral component of foreign policy.

Enormous successes have been obtained against the control of major epidemic diseases, such as SARS, MERS, Ebola, Swine Flu in the past. Dynamic interplay of biological, socio-cultural and ecological factors, together with novel aspects of human-animal interphase, pose additional challenges with respect to the emergence of infectious diseases. The important challenges faced in the control and prevention of emerging and re-emerging infectious diseases range from understanding the impact of factors that are necessary for the emergence, to development of strengthened surveillance systems that can mitigate human suffering and death. The aim of the current study is to assess the awareness of symptomatic differences between viral diseases like COVID-19, SARS, Swine flu and common cold among dental students that support the prevention of emergence or re-emergence. Cross-sectional type of study conducted among the undergraduate students comprising 100 Subjects. A questionnaire comprising 15 questions in total were framed, and responses were collected in Google forms in SPSS Software statistical analysis. The study has concluded that dental students have an awareness of the symptomatic differences between infectious viral disease. The study concluded that the awareness of symptomatic differences between viral diseases like COVID-19, SARS, Swine flu, Common cold is good among the dental students who would pave the way for early diagnosis and avoid spreading of such diseases. A further awareness can be created by regular webinars, seminars and brainstorming sessions among these healthcare professionals.

When the underlying causes and mechanisms of emerging infectious disease problems are studied carefully, human behaviour is often involved. Even more often, the only methods of control or prevention available are to change human behaviour. Several major recent emerging disease problems can be cited. It is sometimes emphasized that it is human carelessness, human excesses, human ignorance or human habits of conquest or leisure which contribute directly to the biological niches that microorganisms are all too capable of exploiting. We must look at ourselves as the engines of microbial opportunism. It is not likely that we will ever conquer the microbial world;we must look instead to control the human factors that contribute to emergence.

Wet markets are important for food security in many regions worldwide but have come under scrutiny due to their potential role in the emergence of infectious diseases. The sale of live wildlife has been highlighted as a particular risk, and the World Health Organisation has called for the banning of live, wild-caught mammalian species in markets unless risk assessment and effective regulations are in place. Following PRISMA guidelines, we conducted a global scoping review of peer-reviewed information about the sale of live, terrestrial wildlife in markets that are likely to sell fresh food, and collated data about the characteristics of such markets, activities involving live wildlife, the species sold, their purpose, and animal, human, and environmental health risks that were identified. Of the 59 peer-reviewed records within scope, only 25% (n = 14) focussed on disease risks; the rest focused on the impact of wildlife sale on conservation. Although there were some global patterns (for example, the types of markets and purpose of sale of wildlife), there was wide diversity and huge epistemic uncertainty in all aspects associated with live, terrestrial wildlife sale in markets such that the feasibility of accurate assessment of the risk of emerging infectious disease associated with live wildlife trade in markets is limited. Given the value of both wet markets and wildlife trade and the need to support food affordability and accessibility, conservation, public health, and the social and economic aspects of livelihoods of often vulnerable people, there are major information gaps that need to be addressed to develop evidence-based policy in this environment. This review identifies these gaps and provides a foundation from which information for risk assessments can be collected.

AbstractBackgroundParamedics respond to emergency scenes in often uncontrolled settings without being aware of potential risks. This makes paramedicine one of the most dangerous occupations. One of these dangers is the risk of contracting infectious diseases. Research in this area is predominantly focused on compliance in the use of protective equipment, attitudes and perceptions of paramedics, infectious disease policy, and exposure rates to blood and body fluids. The purpose of this scoping review was to determine what is known about the impact of infectious disease on the health of paramedics.MethodsUsing the Arskey and O’Malley methodological framework, a scoping review was undertaken, which allows for a broad search of the available evidence.ResultsThe literature search identified eight articles for review that reported on paramedic exposure trends; the lack of reported blood-borne infections contracted, such as hepatitis B, hepatitis C, and human immunodeficiency virus (HIV); instances of severe acute respiratory syndrome (SARS) infections; and the higher prevalence of methicillin-resistant staphylococcus aureus (MRSA) nasal infections amongst paramedics.ConclusionsExposure to infectious diseases is decreasing, yet it remains significant. The decrease is attributed to prevention strategies; however, paramedic knowledge and attitudes as well as the uncontrolled environment paramedics work in can be a barrier. Contraction of infectious diseases is generally low; exceptions to this are MRSA colonization, influenza, and SARS. Paramedics are at greater risk of acquiring these infectious diseases compared to the general public. Prior to 1947, India, despite its dependence upon Great Britain, was represented in most of the bonafide international conferences and organizations that evolved especially during the inter-war years. For example, India participated in the Paris Peace Conference of 1919, the Washington Conference on Naval Armaments of 1921, the London Naval Conference of 1930, the Disarmament Conference of 1932 and the annual inter-war conferences of the International Labour Organization. In addition, India was represented in two important international organizations of the inter-war period—the British Commonwealth, in whose deliberations it was included from 1917 onwards and the League of Nations, of which it was a founding member. For a variety of reasons; Indians involved in the independence movement disassociated themselves from and were critical of official Indian diplomacy conducted through the major international conferences and institutions of the world community and tended to attach greater importance to those non-governmental organizations in which the voice of nationalist India could be fully heard—that is to the deliberations of such bodies as the League Against Imperialism, 1927–1930, the Anti-War Congress of 1932, the World Peace Congress of 1936 and the International Peace Campaign Conference of 1938. Nevertheless, despite the nationalist antipathy for official Indian diplomacy, an examination of such governmental institutions as the League of Nations from the perspective of nationalist India is still important in order to understand some aspects of independent India's foreign policy and more specifically its approach to international organization. Further, even though Indian delegations to the League were unrepresentative, there were subtle ways in which they reflected national Indian opinions and exhibited specifically Indian traits, so that a study of the official Indian role is useful in drawing attention to what were to prove to be some of the earliest and most persisting elements of independent Indian diplomacy via such bodies as the United Nations. It is thus the purpose of this article first to explore nationalist Indian attitudes towards the League (especially the reasons for opposition to the organization), second to analyze the extent to which the official Indian role in the League reflected nationalist Indian concerns, and third to comment upon the impact of the League of Nations on independent India's foreign policy, especially its role in the United Nations.

The Euro-Atlantic relations after the end of the Cold war have been strongly influenced by the impact of three interrelated crises: the existential crisis of NATO, the world economic and financial crisis, and the crisis in the Russia-West relations. The end of bipolarity has changed the threat environment and revealed how different alliance members formulate their threat perception and foreign policy interests. Europe stopped to be the US foreign policy priority. The US pivot to Asia has raised European concerns about American commitments to collective defense. The removal of the threat of a global conflict resulted in the EU initiatives aimed at promoting integration in the field of common security and defense policy (CSDP). Even though the US has officially welcomed a stronger European pillar in NATO, it has become concerned about new approaches that could divide transatlantic partnership and take resources away from military cooperation. At the same time the unilateralist preferences of the Bush administration generated deep political divisions between the United States and the European Union. The world economic and financial crisis contributed to a dangerous gulf between American and European defense spending. The US has complained about the tendency of the alliance’s European members to skimp on defense spending and take advantage of America’s security shield to free ride. In the absence of a clear external threat NATO tried to draft new missions, which were found in NATO’s expansion to the post-Communist space and Alliance’s out of area operations. But these new missions could not answer the main question about NATO’s post-bipolar identity. Moreover, the Kosovo operation of NATO in 1999 fueled Russia’s concerns about NATO’s intentions in the post-Soviet space. The creeping crisis in the Russia-West relations resulted in the Caucasus and Ukrainian conflicts that provided kind of glue to transatlantic relations but did not return them to the old pattern. There can be several representing possible futures lying ahead. But under any scenario EU will be faced with a necessity to shoulder more of the burden of their own security.
