Abstract The impact of digitalization of health services has been profound and is expected to be even more profound in the future. It is important to evaluate whether digital health services contribute to health system goals in an optimal way. This should be done at the level of the service, not the ‘digital transformation’. Decisions to adopt new digital health services, at different levels of the health care system, are ideally based on evidence regarding their performance in light of health system goals. In order to evaluate this, a broad perspective should be taken in evaluations of digital health services. Attainment of the broad health system goals, including quality, efficiency and equity, are objectives against which to judge new digital health services. These goals in a broad sense are unaltered by the process of digitalization. Governance should be designed and tailored in such a way to capture all relevant changes in an adequate way. When evaluating digital health services many specific aspects need to be considered. Like for other innovations and (new) technologies, such promises may or may not materialize and potential benefits may also be accompanied by unintended and/or negative (side) effects in the short or long term. Hence, the introduction, implementation, use and funding of digital health technologies should be carefully evaluated and monitored. Governments should play a more active role in the further optimization both of the process of decision making (both at the central and decentral level) and the related outcomes. They need to find a balance between centralized and decentralized activity. Moreover, the broader preparation of the health care system to be able to deal with digitalization, from education, through financial and regulatory preconditions, to implementation of monitoring systems to monitor its effects on health system performance remains important.

This chapter uses a historical perspective to examine the development trajectory of digital health in Singapore since 1980 and the impact of digital health on the current health care system and doctor-patient relationship. It shows that digital health is able to transform a fragmented and provider-centric health care system into a more integrated and patient-centric health care system. Besides, it improves the operational efficiency of health care providers, reduces administrative costs and turnaround time, and empowers patients to contribute in treatment decisions. It shows that the development of digital health requires the government to have strong political will and long-term commitment to support and promote the use of digital health to its full potential and engage stakeholders in the policy making process so that such policy can suit the special needs of stakeholders.

The purpose of this paper is to present the main data about the health care sector depends on how it is financed and thus the choice of an appropriate model healthcare system. The article shows a comparative analysis of health care systems in European countries of the OECD. Based on the literature, the general characteristics of Beveridge and Bismarck systems are presented. The evolution of the health care system in the world arises from a different history, conditions for economic development, diversity, under State policy, geographical location and cultural. Every country in the world takes part in the financing of health care, which is 20-80% of the expenditure on health. According to t. Szumlicza you must distinguish between concepts: "model" and "standard". The "formula" is understood in the context of the broader concept of "model of the health system". As the author of finding "patterns express different real concept of health policy while the term" pattern "prejudge the specific choice of health policy, which is a reference to the health care system". The World Health Organisation defines the term "health system" as a system covering all organizations, investment and institutions whose concept is to create actions on improved health. According to the Organization's objectives is the basis for the operation of the system of health protection, which targets focus on: constant improving population health, meeting the demand needs of health services, where the recipient is you as a consumer. On the other hand, the term "health care" defined by the WHO as a program of benefits in accordance with medical knowledge necessary to promote and maintain health by sharing individuals and entire populations. C. Wlodarczyk stresses that for the proper definition of the concept of health system you must extract the three spheres of the impact of health policy: health, administrative institutions and finance health and traditional public health activities. Many definitions that appear in the literature points to the narrower scope of the definition of the concept of "health care system" than those WHO suggested.. Author B. McPake and colleagues present the thesis that the health system consists of payers, healthcare providers and regulatory bodies together with relationships that occur between them. These relationships are presented for four health system functions: regulatory, financial, allocation of resources and the provision of services. C. Bailey and S. Poździoch describe that the health system is a whole, consisting of a variety of elements, the associated affinity, between which there are relationships. S. Poździoch is used for the definition of the system: "organized and coordinated team actions, whose aim is the realization of benefits and services and awareness campaign-therapeutic and rehabilitation aimed at protection and improvement of the health status of the individual and the collective ". The fact is the large role played by the State in the health system.

Health services are important lifelines of a community any time and this role is more pronounced during times of disasters. Evidence from various parts of the world presents examples of disaster-induced damage to hospitals and failure of health services at times of need. The impact of disasters-induced damage to health care is three-dimensional: health, social and economic. Damage to health care facilities apart from delaying and complicating relief measures also compromise the achievement of planned national and global health and related goals. The indirect and long-term costs of damage to health sector are greater than direct and immediate costs, compounding the disastrous consequences on the economy. The increasing invasion of nature spaces, climate change and urbanisation are bound to aggravate more natural hazards in future. So a resilient health care system is an immediate necessity for all global states. This paper discusses the international and national endeavours towards a resilient health-care system and analyses the strategies to promote safe hospitals in future.

Abstract Background Brazil have a huge free health care system. Inspired on UK National Health System (NHS), the Sistema Único de Saúde (SUS, that means Unified Health System) was consolidate as a right for all citizens after 1988's Brazilian Federal Constitution. Despite your spread, part of Brazilian citizens have an opposite opinions about SUS. So, the main question is: How to teach the relevance of a universal free health care system as a human right. Objectives The main aim of this work is teaching the recognize of SUS as a human right and ratified by Brazilian Federal Constitution in a High School class. In this sense, we used the cinema and debate for stimulate an effective comprehension of Universal Declaration of Human Rights (UDHR) and health promotion as a human right in a high school class. We used the movie Elysium (Neill Blomkamp, 2013) that show a dystopian world with two social class: citizen and non-citizen of Elysium (an artificial satellite of the Earth with high technology of cure for your citizens). Results The class has three Lessons: in first lesson we teaching about different notions about rights and the UDHR. In the second lesson we teaching about free health care systems in the world and a historical perspective about SUS. The third lesson we show the movie Elysium and proceed a debate. In debate we discuss about citizenship and rights, how the movie show two societies with different rights about work, transport and health, and how this aspects impact the quality of life of human being. Conclusions What options we have? The notion of free health care as a human right isn't an easy concept. So, efforts are needed for this comprehension and cinema can be used, followed by discussion, as a significant teaching tool for achieving humanistic educational objectives about free health care system as a human right in the high school curriculum. Key messages Health care is a human right and free health care system is a concretization of this right. Cinema can be used for stimulate a critical evaluation and effective understanding of rights in a high school classroom.

OBJECTIVESeveral similarities exist between the Massachusetts health care reform law of 2006 and the Affordable Care Act (ACA). The authors’ prior neurosurgical research showed a decrease in uninsured surgeries without a significant change in surgical volume after the Massachusetts reform. An analysis of the payer-mix status and the age of spine surgery patients, before and after the policy, should provide insight into the future impact of the ACA on spine surgery in the US.METHODSUsing the Massachusetts State Inpatient Database and spine ICD-9-CM procedure codes, the authors obtained demographic information on patients undergoing spine surgery between 2001 and 2012. Payer-mix status was assigned as Medicare, Medicaid, private insurance, uninsured, or other, which included government-funded programs and workers’ compensation. A comparison of the payer-mix status and patient age, both before and after the policy, was performed. The New York State data were used as a control.RESULTSThe authors analyzed 81,821 spine surgeries performed in Massachusetts and 248,757 in New York. After 2008, there was a decrease in uninsured and private insurance spine surgeries, with a subsequent increase in the Medicare and “other” categories for Massachusetts. Medicaid case numbers did not change. This correlated to an increase in surgeries performed in the age group of patients 65–84 years old, with a decrease in surgeries for those 18–44 years old. New York showed an increase in all insurance categories and all adult age groups.CONCLUSIONSAfter the Massachusetts reform, spine surgery decreased in private insurance and uninsured categories, with the majority of these surgeries transitioning to Medicare. Moreover, individuals who were younger than 65 years did not show an increase in spine surgeries, despite having greater access to health insurance. In a health care system that requires insurance, the decrease in private insurance is primarily due to an increasing elderly population. The Massachusetts model continues to show that this type of policy is not causing extreme shifts in the payer mix, and suggests that spine surgery will continue to thrive in the current US health care system.
