Abstract Social isolation and loneliness are pressing health concerns in older adults, likely exacerbated by social distancing guidelines enacted during COVID-19. Creating effective interventions to address health issues is challenging. Design is an alternative approach to create innovative interventions and to test their preliminary potential. In the present case study, we describe the processes and outcomes of a four-week project in a graduate design studio. Students were asked to develop a prototype for an intervention using digital technologies to increase social connectedness among older adults. This was an interdisciplinary process guided by faculty with expertise in design (Mejía), healthcare redesign (Doebbeling), and gerontology (Der Ananian). In the first week, the faculty helped the students understand the design goals, the implications of social isolation and loneliness, and technology use in older adults. In the second week, students conducted user interviews. In the third week, students set the problem by defining a specific potential audience and context. They also prototyped two preliminary concepts using storyboards and received feedback from the faculty. In the last week, students presented refined prototypes with storyboards, user flows, and interface mockups. Student design ideas included an audio story-sharing app that facilitates conversations and new friendships, an assistance digital service for immigrant older adults that need support with language or cultural challenges, and an art and crafts subscription service with a sharing platform to connect older adults with similar interests. The students' design projects provided innovative technological approaches for improving social connections and could be used in future R&D.

Objectives: The effectiveness of Intergenerational Mentor-Up (IMU), an innovative intervention that engages college students in tutoring older adults, was examined with regard to eHealthy literacy and social isolation. Method: A total of 55 older adults (mean age = 73.82) participated in the six-session IMU tutorials. In this mixed methods study, quantitative and qualitative data were collected in a parallel fashion. Participants were interviewed after participating in the IMU. Results: Older adults presented significant improvement between pre- and postsurveys in various outcomes such as eHealth literacy, technophobia, self-efficacy, and interest in technology. Intergenerational interaction brought about by IMU helped to decrease social isolation among older adults. Qualitative data revealed that individualized training, modifications, adaptations, and intergenerational interactions can decrease their anxiety and boost their confidence. Discussion: Study findings enable the identification of the training needs to help close the digital divide and allow these older adults to reap the benefits of technology use.

BACKGROUND Technology has become the most critical approach to maintain social connectedness during the COVID-19 pandemic. Older adults (over age 65) are perceived as most physiologically vulnerable to COVID-19 and at risk of secondary mental health challenges related to social isolation imposed by virus containment strategies. To mitigate concerns regarding sampling bias we used a random sampling of older adults to understand uptake and acceptance of technologies to support socialization during the pandemic. OBJECTIVE To conduct a random population-based assessment of the barriers and facilitators to engaging in technology use for virtual socialization amongst older adults in the Canadian province of British Columbia during the COVID-19 pandemic. METHODS We conducted a cross-sectional population-based survey using random-digit dialing to participants over age 65 living in British Columbia. Data were analyzed using SPSS, with open-text responses analyzed using thematic analysis. RESULTS Respondents included 400 older adults with an average age of 72 years old and 63.7% female. Most respondents (89.5%) were aware of how to use technology to connect with others and slightly more than half (56%) reported using technology differently to connect with others during the pandemic. 55.9% of respondents reported adopting new technology since the beginning of the pandemic. Older adults reported key barriers to using technology including: (1) lack of access (including finance, knowledge, and age); (2) lack of interest (including a preference for telephone, and a general lack of interest in computers); and (3) physical barriers (resultant of cognitive impairments, stroke, and arthritis). Older adults reported numerous facilitators, including: (1) knowledge of technologies (whether self-taught or via external courses); (2) reliance on others (family, friends, and general internet searching); (3) technology accessibility (including environments, user-friendly technology, and receiving clear instructions); and (4) social motivation (because everyone else is doing it). CONCLUSIONS Much data on older adults use of technology is limited by sampling biases, but the current study that used random sampling demonstrated that older adults used technology to mitigate social isolation during the pandemic. Virtual socialization is most promising to mitigate potential mental health effects related to virus containment strategies. Addressing barriers by mobilizing telephone training and task lists, and mobilizing facilitators described by participants such as facilitated socialization activities are important strategies that can be implemented within and beyond the pandemic to bolster the mental health needs of older adults.

This research investigated the effects of social isolation on frailty and health outcomes and tested whether these associations varied across different levels of frailty. We performed a multivariate analysis of data from the first wave of the FRéLE study among 1,643 Canadian older adults aged 65 years and over. We assessed social isolation using social participation, social networks, and support from various social ties, namely, friends, children, extended family, and partner. Frailty predicted disability, comorbidity, depression, and cognitive decline. Less social participation was associated with IADLs, depression, and cognitive decline. The absence of friends was associated with depression and cognitive impairment. Less social support from children and partner was related to comorbidity, depression, and cognitive decline. Overall, social isolation is linked to mental health, rather than physical health. The associations of having no siblings, receiving less support from friends, and participating less in social activities with ADL limitations, depression, and cognitive decline were higher among frail than pre-frail and robust older adults. This study corroborates the pivotal role of social connectedness, particularly the quality of relationships, on the mental health of older adults. Public health policies on social relationships are paramount to ameliorate the health status of frail older adults.

Abstract This symposium brings together five studies that examined the relationship between social isolation and well-being. Two used pre-COVID data from the Health and Retirement Study (HRS). One aimed to identify patterns of social isolation trajectory in a 9-year period, where social isolation was conceptualized as a multidimensional construct. It identified four distinct patterns, and the pattern had a gradient relationship with health outcomes. Another examined the association between self-perceptions of aging (SPA) and social well-being among older adults. It found that positive SPA predicted increased social connectedness and reduced loneliness in four years. Two other studies were based on a longitudinal survey (COVID-19 Coping Survey) that began in April 2020. One reports that adults 55+ with comorbidity at pandemic onset had persistently elevated depressive symptoms in a 6-month period, regardless of their social isolation level. Another paper suggests that physical isolation at pandemic onset was associated with elevated symptoms of depression, anxiety, and loneliness throughout the following six months. The fifth paper was based on two-wave data—2019 survey and 2020 COVID supplement—from the National Aging and Health Trend Study (NAHTS). It found that older adults who were very socially isolated and completely homebound before the pandemic experienced less psychological distress during the outbreak than those who were very socially integrated and not homebound. The five studies highlight the multiple dimensions of social isolation, their antecedents and development over time, and their role in shaping mental health in a pandemic context.

Abstract Because of the COVID-19 pandemic, older adults have been advised to stay-at-home to reduce the risk of infection. Social distancing and quarantine measures increase their vulnerability to adverse health outcomes like depression and cardiovascular disease. Technology is an effective tool to promote social connectedness among older adults affected by the pandemic; however, its role in reducing loneliness and health inequities is not well understood. The goal of this project was to construct a model for how technologies may be deployed to mitigate the impact of a pandemic on social isolation, loneliness, and health inequities for older adults. PubMed, SCOPUS, and PsychINFO were searched for the following keywords: “social isolation,” “loneliness,” “social support,” “resilience,” “technology,” “pandemic,” and “health inequities.” Articles selected for full analysis attempted to understand or observe how technology alleviates social isolation and/or loneliness among older adults. Research evidence indicates that using technology reduces loneliness directly and indirectly (by reducing social isolation) and can strengthen social support, which in turn promotes resilience among older adults. Video-based technologies encourage care-seeking behaviors in this population. There is insufficient evidence to determine technology’s relationship to health inequities experienced by older adults. The model we have proposed should help advance research on the relationship between technology and health inequities among older adults that may be aggravated by pandemic-like situations. We hypothesize that technology interventions for social support and functional competence should be sequenced in order to have the best effects on reducing health disparities.

Background: Choose to Move is one of few scaled-up health-promoting interventions for older adults. The authors evaluated whether Choose to Move participants maintained their intervention-related gains in physical activity (PA), mobility, and social connectedness 12 months after the intervention ended. Methods: The authors assessed PA, mobility, loneliness, social isolation, and muscle strength via questionnaire and objective measures in 235 older adults at 0 months (baseline), 6 months (end of intervention), and 18 months (12-months postintervention). The authors fitted linear mixed models to examine the change in each outcome from 6 to 18 months (primary objective) and 0 to 18 months (secondary objective) and reported by age group (60–74 and ≥75 y). Results: In younger participants, PA decreased between 6 and 18 months, but remained significantly higher than at baseline. Intervention-related benefits in loneliness, social isolation, mobility, and muscle strength were maintained between 6 and 18 months in the younger participants. Older participants maintained their intervention benefits in loneliness, mobility, and muscle strength. When compared with baseline values, PA levels in older participants were unchanged, whereas social isolation increased. Conclusions: Older adults maintained some, but not all, health benefits of Choose to Move 12 months after the intervention ended. Long-term commitments are needed to deliver effective health-promoting interventions for older adults if benefits are to be maintained.

Abstract The US population over 65 is projected to increase to 21% by 2050. Given mobility issues arising from health concerns, economic status changes, loss of friends and partners, older people are at a higher risk for social isolation and loneliness. Since the declaration of national emergency for COVID-19 on March 13, 2020, many older adults have not been able to connect with others in traditional ways. Instead, activities and contacts have been facilitated virtually via videoconferencing or phone calls to maintain physical and social distance. Amidst COVID-19, the transition to using technologies to connect socially and reduce loneliness has been a critical factor in preventing social isolation and loneliness. Identifying effective strategies involving the use of technology, designing new ways to deliver services virtually, and developing educational programs to promote technology is vitally necessary. This systematic review explored the relationship between technology use and social isolation or loneliness, and examined interventions that reduced social isolation in older adults. A unified strategy was used to systematically search seven databases (MEDLINE, PubMed, Embase, CINAHL, Web of Science, PsycInfo, and AgeLine) to examine qualitative and quantitative studies published in English between 2010 and 2020. Preliminary results indicate that technology can alleviate social isolation and loneliness in older adults despite some mixed results. The findings of this study will provide a foundation for policymakers and practitioners to shape policies and design programs that help older adults to alleviate social isolation and loneliness, particularly amidst the COVID-19 pandemic.

Abstract With enhanced challenges to maintain social connections especially during times of social distancing due to the COVID-19 pandemic, the need for technology solutions grow. Technologies have become interwoven into the daily lives for many older adults. The Technology Use in Later Life (TILL) study investigated how the perceptions and use of technology both can foster new and leverage existing intergenerational relationships. Through a mixed methods study engaging older adults aged 70 years of age and greater across rural and urban sites in Canada and the UK (N=37), participants described how the interconnection between technology and intergenerational relationships was an integral component to social connectedness with others. Through a qualitative descriptive approach, it was noted that older adults leveraged intergenerational relationships with family and friends to adjust to new technologies and to remain connected to adult children and grandchildren especially when there is high geographic separation between them. Especially during times of COVID-19, younger family members can play an important role to introduce and teach older adults how to use, technologies such as digital devices, computers, and social networking sites. Participants emphasized the benefits of intergenerational connections to adopt and use technology in later life noting flexibility and willingness to overcome barriers to technology adoption and remain connected across the generations. The adoption and uptake of technologies may continue as viable options during times of social distancing to support older persons to remain independent, age in place, in both age-friendly cities and across rural geographies during and post COVID-19.
