BACKGROUND Rasburicase (recombinant urate oxidase) has been proven to be an effective therapy for prevention of tumour lysis syndrome (TLS). The recommended daily dosing regimen of rasburicase is 0.2 mg/kg/day for 5 days which is expensive and unaffordable to many patients in the developing countries. The purpose of the present study was to evaluate the effect of single 1.5 mg dose rasburicase in the management of tumour lysis syndrome. METHODS This is a follow-up study done at our institute. Fifty (50) patients with tumour lysis syndrome who received rasburicase from August 2015 to January 2020 were enrolled in this study RESULTS Single dose of rasburicase is effective in decreasing serum uric acid level in significant number (N = 41) of patients. Percentage of patients having uric acid less than 7 mg after single dose of rasburicase in 48 hours - 82.9 % (N = 34) while 17 % (N = 7) were found to have uric acid levels of more than 7 mg/dl. The percentage of patients with uric acid levels more than 7 mg/dl reduced from 36.5 % after 24 hours to 17 % after 48 hours. This indicates that the uric acid levels show a declining trend even after 24 hours without giving an additional dose of rasburicase. There was no relationship between uric acid levels at 24 hours and percentage change in creatinine level from baseline to 24 hours (correlation coefficient (r) = -0.047, P = 0.770. Patients who required additional dose (N = 9) had high base line value of uric acid and their high value was maintained over the follow up period of three days. Patients with pre exiting kidney disease and high level of baseline uric acid also needed dialysis (N = 3). CONCLUSIONS In majority of patients, a single 1.5 mg dose of rasburicase is an effective way to reduce raised uric acid in appropriate circumstances. This article aims to examine the issue of infrastructure development in Papua Province, Indonesia. This article specifically discusses the problems confronted during the process of infrastructure development in Jayapura and the challenges in expanding them. The method used in this study is the qualitative method with a case study approach, wherein the infrastructure development case selected is the Papua Bangkit Stadium and Holtekamp Bridge located in Jayapura. Both infrastructure development cases serve as sample cases that show the extent of infrastructure development impact in Papua, and the issues that were confronted, starting from the development process up to the expansion. The study results indicate that although previous studies have shown positive correlation between development and welfare, in reality, there are several problems occurring in Papua’s infrastructure development. Preparedness of human resources, cultural issues, and weak coordination between every level of regional government institution, these are the main problems in Papua’s infrastructure development. This article is expected to assist in resolving the issues found in Papua’s infrastructure development, and to provide recommendations for resolving those issues.

Background: A growing population of young adults is presenting to cardiologists with late manifestations of Kawasaki disease (KD) that include cardiomyopathy, ischemia, and infarction. The management of these conditions differs in important ways from atherosclerotic heart disease, and yet there is little awareness in the adult cardiology community regarding the special challenges posed by the cardiovascular sequelae of KD. Methods: Observations were made on a population of 140 adult KD patients enrolled in the San Diego Adult KD Collaborative Study. Results: Coronary artery aneurysms resulting from KD in childhood are associated with a high risk of thrombosis and stenosis at the inlet or outlet of the aneurysm. These aneurysms are often highly calcified and may contain a large thrombus burden that may obscure the true size of the aneurysm. Pitfalls in the management of these patients stem largely from failure to recognize the nature of the lesions, which leads to attempts to dilate highly calcified stenotic segments and undersizing of stents. Intravascular ultrasound is helpful in appreciating the true dimensions of the aneurysm, which may be filled with thrombus. Thrombolysis and use of anti-platelet agents followed by systemic anti-coagulation are appropriate management strategies for patients presenting with acute infarction. Bypass grafting with the internal thoracic arteries can be a successful strategy, but care must be taken to avoid competitive flow through the native vessel leading to graft failure. In contrast to the individuals who developed coronary artery aneurysms, young adults who had documented normal echocardiograms associated with their acute KD in childhood and who have no evidence of calcium deposition in the arterial wall as assessed by computed tomography (CT) calcium score appear to have no increased cardiovascular risk in the medium term. Long-term outcomes for adults post-KD in childhood are still being defined. Conclusions: KD poses special management challenges for the adult cardiologist who must recognize the unique features of the cardiovascular lesions in this growing population of patients.

Abstract A quantitative and a qualitative method for the determination of acetone peroxides were subjected to collaborative study. Quantitative analyses are based upon liberation and titration of hydrogen peroxide from acyclic peroxides and hydroperoxides by dilute sulfuric acid and standardized potassium permanganate, respectively. Single determination of 6 samples (varying in per cent levels of peroxide equivalent) each of baking premixes and milling premixes produced very good collaborative results. Qualitative analyses, achieved by comparing infrared spectra of acetone-extracted organic peroxides with acetone-extracted organic peroxides from a reference premix, gave peaks characteristic of the premixes. No interferences were seen from various starch blanks. Both the quantitative and the qualitative methods are recommended for adoption as official, first action.

The fishing community is a social element that is very important in the structure of coastal communities which they had colored cultural characteristics and socio-cultural behavior of coastal communities in particular that of Bagan Deli, Belawan, Medan. This study views their work ethic differences between the shipmen/crew, skipper/captain and ship owner that caused the differences of their social stratification. Using a qualitative method and non-parametric statistical analysis of kolmogorov smirnov, this study results that the work ethic among the crew and skipper there has no differences, whereas between the crew and the skipper with the owners there are differences in higher ethic.

Abstract An international collaborative study involving 23 collaborators was conducted to test methods, improved over previous methods with respect to speed and solvent use, for the rapid determination and thin layer chromatographic (TLC) confirmation of aflatoxin M1 identity in dairy products. For the quantitative method, collaborators assayed samples of Couda and cheddar cheeses, powdered milk, and butter containing levels of M1 near the anticipated limit of determination. Statistical analysis of the study results indicated that the lower limit of determination and precision of this method were comparable to these parameters of methods previously approved for analysis for aflatoxin M1. A few collaborators found that M1 eluted early from cleanup columns with cheese and butter samples and that emulsions formed during powdered milk sample extraction. The reasons for these problems have been determined and remedies are provided. For the TLC confirmation of identity method, collaborators prepared trifluoroacetic acid derivatives of M1 on the plates after 2-dimensional development. Concentrations as low as 0.3 ng/g cheese and 1.0 ng/g powdered milk were determined in this study. As a result of this study, both methods have been adopted as official first action methods by the AOAC and as reference methods by IUPAC.

Abstract Eleven laboratories collaboratively studied a liquid chromatographic (LC) method for determination of D-malic acid in apple juice. The mobile phase consisted of 16 mM L-valine and 8 mM copper acetate adjusted to pH 5.5 with NaOH. The UV detector was set at 330 nm, and a single reversed-phase LC column was used. Seven paired samples containing various amounts of D-malic acid ranging from 0 to 188 mg/100 mL of 12 Brix pasteurized apple juice were tested by each collaborator. Repeatability and reproducibility coefficients of variation ranged from 1.0 to 3.5% and 7.7 to 11.7%, respectively, within the range of 26 to 188 mg D-malic acid/100 mL of 12 Brix apple juice. The collabora tive study results demonstrated that the method could quantitate the economic adulteration of ap ple juice with DL-malic acid at lower levels than those reported with previous methods. The LC method for determination of D-malic acid in apple juice has been adopted first action by AOAC INTERNATIONAL.
