Abstract For patients recovering from surgery, a video consultation is a suitable alternative to conventional consultations. Video consultations have been found to be beneficial, but little is known about their organization, compared to face-to-face consultations. In this article, we explore potential extra interactional work conducted by participants in video-mediated consultations. We focus on the beginning of the consultation. Our data consist of 39 recorded, postoperative, oncological consultations, both face-to-face and through video-mediated communication (VMC), which we analyzed using conversation analysis. Although surgeons commonly launched the beginning with an announcement of the pathology report as the reason for the consultation, we found that in VMC, “how are you?” was regularly inserted after the testing of the technology. The question is a suitable strategy, as it displays overt other-attentiveness, while also being medically relevant. However, subsequently, surgeons may unilaterally close the elicited self-report to then address the pathology report. Thus, the “extra interactional work” of other-attentiveness is again attenuated.

Abstract Introduction During the current COVID-19 pandemic video consultations are increasingly common in order to minimize the risk of infection for staff and patients. The aim of this study was to evaluate the feasibility of a spine examination via video. Methods A total of 43 patients were recruited. Each participant underwent a video-based (VB) and a conventional face-to-face (FTF) spine examination. Pain intensity, active range of motion, inspection, a neurophysiologic basic exam and provocations tests were evaluated using video-based and face-to-face methods. Results The intra-rater reliability (IRR) was measured between both examinations. Good to very good IRR values were obtained in inspection (Kappa between 0,752 und 0,944), active range of motion and basic neurophysiological examination (Kappa between 0,659 und 0,969). Only moderate matches were found in specific provocation tests (Kappa between 0,407 und 0,938). A video-based spine examination is a reliable tool for measuring pain intensity, active range of motion and a basic neurophysiologic exam. Conclusion A basic spine examination during a video consultation is possible. A good agreement of the test results between video-based and face-to-face examination could be found.

Conversation analysis is a method for the systematic study of interaction in terms of a sequential turn-taking system. Research in conversation analysis has traditionally focused on speakers of English, and it is still unclear to what extent the system observed in that research applies to conversation more generally around the world. However, as this method is now being applied to conversation in a broader range of languages, it is increasingly possible to address questions about the nature of interactional diversity across different speech communities. The approach of pragmatic typology first applies sequential analysis to conversation from different speech communities and then compares interactional patterns in ways analogous to how traditional linguistic typology compares morphosyntax. This article discusses contemporary literature in pragmatic typology, including single-language studies and multilanguage comparisons reflecting both qualitative and quantitative methods. This research finds that microanalysis of face-to-face interaction can identify both universal trends and culture-specific interactional tendencies. BACKGROUND Video consultation is increasingly used in different healthcare settings in order to reach patients. However, little is known about telehealth in psychological treatment for patients with somatic and chronic conditions such as rheumatoid arthritis and diabetes. OBJECTIVE This study aims to provide new insight into the development of an app to support the psychological treatment and video based intervention. METHODS This study was inspired by participatory design and a qualitative approach. The content of the intervention was developed through user involvement and evaluated by individual interviews with patients and psychologists as well as questionnaires. RESULTS We co-created an app that targeted patients with rheumatic diseases and diabetes in relation to the psychological challenges of living with chronic diseases. Video based interventions was described under the four themes “The good relation despite physical distance”, “The comfort of being at home”, “The pros of saving time on transport and energy” and “A therapeutic alliance at a distance”. CONCLUSIONS Psychological treatment in relation to somatic care can be provided by video and supported by tailored information and tools delivered in an app without losing quality of care. To ensure a good alliance between the patient and the psychologist, a first face-to-face meeting is important. Being in their own home provided the patients with a safe environment, increased accessibility, and reduced travel time to the hospital.

Summary In order to understand how mutual understanding was achieved in discursive interactions between the welfare service users and service practitioners, conversation analysis was conducted in four discussion panels set up for building consensus on the appropriate structure for user participation in service management. Conversations in eight panel discussion meetings were audio-taped for analysing the talks-in-interaction therein. Drawing on the conversation analysis, the article uncovers the dynamics of consensus building among participants from different epistemic communities. Findings The study identifies the extent of divergence in views among stakeholders, which could have been obscured by the pressure to acquiesce in platform of face-to-face coordination. In the contest for truth between the welfare service users and service practitioners, personal experience has not been accepted as legitimate resource for supporting truth claims. Having limited argument resources on issues of service management, the welfare service users perceived argumentation in panel discussion a threatening venture that they chose to avoid. Avoidance was also a strategy that panel participants employed to maintain mundane interactions in the face of looming dissents. The article argues that the Habermasian communicative ethics are not panacea to the problem of coordination between the welfare service users and service practitioners. An agonistic model of democracy is called for to shift the objective of communication from gauging consensus to encouraging articulation of disagreements in the intricate user participation project. Application The article provides a new direction for developing the user participation imperative to address necessary pluralities among stakeholders of welfare services.

Abstract The rate of social participation of senior citizens in a senior club’s activities is not equal to the rate of desire for the said participation. Earlier studies mainly examined personal and social factors which influence the participation rate, overlooking the practical methods by which senior citizens can overcome barriers to participating in club activities. Our study aims to clarify the features of a club activity as a resource by analyzing the activity’s interactions. Our study is based on data extracted from videotaped recordings of a senior calligraphy club in Kanto, Japan. In September 2018, one lecturer and 11 participants were videotaped for 3 hours, and the video underwent conversation analysis, which elucidates how people organize activities under specific circumstances. We analyzed how a female newcomer to the activity initiated face-to-face interaction, which is considered the first step of social participation. She talked to other participants who were familiar with the exercise several times by inquiring how to read kanjis on teaching materials. These findings suggest that visualization of skill relative to the other creates an environment for initiating face-to-face interaction. In this case, the newcomer utilized the difference in skill denoted by teaching materials and was given the rational reason to talk to the others already engaging in the activity. Therefore, designing teaching materials that assign the learning level of each participant may be effective in promoting social participation in senior study clubs.

Background: The importance of caring for children with complex and serious conditions means that paediatric palliative care must continue during pandemics. The recent pandemic of Coronavirus Disease 2019 (COVID-19) provides a natural experiment to study health communication during pandemic times. However, it is unknown how communication within consultations might change during pandemics. Aim: This study, a sub-study of a larger project, aimed to examine real-world instances of communication in paediatric palliative care consultations prior to and during the COVID-19 pandemic to understand how clinicians and families talk about the pandemic. Design: Paediatric palliative care consultations prior to, during, and immediately following the initial peak of COVID-19 cases in Australia were video recorded and analysed using Conversation Analysis methods. Setting/participants: Twenty-five paediatric palliative care consultations (including face-to-face outpatient, telehealth outpatient and inpatient consultations) were video recorded within a public children’s hospital in Australia. Participants included 14 health professionals, 15 child patients, 23 adult family members and 5 child siblings. Results: There was a pervasive relevance of both serious and non-serious talk about COVID-19 within the consultations recorded during the pandemic. Topics typical of a standard paediatric palliative care consultation often led to discussion of the pandemic. Clinicians (55%) and parents (45%) initiated talk about the pandemic. Conclusions: Clinicians should not be surprised by the pervasiveness of COVID-19 or other pandemic talk within standard paediatric palliative care consultations. This awareness will enable clinicians to flexibly address family needs and concerns about pandemic-related matters that may impact health and wellbeing.

Purpose The purpose of this paper is to explore what conversation analysis has to offer when analysing a series of life story interviews aiming to capture how reading and texts are used in a rural working-class man’s identity construction. Design/methodology/approach The conversation analysis methodology with its explicit focus on embodied social action, activity and conduct in interaction is integrated with a life story approach when analysing and describing the identity constructing processes that take place in life story interview settings. Findings Through a close and detailed analysis of the interaction between interviewer and interviewee, and by focusing and highlighting the phenomena and identities that are oriented to in the face-to-face interaction here and now (and in relation to there and then), descriptions of the complex and dynamic identity constructing processes that are set into play in the life story interview are possible. Research limitations/implications It is argued that the approach has a lot to offer when approaching life story data, and thus is a method that can increase the transparency in life story interview research. Originality/value The paper explores the intersection of what is often seen as diametrically opposed forms of analysis: conversation analysis and narrative inquiry.

Developmental research on first and second language acquisition is mainly concerned with cognitive, linguistic or pragmatic aspects of individual speech production treated separately and based on the tenets of separate disciplines or approaches (psycholinguistics, psychology of language, constructivism, conversation analysis). However, some studies try to integrate questions of language acquisition into the much broader context of social interaction in general. This paper argues in favour of such integration, taking a conversationalist perspective on speech and discourse production in social — face-to-face — interaction. In particular, it argues for the systematic integration of all kinds of body movements (traditionally called gestures) and prosody into the analysis of empirical data as a fundamental basis for the development of an interactional grammar and its study in an acquisitional research framework.

Abstract This study explores fuzzy boundaries, or weak cesuras, in the particle combination OH + OKAY as used in informing sequences in ordinary English talk-in-interaction. The focus is on the third position in such sequences, where OH + OKAY responds to information that has been solicited by the speaker in a prior turn. Based on a collection of approximately 45 instances of OH + OKAY in recent American English telephone and face-to-face interactions, this study adopts the methodology of Conversation Analysis and Interactional Linguistics to examine the prosody and phonetics of OH + OKAY as a feature of turn design. It asks: (i) when does the speaker use a combination of OH and OKAY in the third position rather than a simple OH or OKAY? and (ii) to what extent does the prosodic–phonetic delivery of OH + OKAY contribute to an interpretation of what the turn is doing? The study finds that (i) OH + OKAY is used when responding to a solicited informing that not only supplies information but also accomplishes another action with consequences for the recipient and (ii) if OH + OKAY is delivered with a strong cesura between its parts, its actions are distributed in separate turn-constructional units, creating a multi-unit turn that proposes sequence closure. If the cesura in OH + OKAY is weak, the component parts are fused into a compound particle that can preface more talk by the same speaker. Weak boundaries between OH and OKAY can be exploited by participants for the purposes of turn construction and epistemic positioning.
