Mindful Attunement

Mindful attunement is the integration of mindful awareness and healthy, sustainable connection with both the internal and external experiences of self that drives intentional thinking and behaviors that support well-being. This chapter details how physiology, emotions, and cognitions influence a sense of internal integration and attunement. Mindful self-care is introduced as a method for cultivating mindful attunement among the internal aspects of self, within the context of external (e.g., social) challenges using the attuned representation model of self (ARMS). The connections between mindful attunement, mindful self-care, and positive embodiment are illustrated using case examples. The Mindful Self-Care Scale, a tool for the assessment of the aspects of self-care addressing each area of the ARMS through actionable practices, is reviewed. This chapter ends by detailing the mindful self-care process, which involves mindful awareness of self-care as essential to well-being, assessment of self-care domains, assessment-driven self-care goal setting, and engagement in self-care.

This chapter explains the ARC principle of being results oriented versus process oriented. The results-oriented principle requires that human service organizations evaluate performance based on how much the well-being of clients improves. The principle addresses deficits in service caused by the conflicting priority of evaluating performance with process criteria such as the number of clients served, billable service hours, or the extent to which bureaucratic procedures such as the completion of paperwork are followed. Results-oriented organizations are described in detail, including case examples from decades of organizational change efforts by the authors in human service organizations. The chapter documents the importance of results-oriented approaches and underlying implicit beliefs to help the reader understand how mindsets and mental models shared among organizational members influence results-oriented approaches and effectiveness in practice. Supporting research, including feedback and goal-setting research are highlighted.

Self-care is a vital component of nursing practice and is essential for the well-being of human beings, that is, both patients and nurses. However, nurses and nursing students tend to focus more on self-care of their patients and often pay little or no attention to personal self-care. This literature review explored self-care behaviors and practices of nursing students in general as well as across academic years of the study. In total, 11 studies were reviewed and critically appraised to identify current trends regarding nursing students’ self-care practices and behaviors in nursing literature. Nursing students realized the importance of maintaining their diet, sleep, and activity level to improve physical and physiological health. However, they tend to neglect self-care practices which could improve their emotional and psychological health because of academic stress, workload, and inadequate knowledge about the strategies to improve self-care in this domain. There is limited evidence for drawing any conclusions regarding students’ use of complementary therapies for self-care, the difference between self-care practices of nursing and non-nursing students, the usefulness of different interventions for improving students’ self-care practices, and the differences between self-care practices and behaviors of students across academic years.

This chapter explains that members of improvement-directed organizations are never satisfied with the status quo and never stop looking for more effective ways to serve their clients. The principle addresses the conflicting priority represented by individuals in formal organizations resisting change and clinging to established protocols, regardless of whether the existing protocols promote improvements in the well-being of clients. The chapter describes improvement-directed organizations, including their application of continuous improvement processes, norms that support ongoing improvement, and behaviors that drive innovation and ongoing growth and development. The chapter presents research evidence and case studies to illustrate how systems and processes, decisions, actions, and behaviors, as well as assumptions and beliefs, need to be addressed to create improvement-directed organizations. Specific case examples illustrate ARC’s application to build improvement-directed organizations.

Objectives: Goal setting and motivational interviewing (MI) may increase well-being by promoting healthy behavior. Since we failed to show improved well-being in a proactive assessment service for community-dwelling older adults applying these techniques, we studied whether implementation processes could explain this. Methods: Goals set during the comprehensive geriatric assessment were evaluated on their potential for behavior change. MI and goal setting adherence wasassessed by reviewing audiotaped interactions and interviewing care professionals. Results: Among the 280 goals set with 230 frail older adults (mean age 77 ± 6.9 years, 59% women), more than 90% had a low potential for behavior change. Quality thresholds for MI were reached in only one of the 11 interactions. Application was hindered by the context and the limited proficiency of care professionals. Discussion: Implementation was suboptimal for goal setting and MI. This decreased the potential for improved well-being in the participating older adults.

Abstract Background In a context where there is no treatment for the current COVID-19 virus, the combination of self-care behaviours together with confinement, are strategies to decrease the risk of contagion and remain healthy. However, there are no self-care measures to screen self-care activities in general population and which, could be briefly in a lockdown situation. This research aims to build and validate a psychometric tool to screen self-care activities in general population. Methods Firstly, an exploratory factor analysis was performed in a sample of 226 participants to discover the underlying factorial structure and to reduce the number of items in the original tool into a significant pool of items related to self-care. Later a confirmatory factor analyses were performed in a new sample of 261 participants to test for the fit and goodness of factor solutions. Internal validity, reliability, and convergent validity between its score with perceived stress and psychological well-being measures were examined on this sample. Results The exploratory analyses suggested a four-factor solution, corresponding to health consciousness, nutrition and physical activity, sleep, and intra-personal and inter-personal coping skills (14 items). Then, the four-factor structure was confirmed as the best model fit for self-care activities. The tool demonstrated good reliability, predictive validity of individuals’ perception of coping with COVID-19 lockdown, and convergent validity with well-being and perceived stress. Conclusions This screening tool could be helpful to address future evaluations and interventions to promote healthy behaviours. Likewise, this tool can be targeted to specific population self-care’s needs during a scalable situation.

Objective: To provide descriptive information about a short-term educational programme for rural carers of people with a mental illness, living in the Loddon Campaspe Southern Mallee region. Method: The Carers Education Exchange Programme is a flexible, needsbased model that can be modified to cater for individual groups. It consists of a number of sessions on topics relevant to caring for someone with a mental illness, held over a period of several weeks. The programme is offered at locations throughout the region, making it accessible to carers in isolated, rural areas. Results: Feedback indicates that the benefits of participating in the programme include the reduction of isolation and stigma, increased understanding of mental illness, development of skills relevant to the caring role and the formation of supportive networks, both professional and personal. Conclusions: Educational group programmes for carers are an effective way of providing both education and support. This programme can assist in reducing some of the distress and difficulties inherent in caring for someone with a mental illness. Carer well-being is enhanced by the promotion of self-care and a positive outlook.

Shifting definitions of health and well-being, prompted by the World Health Organization's International Classification of Functioning (2001), have stimulated changes in traditional clinician-client relationships in rehabilitation. Among these changes, in keeping with the concept of client-centered care, is a trend toward more collaborative goal-setting and joint determination of intervention plans. Evidence suggests that supporting clients' autonomy in prioritizing personally meaningful goals leads to increased engagement in intervention, less emotional anxiety about the rehabilitation process, and improved treatment outcomes. Supporting people with aphasia in a process of collaborative goal formulation may also serve to alter treatment priorities so that they address more relevant communication challenges embedded in post-rehabilitation life.
