In this session, we will take time to review your experiences of “falling down the hole”—having a sustained emotional reaction to something like a cognitive slip-up. This is connected to further cultivation of awareness as a way to track our experience without reacting to it. The discussion of reacting versus responding is broadened to incorporate our experiences with others, and how we communicate with others. We will then discuss different communication styles and for homework, you will be encouraged to track your use of these communication styles, as well as the tendency to respond versus react. Finally, you will continue with Open Monitoring practice and you are invited to track your mindfulness in daily life and to find ways of adding to your mental bank account.

Background. Life events (LE) have consistently been found to influence course and outcome of psychotic disorders. However, the mechanism through which they operate is not known. The present study investigated whether LE have their effect by impacting on the emotional sensitivity for daily hassles.Method. Patients with clinically remitted psychotic illness (N=42) were studied with the Experience Sampling Method (a structured diary technique assessing current context and mood in daily life) to assess: (1) appraised subjective stress related to daily events and activities; and (2) emotional reactivity conceptualized as changes in both negative affect (NA) and positive affect (PA). LE were assessed with the Brown & Harris Life Event and Difficulties Schedule.Results. Multilevel regression analyses showed that previous exposure to LE influenced the appraised stressfulness of neither daily events nor the activities in which the subjects were involved. However, a history of LE did modify the emotional reaction to daily life stress, both in models predicting NA and in models predicting PA.Conclusions. By their cumulative effect on emotional reactivity to daily activities and events, LE may render individuals more vulnerable to the onset or persistence of psychotic experiences.

Medical treatment for acromegaly commonly involves receiving intramuscular or deep subcutaneous injections of somatostatin receptor ligands (SRLs) in most patients. In addition to side effects of treatment, acromegaly patients often still experience disease symptoms even when therapy is successful in controlling GH and IGF-1 levels. Symptoms and side effects can negatively impact patients’ health-related quality of life. In this study, we examine the disease- and treatment-related burden associated with SRL injections as reported through the use of the Acromegaly Treatment Satisfaction Questionnaire (Acro-TSQ ©) and clinician-reported symptom severity through the Acromegaly Index of Severity (AIS). Patients included in this analysis were enrolled in a randomized phase 3 study, were biochemically-controlled (an IGF-1 < 1.3 × the upper limit of normal [ULN] and average GH < 2.5 ng/ml) and receiving SRL injections for ≥6 months with a stable dose of either long-acting octreotide or lanreotide monotherapy for ≥4 months. The sample (N = 91) was 65% female, 91% Caucasian, with a mean [standard deviation (SD)] age of 53 (1) years. Two-thirds of patients reported that they still experience acromegaly symptoms; 82% of these said they experience symptoms all of the time. Three-fourths experienced gastrointestinal (GI) side effects after injections, and 77% experienced treatment-related injection site reactions (ISRs). Patients commonly reported that these interfered with their daily life, leisure, and work activities. Those with higher symptom severity, as measured by the AIS, scored significantly worse on several Acro-TSQ domains: Symptom Interference, GI Interference, Treatment Satisfaction, and Emotional Reaction. Despite being biochemically controlled with injectable SRLs, most patients reported experiencing acromegaly symptoms that interfere with daily life, leisure, and work. GI side effects and ISRs were also common. This study highlights the significant disease burden that still persists for patients with acromegaly that have achieved biochemical control with the use of injectable SRLs.

After a discussion of the prior week’s practices, participants are then introduced to Open Monitoring as a way to cultivate awareness and make space for our emotional reactivity. In this context, a discussion is raised about “falling down a hole”—when we have a sustained negative emotional reaction, for example, in response to a cognitive slip-up. Participants are assigned the practice “Emotional Weather” as a way to actively work with emotions. Walking meditation (“Aimless Wandering”) is also provided for home practice.

This thesis investigates the interaction between the obligation to contract and the limitations to the free bargaining of the conditions of these contracts, in theory as well as in practice, focusing on the new right of consumers to open a basic bank account. It is the aim of the EU Directive and the German “Zahlungskontengesetz (ZKG)” to guarantee consumers’ access to a basic bank account in order to ensure that consumers may participate in non-cash payments. Yet, the effectivity of the ZKG’s obligation to contract is significantly impaired at the moment, resulting from the fact that the banks may charge an “appropriate” account fee for the opening and the account processing of the basic bank accounts. Hence, the thesis focuses on the problems related to the account fees, especially on the question which account fees are actually “appropriate”. Furthermore, the thesis investigates the general significance of bank accounts in daily life, the legal and political discussion which ultimately led to the enactment of the EU Directive and the German “Zahlungskontengesetz (ZKG)” and the new restrictions for the banks in connection with the dismissal of basic bank accounts.

AbstractPrevious studies have shown that specific personality dimensions, -e.g., the Big Five-, consistently intercorrelate, such that they form a general factor of personality (GFP). It has been hypothesized that the GFP reflects social effectiveness. Similarly, in the clinical domain, overlap between various psychopathological symptoms has also been reported, leading to a general factor of Psychopathology, or p factor. The aim of this study was to test the overlap between the higher-order factors in personality and psychopathology, and how they relate to daily life functioning and communication style. We tested a sample of 165 outpatients of a psychological therapy institute, using a multi-source approach that included self-reports and other ratings. The outpatients’ self-reports of personality, general psychological problems, and interpersonal problems were available. Psychotherapists rated the outpatients’ functioning in daily life with the well-known Global Assessment of Functioning (GAF) scale. A spouse or friend also rated the impact of the patient’s communication/social behavior. Patients with lower GFP scores and higher scores on general psychopathology, displayed more distress and daily functioning deficits (i.e., lower GAF scores) and, in terms of communication styles, were also rated as being less dominant, less in control socially, and more submissive and aggressive. We proposed that part of the overlap between the general factors (GFP, psychopathology factors) may relate to a lower general life functioning and less social effectiveness.

Abstract Audiologists have a responsibility to counsel patients with auditory concerns on methods to manage the inherent challenges associated with hearing loss at every point in the process: evaluation, hearing aid fitting, and follow-up visits. Adolescents with hearing loss struggle with the typical developmental challenges along with communicative challenges that can erode one's self-esteem and self-worth. The feeling of “not being connected” to peers can result in feelings of isolation and depression. This article advocates the use of a Narrative Therapy approach to counseling adolescents with hearing loss. Adolescents with hearing loss often have problem-saturated narratives regarding various components of their daily life, friendships, amplification, academics, etc. Audiologists can work with adolescents with hearing loss to deconstruct the problem-saturated narratives and rebuild the narratives into a more empowering message. As the adolescent retells their positive narrative, they are likely to experience increased self-esteem and self-worth.

Abstract. Most research in health psychology is based on retrospective self reports, which are distorted by recall biases and have low ecological validity. To overcome such limitations we developed computer assisted diary approaches to assess health related behaviours in individuals’, couples’ and families’ daily life. The event- and time-sampling-based instruments serve to assess appraisals of the current situation, feelings of physical discomfort, current emotional states, conflict and emotion regulation in daily life. They have proved sufficient reliability and validity in the context of individual, couple and family research with respect to issues like emotion regulation and health. As examples: Regarding symptom reporting curvilinear pattern of frequencies over the day could be identified by parents and adolescents; or psychological well-being is associated with lower variability in basic affect dimensions. In addition, we report on preventive studies to improve parental skills and enhance their empathic competences towards their baby, and towards their partner.

Background: A substantial proportion of therapists experience the loss of a patient to suicide at some point during their professional life. Aims: To assess (1) the impact of a patient’s suicide on therapists distress and well-being over time, (2) which factors contribute to the reaction, and (3) which subgroup might need special interventions in the aftermath of suicide. Methods: A 63-item questionnaire was sent to all 185 Psychiatric Clinics at General Hospitals in Germany. The emotional reaction of therapists to patient’s suicide was measured immediately, after 2 weeks, and after 6 months. Results: Three out of ten therapists suffer from severe distress after a patients’ suicide. The item “overall distress” immediately after the suicide predicts emotional reactions and changes in behavior. The emotional responses immediately after the suicide explained 43.5% of the variance of total distress in a regression analysis. Limitations: The retrospective nature of the study is its primary limitation. Conclusions: Our data suggest that identifying the severely distressed subgroup could be done using a visual analog scale for overall distress. As a consequence, more specific and intensified help could be provided to these professionals.
