The majority of extradural spinal tumors are metastatic. Magnetic resonance imaging (MRI) and computed tomography (CT) imaging are both useful for evaluation of epidural cord compression and biomechanical instability. For the patient with an extradural thoracic tumor, goals of therapy may include recovery or preservation of neurologic status, local oncologic control, recovery or maintenance of mechanical stability, and improvement of quality of life. This chapter discusses the key therapeutic decision-making criteria and surgical considerations in the management of patients with epidural cord compression due to an extradural thoracic tumor.

Endometriosis is a gynecological pathology with chronic symptoms, which negatively affects the patient’s quality of life. The prevalence of endometriosis in asymptomatic women is between 2% and 50%, depending on the populations studied and the method of diagnosis. The severity of the symptoms as well as the probability of diagnosing endometriosis increases with age9. Because endometriosis is a gynecological condition with a nonspecific clinical picture, sometimes even asymptomatic, imaging technology can be considered the first line of diagnosis for this pathology. The main objective of this study is to evaluate the sensitivity and specificity of nuclear magnetic resonance imaging (MRI) used in the diagnosis of endometriotic lesions depending on their location, and compare the results obtained with the intraoperative appearance considered a reference standard in the diagnosis of endometriosis. Our study revealed the highest specificity for MRI in the case of endometriotic bladder invasion, respectively the highest sensitivity for endometriotic rectal nodules.

Abstract Immune thrombocytopenia (ITP) comprises a heterogeneous group of disorders characterized by autoimmune-mediated platelet destruction and impairment of thrombopoiesis. ITP may occur in the absence of an evident predisposing etiology (primary ITP) or secondary to a growing list of associated conditions (secondary ITP), and must be differentiated from other causes of thrombocytopenia. This review focuses on primary ITP in adults. The traditional goal of therapy in this population is to achieve a hemostatic platelet count of 30 × 109/L or above for most patients while minimizing treatment-related morbidity. This approach has been called into question by the recent advent of well-tolerated and effective agents for the management of ITP, including pulse-dose dexamethasone, rituximab, and the thrombopoietin receptor agonists. Recent studies suggest the potential for aggressive therapy at the time of diagnosis to alter the natural history of ITP and point to the importance of quality-of-life considerations in therapeutic decision making.

Pediatric spinal tumors are rare and account for 10% of all central nervous system tumors in children. Onset usually occurs with chronic nonspecific symptoms and may depend on the intra- or extradural neoplastic location. Meningiomas, schwannomas, and neurofibromas are the most common intradural-extramedullary lesions, while astrocytomas and ependymomas represent the majority of intramedullary tumors. The new molecular discoveries regarding pediatric spinal cancer currently contribute to the diagnostic and therapeutic processes. Moreover, some familial genetic syndromes can be associated with the development of spinal tumors. Currently, magnetic resonance imaging (MRI) is the standard reference for the evaluation of pediatric spinal tumors. Our aim in this review was to describe the imaging of the most frequent intradural intra/extramedullary pediatric spinal tumors and to investigate the latest molecular findings and genetic syndromes.

Lumbar Foraminal Stenosis (LFS) dapat secara signifikan mengurangi fungsi dan kualitas hidup pasien dan Magnetic Resonance Imaging (MRI) adalah alat pendukung yang umum digunakan untuk mengukur beratnya stenosis. Lee score umumnya digunakan untuk mengukur derajat LFS pada MRI sagital. Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) dan Oswestry Disability Index (ODI) digunakan untuk menilai disabilitas dan skor fungsional pada pasien LFS. Penelitian ini bertujuan untuk mengetahui korelasi antara derajat LFS pada MRI sagital dengan kualitas hidup pada pasien dengan LFS. Penelitian ini merupakan penelitian analitik observasional yang melibatkan 25 pasien dengan gejala klinis LFS di RS. X Surakarta. Pasien dinilai dengan mengisi kuesioner JOABPEQ dan ODI, kemudian dilakukan evaluasi MRI sagittal lumbar untuk menentukan derajat Lee score, kemudian melakukan uji korelasi pada data yang diperoleh. Penelitian ini menunjukkan korelasi yang signifikan antara Skor Lee dengan JOABPEQ dan ODI. Tingkat LFS berdasarkan Lee Score memiliki korelasi yang signifikan dengan tingkat disabilitas menggunakan JOABPEQ dan ODI. JOABPEQ memiliki korelasi yang lebih signifikan dengan Skor Lee dibandingkan dengan ODI. Kata Kunci : Lumbar Foraminal Stenosis, Lee Score, JOABPEQ, ODI Lumbar Foraminal Stenosis (LFS) can significantly reduce the patient’s function and quality of life and Magnetic Resonance Imaging (MRI) is commonly used supporting tool to measure the degree of stenosis. Lee Score is commonly used to measure the degree of LFS on sagittal MRI. Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) and Oswestry Disability Index (ODI) to assess disability and functional scores in LFS patients. This study was conducted to determine the correlation between the degree of LFS on sagittal MRI images with quality of life in patients with LFS.This study is an observational analytics study involving 25 patients with clinical symptoms of LFS in X Hospital Surakarta. Patients were assessed by filling JOABPEQ dan ODI questionnaires, then performed sagittal lumbar MRI evaluation to determine the degree of Lee Score, then performed correlation test on the data obtained.This study shows a significant correlation between Lee Score with JOABPEQ and ODI. The degree of LFS based on Lee Score has a significant correlation with the degree of disability using JOABPEQ and ODI. The imaging mimics, acute osteoporotic compression fractures, metastasis and malignant melanoma or plasmacytoma pathological fractures are the important clinical problems in geriatric age group that need to be differentiated due to their grossly differing prognostic and therapeutic implications. There are few suggestive features on magnetic resonance imaging (MRI) that help differentiate between these entities. Hemangiomas are very common benign spinal tumors that have characteristic features on MRI. In the setting of multiple vertebral hemangiomas causing cord compression in elderly patients, the scenario is even more complex with four different entities with different prognostic profiles. We report such a diagnostic dilemma we encountered in a middle aged female patient with multiple vertebral hemangiomas and compression fracture in D10 vertebra.

The detection and delineation of spinal tumors by magnetic resonance imaging (MRI) after intravenous administration of gadolinium (Gd)-diethylenetriaminepenta-acetic acid (DTPA) is demonstrated in eight cases of neurinoma or meningioma. The advantages of Gd-DTPA-enhanced MRI over other MRI techniques used in more than 100 cases of spinal cord diseases are described.

Nowadays, brachytherapy is one of the major components to treat inoperable cervical cancer. Brachytherapy yields a higher dose to the target (cervix) while sparing normal tissues. Developments of brachytherapy stepped forward in the previous decade by image-guided brachytherapy (IGBT) turning brachytherapy from point-based planning to volume-based planning and IGBT improves the treatment quality for cervical cancer. Magnetic resonance imaging (MRI) or computed tomography (CT) is utilized in brachytherapy and showed promising results internationally. However, in a limited-resource area, the implementation of IGABT is difficult due to many causes (manpower, equipment, or budgets). To improve the quality in limited resources, ultrasound is introduced. The utilization of ultrasound in brachytherapy practice is to prevent uterine perforation during application. With present data, measurement by ultrasound showed the correlation to MRI measurement in uterine dimensions. With these aspects, there are many researches using ultrasound to improve the quality of treatment in brachytherapy, for example, to guide contouring on CT or to support brachytherapy planning. The use of ultrasound improves the quality of brachytherapy in comparison to conventional planning and supports the improvement in brachytherapy for cervical cancer.

Gadolinium-Based Contrast Agents (GBCA) are intravenous drugs used in diagnostic imaging procedures to enhance the quality of Magnetic Resonance Imaging (MRI) or Magnetic Resonance Angiography (MRA). FDA alerts concerning potential side effects increased patient and medical concerns. More recent FDA information indicate that ionic gadolinium is released from some GBCAs, potentially causing gadolinium toxicity. We checked if gadolinium is excreted renally without intervention after previous administration of GBCAs, and if chelating agents are effective in removing gadolinium that may have remained in the body after GBCA administration. Through the evaluation of our internal database and the studies of others, we concluded that no clear consensus exists at this time. While the DTPAs may be the choice of chelators for the removal of gadolinium from the human body, further studies are needed to prove this. It seems clear, however, that microcyclic GBCAs provide a lesser risk of causing gadolinium retention and gadolinium toxicity symptoms.
