A comprehensive dementia service delivery is not well established in Indonesia, even though geriatric teams have been in place since 1997. Psycho-geriatric services are mainly provided by departments of psychiatry, as well as the geriatric division of internal medicine or neurology departments. Screening is routinely performed using the Mini-Mental State Examination and the Abbreviated Mental Test. In the near future, the division of geriatric psychiatry in psychiatry departments will have comprehensive services for dementia, from initial assessment to cognitive and functional rehabilitation. This will comprise screening, home visits, day care, home care, educational seminars, and specific programmes tailored to the needs of the person with dementia. Pre-dementia detection and any activities promoting active ageing should be prioritized in Indonesia.

Introduction: The aim of the dissertation was to compare the psychophysical state of people over 50 years of age from different societies.Material and methods: The first group consisted of 30 care home residents, the second group consisted of 30 participants of Active Senior Fair. In the research, two scales were applied: Mini­‍‑Mental State Examination, measuring the mental state, and Instrumental Activity of Daily Living, measuring agility in the field of complex everyday tasks.Conclusiona: The study indicates the difference in the psychophysical state of both researched groups. The results may be connected with both age difference between groups and different living environment.

THE IMPACT OF GRANDPARENTING ACTIVITY ON COGNITIVE FUNCTION IN ELDERLY WOMANABSTRACT Introduction: Cognitive impairment is a neurodegenerative process which often experienced by the elderly. One of the factors associated with cognitive function is grandparenting which is common and has become a social norm in Indonesia.Aims: To determine the association between characteristics of the subjects and grandparenting activity with cognitive function in elderly woman.Methods: This study was conducted on July 2016–January 2017 in West Jakarta and as a part of Active Ageing Research in Atma Jaya University. The data were collected through grandparenting activity questionnaire and cognitive function assessment using Mini Mental State Examination (MMSE).Results: Based on 93 participants, the age mean was 67,51±5,058 and majority had education <9 years (54,8%). Data analysis showed that education, grandparenting, the quality of grandparenting, types of grandparenting activity specifically preparing food and/or feeding child and storytelling had a significant association with global cognitive function.Discussions: Physical, social activity, and cognitive stimulation while caring for grandchildren can prevent decline in cognitive function.Keywords: Elderly, cognitive function, grandparentingABSTRAK Pendahuluan: Penurunan fungsi kognitif merupakan suatu proses neurodegeneratif yang sering dialami oleh lansia. Salah satu faktor yang dianggap berpengaruh terhadap fungsi kognitif adalah aktivitas pengasuhan cucu yang masih umum dilakukan dan telah menjadi bagian dari norma masyarakat di Indonesia.Tujuan: Mengetahui hubungan antara karakteristik subjek dan aktivitas pengasuhan cucu terhadap fungsi kognitif pada perempuan lansia.Metode: Penelitian dilakukan pada bulan Juli 2016–Januari 2017 di Jakarta Barat dan merupakan bagian dari penelitian Active Ageing di Universitas Atma Jaya. Pengumpulan data dilakukan melalui pengisian kuesioner aktivitas pengasuhan cucu dan pengukuran fungsi kognitif menggunakan Mini Mental State Examination (MMSE).Hasil: Penelitian terhadap 93 subjek dengan rerata usia 67,51±5,058 dan sebagian besar berpendidikan <9 tahun (54,8%). Pendidikan, aktivitas pengasuhan cucu, kualitas pengasuhan cucu, serta jenis aktivitas pengasuhan cucu, yaitu menyiapkan dan/atau menyuapi makanan dan membacakan cerita memiliki hubungan yang bermakna dengan fungsi kognitif global.Diskusi: Aktivitas fisik, sosial dan stimulasi kognitif pada saat mengasuh cucu dapat mencegah terjadinya penurunanfungsi kognitif.Kata kunci: Aktivitas pengasuhan cucu, fungsi kognitif, lanjut usia

The number and diversity of rating scales and tests of psychiatric status has increased dramatically in the last 20 to 30 years. Tests are in screening, e.g. for dementia the Mini Mental State Examination and the Abbreviated Mental Test; to aid clarification of the diagnosis in difficult cases, e.g. Walton-Black New Word Learning Test, to differentiate between depression and dementia; and in assessing severity of symptoms, e.g. Hamilton Depression Rating Scale.

SynopsisOne hundred and fifty unselected elderly community subjects were assessed by Mini Mental State Examination (MMSE), Abbreviated Mental Test (AMT) and Mental Status Questionnaire (MSQ). The effects on cognitive test scores of potential confounding (non-cognitive) variables were evaluated. Sensitivities and specificities were: MMSE 80% and 98%; AMT 77% and 90%; and MSQ 70% and 89%. The MMSE identified significantly fewer false positives than the AMT and MSQ. The major effect of intelligence on cognitive test scores has previously been underestimated. Age, social class, sensitivity of hearing and history of stroke were also significantly correlated with cognitive test scores. Years of full time education and depression only affected the longer MMSE and CAMCOG. The MMSE (cut-off 20/21) can be recommended for routine screening. However, as scores are affected by variables other than cognitive function, particularly intelligence, further assessment of identified cases may fail to reveal significant functional impairment.


The chapter outlines traditional categories used to capture the current condition of a patient’s state of mind such as appearance and behaviour, mood, speech, thought content, abnormal beliefs and experiences. It suggests questions to ask and how to organize the material. The chapter encourages a descriptive approach in which examples from the interview are recorded to help future reference and help other clinicians make judgements as to significance. Guidance is also given on examining the cognitive state and interpreting intelligence. Further advice is offered on examining the mental state in the elderly, particularly interpreting cognitive impairment using scales–the Addenbrookes’ Cognitive Examination, the Mini-Mental State Examination, and the Abbreviated Mental Test .

Im folgenden Artikel werden fünf verschiedene Versionen der Mini-Mental-State-Examination dargestellt, die alle auf der Grundlage des Originals von Folstein erstellt wurden, sich jedoch deutlich voneinander unterscheiden und zu unterschiedlichen Ergebnissen kommen, unabhängig davon, ob das Screening von erfahrenen Untersuchern durchgeführt wird oder nicht. Besonders auffällig ist, dass Frauen die Aufgaben «Wort rückwärts» hoch signifikant besser lösten als das «Reihenrechnen». An Hand von Beispielen werden Punkteunterschiede aufgezeigt.

Zusammenfassung. Beim Einsatz der Center for Epidemiological Studies Depression Scale (CES-D) stellt sich das Problem der Dimensionalität des Instruments, dessen Lösung durch die Konfundierung eines Teilkonstruktes (“Wohlbefinden”) mit Besonderheiten der Itemformulierung Schwierigkeiten bereitet, da Antwortartefakte zu erwarten sind. Dimensionsstruktur und Eignung der CES-D zur Erfassung der Depression bei älteren Menschen wurden an einer Stichprobe von 663 über 75-jährigen Teilnehmern der “Leipziger Langzeitstudie in der Altenbevölkerung” untersucht. Da sich die Annahme der Gültigkeit eines partial-credit-Rasch-Modells sowohl für die Gesamtstichprobe als auch für eine Teilpopulation als zu restriktiv erwies, wurde ein 3- bzw. 4-Klassen-latent-class-Modell für geordnete Kategorien berechnet und die 4-Klassen-Lösung als den Daten angemessen interpretiert: Drei Klassen zeigten sich im Sinne des Konstrukts “Depression” geordnet, eine Klasse enthielt jene Respondenten, deren Antwortmuster auf ein Antwortartefakt hinwiesen. In dieser Befragtenklasse wird der Depressionsgrad offensichtlich überschätzt. Zusammenhänge mit Alter und Mini-Mental-State-Examination-Score werden dargestellt. Nach unseren Ergebnissen muß die CES-D in einer Altenbevölkerung mit Vorsicht eingesetzt werden, der Summenscore sollte nicht verwendet werden.
