The ethics of incomprehensibility

What can psychiatrists learn today from Karl Jaspers, who at the dawn of XX Century, held that the future of medicine was in binding philosophy to science ? How can young psychiatrists, who are so hungry for handbook knowledge, structured interviews, decision-making criteria, and therapeutic protocols be so patient as to listen to such a hybrid clinician-philosopher arguing for a kind of knowledge which is stubbornly aware of its limits, and breathlessly revolting against all sorts of objectification and dogmatism? How can those who are looking for ‘expert knowledge’ be satisfied with a kind of knowledge which conceives of itself as an ‘unlimited task’ which takes place in the face-to-face, here-and-now encounter between two persons? How can they be happy with a mentor whose main teaching can be condensed into one sentence: ‘[Q]uestions are more essential than answers, and every answer becomes a new question’? To respond to these interrogations, we need to tackle another more fundamental one: On what kind of knowledge can we rely to establish the foundations of psychiatry? Jaspers’ answer can be condensed in one single word: Psychopathology.

The last two decades have witnessed a tremendous growth in the body of literature addressing the importance and the impact of contracting and public procurement within the context of devolution of government. The austere budgetary and financial outlooks of the future suggest that the significance of the area will only continue to grow. As such, generating explanatory frameworks, within dimensions such as decisionmaking and accountability in public procurement, becomes crucial. Drawing from original research this article suggests one possible frame for understanding administrative decision-making in complex environments. Based on semi-structured interviews with public procurement specialists, the study identifies two decision-making patterns− broker and purist. It is asserted that the decision-making dynamics exhibited by administrators are contingent on their perceptions regarding environmental instability, in particular the political volatility surrounding their work.

Purpose This study aims to explore the current and future employment opportunities for library and information science (LIS) graduates. It identifies future employers and emerging competitors in the information landscape. The strategies that may be used to respond to and capture the future job market are also presented for LIS schools in the light of the study findings. Design/methodology/approach The qualitative approach coupled with the interview technique was used for collecting insight into the phenomena. Face-to-face and telephonic semi-structured interviews were conducted with 17 faculty members (professors, associate professors and assistant professors) from 8 LIS departments. A thematic analysis approach was used to analyze the data to present the participants’ opinions regarding various aspects of the job market in the country. Findings It was found that the job market was still traditional (library) and would continue to be so for the next five to seven years. However, the findings also indicated the emergence of a non-traditional job market in the country. New job titles and functions relevant to the emerging job sector other than libraries in the information environment were noted. Originality/value Findings may have a direct impact on the future orientation, survival and sustainability of LIS programs in the country. The study may inform curriculum revision and updating efforts to make it more relevant and responsive to future needs.

The present study intends to explore the motivation of Kurdish students to study in Hungary. To answer the query of the study; an in-depth qualitative interview method was employed. Nine higher education level students (BA, MA or PhD), five males and four females who are studying at four different universities in Hungary were interviewed. Individual face to face interviews were organised to engage into the details of what could be achieved from the participants and to discover their study experiences deeply. Based on the collected data outcomes; a process model of Kurdish student motivation to study in Hungary was designed which identifies the main stages of their motivational behavior in a process way from the initial wishes to accomplishments. The findings reveal that several reasons have impacts on responders` decision making and the choice of country. Also, the participants are highly motivated on their studies because of the future accomplishments they could gain as the result of pursuing their studies abroad and getting an international qualification

Background: Currently, many initiatives are devoted to optimizing informed consent for participation in clinical research. Due to the digital transformation in health care, a shift toward electronic informed consent (eIC) has been fostered. However, empirical evidence on how to implement eIC in clinical research involving neonates is lacking.Methods: Semi-structured interviews were conducted with 31 health care professionals active in Belgium or the Netherlands. All health care professionals had experience in conducting clinical research involving neonates. Interviews were audio-recorded, transcribed and analyzed using the framework method.Results: Interviewees generally supported the use of eIC in clinical research involving neonates. For example, eIC could enable parents to receive study feedback via the eIC system. Requirements were expressed for parental involvement to decide on which feedback would be appropriate to return. Moreover, experts specialized in presenting information and designing electronic systems should be involved. Broad consensus among health care professionals indicates that the face-to-face-interaction between parents and the research team is vital to establish a relationship of trust. Therefore, it is necessary that the use of eIC runs alongside personal interactions with the parents. Concerns were raised about the accessibility of eIC to parents. For this reason, it was suggested that parents should always be given the possibility to read and sign a paper-based informed consent form or to use eIC.Conclusions: Health care professionals' views indicate that the use of eIC in clinical research with neonates may offer various opportunities. Further development and implementation will require a multi-stakeholder approach.

Images and practices of relations, aliveness, and love provide a way to reconcile knowledge and its schooled pursuit with the wisdom required in our current, ecologically desperate times. This desperation is rooted, in part, in threads of the efficiency movement that were inherited by education in the early 1900s and left schools with a curriculum legacy that has become exhausted and counterproductive. This inheritance can be countered with ideas from the traditions of hermeneutics and ecological thought. But they are also countered with life-affirming and life-sustaining Cree ideas: wahkohtowin, wicihitowin, and sakihitowin. Practicing these ideas can help align work inside and outside schools with the characteristic spirit (ethos) of our earthly being, and can provide the grounds for a pointed critique of, and alternative to, the regnant regimes of contemporary schooling. wahkohtowin means, briefly put, “all things are related/all things are our relations” and wicihitowin refers to “the life-giving energy that is generated when people face each other as relatives and build trusting relationships by connecting with others in respectful ways.” sakihitowin means “love.” Reimagining curriculum as constituted by living fields of relations while also considering not only the energeia, the “aliveness” that is generated in the face-to-face care of and learning the ways of such living fields, but also the deep affection that is both needed for and produced by such reimagining, increases the prospects of our ecological future and the future of the more-than-human world.

OBJECTIVETo understand the professional and psychosocial factors that influence physician antibiotic prescribing habits in the inpatient setting.DESIGNWe conducted semi-structured interviews with 30 inpatient physicians. Interviews consisted of open-ended questions and flexible probes based on participant responses. Interviews were audio recorded, transcribed, de-identified, and reviewed for accuracy and completeness. Data were analyzed using emergent thematic analysis.SETTINGTwo teaching hospitals in Indianapolis, IndianaPARTICIPANTSA total of 30 inpatient physicians (10 physicians-in-training, 20 supervising staff) were enrolled in this study.RESULTSParticipants recognized that antibiotics are overused, and many admitted to prescribing antibiotics even when the clinical evidence of infection was uncertain. Overprescription was largely driven by anxiety about missing an infection, whereas potential adverse effects of antibiotics did not strongly influence decision making. Participants did not routinely disclose potential adverse effects of antibiotics to inpatients. Physicians-in-training were strongly influenced by the antibiotic prescribing behavior of their supervising staff physicians. Participants sometimes questioned their colleagues’ antibiotic prescribing decisions, but they frequently avoided providing direct feedback or critique. These physicians cited obstacles of hierarchy, infrequent face-to-face encounters, and the awkwardness of these conversations.CONCLUSIONA physician-based culture of prescribing antibiotics involves overusing antibiotics and not challenging the decisions of colleagues. The potential adverse effects of antibiotics did not strongly influence decision making in this sample. A better understanding of these factors could be leveraged in future efforts to improve antibiotic prescribing practices in the inpatient setting.Infect. Control Hosp. In Primary Care, the field of nursing comes face-to-face with the complexity of violence, leading these professionals to constantly re-evaluate their habitus. OBJECTIVE: to analyze how cases of violence against children and adolescents are approached by primary care nurses, identifying limits and possibilities for dealing with these cases. METHOD: a qualitative study, undertaken in 2011, through semi-structured interviews with 8 out of 48 nurses in the Family Health teams in the city of Uberaba in the state of Minas Gerais, the analysis of which followed the interpretation of meanings, based in dialectical hermeneutics. RESULTS: the following stand out: non-identification of violence as a problem for the nurses; denunciations and notifications as a role of the nurses; and the limits found in the face of violence. CONCLUSION: it is determined that the habitus of nursing directed at health promotion and prevention of violence must be restructured, overcoming the biomedical paradigm and involving intersectorial and multidisciplinary actions.

BackgroundNHS policy encourages general practices to introduce alternatives to the face-to-face consultation, such as telephone, email, e-consultation systems, or internet video. Most have been slow to adopt these, citing concerns about workload. This project builds on previous research by focusing on the experiences of patients and practitioners who have used one or more of these alternatives.AimTo understand how, under what conditions, for which patients, and in what ways, alternatives to face-to-face consultations present benefits and challenges to patients and practitioners in general practice.Design and settingFocused ethnographic case studies took place in eight UK general practices between June 2015 and March 2016.MethodNon-participant observation, informal conversations with staff, and semi-structured interviews with staff and patients were conducted. Practice documents and protocols were reviewed. Data were analysed through charting and the ‘one sheet of paper’ mind-map method to identify the line of argument in each thematic report.ResultsCase study practices had different rationales for offering alternatives to the face-to-face consultation. Beliefs varied about which patients and health issues were suitable. Co-workers were often unaware of each other’s practice; for example, practice policies for use of e-consultations systems with patients were not known about or followed. Patients reported benefits including convenience and access. Staff and some patients regarded the face-to-face consultation as the ideal.ConclusionExperience of implementing alternatives to the face-to-face consultation suggests that changes in patient access and staff workload may be both modest and gradual. Practices planning to implement them should consider carefully their reasons for doing so and involve the whole practice team.
