Tieto kuntoutuksen kentän kokonaisuudesta ja erilaisten kuntoutuspalveluiden käytöstä samoilla ihmisillä on varsin hajanaista ja puutteellista. Tutkimuksen tavoitteena on selvittää kuntoutukseen osallistumisen yleisyyttä ja päällekkäisyyttä eri osajärjestelmissä Oulun asukkailla vuonna 2018 laajalla rekisteriaineistolla (N = 192 844). Tutkimuksessa tarkastellaan julkisen sosiaali- ja terveyspalvelujärjestelmän kuntoutusta, Kelan kuntoutusta, työeläkekuntoutusta, työterveyshuollon fysioterapiaa ja Kelan korvaamaa yksityistä fysioterapiaa. Vuonna 2018 oululaisista 18 prosenttia sai vähintään yhden tutkitun osajärjestelmän kuntoutusta (N = 34 061). Yleisintä oli julkisen sosiaali- ja terveyspalvelujärjestelmän kuntoutus, harvinaisinta työeläkekuntoutus. Naiset osallistuivat kuntoutukseen miehiä yleisemmin. Kuntoutukseen osallistuminen oli miehillä yleisintä 65 vuotta täyttäneillä, naisilla 45–64-vuotiailla. Harvinaisinta se oli 16–24-vuotiailla miehillä ja alle 16-vuotiailla naisilla. Suurin osa (90 %) kuntoutukseen osallistuneista oli osallistunut vain yhden osajärjestelmän kuntoutukseen. Useamman osajärjestelmän kuntoutukseen osallistuminen oli naisilla miehiä yleisempää. Ikäryhmistä se oli yleisintä 45–64-vuotiailla ja harvinaisinta alle 16-vuotiailla. Vähintään kahden osajärjestelmän kuntoutukseen osallistuneet olivat keskimäärin vanhempia kuin vain yhden osajärjestelmän kuntoutukseen osallistuneet ja myös naisten osuus oli heillä suurempi. Rekisteritietoja kuntoutuksesta on hankala koota kattavasti, sillä järjestelmä on hyvin hajanainen ja toimijoita ja rekisterinpitäjiä on lukuisia. Myös kuntoutuksen määrittely aineistoista osoittautui vaikeaksi. Yhtenäiset tietojärjestelmät kuntoutuksesta palvelisivat paitsi tutkijoita, myös kuntoutujia. Abstract Prevalence and overlap of participation in rehabilitation in different subsystems – a register-based study among residents of the city of Oulu, Finland, in 2018 Knowledge of the whole spectrum of rehabilitation and of the use of different rehabilitation services by the same individuals is quite fragmented and incomplete. The aim of this study is to examine the prevalence and overlap of participation in rehabilitation in different subsystems among residents of the city of Oulu, Finland, in 2018 using extensive register-based data (N=192,844). The study examines rehabilitation organized by the public social and health care system, by the Social Insurance Institution of Finland, by the earnings-related pension system, as well as physiotherapy in occupational health care and private physiotherapy reimbursed by the Social Insurance Institution of Finland. In 2018, 18 per cent of the residents of Oulu received rehabilitation of at least one of the examined subsystems (N=34,061). Receiving rehabilitation of public social and health care was the most common; the rarest was rehabilitation within the earnings-related pension system. Women participated in rehabilitation more often than men. Using rehabilitation services was most common in men aged 65 and over, and in women aged 45–64. It was least common in men aged 16–24 years and in women under 16 years of age. The majority (90%) of those who participated in rehabilitation had participated in rehabilitation of only one subsystem. Participation in rehabilitation of several subsystems was more common in women than in men. It was most common in those aged 45–64 years and least common in those under 16 years of age. Those who received rehabilitation of at least two subsystems were, on average, older than those who received rehabilitation of only one subsystem, and more often women. It is difficult to compile comprehensive register data on rehabilitation, as the system is very fragmented and there are many organizers and registrars. Defining rehabilitation from the data also proved difficult. Unified information systems on rehabilitation would serve not only researchers but also rehabilitees. This paper deals with the Korean welfare reform carried out due to the extremely unstable societal atmosphere, caused by the 1997 economic crisis. Introduction of the National Basic Livelihood Security Act and expansion of the coverage of social insurance are characterized as the projected reform. Nevertheless, the newly introduced public assistance program and the social insurance schemes still have room for improvement. For example, social insurance schemes had provided a protective umbrella in favor of regular workers, but it did not do the same for temporary contract-based workers or the self-employed. The government's efforts have not been sufficient to overcome these problems. This paper suggests some measures to resolve problems still existing in the Korean welfare system today.

Convicts and undocumented immigrants are similarly excluded from full social and political membership in the United States. Disfranchised, denied core protections of the social welfare state and subject to forced removal from their homes, families, and communities, convicts and undocumented immigrants, together, occupy the caste of outsiders living within the United States. This essay explores the rise of the criminal justice and immigration control systems that frame the caste of outsiders. Reaching back to the forgotten origins of immigration control during the era of black emancipation, this essay highlights the deep and allied inequities rooted in the rise of immigration control and mass incarceration.

The Federal Social Security Act, which may be regarded as the central core of the social security program, is an omnibus act, containing the following features: (1) a national, compulsory oldage insurance plan, covering all employees except certain exempted groups; (2) two measures designed to stimulate the states to enact state unemployment compensation laws, namely, (a) a uniform nation-wide tax upon employers, against which a credit is allowable for contributions made to approved state unemployment compensation plans, and (b) subsidies to the states to cover the administrative costs of unemployment compensation; and (3) grants-in-aid to the states for old-age assistance, pensions for the blind, aid to dependent children, child welfare, maternal and child health, vocational rehabilitation, and public health activities. It is estimated that each of the two forms of social insurance will apply to about 25,000,000 wage-earners, and, when the maximum rates become effective in 1949, will involve annual contributions of nearly $3,000,000,000. This amount is approximately equal to the normal annual expenditure of the federal government prior to 1930. In addition, the grants-in-aid to the states were estimated by the actuaries of the President's Committee on Economic Security to reach a total of a half-billion dollars annually within a few years.History of the Federal ActWhen, in a message to Congress on June 8, 1934, the President indicated that he would submit a program of social insurance for consideration at the following session, the Wagner-Lewis unemployment insurance bill and the Dill-Connery old-age assistance bill were pending. Shortly afterwards, the President, by executive order, created the Committee on Economic Security, consisting of the Secretaries of Labor (chairman), Treasury, and Agriculture, the Attorney-General, and the Federal Emergency Relief Administrator. This committee appointed Professor Edwin E. Witte, of the University of Wisconsin, as executive director, and proceeded to build up a staff of actuaries and experts to study the whole problem of economic insecurity, and to prepare recommendations.

This chapter focuses on the Social Security Act and the disparate treatment of blacks, Mexicans, and European immigrants in the administration of Social Security, Unemployment Insurance, Aid to Dependent Children, and Old Age Assistance. Though framed as legislation that would help the “average citizen,” scholars have shown that the Social Security Act in fact excluded the vast majority of blacks from the most generous social insurance programs, relegating them to meager, decentralized, and demeaning means-tested programs. European immigrants, by contrast, benefited from many of the provisions of the Social Security Act, and in at least some respects, they benefited more than even native-born whites. The net result of these policies was that blacks were disproportionately shunted into categorical assistance programs with low benefit levels, European immigrants were disproportionately covered under social insurance regardless of citizenship, and Mexicans were often shut out altogether.

The Church of Jesus Christ of Latter-day Saints (LDS) has not addressed the structure of health care delivery in the United States that is simultaneously expensive far beyond the levels of any other nation and yet fails to provide access to basic health services for nearly 10% of the population. The concept of adequate health care in LDS teaching on welfare principles provides a basis for constructing an LDS argument for universal access to basic health care. This epilogue draws on the examples of Church advocacy of health care reform in Utah—and Church priorities in international humanitarian assistance programs—to construct a framework of ethical principles to assess proposals and criteria for a social commitment to provide adequate health care to all citizens.

Four decades of neoliberal health policies have left the United States with a health care system that prioritizes the profits of large corporate actors, denies needed care to tens of millions, is extraordinarily fragmented and inefficient, and was ill prepared to address the COVID-19 pandemic. The payment system has long rewarded hospitals for providing elective surgical procedures to well-insured patients while penalizing those providing the most essential and urgent services, causing hospital revenues to plummet as elective procedures were cancelled during the pandemic. Before the recession caused by the pandemic, tens of millions of Americans were unable to afford care, compromising their physical and financial health; deep-pocketed corporate interests were increasingly dominating the hospital industry and taking over physicians’ practices; and insurers’ profits hit record levels. Meanwhile, yawning class-based and racial inequities in care and health outcomes remain and have even widened. Recent data highlight the failure of policy strategies based on market models and the need to shift to a nonprofit social insurance model.
