Global population projections indicate that the proportion of older people is increasing and will continue to do so for the foreseeable future. Many countries are already experiencing the challenges of managing an ageing population, including increased pension contributions from workers, rises to national retirement ages, and spiralling healthcare costs. In oral health terms, in addition to simply an ageing population, epidemiological studies have demonstrated significant changes in the oral health of older adults in recent years. As the numbers of edentulous older adults has declined, there has been a significant increase in the number of partially dentate elderly. Changing attitudes, improved access to dental care and more effective preventative programmes have meant that large numbers of patients are now retaining natural teeth into old age. However, as older patients retain natural teeth for longer, the dental profession is charged with controlling chronic dental diseases in an increasingly challenging oral environment.

The increased percentage of older people retaining their natural dentition was associated with a burden of poor oral health and increased service demands. This study analyses the dental service utilisation of the ageing population in Australia and develops a modelled cost design that estimates the dental expenditure required to cover dental services for the aged population. Using the Australian Census of Population and Housing, ageing population and socioeconomic data were mapped to geographic boundaries and integrated with dental service provision data to estimate a model for the utilisation of dental services. The estimated financial cost of dental services was calculated based on the mean fees as per the Australian Dental Association’s Dental Fees Survey. The utilisation of the services varied considerably across the states and also by type of service, with limited numbers using periodontic services. However, there was an increase in cost for replacement and restorative services (5020 million AUD), most evident in the socioeconomic deprivation areas. In addition, the average dental services utilisation cost increased noticeably in the lower socioeconomic deciles of all regions outside major cities. The geographic maldistribution of older people significantly affects the utilisation of dental services, especially among disadvantaged communities. A predicted cost model of 6385 million AUD would cover the oral health needs of older Australians.

BACKGROUND The constant progression in number and share of the ageing population will likely have deep effects in most of the industrialized countries. The Internet of Things (IoT) paradigm can play a key role in facilitating independent living of the ageing population thus trying to reduce the burden on the society. Considering that ageing is a multi-factorial physiological process, the development of novel IoT systems, tools and devices, specifically targeted to older people, must be based on a holistic framework built on robust scientific knowledge in different scientific domains. OBJECTIVE A novel semantic formalization was developed, based on a multidomain healthy ageing model, to support structuring and standardizing heterogeneous scientific knowledge about ageing. The main aim of the paper is to present the new NESTORE ontology, with the purpose thus extending the available ontologies provided by universAAL-IoT (uAAL-IoT). METHODS Well-assessed scientific knowledge, specifically selected to target older adults aged between 65 and 75, was formalized into a holistic model using a multi-domain approach including three main different dimensions related to well-being: (i) Physiological Status and Physical Activity Behaviour, (ii) Nutrition, and (iii) Cognitive and Mental Status and Social Behaviour. Based on this model, within the NESTORE H2020 project, a new ontology was developed in the uAAL-IoT framework, which provides modelling tools and a set of core ontologies. RESULTS The NESTORE ontologies cover all the needed concepts to represent 5 significant domains of ageing. In total, 12 sub-ontologies were modelled with more than 60 classes and sub-classes referenced among them by using more than 100 relations and around 20 enumerations. NESTORE increases the uAAL ontologies collection by 40% and expand the uAAL domain usage for Physiological Status and Physical Activity Behaviour (8 ontologies), Nutrition (3 ontologies) and Cognitive and Mental Status and Social Behaviour (4 ontologies). CONCLUSIONS NESTORE ontology provides innovation both in terms of semantic content and technological approach. The thoroughly use of this ontology can support the development of a decision support system, to promote healthy ageing, with the capacity to do dynamic multi-scale modelling of user-specific data based on the semantic annotations of users’ profile.

Abstract Background Globally the population of older adults is increasing. It is estimated that by 2050 the number of adults over the age of 60 will represent over 21% of the world’s population. Frailty is a clinical condition associated with ageing resulting in an increase in adverse outcomes. It is considered the greatest challenge facing an ageing population affecting an estimated 16% of community-dwelling populations worldwide. Aim The aim of this systematic review is to explore how wearable sensors have been used to assess frailty in older adults. Method Electronic databases Medline, Science Direct, Scopus, and CINAHL were systematically searched March 2020 and November 2020. A search constraint of articles published in English, between January 2010 and November 2020 was applied. Papers included were primary observational studies involving; older adults aged > 60 years, used a wearable sensor to provide quantitative measurements of physical activity (PA) or mobility and a measure of frailty. Studies were excluded if they used non-wearable sensors for outcome measurement or outlined an algorithm or application development exclusively. The methodological quality of the selected studies was assessed using the Appraisal Tool for Cross-sectional Studies (AXIS). Results Twenty-nine studies examining the use of wearable sensors to assess and discriminate between stages of frailty in older adults were included. Thirteen different body-worn sensors were used in eight different body-locations. Participants were community-dwelling older adults. Studies were performed in home, laboratory or hospital settings. Postural transitions, number of steps, percentage of time in PA and intensity of PA together were the most frequently measured parameters followed closely by gait speed. All but one study demonstrated an association between PA and level of frailty. All reports of gait speed indicate correlation with frailty. Conclusions Wearable sensors have been successfully used to evaluate frailty in older adults. Further research is needed to identify a feasible, user-friendly device and body-location that can be used to identify signs of pre-frailty in community-dwelling older adults. This would facilitate early identification and targeted intervention to reduce the burden of frailty in an ageing population.

Abstract Oral health status and dental care utilization is strongly associated with social and behavioral factors and health outcomes. The five papers in this symposium address how several of these factors affect oral health and dental care among diverse groups of older adults. Using data from the Health and Retirement Study, the first paper examined the impact of early childhood disadvantages on oral health in later life among adults age 51 and above in the U.S. The second paper used large-scale epidemiological data that addressed the relationship between acculturation and subsequent oral health problems. It also tested the moderating role of neighborhood disorder in such a relationship among older Chinese Americans. The third paper demonstrated the importance of examining different pathways among foreign-born and native-born Chinese older adults with regard to offspring’s support on their oral health outcomes. While increasing evidence shows that cognitive function is associated with oral health, limited studies have been conducted to examine the impact of cognitive impairment, e.g., Alzheimer’s Disease (AD) and related dementias (RD), on dental care utilization and costs in older adults. The fourth paper aimed to address this knowledge gap. Results showed that AD and RD had different impacts on different types of dental care utilization and costs. The fifth paper further displayed that individuals with cognitive impairment face a significant challenge in handling dental-related medications. This symposium provides policy and clinical implications on improving oral health and dental care utilization among older adults in the U.S. Oral Health Interest Group Sponsored Symposium.

ABSTRACTIt is noteworthy that while modernisation, together with population ageing, has happened in Western countries for a rather long period of time, China must confront the challenges of an ageing population before it really becomes an advanced industrial society. Facing rapid societal changes, citizens may react differently to this process. By conducting questionnaire surveys with 445 elders in Wuhan, China, this study examines Chinese older adults' individual modernity modes and reveals its socio-demographic correlates. The results show that facing rapid changes in societal modernisation, participants have reacted differently to this process, with 23.8 per cent of Chinese older participants using an accommodation mode, 27.0 per cent using a resistance mode, 13.3 per cent using a withdrawal mode and 36.2 per cent using a coping mode. In addition, socio-demographic factors such as urban–rural residence, age, gender, socio-economic status, living arrangement and daily activities have been identified as significant correlates of Chinese older participants' individual modernity modes. It is further suggested that governments at all levels should allow for options and opportunities for older people to align themselves in the modernisation process by removing the barriers that may hinder their integration and empowerment efforts. While it is important to respect older adults' different responses in the face of modernisation, their participation in societies should be encouraged and respected by the wider society in the context of modernisation.

As a rapidly ageing population becomes an increasingly serious social challenge for Chinese megacities, issues affecting older adults’ subjective well-being (SWB) attract greater concern. However, it is difficult to gain a comprehensive understanding of older adults’ SWB, since most SWB theories focus only on specific factors. Moreover, residential environmental factors are hardly considered in studies of older adults’ SWB. In this paper we therefore investigate the effects of residential environment and individual resources on the SWB of older adults in Shanghai, using the integrative theoretical framework proposed by Lindenberg. We investigate the relationships between resources (residential environment and individual resources), needs satisfaction and SWB using multiple regression analysis. Our results show that the residential environment exerts a stronger impact on SWB than individual resources. Good quality residential building, good accessibility to medical and financial facilities, higher economic status of a neighbourhood, and a lower proportion of older adults in a neighbourhood are important environmental correlates of SWB. Health appears to be the most significant individual resource; other important individual resources include household income, a high-skilled occupation, a job in the public sector and living with grandchildren. Comfort is the most important basic need for older adults.
