Family home visiting programs delivering early childhood services are supported by politicians and policy makers in many countries. This study focuses on a home visiting program for first-time parents in a county in Sweden. The program comprises six home visits conducted by interprofessional teams, including child healthcare nurses, midwives, social workers and dental hygienists, with the aim to increase accessibility to child healthcare and to promote more equal health in young children. Child healthcare, maternal care, social services and dental care organisations participated voluntarily in the program. This study explores how middle managers of the participating organisations view the program. Data were collected from semi-structured interviews with ten middle managers. The interviews were analysed using qualitative content analysis as a method. The results show that the middle managers saw the home visiting program as beneficial for society, parents and children, and the participating organisations and professionals. In other words, they expressed both altruistic goals and a self-interest in participating. The study is of importance as middle managers’ decision to participate in a home visiting program might be grounded on their perceptions of the program.

While home visiting programs are among the most widespread interventions to support at-risk families, there is a paucity of research investigating these programs under real-world conditions. The effectiveness of Families First home visiting (FFHV) was examined for decreasing rates of being in care of child welfare, decreasing hospitalizations for maltreatment-related injuries, and improving child development at school entry. Data for 4,562 children from home visiting and 5,184 comparison children were linked to deidentified administrative health, social services, and education data. FFHV was associated with lower rates of being in care by child’s first, second, and third birthday (adjusted risk ratio [aRR] = 0.75, 0.79, and 0.81, respectively) and lower rates of hospitalization for maltreatment-related injuries by third birthday (aRR = 0.59). No differences were found in child development at kindergarten. FFHV should be offered to at-risk families to decrease child maltreatment. Program enhancements are required to improve child development at school entry.

Abstract The purpose of this qualitative content analysis was to explore the experiences and perceived benefits for older adult members who participated in an intergenerational rock band (IGRB) pairing older adults with college students. Participants (n = 29) were first-time older adult IGRB members who completed semi-structured interviews over the course of 5 years. Interviews were completed within 1 week of the conclusion of the semester-long participation in the IGRB. Each interview was digitally recorded, manually transcribed, and analyzed using an iterative approach to thematic analysis. Experiences and perceived benefits associated with participation in the IGRB for these older adults were encompassed by four themes: Staying Active, Wanting to Try Something New, Feeling Connected, and Seeking Positive Experiences. Findings indicate that an IGRB is an innovative, collaborative, and inclusive intergenerational music-making experience that leaves a multifaceted and overall positive impact on its older adult members. Themes suggest that older adults with musical backgrounds may enjoy an experience like an IGRB because it brings a level of familiarity coupled with the opportunity to try something new and different from their previous experiences with music. A key contribution of this study is the importance of promoting a judgment-free environment for singing that is inclusive of all ability levels. Developing a better understanding of older adults’ motivations for participating in an IGRB may help us to plan future music experiences for older adults and broaden our reach to the older adult community.

Purpose – Audio-diary methods are under-utilised in contemporary qualitative research. The purpose of this paper is to discuss participants and researchers’ experiences of using audio-diaries alongside semi-structured interviews to explore breastfeeding experiences in a short-term longitudinal study with 22 first-time mothers. Design/methodology/approach – The authors provide a qualitative content analysis of the participants’ feedback about their experiences of the audio-diary method and supplement this with the perspectives of the research team based on fieldwork notes, memos and team discussions. The authors pay particular attention to the ways in which the data attained from diaries compared with those from the interviews. Findings – The diaries produced were highly heterogeneous in terms of data length and quality. Participants’ experiences with the method were varied. Some found the process therapeutic and useful for reflecting upon the development of breastfeeding skills whilst negative aspects related to lack of mobility, self-consciousness and concerns about confidentiality. Researchers were positive about the audio-diary method but raised certain ethical, epistemological and methodological concerns. These include debates around the use of prompts, appropriate support for participants and the potential of the method to influence the behaviour under scrutiny. Interview and diary accounts contrasted and complemented in ways which typically enriched data analysis. Practical implications – The authors conclude that audio-diaries are a flexible and useful tool for qualitative research especially within critical realist and phenomenological paradigms. Originality/value – This appears to be the first paper to evaluate both participants and researchers’ experiences of using audio-diaries in a detailed and systematic fashion.

Purpose – Middle managers in health care today are expected to continuously and efficiently decide and act in administration, finance, care quality, and work environment, and strategic communication has become paramount. Since dialogical communication is considered to promote a healthy work environment, the purpose of this paper is to investigate the ways in which health care managers experienced observing subordinates’ dialogue training. Design/methodology/approach – A qualitative study using semi-structured interviews and documents from eight middle managers in a dialogue programme intervention conducted by dialogue trainers. Focus was on fostering and assisting workplace dialogue. Conventional qualitative content analysis was used. Findings – Managers’ experiences were both enriching and demanding, and consisted of becoming aware of communication, meaning perceiving interaction between subordinates as well as own silent interaction with subordinates and trainer; Discovering communicative actions for leadership, by gaining self-knowledge and recognizing relational leadership models from trainers – such as acting democratically and pedagogically – and converting theory into practice, signifying practising dialogue-promoting conversation behaviour with subordinates, peers, and superiors. Research limitations/implications – Only eight managers participated in the intervention, but data afforded a basis for further research. Practical implications – Findings stressed the importance of listening, and of support from superiors, for well-functioning leadership communication at work. Originality/value – Studies focusing on health care managers’ communication and dialogue are few. This study contributes to knowledge about these activities in managerial leadership.

BACKGROUND The demand for rehabilitation services, including Orthotics and prosthetics services, is increasing. Traffic injuries, Eight-years imposed war, natural disasters, aging, and chronic diseases are the main causes of disability in Iran that lead to the great need for receiving these services. This study will attempt to identify the challenges in the financing and provision of Orthotics and Prosthetics services in Iran and declare the policy solutions to improve the financing and delivery of these services in line with the universal health coverage. OBJECTIVE This study aims to explore the challenges in the financing and provision of the Orthotics and Prosthetics services in Iran and propose policy solutions to strengthen this sector. METHODS This study will use the qualitative content analysis method for understanding the challenges. The study population of this study will be all clinicians, academics, and policy-makers who involved in the financing and provision of the Orthotics and Prosthetics services. The purposeful sampling, as well as the snowball sampling, will be used to select the informants. The individual face-to-face semi-structured interviews will be conducted in Tehran, Isfahan, and Shiraz. Furthermore, telephone and Skype interviews will be applied to participants who lived in other regions. This project will be performed from 2019 to 2020. RESULTS We believe the results will lead to identifying challenges and also policy solutions to strengthen the Orthotics and Prosthetics services in the healthcare system and improve the utilization in Iran. CONCLUSIONS The findings will inform the policy-makers to better planning in Iran and internationally.

Abstract Background Ensuring women receive optimal breastfeeding support is of key importance to the health of mothers and their infants. Early discharge within 24 h of birth is increasingly common across Australia, and the practice of postnatal home visiting varies between settings. The reduction in length of stay without expansion of home visits reduces midwives’ ability to support breastfeeding. The impact of early discharge on first-time mothers establishing breastfeeding was unknown. The study aim was to understand the experiences of first-time Australian mothers establishing breastfeeding when discharged from the hospital within 24 h of a normal vaginal birth. Methods A qualitative interpretive method was used. Semi-structured interviews with 12 women following early discharge were conducted. Data were audio recorded, professionally transcribed, and subjected to a thematic analysis. Results Three interconnected themes of ‘self-efficacy’, ‘support’ and ‘sustainability’ were identified. Self-efficacy influenced the women’s readiness and motivation to be discharged home early and played a role in how some of the mothers overcame breastfeeding challenges. Social, semi-professional and professional breastfeeding supports were key in women’s experiences. Sustainability referred to and describes what women valued in relation to continuation of their breastfeeding journey. Conclusion This study found accessible people-based breastfeeding services in the community are valued following early discharge. Furthermore, there is demand for more evidence-based breastfeeding educational resources, potentially in the form of interactive applications or websites. Additionally, a focus on holistic and individualised breastfeeding assessment and care plans prior to discharge that link women with ongoing breastfeeding services is paramount.

Purpose The purpose of this paper is to look at family carers’ views and experiences of self-funded care for older people with an emphasis on attitudes to public intervention. Design/methodology/approach Semi-structured interviews were conducted with family carers in a densely populated city in Northern England. Study participants were recruited according to a purposive sampling strategy; data analysis was based on a qualitative content analysis approach. Findings The paper concludes that it may not be straightforward for local authorities to engage with family carers as appropriate under the Care Act 2014. An issue is that family carers do not envisage an intervention of the local authority in circumstances involving the use of privately paid social services. Research limitations/implications Qualitative information gathered within a broad study of family carers’ views, attitudes and practices of care of dependent older people have been interpreted in the light of the provisions of the Care Act 2014 concerning self-funders. The qualitative approach and the limited number of study participants are issues with the generalisation of findings. Originality/value Only a handful of studies have attempted to look into family carers’ experiences of self-funded care and the paper aims to contribute to such limited literature. It also provides an evidence-based assessment of the challenges associated with the implementation of the Care Act 2014.

David Olds developed nurse home visiting as an intervention for poor, first-time mothers. As demonstrated by three randomized trials in Elmira, Memphis, and Denver, the Nurse-Family Partnership became the basis for significant federal funding under the Maternal, Infant, and Early Childhood Home Visiting Program. The transition from pilot program to federal legislation included tiered funding, by which programs demonstrated by Randomized Controlled Trials were prioritized over those with less empirical support.
