Free Open Access Medical Education (FOAM) and its relevance to military clinicians

AbstractClinicians are expected to regularly update their professional knowledge, and in doing so, they navigate more complex and wide-ranging educational resources, putting greater pressure on their time. Free open-access medical education (FOAM) is an environment led predominantly through social media which enables discussion, education and rapid dissemination of journal articles and other educational material.The Defence Medical Services have many different employment groups spread across a variety of medical specialties. FOAM can provide benefits to military clinicians in promoting and disseminating the work being done, and in terms of recruitment and education of clinical personnel, both in the firm base and in deployed settings. There are several potential pitfalls among FOAM resources which can be addressed by carefully appraising the resources being used and being aware of the possible biases. This article aims to highlight the possible uses of FOAM within the military as well as some methods to help engage users and ensure they are able to negotiate the hazards of the FOAM world.

Innovation Concept: Free open access medical education (FOAM) is a quickly growing field. While there is an abundance of resources online, and on social media, the quality of those resources should always be questioned and reviewed. Furthermore, as medical learners progress in their training, they become lead consumers and producers of FOAM. Our educational innovation concept was the introduction of two FOAM streams into our residency program to assist learners to produce their own content with mentorship from our emergency medicine faculty. Methods: Medical students and residents training in the emergency department were encouraged to submit content to either our department website in the form of a clinical PEARL, or a research paper to the departmental Cureus online journal. All website content was reviewed by an attending physician and all Cureus content was submitted for further peer review and publication if approved. All published content was shared on social media through our department's Twitter account. A select number of residents were also mentored in reviewing and editing FOAM content and publishing it to our departmental website. Curriculum, Tool or Material: sjrhem.ca is the Saint John Regional Hospital Department of Emergency Medicine's website. A portion of the website is dedicated to posts arising from departmental rounds, case reviews as well as posts from learners in the form of clinical PEARLS. They are designed as succinct and informative clinical summaries and allow learners to share their content to a wider audience online. Cureus.com is an online journal of medical science, with a dedicated Dalhousie Emergency Medicine Channel. The editors are local emergency medicine faculty and senior residents, while reviewers are independent. In the last year, the clinical pearls received 5672 views, and the Cureus channel received 1143 content views. Conclusion: Feedback from learners regarding publication of their own FOAM has been positive and has allowed them to share their content to a much wider audience through our Departmental Website, Cureus Channel and Twitter stream. Furthermore, we are helping to prepare residents to produce their own high quality content, allowing our FOAM program to grow.

AbstractDespite the rapid expansion of online educational resources for emergency medicine, barriers remain to their effective use by emergency physicians and trainees. This article expands on previous descriptions of techniques to aggregate online educational resources, outlining four strategies to help learners navigate, evaluate, and contribute online. These strategies include 1) cultivating digital mentors, 2) browsing the most popular free open access medical education (FOAM) websites, 3) using critical appraisal tools developed for FOAM, and 4) contributing new online content.

 There is scant research-based evidence on the development and adoption of open access (OA) and institutional repositories (IRs) in Africa, and in Kenya in particular. This article reports on a study that attempted to fill that gap and provide feedback on the various OA projects and advocacy work currently underway in universities and research institutions in Kenya and in other developing countries. The article presents the findings of a descriptive study that set out to evaluate the current state of IRs in Kenya. Webometric approaches and interviews with IR managers were used to collect the data for the study. The findings showed that Kenya has made some progress in adopting OA with a total of 12 IRs currently listed in the Directory of Open Access Repositories (OpenDOAR) and five mandatory self-archiving policies listed in the Registry of Open Access Repositories Mandatory Archiving Policies (ROARMAP). Most of the IRs are owned by universities where theses and dissertations constitute the majority of the content type followed by journal articles. The results on the usage and impact of materials deposited in Kenyan IRs indicated that the most viewed publications in the repositories also received citations in Google Scholar, thereby signifying their impact and importance. The results also showed that there was a considerable interest in Swahili language publications among users of the repositories in Kenya.

UNSTRUCTURED This research study was designed to understand physician awareness and engagement with social media (sometimes referred to as #SoMe) within the digital landscape of healthcare delivery. An audience-response survey focused on social media “Social media in Healthcare: Physician Survey”, was administered during the “A Systematic Approach to Medically Unexplained Symptoms” continuing medical education conference. The Conference took place between August 22, 2019 and August 24, 2019. Data was collected on August 23, 2019. A range of 103 (59.5%) to 112 (64.7%) of the total 173 attendees participated in this cross-sectional audience-response survey, depending on the questions answered. Most responders were between the ages of 35 and 65 years (79.6%) and female (60.2%). A majority of responders were aware of social media terminology (88.7%), and many had used it personally (46.7%), but only 12% knew how to use social media to search medical topics, 18% used it to network professionally and most (68.9%) had a distrust of social media when it came to the protection of their privacy or their patients’ privacy. Overall, about 29.6% indicated an interest in future continued medical education focused on social media (and 27.4% were neutral). Awareness of social media’s role in healthcare has increased among physicians in practice, however their participation and knowledge of opportunities remains limited. Distance from academic institutions and concerns about privacy play the largest roles in the aversion towards professional use of social media.

BACKGROUND The COVID-19 pandemic has resulted in over 1,000,000 cases across 181 countries worldwide. The global impact of COVID-19 has resulted in a surge of related research. Researchers have turned to social media platforms, namely Twitter, to disseminate their studies. The online database Altmetric is a tool which tracks the social media metrics of articles and is complementary to traditional, citation-based metrics. Citation-based metrics may fail to portray dissemination accurately, due to the lengthy publication process. Altmetrics are not subject to this time-lag, suggesting that they may be an effective marker of research dissemination during the COVID-19 pandemic. OBJECTIVE To assess the dissemination of COVID-19 research articles as measured by Twitter dissemination, compared to traditional citation-based metrics, and determine study characteristics associated with tweet rates. METHODS COVID-19 studies obtained from LitCovid published between January 1st to March 18th, 2020 were screened for inclusion. The following study characteristics were extracted independently, in single: Topic (General Info, Mechanism, Diagnosis, Transmission, Treatment, Prevention, Case Report, and Epidemic Forecasting), open access status (open access and subscription-based), continent of corresponding author (Asia, Australia, Africa, North America, South America, and Europe), tweets, and citations. A sign test was used to compare the tweet rate and citation rate per day. A negative binomial regression analysis was conducted to evaluate the association between tweet rate and study characteristics of interest. RESULTS 1328 studies were included in the analysis. Tweet rates were found to be significantly higher than citation rates for COVID-19 studies, with a median tweet rate of 1.09 (SD 156.95) tweets per day and median citation rate of 0.00 (SD 3.02) citations per day, resulting in a median of differences of 1.09 (95% CI 0.86-1.33, P < .001). 2018 journal impact factors were positively correlated with tweet rate (P < .001). The topics Diagnosis (P = .01), Transmission (P < .001), Treatment (P = .01), and Epidemic Forecasting (P < 0.001) were positively correlated with tweet rate, relative to Case Report. The following continents of the corresponding author were negatively correlated with tweet rate, Africa (P <.001), Australia (P = .03), and South America (P < .001), relative to Asia. Open access journals were negatively correlated with tweet rate, relative to subscription-based journals (P < .001). CONCLUSIONS COVID-19 studies had significantly higher tweets rates compared to citation rates. This study further identified study characteristics that are correlated with the dissemination of studies on Twitter, such as 2018 journal impact factor, continent of the corresponding author, topic of study, and open access status. This highlights the importance of altmetrics in periods of rapidly expanding research, such as the COVID-19 pandemic to localize highly disseminated articles.
