IntroductionIn 2010, the International Agency for Research on Cancer (IARC) classified ‘Shiftwork that involves circadian disruption’ as a probable human carcinogen (group 2A), based on limited evidence from epidemiological studies and sufficient evidence from experimental animal studies. The epidemiological results were consistent for breast cancer. More recent publications suggested an increase in risk for ovarian and endometrial cancer, prostate cancer, and colorectal cancer.Sleep deprivation is common in shift workers, and it leads to disruption of the transcription of genes implicated in the regulation of the immune response.ObjectivesAs depression of the immune system is a known risk factor for lymphomas, we explored the association between risk of lymphoma subtypes and nightshift work with a case-control study design.Materials and methodsBased on the lifetime occupational history available for 323 cases and 463 controls who participated in a case-control study on lymphoma etiology conducted in Sardinia (Italy) in 1998–2006, expert occupational physicians assessed nightshift work for each job entry of each study subject. We calculated risk of major lymphoma subtypes, namely diffuse large B cell lymphoma, follicular lymphoma, chronic lymphocytic leukemia, multiple myeloma and Hodgkin lymphoma, associated with cumulative days of nightshift work with unconditional logistic regression, adjusting by age, gender, and education.ResultsNone of the lymphoma subtypes we investigated showed an association with lifetime cumulative days of nightshift work. Contrary to our expectation, risk of all lymphomas combined tended to decrease with cumulative days of nightshift work.ConclusionsFurther studies are warranted to investigate in more detail whether sleep deprivation resulting from nightshift work might be implicated in lymphomagenesis. Future investigations should include information on the chronotype of study subjects, the specific shift rotation schedule applied in each workplace, usual hours of sleep per day, and occurrence of daytime sleepiness.

We inquired into the association between occupational exposure to glyphosate and risk of lymphoma subtypes in a multicenter case-control study conducted in Italy. MethodsThe Italian Gene-Environment Interactions in Lymphoma Etiology (ItGxE) study took place in 2011-17 in six Italian centres. Overall, 867 incident lymphoma cases and 774 controls participated in the study. Based on detailed questionnaire information, occupational experts classified duration, confidence, frequency, and intensity of exposure to glyphosate for each study subject. Using unconditional regression analysis, we modelled risk of major lymphoma subtypes associated with exposure to glyphosate adjusted by age, gender, education, and study centre. ResultsVery few study subjects (2.2%) were classified as ever exposed to glyphosate. Risk of follicular lymphoma (FL) was elevated 7-fold in subjects classified as ever exposed to glyphosate with medium-high confidence, 4.5-fold in association with medium-high cumulative exposure level, 12-fold with medium-high exposure intensity, and 6-fold with exposure for 10 days or more per year. Significant upward trends were detected with all the exposure metrics, but duration. The overall p-value for an upward trend with four independent metrics was 1.88 x 10-4. There was no association with risk of lymphoma (any subtype), Non Hodgkin Lymphoma, B-cell lymphoma, or the major lymphoma subtypes other than FL. ConclusionsOur findings provide limited support to the IARC decision to classify glyphosate as Group 2A human carcinogen.

Nonparticipation limits the power of epidemiological studies, and can cause bias. In a case–control study on pleural malignant mesothelioma (MM), we found low participation in interviews (63%) among controls. Our goal was to characterize nonresponder controls and assess nonresponse bias in our study. We selected all nonresponder controls (204) and a random sample of responder controls (174). Data were obtained linking hospital admissions and town registrars, and concordance between sources was assessed. Nonresponse bias was evaluated using a logistic regression model applying the inverse probability weighting approach. The odds ratio (OR) for the status of the respondents was 0.61 (95% confidence interval (CI): 0.33–1.16) for controls aged 61–70, 0.37 (CI: 0.20–0.66) for those aged 71–80, and 0.40 (CI: 0.20–0.80) for those aged above 80 (reference group: ≤60 years). Controls with low education level had lower OR (0.47; CI: 0.26–0.84). After adjustment, the ORs for MM by categories of cumulative exposure to asbestos were similar to the unadjusted results, ranging from 4.6 (CI: 1.8–11.7) for cumulative exposures between 0.1 and 1 f/mL-y to 57.5 (CI: 20.2–163.9) above 10 f/mL-y. Responder controls were younger and had higher education level. Nevertheless, there was little evidence of bias from nonresponse in the risk estimates of MM.

Background and Purpose— Epidemiological studies of intracerebral hemorrhage (ICH) have consistently demonstrated variation in incidence, location, age at presentation, and outcomes among non-Hispanic white, black, and Hispanic populations. We report here the design and methods for this large, prospective, multi-center case–control study of ICH. Methods— The Ethnic/Racial Variations of Intracerebral Hemorrhage (ERICH) study is a multi-center, prospective case–control study of ICH. Cases are identified by hot-pursuit and enrolled using standard phenotype and risk factor information and include neuroimaging and blood sample collection. Controls are centrally identified by random digit dialing to match cases by age (±5 years), race, ethnicity, sex, and metropolitan region. Results— As of March 22, 2013, 1655 cases of ICH had been recruited into the study, which is 101.5% of the target for that date, and 851 controls had been recruited, which is 67.2% of the target for that date (1267 controls) for a total of 2506 subjects, which is 86.5% of the target for that date (2897 subjects). Of the 1655 cases enrolled, 1640 cases had the case interview entered into the database, of which 628 (38%) were non-Hispanic black, 458 (28%) were non-Hispanic white, and 554 (34%) were Hispanic. Of the 1197 cases with imaging submitted, 876 (73.2%) had a 24 hour follow-up CT available. In addition to CT imaging, 607 cases have had MRI evaluation. Conclusions— The ERICH study is a large, case–control study of ICH with particular emphasis on recruitment of minority populations for the identification of genetic and epidemiological risk factors for ICH and outcomes after ICH.

Abstract Background BPA and DEHP are endocrine disruptors. BPA-G and MEHP are their main metabolites. The main exposure route for human is the diet. Endometriosis is a pathology with uncertain etiopathogenesis, characterized by disturbances in sex hormones balance with a prevalence in women ranged from 1% to 7% in women aged 15-45 years. Methods A case-control study should highlight a possible relationship between exposure to DEHP and BPA and endometriosis. Patients (n = 40) with a surgical diagnosis of endometriosis will compose the group of cases; healthy women (n = 40) will compose the control group. Cases and controls will be studied by means of questionnaires and by means of urinary analysis of these endocrine disruptors concentration. Results BPA values ranged between 0.05-8.55 and 0.05-1.84 in cases and in controls, respectively; BPA-G values ranged between 0.05-16.1 and 0.05-7.38 in cases and in controls, respectively; total BPA values ranged between 0.05-3.26 and 0.05-3.51 in cases and in controls, respectively. DEHP values ranged between 13.63-57.78 and 16.66-61.93 in cases and in controls, respectively; MEHP values ranged between 1.74-6.16 and 4.26-14.16 in cases and in controls, respectively; total DEHP ranged between 15.27-63.93 and 21.72-72.73 in cases and in controls, respectively. A reverse correlation was found between endometriosis and phthalates, while a direct correlation was found between BPA and endometriosis. Conclusions The direct correlation between BPA and endometriosis suggests the decreasing the use of plasticizer in food packaging and the importance to implement interventions and strategy to minimize exposure. The reverse correlation between endometriosis and DEHP could be explained by the small size of sample and by the monitoring of urine that are representative of a short-time exposure. Key messages Improve epidemiological studies with analysis of alternative matrices that are more representative of long-time exposure. Implement interventions and strategy to minimize exposure to plasticizer.

IntroductionPhysical activity is known to convey protection against several cancers, but results on risk of lymphoma and its subtypes have been inconsistent. A possible reason might be confounding by workplace exposures associated with occupational energy expenditure, which was not considered in studies of recreational physical activity. It is also unclear whether energy expenditure acts directly, or through preventing obesity.ObjectivesTo investigate the role of energy expenditure, including occupational and recreational physical activity, on risk of lymphoma subtypes.Materials and methodsBased on questionnaire information on lifetime recreational physical activity and lifetime occupational history available for all participants to the multicenter EpiLymph case-control study, we estimated energy expenditure at work by occupational ISCO68 code, and we applied it to the work histories of study subjects. We also categorized hours of lifetime recreational physical activity into quartiles. We calculated risk of lymphoma subtypes with unconditional polytomous regression analysis, associated with increasing categories of lifetime energy expenditure at work (EEW), increasing categories of recreational physical activity (RPA), and their interaction term (total energy expenditure, TEE), adjusting by age, gender, education, body mass index (BMI), and history of farm work and solvents use.ResultsRisk of lymphoma overall, diffuse large B cell lymphoma and multiple myeloma was not associated with EEW, RPA and TEE. Risk of follicular lymphoma and chronic lymphocytic leukemia associated were elevated with medium and high EEW (OR 3.1, 95% CI 1.5–6.1; (OR 2.5 95% CI 1.2–5.1, respectively), but there was not a significant upward trend.ConclusionsFurther epidemiologic and mechanistic research is warranted to assess the role of physical activity in the etiology of lymphoma subtypes. New standardized energy expenditure assessment methods, as the ones herein developed, might contribute to a better understanding of the nature of the observed inconsistent findings.

Background: Several epidemiological studies have identified the association of abnormal ABPI with ischemic stroke. So the goal of this study was to determine the actual relationship of ABPI with ischemic stroke in the context of our country. Materials and Methods: This case control study was carried out in the Department of Neurology, Sir Salimullah Medical College and Mitford Hospital, Dhaka, Bangladesh. ABPI was measured by Doppler ultrasound machine of 100 patients who were admitted to the Mitford Hospital during the study period. Among them 50 patients with Ischemic stroke, confirmed by CT/MRI scan of brain were considered as ‘case’ and 50 age- sex matched individuals with one or more vascular risk factors (VRF) but without stroke were considered as ‘control’. Then the results of ABPI were compared between the two groups. Results: Among the 50 patients with ischemic stroke (case group) , 74% had normal ABPI and 26% had ABPI< 0.9; on the other hand among 50 age and sex matched individuals (control group) 90% had normal ABPI and 10% had ABPI <0.9. The difference was statistically significant between two groups (p=<0.05).This association remained significant even after adjustment for potential confounders (age, gender, high BMI, hypertension, diabetes mellitus, hyperlipidemia, smoking, ischemic heart disease and family history) in a multiple logistic regression model. Conclusion: The incidence of low ABPI is significantly higher in ischemic stroke patients than the age- sex matched control. Bangladesh Journal of Neuroscience 2013; Vol. 29 (1) : 31-42
