Familial aggregation ofV˙o 2 max response to exercise training: results from the HERITAGE Family Study

The aim of this study was to test the hypothesis that individual differences in the response of maximal O2 uptake (V˙o 2 max) to a standardized training program are characterized by familial aggregation. A total of 481 sedentary adult Caucasians from 98 two-generation families was exercise trained for 20 wk and was tested for V˙o 2 max on a cycle ergometer twice before and twice after the training program. The mean increase inV˙o 2 max reached ∼400 ml/min, but there was considerable heterogeneity in responsiveness, with some individuals experiencing little or no gain, whereas others gained >1.0 l/min. An ANOVA revealed that there was 2.5 times more variance between families than within families in theV˙o 2 max response variance. With the use of a model-fitting procedure, the most parsimonious models yielded a maximal heritability estimate of 47% for the V˙o 2 max response, which was adjusted for age and sex with a maternal transmission of 28% in one of the models. We conclude that the trainability ofV˙o 2 max is highly familial and includes a significant genetic component.

Objective: This study examined the effect of hormone-replacement therapy (HRT) use on the incremental GH response to aerobic exercise in postmenopausal women and established whether racial differences in the GH response were seen at rest and in response to exercise. Methods: 13 white (n = 6, HRT; n = 7, no HRT) and seven black women (no HRT) were studied on two occasions, a control day and an exercise day (30 min at 70% VO2max on a cycle ergometer). Blood was sampled every 10 min for a 4-h period and analyzed for GH using an ultrasensitive chemiluminescent assay. Results: The mean 4-h GH concentration was higher on both study days in the HRT women than the non-HRT users. The integrated GH concentrations were greater in the HRT women both at rest and in response to exercise (rest, 352 ± 53 min μg l−1; exercise, 711 ± 57 min μg l−1; P < 0.01) than in the non-HRT women (rest, 157 ± 87 min μg l−1; exercise, 248 ± 94 min μg l−1). The incremental GH response was greater in the HRT users than in the non-HRT women (358 ± 130 versus 90.8 ± 94 min μg l−1, respectively; P < 0.05). GH-production rate during the 4-h period was greater in the HRT women than in the non-HRT women (P < 0.01), due to an increase in the GH mass secreted/pulse (P < 0.05), with no change in GH pulse number or GH half-life. No racial differences in the mean 4-h GH concentrations or integrated GH concentrations were found at rest or in response to exercise. Conclusion: HRT use resulted in a greater incremental exercise response compared with non-HRT users, due to changes in the secretory pulse characteristics in the HRT users. This study also demonstrated that no racial differences exist at rest and in response to exercise in the morning hours.

AimTo investigate the heritability of corneal thickness at the apex, entrance pupil centre, thinnest point, pachymetric progression index (PPI) and maximum Ambrósio relational thickness (ARTmax) using Pentacam.MethodsThe present cross-sectional and population-based study was conducted in two rural districts that were selected randomly. Individuals 5 years and older and data from households where one of the parents and at least one child participated in the study were considered for this analysis. All subjects were examined for visual acuity, refraction, biomicroscopy and, finally, Pentacam imaging. The heritability estimate was used to calculate familial aggregation of pachymetric indices.ResultsOf the 3851 selected individuals, 3314 participated in the study. After applying the exclusion criteria, a total of 1383 individuals from 382 households were included in the analysis; of these, 754 (54.52%) were female. The mean age of the subjects was 37.23±19.35 years (from 6 to 93 years). The highest heritability was observed for corneal thickness at the apex (85%), and the lowest was for ARTmax (27.62%). The heritability of other studied parameters, including corneal thickness at the entrance pupil centre, thinnest point and average PPI, was 82.0%, 77.0% and 31.49%, respectively.ConclusionThe high heritability of the mentioned pachymetric parameters confirms the high correlation between these phenotypes and genetic factors and calls for genetic and molecular research to find related genes and to understand the aetiology of associated diseases, especially glaucoma and keratoconus.

Abstract Background The objective of this study is to characterize participants in a laparoscopic cadaveric neuroanatomy course and assess knowledge of pelvic neuroanatomy before and after this course. Methods This is a survey-based cohort study with a setting in a university educational facility. The participants are surgeons in a multiday laparoscopic cadaveric pelvic neuroanatomy course. Participants completed a precourse survey, including demographics and comfort with laparoscopic surgery. They then completed an identical precourse and postcourse anatomic knowledge test. Main outcomes are scores on the anatomic knowledge test precourse and postcourse. Results 44 respondents were included: 25 completed fellowship, 15 completed residency, 2 were residents, and 2 were fellows. Participants were on average 11.09 years post training, with an average of 8.67 years from training if they completed fellowship and 18.62 years if they completed residency only. 22 of 42 respondents strongly agreed or agreed they are comfortable performing complex laparoscopic hysterectomies. The average precourse score was 32.18/50 points and the mean difference score (MDS, defined as mean of Postcourse scores minus Precourse scores) was 9.80, showing significant improvement (p <  0.001). Precourse and MDS scores were not significantly different when comparing country of practice, level of training, or time since training. Conclusion Baseline knowledge of pelvic neuroanatomy was similar among groups when comparing fellowship status, place of training, or time since training. There was significant improvement in knowledge after training in this dissection method. This course garnered interest from surgeons with broad training backgrounds.

Research in primary health care (PHC) is underdeveloped and scarce, especially in developing countries. It is important to understand the attitudes and aspirations of PHC physicians for the promotion of research. The aim of this study was to determine the attitudes of PHC physician managers toward research in Qassim province and to identify barriers that impede performing research in the PHC system. The study was based on social cognitive theory framework, and was pre-experimental with a ‘one-group pre-test–post-test’ design. The study participants were physician managers in PHC administration, Qassim. The participants’ attitudes were measured by adapting statements from the Attitude Towards Research scale. The intervention was the 1-day training program ‘Introduction to Research in Primary Health Care’. A total of 23 PHC physicians participated in the study. The mean age of the participants was 45.4 (±1.6) years, and the mean years of work experience was 16.2 (±2.2) years. Only one participant had an article published in a peer-reviewed journal. The results of the study showed that PHC physicians had a baseline positive attitude toward research that was further enhanced after participating in an introductory research-training program. During the pre-test, out of the total score of 63, the mean score on attitude toward research was 48.35 (±6.8) while the mean total attitude score in the post-test was 49.7 (±6.6). However, the difference was not statistically significant at P < 0.05. The item with the highest score regarded the role of research in the improvement of health care services, while the lowest-scoring item was about support from administration to conduct research. The participants recognised lack of skills, lack of training and inadequate resources as major barriers in conducting research. Our study results suggest that the PHC physicians’ positive attitudes toward research can be further improved through in-service training. To promote research in PHC in Qassim, regular research-training programs and incentives for conducting research should be provided to the PHC physicians.

We present the use of Extended Modified Lambert–Beer model for optical monitoring of mean blood oxygen saturation ( S m O 2) via a fitting procedure. This work focuses on the absorption characteristics of hemoglobin derivatives in the wavelength range of 520–600 nm to give the best estimates of S m O 2. The study of the feasibility of applying this analytic method to skin oximetry is via spectroscopy data collected from fingertips of four healthy volunteers both at rest and during arterial blood occlusion condition. The results revealed a decrease in the mean of mean and standard deviation of S m O 2 value of fingertips from 94.5 ± 2.19% when volunteers were at rest to 56.76 ± 5.8% during the arterial blood occlusion measurement. The larger variation in the value estimated for blood occlusion condition could be a result of differences in volunteers' physical fitness and hypertension status. These estimated S m O 2 values agreed reasonably well with the value reported in most of the previous studies. This work concluded that the proposed technique can potentially be used as a complementary technique to clinical assessment of skin grafts and burnt skin.

To examine the effect of various carbohydrates on the metabolic and hormonal response to exercise, 75 g glucose, fructose, or placebo were given to nine well-trained males (VO2 max 60 +/- 1 ml . kg-1 . min-1) 45 min before cycle ergometer exercise performed at 75% VO2 max for 30 min. After glucose ingestion, the rise in plasma glucose was 3-fold (P less than 0.005) in plasma insulin 2.5-fold (P less than 0.01) greater than after fructose. During exercise, after glucose administration plasma glucose fell from 5.3 +/- 0.3 to 2.5 +/- 0.2 mmol/l (P less than 0.001) and after fructose from 4.5 +/- 0.1 to 3.9 +/- 0.3 mmol/l (P less than 0.05). The fall in plasma glucose was closely related to the preexercise levels of plasma insulin (r = 0.82, P less than 0.001) and glucose (r = 0.81, P less than 0.001). Both glucose and fructose ingestion decreased the FFA levels by 40–50% (P less than 0.005) and during exercise they remained 30–40% lower after carbohydrate than placebo administration (P less than 0.02). This study suggests that glucose ingestion prior to exercise results in hypoglycemia during vigorous exercise, this rapid fall in plasma glucose is mediated, at least in part, by hyperinsulinemia, and fructose ingestion is associated with a modest rise in plasma insulin and does not result in hypoglycemia during exercise.

Seventeen women (mean age 31 yr) participated in a training program divided into an initial 9-wk period and a subsequent 52-wk period, during which time 6 continued to exercise and the remainder detrained. Improvements in VO2max were significant (+34%) during the initial 9 wk and small (+5%) for the final 52 wk. Four women who stopped training showed a decrease in VO2max (-10%) during the last phase. During the initial 9 wk, central adaptation was important, with SV showing an increase of 28% at 80% VO2max. Peripheral adaptation (a-v O2 difference) was unchanged. Subjects who trained an additional 52 wk showed a slight drop in SV at submaximal work loads from the initial increase following the first 9 wk. When compared with the initial test the change at 9 wk in peripheral adaptation was a small and nonsignificant rise, followed by a significant increase at 61 wk. Women who are very unfit initially (predicted VO2max of 28 ml/kg-min), apparently adapt to the initial training with a central change followed by a much stronger peripheral adaptation during a longer training program.

The Hertzprung-Russel (HR) diagram luminosity calibration relies basically on three kinds of data: trigonometric parallaxes, kinematical data (proper motions and radial velocities) and cluster distances obtained by the zero-age main sequence fitting procedure. The most fundamental method to calculate the absolute magnitude is the use of trigonometric parallaxes, but up to now, accurate data only exist for stars contained in a small volume around the sun. Individual absolute magnitudes are obtained using trigonometric parallaxes or photometric and spectroscopic calibrations. In these calibrations the accuracy on the absolute magnitude determination ranges from ±0.m2 in the main sequence to ±0m5 in the giant branch. On the other hand, trigonometric parallaxes, kinematical data or cluster distances have been used to make statistical calibrations of the absolute magnitude. The standard error on the mean absolute magnitude calibrations ranges from ±0m3 to ±0m6 on the mean sequence, from ±0m5 to ±0m7 on thegiant branch and is of about 1mfor supergiants.Future improvements in the absolute magnitude determination will depend on the improvement of the basic data from the ground and space. A brief overview of the new available data is presented. In particular, the analysis of the first 30 months data of the Hipparcos mission (H30) (from the 37 months data of the whole mission) allows to perform a statistical evaluation of the improvements expected in the luminosity determination.

ABSTRACTThis paper presents first chronological results for a Holocene marshland system from the southern part of the Danube-Tisza Interfluve. Radiocarbon (14C) ages were used to build age-depth models relying of probabilistic tools. Four models have been built: a linear one using dates gained via simple calibration, a P_Sequence model, fitting a polynomial function to calibrated dates; a Gamma_Sequence considering priori given and posterior accumulation rates have been constructed. As there was no significant difference between the mean values of individual models all seem suitable for establishing a reliable chronology despite differences in 95% CI ranges. While P_Sequence models underestimated SR, values calculated from the polynomial model were not significantly different from those of the G_Sequence. Based on multiproxy geochemical, sedimentological, paleoecological data the evolution of the system was reconstructed, covering a timespan of ca. 13,000 years starting from 12,000 BC and lasting until 1300 AD. Highest accumulation rates are dated to the Early Middle Ages from the 11th century. Several climate changes could have been identified which are present in other Hungarian and Western European records too, such as the 5b IRD event at ca. 5800 BC, a humid phase around 1600 BC, and a cool humid phase around the 6th century AD.
