Poria cocos, Bai Fu Ling in Chinese, is used in traditional Chinese medicine to treat diabetes. However, its claimed benefits and mechanism are not fully understood. This study aimed to investigate the effect and action ofP. cocoson type 2 diabetes. We first performed phytochemical analysis on the crude extract and factions ofP. cocos.P. cocoscrude extract at 50 mg/kg body weight or more significantly decreased blood glucose levels in db/db mice. Based on a bioactivity-directed fractionation and isolation (BDFI) strategy, chloroform fraction and subfractions 4 and 6 of theP. cocoscrude extract possessed a blood glucose-lowering effect. Dehydrotumulosic acid, dehydrotrametenolic acid, and pachymic acid were identified from the chloroform sub-fractions 4, 3, and 2, respectively. Dehydrotumulosic acid had anti-hyperglycemic effect to a greater extent than dehydrotrametenolic acid and pachymic acid. Mechanistic study on streptozocin- (STZ-) treated mice showed that the crude extract, dehydrotumulosic acid, dehydrotrametenolic acid, and pachymic acid ofP. cocosexhibited different levels of insulin sensitizer activity. However, theP. cocoscrude extract and triterpenes appeared not to activate PPAR-γpathway. Overall, the data suggest that theP. cocosextract and its triterpenes reduce postprandial blood glucose levels in db/db mice via enhanced insulin sensitivity irrespective of PPAR-γ.

Baroreflex sensitivity (BRS) is abnormal in the prediabetic state. This study was conducted to determine effects of chronic rosiglitazone (RSG), an insulin sensitizer, on BRS in prediabetic hyperglycemic (PDH) rats induced by nicotinamide and streptozotocin. The fasting and postprandial blood glucose levels were 5.6–6.9 and 7.8–11.0 mmol/l, respectively. Rats were treated with RSG or saline for 12 weeks. BRS response to phenylephrine (PE-BRS) or sodium nitroprusside (NP-BRS) was determined by linear regression method. Cardiac sympathetic and parasympathetic influences were determined by autonomic blockades. In the saline-treated PDH rats, PE-BRS was enhanced early at week 4 and became greater at week 12. Abnormalities in NP-BRS and cardiac autonomic influences were found only after week 12. Four weeks of RSG treatment normalized blood glucose levels but not PE-BRS. All altered cardiovascular variables were completely restored by 12 weeks of RSG treatment. The correlation between BRS and blood glucose levels in saline-treated PDH rats was significant at week 12, but no correlation was found in RSG-treated rats. In conclusion, hyperglycemia, even in the prediabetic state, may play a role in BRS abnormalities. RSG treatment early in the prediabetic state may normalize BRS via cardiac autonomic modulation, besides its anti-hyperglycemic action.

Gynura divaricata (GD) also known as Dewa leaf is one of the Indonesian medicinal plants which are also native to India, Thailand and China. GD is widely used as antidiabetic, antihypertensive and other diseases including several tumors. However, research on the effects of GD as antidiabetic is still not widely practiced, especially in Indonesia. The aim of this research is to investigate the effect of GD ethanolic extract on blood glucose levels of alloxan-induced diabetes mice. This research is an experimental research with pre- and post-test design using 24 male DDY strain mice. Animals experiment is divided into four groups, that are one normal group and three alloxan-induced diabetes mice group that give 1% CMC (control), 400[Formula: see text]mg/kg GD ethanolic extract and 0.65[Formula: see text]mg/kg glibenclamide, respectively. Treatment was given for 14 days. Blood glucose levels were measured before and after inducing alloxan, and after 14 days treatment. At the end of study, all of animals experiment were sacrificed for histological examination. Phytochemical analysis revealed that polyphenols, flavonoids, steroids, triterpenoids, terpenoids and and quinones were present in ethanol extract of GD. The results of this study showed significant ([Formula: see text]) decrease of blood glucose levels after GD ethanolic extract and glibenclamide treatment. The results of histological examination showed improvement in pancreas damage and significant increase in the number of beta cells in GD and glibenclamide groups. The results indicate that dewa leaf ethanolic extract has antidiabetic activity and the effectiveness is the same as glibenclamide. GD also could repair damage of pancreas caused by alloxan induction.

Objective: Diabetes mellitus is a leading cause of death and disability in the world and its prevalence is predicted to rise to 10% by 2030. Hence, this study is conducted with objectives to find out the effect of aerobic exercises and resisted exercises on blood glucose levels in type 2 diabetes mellitus (T2DM) subjects and to compare the effect of both exercises on blood glucose level.Method: The comparative study was conducted at Krishna Institute of Medical Sciences Deemed University, Physiotherapy department, Karad. 30 participants with age group between 30 and 65 years were taken. Subjects were selected as per inclusion and exclusion criteria. Group A (15) participants were given aerobic exercise on static bicycle, and Group B (15) participants were given resistance training using dumbbells and weight cuffs for 5 days/week for 4 weeks. Diet recommendations were given to every participant.Results: Statistical analysis was performed using paired and unpaired t-test. Analysis showed statistically extremely significant difference in fasting blood glucose level and postprandial blood glucose level in both the groups (p≤0.0001).Conclusion: Thus, this study concludes that both aerobic exercises and resistance training prove to be beneficial in controlling blood glucose levels in T2DM subjects.

ObjectiveRecent studies have demonstrated that gut microbiota was closely related to metabolic disorders such as type 2 diabetes. Oral antidiabetic medications including metformin, acarbose and sitagliptin lowered blood glucose levels via acting on the gastrointestinal tract. The aim of the study was to observe the comparisons among those medications on gut microbiota composition.Research design and methodsZucker diabetic fatty rats (n=32) were randomly divided into four groups, and had respectively gastric administration of normal saline (control), metformin (215.15 mg/kg/day), acarbose (32.27 mg/kg/day), or sitagliptin (10.76 mg/kg/day) for 4 weeks. Blood glucose levels were measured during an intragastric starch tolerance test after the treatments. 16S rRNA gene sequencing was used to access the microbiota in the fecal samples.ResultsMetformin, acarbose, and sitagliptin monotherapy effectively decreased fasting and postprandial blood glucose levels (p<0.001). Acarbose group displayed specific cluster and enterotype mainly composed byRuminococcus 2whileLactobacilluswas the dominant bacterium in the enterotype of the other three groups. The relative abundance of generaRuminococcus 2andBifidobacteriumwas dramatically higher in acarbose group. Metformin and sitagliptin increased the relative abundance of genus Lactobacillus. Metagenomic prediction showed that the functional profiles of carbohydrate metabolism were enriched in acarbose group.ConclusionsMetformin, acarbose and sitagliptin exerted different effects on the composition of gut microbiota and selectively increased the beneficial bacteria. Supplementation with specific probiotics may further improve the hypoglycemic effects of the antidiabetic drugs.

1. The degree to which disruption by mastication affects the glycaemic response to four different carbohydrate foods was investigated in healthy human volunteers; each food was eaten by six subjects.2. Subjects ate meals of sweetcorn, white rice, diced apple or potato on two occasions; on one occasion they chewed the food thoroughly, on the other occasion they swallowed each mouthful without chewing it.3. When the foods were chewed the postprandial blood glucose levels rose to levels which vaned according to the food ingested.4. Swallowing without chewing reduced the glycaemic response to each food, achieving a similar effect as administration of viscous polysaccharides or ‘slow-release’ carbohydrates.
