Purpose. To evaluate the ability of frequency-doubling technology (FDT) perimetry in detecting visual field defects in young adults with type I diabetes prior to retinopathy or with minor retinovascular changes.Methods. This comparative cross-sectional study included 30 healthy subjects and 73 age-matched patients with type I diabetes mellitus. All subjects underwent a full ocular examination including an FDT with the threshold C-20-5 strategy. Only one eye per subject was randomly included in the statistical analysis. FDT results and time to perform the test were compared between the groups.Results. The mean age was 27.1 years in the control group and 26.6 years in the diabetic group (P=0.875). The mean period from the onset of diabetes was12.6±6.7years, while minimal retinovascular changes were observed in 18 eyes. Mean deviation of FDT did not differ between the groups. Although global indices of FDT were within normal limits, pattern standard deviation of FDT was higher in the diabetic group (P=0.035). The area under the receiver operating characteristic curve was 0.647 for pattern standard deviation of FDT (standard error = 0.052;P=0.017).Conclusion. FDT can detect retinal dysfunctions in diabetic patients prior to the onset of significant vascular complications.

Background. Type I diabetes (TΙDM) is a genetic or autoimmune disorder, which may be stimulated by induced immune system components due to the underlying infectious diseases. This study was undertaken to find out any possible association between Toxoplasma gondii infection and TIDM. Materials and Methods. One hundred and eighty-two blood samples were taken from individuals who were referred to outpatient clinics in Shiraz city, Southern Iran, during a 6-month period. The age of type I diabetic subjects ( n = 91 ) and the control group ( n = 91 ) was identical, which were less than 30 years. The sera were examined for IgG and IgM antibodies by ELISA and correlated with epidemiological factors such as age, sex, and family history of diabetes. Results. Out of 91 diabetic patients, 54 (59.3%) were female and 37 (40.7%) were male. The highest frequency of diabetes belonged to 6-10- and 11-15-year groups ( P = 0.17 ). Toxoplasma infection prevalence in diabetic and control groups was 28.6% and 7.7%, respectively ( P = 0.001 ). A significantly positive family history of diabetes was observed between diabetic patients (31 cases, 34.1%) and the control group (3 cases, 3.3%) ( P = 0.01 ). Interestingly, IgG positivity was seen in 13 cases (41.9%) of patients with positive family history of type I diabetes and 13 cases (21.7%) of subjects with no positive family history of type I diabetes ( P = 0.04 ). Conclusion. Our study showed a higher prevalence of Toxoplasma infection in type I diabetes patients. It is likely that the prevalence of TIDM decreases by increasing hygiene and preventing toxoplasmosis.

Background: Diabetes mellitus is associated with structural changes in corneal endothelial cells and their thickness. The present study was done to compare the endothelial cell density (ECD), central corneal thickness (CCT) and morphology in diabetic and non-diabetic patients.Methods: A cross-sectional study was conducted at Minto Ophthalmic hospital, BMC and RI Bangalore for a period of 20 months (October 2013 - May 2015). A total of 200 study subjects, 100 diabetics and 100 non-diabetic age matched controls were selected, and complete timed ophthalmic evaluation was performed. Specular microscopy was performed on all patients for endothelial cell count assessment and corneal thickness was measured by Pachymeter. The data was analyzed and represented using descriptive statistics. ‘t’ test was used for comparing the two groups.Results: The mean endothelial cell density in diabetic group was significantly lower (2438.73±250.23cells/mm2) compared to non-diabetic group (2599.88±168.16cells/mm2) (p<0.0001). The mean Central corneal thickness in diabetic group was significantly higher (518.40±28.13 μm) compared to control group (490.14±24.31 μm) (p<0.001). The Co-efficient of variation percentage of the diabetics was higher than the non-diabetics but this difference was not statistically significant (P>0.05). The hexagonality percentage was significantly lower in diabetic group compared to the controls suggesting less pleomorphism in the diabetic group.Conclusions: The study concludes that the endothelial cell density was lower and central corneal thickness was higher in diabetic patients compared with the non-diabetics. The altered endothelial morphology was significantly seen in the form of pleomorphism (hexagonality) but polymegathism was not significantly altered.

AbstractObjectivesReduced levels of α-Klotho is associated with the pathogenesis of various diseases including diabetes. In type I diabetes, decrease in Klotho leads to apoptosis of β-cells of pancreases. The aim of this study was to evaluate the levels of α-Klotho in type I diabetic pediatric patients.MethodsIn this cross-sectional single centered study, 46 patients presenting type I diabetes mellitus (case group) and 78 control group under the age of 12, referred to our clinic were included in our study. Serum levels of soluble Klotho were measured by sandwich ELISA in case and control groups. Statistical analysis was conducted for the data recorded via questionnaire.ResultsMean age of the patients in the case and control group was 7.65 ± 3.09 and 7 ± 2.37, respectively. Type I diabetes patients had a significant reduction in the levels of serum Klotho, as compared to controls (p<0.001). However, gender and age-based comparison between patient and control group was not significant.ConclusionsThis study reports a significant decrease in the serum levels of α-Klotho in type 1 diabetic patients. Low levels of Klotho can be associated with diabetic nephropathy and other comorbidities in these patients.

Introduction: The number of diabetes patients are increasing. If the blood glucose is not controlled in diabetics, patients will face with complications. Self-care is important to control the blood glucose of diabetic patients and prevention of complications such as neurological complications, heart, eye disease, amputations and so on. Therefore aim of this study is to identify the effect of Continuous blood Glucose Monitoring or CGM on the blood glucose control in type I diabetes.Materials and methods: The study was done on 20 patients chosen from an online data set available in http://www.jaeb-diabetes.net/ . These patients used CGM to test blood glucose and the result of three month test was measured. To analyze the data, the software SPSS 21, independent Mann- Whitney test was used.Results: The results showed that blood glucose in patients of the experimental group reduced significantly rather than patient of the control group in the second and third month. Also, HbA1C of the patients has reduced.Conclusion: The CGM has an effective role in controlling blood glucose and prevention of complications. Therefore it is recommended that educational programs on the use of monitoring blood glucose, such as CGM as a requirement to be developed.

Background: Although myocardial ischemia is known to be significantly related to the development of coronary collateral vessels (CCVs), there is considerable variation between patients with ischemic heart disease in the presence of collateral development. The nature of this variability is not well known. Likewise, it remains unclear whether diabetes mellitus (DM) has any effect on CCVs. The aim of this study was to evaluate the effect of DM on CCVs. Methods: A total of 100 patients who had a stenosis of >95% in any major coronary artery in angiograms were included in the study. Of these patients, 30 patients constitute the diabetic group. Remaining 70 patients were non-diabetic. For case-control matching, 30 non-diabetic patients (mean age, 52.03± 8.69 years) were selected randomly and were included in the control group. The CCVs were graded according to the Rentrop scoring system, and the collateral score was calculated by summing the Rentrop numbers of every patient. Results: There was no statistical difference between patients with and without DM in clinical baseline characteristics. The mean number of diseased vessels in the DM group (2.10±0.76) was higher than that in the nondiabetic group (1.63± 0.72 P.017). The mean collateral score was (1.13± 0.86) in the DM group and (1.97± 1.61) in the control group. After confounding variables were controlled for, the collateral score in the diabetic group was significantly different from that in the nondiabetic group (p=0.015). Conclusions: Our findings suggest that CCV development is poorer in patients with DM than in patients without DM. Thus, we can speculate that DM is an important factor affecting CCV development. Retinal vein occlusion (RVO) not infrequently occurs in diabetic patients. Although the aetiology is unclear, it could relate to the other microvascular complications of diabetes. In the non-diabetic, both the central (CRVO) and branch (BRVO) forms are commonly associated with hypertension and hyperlipidaemia. We have therefore studied fifty type II diabetic patients with RVO compared to a carefully matched diabetic control group (n = 50) to elucidate underlying medical conditions and hence the aetiology of RVO in diabetic patients. The two groups were well matched. Diabetics with RVO showed a strikingly high prevalence of hypertension compared to the controls (72% versus 32%: p < 0.001) and a trend to increased hyperlipidaemia (54% versus 36%). Diabetic microvascular complications were more common in the control group (diabetic retinopathy and proteinuria). No significant differences were observed in mean HbA1 or weight, but current smoking habits and blood pressure levels were increased in the diabetics with RVO. 80% of diabetic patients with the BRVO form, were hypertensive. We conclude that the main underlying medical conditions for RVO in diabetics are hypertension and hyperlipidaemia, and these may be important in the aetiology as in the non-diabetic. RVO is more common in type II rather than type I diabetes, and does not associate with the presence of diabetic microvascular complications. Clinical assessment for hypertension and hyperlipidaemia is therefore important in diabetic patients with RVO, especially if recurrence of the condition and further visual loss is to be prevented.

     Diabetes mellitus is a form of metabolic disorder where patients are incapable to organize glucose metabolism. The most common types are Type I and Type II, constituting about 10% and 90% of cases, respectively. The cause of type I diabetes, which usually spreads in children and adolescents, is the disability of the endocrine system to produce insulin. On the other hand, The most common type of diabetes,  type II diabetes, is often presented in adults. It is usually presented as a collection of insulin deficiency and insulin resistance. This work was done to estimate the count of microbiota in diabetics to find an appraoch for detection and follow-up treatment. The count of two types of bacteria Lactobacillus and Bifidobacterium was determined using qPCR based on the standard curve that was created from the serial decimal dilution of samples containing an unknown number of bacteria taken from probiotic capsules. The main results of this study show that the Lactobacillus count was affected by diabetes types, where a decrease was observed in the mean value in the case of diabetes type I group (32978.13) compared with the control group (610680.26). The mean value in diabetes type II was close to that of the control group (682199.27). While, the count of the Bifidobacterium showed a significant reduction in the mean value in both type I and type II diabetes groups (7521.70, 51880.82, respectively), compared with the control group (63405999.00).

Purpose To test the capability of rarebit perimetry (RP), a recent non-conventional perimetric technique, in detecting early functional damage in subjects with ocular hypertension (OHT) and to compare RP findings with those obtained by frequency-doubling technology (FDT) perimetry. Methods Thirty patients with OHT were matched with 30 healthy subjects. All were tested with RP and FDT. Frequency-doubling technology mean deviation (MD) and pattern standard deviation (PSD), as well as RP mean hit rate (MHR), of the two groups were analyzed. The agreement between the two techniques was tested by Kappa analysis. Results In the OHT group the mean (SD) FDT MD was 0.5 (2.1), the mean (SD) FDT PSD was 4.2 (1.6), and the mean (SD) RP MHR was 81.4 (6.7). In the control group, corresponding values were mean (SD) FDT MD 1.1 (1.4), mean (SD) FDT PSD 3.0 (0.3), mean (SD) RP MHR 96.2 (2.0). The differences between the two groups were not significant for the studied indexes. Eleven (36.6%) out of the 30 OHT eyes had abnormal RP results; 12 (40.0%) eyes had abnormal FDT results. Five (16.6%) eyes had abnormal RP and FDT findings. Only 1 eye (3.3%) in the control group had abnormal RP results and 3 eyes (10.0%) had abnormal FDT results. RP and FDT showed a moderate agreement (Kappa = 0.43; 95% CI: 0.42 to 0.51). Conclusions RP and FDT showed VF defects not shown in standard automated perimetry in the OHT group. This may be indicative of an increased risk in developing glaucoma, even if a gold standard for detecting subtle defects is not currently available. RP has the additional advantage of not requiring any expensive device to be used. The poor agreement between these techniques in identifying eyes with early damage warrants further investigations. Large longitudinal studies are needed before defining the role of RP in early glaucoma diagnosis.

Abstract Introduction: Diabetes mellitus has emerged as a major global health problem. Diagnosing and monitoring diabetes is the best way for its prevention, thus reducing the burden of disease. As saliva is easily available and accessible when compared with serum, salivary biomarkers have gained importance in recent years and, therefore, can be helpful in diagnosing the disease. Aims: The purpose of this study was to estimate the salivary amylase level in patients with types I and II diabetes mellitus and to correlate these findings with those of nondiabetic individuals in order to ascertain its value as a biochemical indicator for diagnosing and monitoring the patients. Materials and methods: Three groups of patients were selected for the present study. Group A: 20 nondiabetic, healthy individuals; group B: 20 type I diabetes mellitus patients; group C: 20 type II diabetes mellitus patients. Fasting and postprandial unstimulated saliva samples were collected and subjected for analysis of salivary amylase. Estimation of salivary amylase was determined by direct substrate method. Results: The mean fasting salivary amylase level for types I and II diabetic and nondiabetic individuals was 245.60, 239.10, and 149 U/dL respectively, whereas the mean postprandial salivary amylase was 257.35, 246.15, and 154.2 U/dL respectively. The mean value of variables was compared using Student's t test and one-way analysis of variance test. Conclusion: The mean salivary amylase level was significantly increased in both types I and II diabetic individuals as compared with healthy nondiabetic subjects (p < 0.05). However, there was no significant difference in the mean of types I and II diabetic patients.

The proteins β-thromboglobulin, platelet factor 4 and thrombospondin are stored in platelet α-granules and released from the platelet by the release reaction. The assays of these proteins were studied in patients with type I diabetes mellitus ( n = 30) and a healthy control group ( n = 15). Platelet factor 4 and β-thromboglobulin levels were not significantly different in both groups but thrombospondin concentrations in diabetic patients were significantly higher than those of the control group (136.6 ± 14.2 ng/ml vs 91.2 ± 14.3 ng/ml, P < 0.05). When the diabetic patients were divided into those with or without complications, the diabetic patients with complications ( n = 11) had significantly elevated plasma thrombospondin concentrations compared with the control group (150.4 ± 23.7 ng/ml vs 91.2 ± 14.3 ng/ml, P < 0.05), while thrombospondin concentrations in the control group were not statistically different from the diabetic patients without complications. Plasma β-thromboglobulin and platelet factor 4 levels were not significantly different between the diabetic and the control group. It is suggested that thrombospondin may be a convenient marker of in vivo platelet release reaction.
