Rationale and Objectives. To develop an optimal scanning protocol for multislice spiral CT perfusion (CTP) imaging to evaluate hemodynamic changes in liver cirrhosis with diethylnitrosamine- (DEN-) induced precancerous lesions.Materials and Methods. Male Wistar rats were randomly divided into the control group (n=80) and the precancerous liver cirrhosis group (n=40). The control group received saline injection and the liver cirrhosis group received 50 mg/kg DENi.p.twice a week for 12 weeks. All animals underwent plain CT scanning, CTP, and contrast-enhanced CT scanning. Scanning parameters were optimized by adjusting the diatrizoate concentration, the flow rate, and the delivery time. The hemodynamics of both groups was further compared using optimized multislice spiral CTP imaging.Results. High-quality CTP images were obtained with following parameters: 150 kV; 150 mAs; 5 mm thickness, 5 mm interval; pitch, 1; matrix,512×512; and FOV, 9.6 cm. Compared to the control group, the liver cirrhosis group had a significantly increased value of the hepatic arterial fraction and the hepatic artery perfusion (P<0.05) but significantly decreased hepatic portal perfusion and mean transit time (P<0.05).Conclusion. Multislice spiral CTP imaging can be used to evaluate the hemodynamic changes in the rat model of liver cirrhosis with precancerous lesions.

Background: The efficacy of color Doppler ultrasound, multislice spiral CT combined with serum alpha-fetoprotein (AFP) and alpha-fetoprotein heterogeneity (AFP-L3) in the diagnosis of primary hepatic carcinoma was evaluated. Methods: Seventy-nine patients with primary hepatic carcinoma (PHC group) and 50 patients with benign liver lesions (benign control group) admitted in Yantaishan Hospital (Yantai, China) from January 2016 to December 2018 were selected. The liver was scanned by color Doppler ultrasound and multiple multislice spiral CT. The serum AFP and AFP-L3 levels were detected by electrochemiluminescence. The value of color Doppler ultrasound, multislice spiral CT combined with serum AFP and AFP-L3 in diagnosis of primary liver cancer was retrospectively analyzed. Results: The color Doppler flow imaging (CDFI) showed a high-speed and high-resistance spectrum. The serum AFP and AFP-L3 levels of patients with primary hepatic carcinoma were significantly higher than those of the benign control group were (U=138.000 and 155.500, P=0.000 and 0.000), P<0.01. The sensitivity, accuracy and negative predictive value of color Doppler ultrasound, multislice spiral CT combined with serum AFP and AFP-L3 examinations for diagnosis of primary hepatic carcinoma were 96.20, 90.70 and 93.18%, which was significantly improved compared with each single examination (X2=27.888, 17.511 and 16.202, P=0.000, 0.002 and 0.003), P<0.01. Conclusion: Color Doppler ultrasound, multislice spiral CT combined with AFP and AFP-L3 examination could significantly improve the diagnosis efficiency of primary hepatic carcinoma, which was beneficial to early clinical diagnosis and early treatment.

Respiratory muscle function has a significant effect on stroke. Stroke is one of the most common cardiovascular and cerebrovascular diseases in the clinic and has a significant impact on the quality of life of patients. Hemiplegia, cerebral hemorrhage, and even death can occur, mainly in the elderly. In this paper, we meta-analyzed the effect of inspiratory muscle training on respiratory muscle function. In this article, we used a topic search method to search for relevant literature on respiratory muscle training and obtained 58 and 32 literature studies from CNKI and Wanfang Data, respectively. As a result of the screening, 36 and 28 documents were obtained. In this paper, 64 selected articles were studied. The authors make statistics on the literature of designing serum content index and multislice spiral CT (Member of the Society of Cardiological Technicians) image of patients, so as to analyze the influence of CT image and inspiratory muscle training on respiratory muscle function. The study showed that FVC, FEV1, MIP, and diaphragm mobility of the experimental group were significantly improved after treatment in more than 85% of the studies ( P < 0.05 ), while those of the control group were not significantly improved ( P > 0.05 ). The comparison between the two groups after treatment showed that FVC, FEV1, MIP, and diaphragm mobility of the experimental group were higher than those of the control group ( P < 0.05 ). The application of multislice spiral CT image analysis technology can effectively evaluate the effect of inspiratory muscle training on respiratory dysfunction in stroke patients, the mechanism of which regulates the expression of related pathways, suppresses the inflammatory response, and can reduce oxidative stress damage. Therefore, respiratory muscle training can improve the function of respiratory muscle and reduce the death rate of cerebellar hemorrhage in patients with stroke and other vascular diseases.

Abstract Background: Accumulation of cholesterol and depletion of calcium induce hepatic injury through the endoplasmic reticulum (ER) stress response. ER stress regulates the calcium imbalance between the ER and mitochondria. Previously, we reported that phosphatase of regenerating liver-1 (PRL-1)-overexpressing placenta-derived mesenchymal stem cells (PD-MSCsPRL-1) promoted liver regeneration through mitochondrial dynamics in a cirrhotic rat model. However, whether PRL-1 is involved in ER stress-dependent calcium is unclear. So, we demonstrated that PD-MSCsPRL-1 improves hepatic functions by regulating ER stress and calcium channels in a rat model of bile duct ligation (BDL). Methods: Liver cirrhosis was induced in Sprague-Dawley (SD) rats using surgically induced BDL for 10 days. PD-MSCs and PD-MSCsPRL-1 (2x106 cells) were intravenously administered to animals, and their therapeutic effects were analyzed. WB-F344 cells exposed to thapsigargin (TG) were cocultured with PD-MSCs or PD-MSCsPRL-1. Results: ER stress markers (e.g., eIF2α, ATF4, and CHOP) were increased in the non-transplantation group (NTx) compared with the control group. PD-MSCsPRL-1 significantly decreased ER stress markers compared to NTx and induced dynamic changes in calcium channel markers (e.g., SERCA2b, IP3R, MCU, and VDAC1) (*p<0.05). In TG-treated WB-F344 cells, cocultivation with PD-MSCsPRL-1 decreased cytosolic CaM expression and cytosolic and mitochondrial Ca2+ concentrations; however, the ER Ca2+ concentration was increased compared to that in PD-MSCs (*p<0.05). Additionally, PRL-1 through epidermal growth factor receptor (EGFR) activated phosphatidylinositol-3-kinase (PI3K) signaling, resulting in calcium increases via CaM expression. Conclusions: These findings suggest that PD-MSCsPRL-1 improved hepatic functions through calcium changes and attenuated ER stress in a BDL-injured rat model. Therefore, these results provide useful data for the development of next-generation MSC-based stem cell therapy for regenerative medicine in chronic liver disease.

To investigate the differences in the diagnosis and treatment of occult fractures between multi-slice spiral CT (CT) and magnetic resonance imaging (MRI) using I/Fe3O4 nanometer contrast agent. This retrospective study analyzed the clinical data of 60 patients with occult fractures and compared the diagnosis results of multislice CT (observation group) and MRI (control group). All the contrast agents used were I/Fe3O4 nanometer contrast agents. In the control group, 55 cases (91.67%) were detected on T1-weighted imaging versus 58 cases (96.67) on T2-weighted imaging. In the observation group, 47 cases (78.33%) were diagnosed. The intergroup difference was significant (P < 0.05). Clinically, I/Fe3O4 nanometer contrast agent can be used to affect imaging. The detection rate of MRI is higher than that of CT, and the ability to obtain detailed data of occult fractures is better.

The objective is to study the application of spiral CT in the diagnosis of the trachea in children. In this study, the effect of 64-slice multislice spiral CT in diagnosing infant bronchial bridge was studied based on an artificial neural network. From June 2020 to December 2020, 100 cases of children with the trachea in the outpatient department of our hospital were selected as the research object. They were divided into the study group and the control group, with 50 cases in each group. The results showed that among the 50 cases in the control group, 42 cases were found to have a bronchial foreign body and 8 cases were missed; the detection rate was 84%. There were 7 cases of trachea foreign body, 15 cases of left bronchial foreign body, 14 cases of right bronchial foreign body, and 6 cases of medium bronchial foreign body. The detection rate of the study group was significantly higher than that of the control group, with a statistical significance ( P < 0.05 ). Conclusion. The detection rate of neural networks in MSCT is higher than that of X-ray, and the MSCT based on the artificial neural network can clearly show the morphology, position, and the relationship between the foreign body and trachea, which is worthy of clinical promotion and application.

Background. Experimental research in hepatology has focused on developing traditional medicines into potential pharmacological solutions aimed at protecting liver from cirrhosis. Along the same line, this study investigated the effects of ethanol-based extract from a traditional medicine plantBoesenbergia rotunda(BR) on liver cirrhosis.Methodology/Results. The BR extract was tested for toxicity on 3 groups of rats subjected to vehicle (10% Tween 20, 5 mL/kg) and 2g/kg and 5g/kg doses of the extract, respectively. Next, experiments were conducted on a rat model of cirrhosis induced by thioacetamide injection. The rats were divided into five groups and, respectively, administered orally with 10% Tween-20 (5 mL/kg) (normal control group), 10% Tween-20 (5 mL/kg) (cirrhosis control group), 50 mg/kg of silymarin (reference control group), and 250 mg/kg and 500 mg/kg of BR extract (experimental groups) daily for 8 weeks. The rats in normal group were intraperitoneally injected with sterile distilled water (1 mL/kg) 3 times/week, and those in the remaining groups were injected intraperitoneally with thioacetamide (200 mg/kg) thrice weekly. At the end of the 8 weeks, the animals were sacrificed and samples were collected for comprehensive histopathological, coagulation profile and biochemical evaluations. Also, the antioxidant activity of the BR extract was determined and compared with that of silymarin. Data from the acute toxicity tests showed that the extract was safe to use. Histological analysis of the livers of the rats in cirrhosis control group revealed uniform coarse granules on their surfaces, hepatocytic necrosis, and lymphocytes infiltration. But, the surfaces morphologically looked much smoother and the cell damage was much lesser in those livers from the normal control, silymarin and BR-treated groups. In the high-dose BR treatment group, the livers of the rats exhibited nearly normal looking lobular architecture, minimal inflammation, and minimal hepatocyte damage, the levels of the serum biomarkers and liver enzymes read nearly normal, and these results were all comparable to those observed or quantified from the normal and silymarin-treated groups. The BR extract had the antioxidant activity about half of what was recorded for silymarin.Conclusion. The progression of the liver cirrhosis can be intervened using the ethanol-based BR extract, and the liver’s status quo of property, structure, and function can be preserved. This capability of the extract warrants further studies exploring the significance of its pharmacologic potential in successfully treating the liver cirrhosis in humans.

Absztrakt A szerzők a világirodalmi ritkaságnak számító atraumaticus (spontán) mellkasfali tüdősérvet ismertetik. Két operált betegük kapcsán bemutatják a sérv kialakulását elősegítő körülményeket és a kórkép klinikai jellemzőit. Mindkét betegük spontán mellkasfali (intercostalis) sérvét chronicus obstructiv syndroma (COPD) okozta makacs köhögés váltotta ki. Az elvégzett multislice spirál CT (MSCT) vizsgálat, valamint a speciális szoftver segítségével készített másodlagos 3D „volume-rendering” (VRT) rekonstrukciós képek egyértelműen utaltak a ritka kórformára. Az MSCT-vizsgálatok a Kaposi Mór Oktató Kórházban készültek, Siemens Somatom Emotion 6 MSCT-berendezéssel. A pontos diagnózis birtokában végzett mellkasfali korrekciók tartós gyógyuláshoz vezettek. A szerzők röviden ismertetik a mellkasfali sérvekkel kapcsolatos hazai és fontosabb külföldi irodalmi vonatkozásokat. A kórkép rendkívüli ritkasága ellenére a kialakulásában szerepet játszó COPD elterjedtsége miatt érdemel figyelmet. A hasonló esetek diagnosztikájában nagy segítséget jelenthet a bemutatott korszerű képalkotó eljárás alkalmazása.

Introduction: For over 30 years, ascites has been postulated to facilitate fibrinolysis in patients with liver cirrhosis. In contrast to previous research employing conventional coagulation tests, this study aimed to characterize hemostatic interactions between blood and ascites using the rotational thromboelastometry (ROTEM). Methods: Blood samples – pure or mixed with ascites in a ratio of 1:1 – from cirrhotic patients (n = 10) were subjected to ROTEM analysis. In addition, a negative control group was built with cirrhotic patients (n = 10) whose blood was mixed with physiologic sodium chloride (0.9% NaCl) solution in a ratio of 1:1. Subsequently, ROTEM measurements were subjected to statistical analysis. Results: During ascites challenge, clotting time (CT, measured in seconds) was significantly prolonged in EXTEM (blood: 70.40 ± 20.40 vs. ascites/blood: 109.8 ± 47.7) and APTEM (blood: 66.50 ± 14.55 vs. ascites/blood: 138.7 ± 105.8), likely reflecting a dilution effect. However, CT in INTEM remained unchanged, suggesting a sustained intrinsic pathway function. Maximal clot firmness (measured in millimeters) in FIBTEM decreased significantly (blood: 14.70 ± 9.55 vs. ascites/blood: 6.00 ± 5.66), thus indicating depletion of fibrinogen in ascites. Strikingly, maximum lysis (measured in %) significantly decreased in EXTEM (blood: 9.30 ± 2.79 vs. ascites/blood: 5.50 ± 2.84), APTEM (blood: 8.50 ± 3.10 vs. ascites/blood: 5.60 ± 2.88), and INTEM (blood: 7.50 ± 2.27 vs. ascites/blood: 5.10 ± 3.48). Conclusions: ROTEM provided new evidence that ascites may not primarily induce fibrinolysis in cirrhotic patients. This finding seems to be of significant importance for the clinical management of cirrhotic patients experiencing complications, for example, abdominal hemorrhage after liver biopsy or paracentesis; here, replacement of prothrombin complex concentrates and/or fibrinogen concentrates may prove more beneficial than the use of fresh frozen plasma or antifibrinolytic drugs.

Granulocyte colony-stimulating factor (G-CSF) was shown to promote bone regeneration and mobilization of vascular and osteogenic progenitor cells. In this study, we investigated the effects of a systemic low dose of G-CSF on both bone consolidation and mobilization of hematopoietic stem/progenitor cells (HSPCs), endothelial progenitor cells (EPCs) and mesenchymal stromal cells (MSCs) in a rat model of distraction osteogenesis (DO). Neovascularization and mineralization were longitudinally monitored using positron emission tomography and planar scintigraphy. Histological analysis was performed and the number of circulating HSPCs, EPCs and MSCs was studied by flow cytometry. Contrary to control group, in the early phase of consolidation, a bony bridge with lower osteoclast activity and a trend of an increase in osteoblast activity were observed in the distracted callus in the G-CSF group, whereas, at the late phase of consolidation, a significantly lower neovascularization was observed. While no difference was observed in the number of circulating EPCs between control and G-CSF groups, the number of MSCs was significantly lower at the end of the latency phase and that of HSPCs was significantly higher 4 days after the bone lengthening. Our results indicate that G-CSF accelerates bone regeneration and modulates mobilization of progenitor cells during DO.
