Objective. First bite syndrome, characterized by pain in the parotid region after the first bite of each meal, predominantly develops in patients who have had head and neck surgery. Idiopathic parotid pain (IPP) that mimics first bite syndrome may present in patients without a history of surgery or evidence of an underlying tumor, but its clinical features are unclear. This study characterized the clinical characteristics of IPP in patients with diabetes. Study Design. A retrospective case review involving the clinical findings and pain characteristics of nine patients with IPP and diabetes who presented to our department between 2013 and 2016. Results. All the patients were men diagnosed with type 2 diabetes (median age, 43 years). IPP developed unilaterally in seven patients and bilaterally in two. The median intensity of the first bite pain was 8 on a numerical rating scale of 0–10. The trigger factor was gustatory stimuli, and the trigger area was the posterior section of the tongue. Postprandial pain occurred within 1–10 min after meals in six patients. Conclusions. IPP may be considered a separate disorder, in which the pain characteristics are similar to those of first bite syndrome but the clinical features and pathophysiology are different.

Penelitian ini bertujuan untuk menghasilkan model latihan relaksasi gabungan dari latihan Yoga, Tai Chi, dan Makko Ho (YOCHIHO) bagi penderita diabetes mellitus tipe 2. Diharapkan latihan relaksasi ini dapat menaikkan relaksasi otot, memperbaiki kapasitas fisik, dan mengelola kadar glukosa darah pada penderita diabetes mellitus tipe 2. Penelitian pengembangan ini mengadaptasi langkah-langkah Borg & Gall, sebagai berikut: (1) studi pendahuluan, (2) perencanaan, (3) mengembangkan produk awal (draf produk), (4) validasi ahli, (5) revisi produk I, (6) uji coba skala kecil, (7) revisi produk II, (8) uji coba skala besar, (9) revisi produk final, dan (10) Produk hasil pengembangan. Dalam uji skala kecil (uji keberterimaan) menggunakan metode survei dan uji skala besar (uji kemanfaatan) dengan metode eksperimen menggunakan rancangan one-group pretest-posttes design. Perlakuan latihan relaksasi YOCHIHO dilakukan 2 kali seminggu selama 6 minggu. Data dianalisis secara deskriptif dan inferensial. Data ordinal (derajat relaksasi otot) dianalisis secara non parametrik mengguna-kan kruskal-wallis test, dilanjutkan dengan U-mann-whitney test, sedangkan data rasio (denyut jantung istirahat, frekuensi nafas, tekanan darah, dan glukosa) dianalisis secara parametrik dengan uji ANOVA, dilanjutkan dengan uji LSD (Least Square Different) dengan taraf signifikansi 0,05. Hasil penelitian berupa model latihan relaksasi YOCHIHO yang sudah divalidasi oleh 3 ahli, terdiri dari 9 gerakan pemanasan, 25 gerakan inti, dan 7 gerakan pendinginan. Latihan relaksasi YOCHIHO dapat diterima oleh penderita diabetes mellitus tipe 2 dan teruji kemanfaatannya untuk meningkatkan relaksasi otot (Numerical Rating Scale), memperbaiki kapasitas fisik (pulse rate, dan tensi meter), dan menurunkan kadar glukosa darah (Easy Touch GCU dan glucose test strips).   DEVELOPING YOCHIHO RELAXATION TRAINING MODEL FOR PEOPLE WITH TYPE 2 DIABETES MELLITUS Abstract This research is aimed to produce combination relaxation training model of yoga, tai chi, and Makko ho training (YOCHIHO) for people with type 2 diabetes mellitus. This training model is expec-ted to increase muscle relaxation, improve physical capacity, and manage glucose level. The develop-ment of research was adapting the steps as follows: (1) preliminary study, (2) plans, (3) developing initial products (draft product), (4) validation expert, (5) revising products I, (6) smaller-scale trials, (7) revising product II, (8) larger-scale trials, (9) revision of the final product, and (10) products of development. In smaller-scale trials (acceptance test) used survey methods, and larger-scale trials (usefulness test) with the experimental method used the design of one-group pretest-posttest design. YOCHIHO relaxation training was done 2 times a week for 6 weeks. Data were analyzed by descrip-tive and inferential statistics. Ordinal data (degree of muscle relaxation) were analyzed by non-para-metrical using Kruskal-Wallis test, continued with U-mann-Whitney test, while the ratio data (resting heart rate, breath frequency, blood pressure, and glucose) were analyzed by parametric ANOVA test and then continued with LSD (Least Square Different) with a significance level of 0.05. The results of this research indicate that YOCHIHO relaxation training-that have been validated by 3 experts, con-sists of 9 warming-up movements, 25 core movements, and 7 cooling down movements. YOCHIHO re-laxation training can be receive by people with type 2 diabetes mellitus and was tested for usefulness to increase muscle relaxation (Numerical Rating Scale), improves physical capacity (pulse rate and device to measure blood pressure), and lower glucose levels (Easy Touch GCU and glucose test strips). Background: The classic phenotype of CLN2 disease (neuronal ceroid lipofuscinosis type 2) typically manifests between ages 2 and 4 years with a predictable clinical course marked by epilepsy, language developmental delay, and rapid psychomotor decline. Atypical phenotypes exhibit variable time of onset, symptomatology, and/or progression. Intracerebroventricular-administered cerliponase alfa (rhTPP1 enzyme) has been shown to stabilize motor and language function loss in patients with classic CLN2 disease, but its impact on individuals with atypical phenotypes has not been described. Methods: A chart review was conducted of 14 patients (8 male, 6 female) with atypical CLN2 phenotypes who received cerliponase alfa. Pre- and posttreatment CLN2 Clinical Rating Scale Motor and Language (ML) domain scores were compared. Results: Median age at first presenting symptom was 5.9 years. First reported symptoms were language abnormalities (6 [43%] patients), seizures (4 [29%]), ataxia/language abnormalities (3 [21%]), and ataxia alone (1 [7%]). Median age at diagnosis was 10.8 years. ML score declined before treatment in 13 (93%) patients. Median age at treatment initiation was 11.7 years; treatment duration ranged from 11 to 58 months. From treatment start, ML score remained stable in 11 patients (treatment duration 11-43 months), improved 1 point in 1 patient after 13 months, and declined 1 point in 2 patients after 15 and 58 months, respectively. There were 13 device-related infections in 8 patients (57%) and 10 hypersensitivity reactions in 6 (43%). Conclusions: Cerliponase alfa is well tolerated and has the potential to stabilize motor and language function in patients with atypical phenotypes of CLN2 disease.

The aim of this study was to evaluate the incidence, clinical features, and risk factors of sustaining inferior rectus (IR) palsy in a group of pediatric patients with orbital floor blowout fractures. We performed a retrospective case review of sequential cases of pediatric orbital floor blowout fractures (<18 years old) from 2000 to 2013 in a tertiary ophthalmic center in Singapore. A total of 48 patients were included in our study, of whom 5 had IR palsy (10.4%). Patients with IR palsy had a higher mean age (16.4 ± 1.5 years) compared with patients without IR palsy (12.4 ± 3.3 years), had significantly ( p < 0.05) worse preoperative motility, and had significantly greater proportion developing postoperative hypertropia (100%) compared with patients without IR palsy (4.7%). Our series of pediatric blowout fractures demonstrated IR palsy prevalence and clinical features for IR palsy which may be distinct to the pediatric group.

The study aimed to determine the predictive capabilities of the Gastrointestinal Symptom Rating Scale (GSRS) for assessing the effectiveness of eradication therapy in Helicobacter pylori (HP) infected patients with type 2 diabetes mellitus. The study involved 60 patients of both sexes with a diagnosis of type 2 diabetes mellitus and chronic HP-associated gastritis, the average age of patients was 61.9 years, average duration of type 2 diabetes was 10.1 years. For statistical analysis of the data, we used the method of constructing ROC curves, which allowed us to identify prognostic factors (in this case, gastroenterological syndromes, such as abdominal pain, refl ux, indigestion, diarrhea and constipation) that influence the effectiveness of eradication therapy in patients with type 2 diabetes. It was found that high values of refl ux and indigestion syndromes scales of the GSRS are associated with the probability of low eradication therapy response in patients with type 2 diabetes, i.e. these syndromes can act as complicating factors for the elimination of Helicobacter pylori. The obtained data suggest the need for screening diagnostics of Helicobacter pylori, clinical manifestations of gastrointestinal pathology and subsequent etiological and pathogenetic treatment.
