High-Level Mupirocin Resistance in Staphylococcus spp. among Health Care Workers in a Tertiary Care Hospital

Background: Nasal carriage of Staphylococcus aureus is very common among health care workers, and treatment with mupirocin is one of the choicest antibiotics available. But with the rampant usage of mupirocin like other antibiotics, the emergence of mupirocin resistance is also on rise. This resistance is both low level as well as high level among the isolated strains. Aim: To screen for the high-level mupirocin resistance among the isolated Staphylococcus strains by Kirby Bauer disk diffusion method. Materials and Methods: A total of 200 clinical isolates were tested for high level mupirocin resistance by disk diffusion method using Himedia disks. Results: Among the 200 nasal swabs, 26 (13%) showed growth of S. aureus, whereas 174 (87%) showed the growth of coagulase negative staphylococcus (CONS) spp. Mupirocin resistance was observed only among CONS spp, which was 15% for low-level mupirocin and 8% for high-level mupirocin resistance. No mupirocin resistance was observed among the Staphylococcus spp. Conclusion: The identification of Mupirocin resistance will guide us to utilize the antibiotic in a judicious way to treat the nasal carriage effectively.

Staphylococcus aureus is one of the common causes of Healthcare-associated infection. Staphylococcus colonizes the anterior nares of the nose and tends to disseminate and secondarily colonize several other body sites including the skin and the gut. Colonized hospital personnel may be an important factor in dissemination. Staphylococcus aureus to patients and vice-versa. Mupirocin is an excellent topical anti-staphylococcal antimicrobial agent used for eradicating nasal carriage. Resistance to Mupirocin is a threat for future use of this drug in eliminating nasal carriage of Staphylococcus aureus. Thus, this study was conducted to determine the rate of Mupirocin resistance among Staphylococcus aureus isolated from nasal swabs of Health care workers (HCWs ) of Operation Theatres (OTs) and Intensive Care Units (ICUs). A single nasal swab was collected from both the anterior nares of participating health care workers of ICU and OT once at the end of their shift. Antibiotic susceptibility testing of Staphylococcus aureus to various antibiotics was done by Kirby-Bauer disk diffusion method using CLSI guidelines. High and low-level Mupirocin resistance was determined. Among 282 nasal swabs collected, Staphylococcus aureus was isolated in 62 samples. Of Staphylococcus aureus 19 came out to be Methicillin-resistant (MRSA) and the remaining 43 Methicillin sensitive (MSSA). Mupirocin resistance was seen in 3 MRSA strains and 1 MSSA strain. Thus, overall 4/62 (6.5%) strains were MupR strains. Mupirocin is the most effective antibiotic used against colonization of Staphylococcus aureus in anterior nares. Resistance to this antibiotic is thus an alarm as well as a matter of great concern. Necessary steps, policies and guidelines need to be framed to stop the spread of this resistance.

Aims: The emergence of COVID-19 disease has created significant stress and anxiety for health care workers. This study aimed to investigate the stressors and coping strategies in the staff of Allameh Bohlool Hospital in Gonabad City, Iran, during the outbreak of the COVID-19 disease. Methods & Materials: This cross-sectional study was performed on 252 employees of Allameh Bohlool Hospital in Gonabad from March 2020 to April 2020. Study tools included a brief form of coping styles (Brief-COPE) and a researcher-made questionnaire of stressors due to the emergence of COVID-19 among health care workers. The obtained data were analyzed using linear regression and ordinal regression models at the significance level of 0.05. Findings: About 74.2%, 69.4%, 52.7%, 52.7%, and 99.2% of the hospital staff had moderate to high stress in the domains of internal, family-social, workplace-related, infection control, and government measures, respectively. The degree of using adaptive and maladaptive coping styles used by staff were 52.0% and 23.8% at the moderate to the high level, respectively. Maladaptive coping styles had a positive and significant relationship with stress intensity so that for each unit increase in maladaptive coping score, the odds of experiencing higher levels of stress increased 1.24 to 1.45 times (P<0.001). Also, the odds of experiencing higher levels of stress in those who had sports activities was 24% to 76% lower (P=0.003). Conclusion: The findings of the present study indicated a high level of stress among hospital staff. Also, the use of maladaptive coping styles and sports activities had a significant positive and negative relationship with stress intensity, respectively. Therefore, designing effective interventions focusing on reducing maladaptive coping patterns among hospital staff and encouraging people to engage in sports activities can help manage stress as much as possible due to the outbreak of COVID-19 disease.

Background: Methicillin-resistant Staphylococcus aureus (MRSA) colonization is considered a major risk factor for nosocomial infections and its decolonization has reduced these infections. Mupirocin (MUP) is the topical antibiotic of choice for decolonization. MUP decolonization failure is attributed to MUP resistance. Objective: The aim of the current study is to assess MUP resistance among MRSA isolates phenotypically and genotypically. Methodology: Fifty MRSA isolates were identified in Microbiology Department in the Medical Research Institute hospital, Alexandria University. Antibiotic susceptibility to different classes of antibiotics by disk diffusion method was done. MUP minimum inhibitory concentration (MIC) was determined phenotypically by MUP Ezy MIC™ Strips. MUP resistance was determined genetically by multiplex PCR detection of mupA and mupB. Results: Of all MRSA isolates, 6% exhibited high level and none showed low level MUP resistance. Only mupA was detected in all resistant isolates. Conclusion: Despite low prevalence of MUP resistance, it is appropriate to test MUP resistance prior nasal decolonization

SUMMARYCase reports have indicated transmission ofStaphylococcus aureusbetween humans and pets. We investigated associations between level of contact between dog and owner, andS. aureuscolonization. In a cross-sectional study, nasal carriage and antibiotic susceptibility ofS. aureuswas determined for 830 dogs and 736 owners. Relatedness of isolates was investigated using antibiograms and pulsed-field gel electrophoresis (PFGE). Associations between carriage and demographics or amount of contact between owners and dogs were documented.S. aureuswas isolated in 24% of humans and 8·8% of dogs. Antibiotic resistance was significantly more common in canine isolates. Of 17 owner/dog colonized pairs, six were indistinguishable by PFGE. Colonization of dogs was not associated with close human contact, but was strongly associated with health-care occupations (OR 3·29, 95% CI 1·49–7·26,P=0·002). In outbreak situations health-care workers' pets should be considered as a source ofS. aureus. High rates of resistance indicate increased monitoring of antibiotic use in veterinary practice is needed.

Abstract Background: Hepatitis B Virus is a DNA virus of the hepadnaviridae family of viruses. It replicates within infected liver cells (hepatocytes) and may cause acute and chronic hepatitis.The hepatitis B vaccine is the mainstay of hepatitis B prevention. Since 1982, safe and effective hepatitis B virus vaccines have been commercially available. the aim of this study was to assess knowledge towards Hepatitis B Virus among health care workers in relation to their vaccination status in private clinics of Addis ketema sub-city, Addis Ababa, Ethiopia. Methods(design): A cross-sectional studywas conducted and multi-stage sampling technique was used and in total 231 healthcareworkers were enrolled in the study, which was conducted in private clinics of Addisketema sub-city. self-administered questionnaires were distributed to the health care workers. The collected data was entered intoEpi-data and analyzed using SPSS 25. Results: Two hundred thirty one healthcare workers completed and returned the questionnaires giving an overall response rate of 97%.Only 66 (28.6%) respondents reported that they received one or more doses of Hepatitis B vaccine. From these, 43 (65.2%) received three doses which was only 18.6% of the total Workers.participants with poor and moderate knowledge had equal proportion 92(39.8%) ,while only 47 (20.3%) were found to have high-level of knowledge .Having high-level of knowledge was not a statistically significant predictor of vaccination uptake. Conclusions: Hepatitis B Virus vaccination coverage and knowledge were poor among health care workers. Therefore: educational intervention and provision of vaccinationis recommended for these vulnerable workers of Addis-ketema sub city, Addis Ababa, Ethiopia.
