In most printing factories, the stitching machine is considered as a significant tool in accomplishing the printing process cycle, such as in the Printing House of the University of Kufa (PHUK), complete their jobs using a cheap manual machine, and thus this leads to an increase in the number of employees and work hours. That is because the automated stitching machine of production is very costly. A decent printing house design maximizes production with a minimum investment in new equipment parts. However, a decent PHUK layout alone cannot reach the intended aims unless firmly linked with a developed production line of an automated stitching machine for the purpose of reducing cost, time, and efforts. This article focused on designing and developing automatic saddle stitching machines for folded paper sheet products such as newspapers, magazines, catalogs, exam sheets, etc. using accommodate devices such as Arduino and infrared sensors. Furthermore, the proposed design is applied in PHUK successfully and it showed that the cost of the stitching machine and the manpower is reduced by 60 percent, also the time is reduced by 70 percent. Finally, one of the significant implications of this work is using IT in management of resources.

Abstract:The development of an innovative clinical decision-support project such as the University of Minnesota’s Clinical Workstation initiative mandates the use of modern client-server network architectures. Preexisting conventional laboratory information systems (LIS) cannot be quickly replaced with client-server equivalents because of the cost and relative unavailability of such systems. Thus, embedding strategies that effectively integrate legacy information systems are needed. Our strategy led to the adoption of a multi-layered connection architecture that provides a data feed from our existing LIS to a new network-based relational database management system. By careful design, we maximize the use of open standards in our layered connection structure to provide data, requisition, or event messaging in several formats. Each layer is optimized to provide needed services to existing hospital clients and is well positioned to support future hospital network clients.

IntroductionTreatment resistance to clozapine is estimated at 40–70% of the treated population. Several clozapine potentiation strategies have come into clinical practice although often without evidence-based support.ObjectiveThe aim of our work was to identify the potentiation strategies in ultra-resistant schizophrenia depending on the subtype of schizophrenia.MethodologyThis is a prospective study conducted on patients with the diagnosis of schizophrenia, based on DSM-IV-TR criteria, and hospitalized in the psychiatric department of the university hospital in Mahdia, Tunisia. The study sample consisted of patients meeting the resistant schizophrenia criteria as defined by national institute for clinical excellence (NICE), and the prescription of clozapine for 6 to 8 weeks was shown without significant improvement.Resultswe have collected 10 patients. The mean serum level of clozapine was 462.25 mg/L. The potentiation strategies were different depending on the subtype of schizophrenia. For the undifferentiated schizophrenia, we have chosen ECT sessions. For the disorganized schizophrenia, we opted for amisulpiride and aripiprazole. For the paranoid forms, we have chosen the association of risperidone and ECT. A psychometric improvement was noted in BPRS ranging from 34 to 40%.ConclusionEvery potentiation strategy entails a cost, whether it is an additional monetary cost, adverse effects or greater stress to caregivers. Currently embedded system designs aim to improve areas such as speed, energy efficiency and the cost of an application. Application-specific instruction set extensions on reconfigurable hardware provide such opportunities. The article presents a new approach for generating soft core processors that are optimized for specific tasks. In this work, we describe an automatic method for selecting custom instructions for generating software core processors that are based on the machine code of the application program. As the result, a soft core processor will contain the logic that is absolutely necessary. This solution requires fewer gates to be synthesized in the field programmable gate arrays (FPGA) and has a potential to increase the speed of the information processing that is performed by the system in the target FPGA. Experiments have confirmed the correct operation of the method that was used. After the reduction mechanism was enabled, the total number of slices blocks that were occupied decreased to 47% of its initial value in the best case for the Xilinx Spartan3 (xc3s200) and the maximum frequency increased approximately 44% in the best case for Xilinx Spartan6 (xc6slx4).

This paper is based on a research report published at the University of Queensland in November 1980, which emanated from research commissioned by the University's Research Committee and carried out by the authors. The study was concerned with the problem of distributing resources available for research and concluded that there was not an efficient use of such resources in the University of Queensland. Part of the study considered attempts to increase efficiency by funding those research projects which seemed to possess most merit. Such policy is becoming more common in Australian universities and this is understandable during a period of financial stringency. However, the policy seems to ignore the substantial costs associated with applying for merit grants, and to assume that any scheme funding the most deserving research automatically improves the efficiency of research funding. That is not necessarily so. Most research funding in Australian universities is provided in the form of staff salaries. When staff time is occupied by the merit application and assessment process, it is not available for research. Consequently there is a cost to research, a cost that is not widely appreciated and one which may well exceed the benefits of ill-considered merit schemes.

Femoral fractures increase the length of hospital stay for our patients for several reasons such as lack of blood, economic resources, and lack of infrastructure. The use of a C-arm has been shown to reduce patient morbidity due to early functional recovery and reduced hospital stay. Objective: To develop an intramedullary nailing technique without c-arm with a closed focus to reduce the duration of hospitalization of its patients as well as the cost related to the equipment used for follow-up. Methodology: prospective study on 35 patients for 1 year August 2020 to August 2021 Results: We followed 35 patients in which the mean age was 37.83 years with extremes of 18 and 78 years. The male sex predominates 21 against 14 women or 60% against 40% respectively. The sex ratio is 1.5. A total of 19 diaphyseal fractures (54.3%) were nailed, 9 supracondylar (25.7%) and 7 subtrochanteric (20%). Twenty-seven were closed fractures (71.1%), and 8 were open fractures (22.9%). The length of hospitalization was less than 3 days for 30 patients (85.7%), and more than 3 days for 5 patients (14.3%). Conclusion: We recommend that we promote this closed-hearth technique because it improves the postoperative follow-up of patients. Additionally, it would reduce exposure to radiation from c-arm in hospitals that have this equipment.

The Department of History, Faculty of Cultural Sciences, the University of Gadjah Mada in 2015 made a milestone in the development of historiography in Indonesia. They made a bold move to produce a scholar with a documentary film work instead of a thesis. In the future, it is not impossible that this step will soon be followed by other universities in Indonesia. This paper was written in response to these developments. In this digital era-and in the midst of still low interest in reading in Indonesia-emerged the discourse to seek new media for historiography in Indonesia. The film, especially documentary films are seen as new media that match the characteristics of history because of they both present real-life reality. Moreover, Indonesia with the diversity of tribes and culture and history, of course, save a variety of themes that will not run out to be appointed a documentary. Based on that, this paper will discuss the types, forms, and format of the documentary that is suitable and possible to be produced by history students as a substitute for thesis-considering the cost of film production tends to be higher than thesis research. Abstract Background In July 2017, The University of Chicago Comer Children’s Hospital Emergency Department (ED) transitioned from a 5-day to a 28-day HIV nPEP (non-occupational post-exposure prophylaxis) dispensation model in an effort to increase adherence. Anecdotal reports of patients lost to follow-up after ED discharge called into question the utility and cost-effectiveness of this practice. We analyzed HIV nPEP follow-up rates in our clinic, explored reasons for nonadherence, and performed basic cost-savings analyses to inform potential changes to our dispensation model. Methods A retrospective review of both electronic health and pharmacy records was conducted for patients prescribed 28-days of HIV nPEP in the ED and scheduled for outpatient follow-up in Pediatric ID clinic from July 2017-June 2019. Clinic provider documentation of nPEP adherence and reasons for nonadherence were examined. Patients were given an initial dose of nPEP regimen in the ED and provided all subsequent doses to complete at home. Using average wholesale price (AWP), we calculated the total cost of each regimen and potential savings if a shorter duration of HIV nPEP supply was dispensed. Results 50 patients received a 28-day supply of HIV nPEP. Please refer to Table 1 regarding baseline patient characteristics. Of these, only 19 (38%) patients had documented outpatient follow-up after nPEP initiation. Median time to follow-up was 6 days (IQR: 3.0-9.0 days). Of the 19 patients with follow-up, 3 admitted to medication non-adherence. Although side effects were elicited in a total of 9 patients (18%), only 1 cited medication intolerance as the reason for discontinuing their nPEP. Given the relatively short time to follow-up, a potential savings of $1720-2211/patient could be achieved if a 10-14 day supply was dispensed. Conclusion Outpatient follow-up after 28-day HIV nPEP dispensation in our ED was &lt; 40%, calling into question the cost-effectiveness of this dispensation model. While our current practice alleviates nPEP interruption due to potential insurance issues and pick-up delays, follow-up and adherence are not assured. The significant cost-savings with a shorter supply at the outset may encourage more robust follow-up and adherence. Disclosures All Authors: No reported disclosures

The paper presents the issues of higher education institutions research activities efficiency and financing. Empirical data on the Russian higher education institutions research activities efficiency have been investigated. A new model for higher education institutions research activities public financing has been suggested. The model developed a methodology for calculating subsidies for basic, guaranteed funding of scientific activities of universities, designed to compensate for the cost of simple reproduction of their scientific potential. The integral index, which allows accounting the influence of statistically significant factors on the total amount of research and development work performed by the University, is formed and justified by methods of econometric modeling. The proposed approach allows us to calculate the amount of guaranteed funding for the scientific activities of each university in the planning period with a known amount of financial resources allocated for the basic financing of scientific activities of universities in the country.
