An informative experiment involved two groups of surgeons during the endoscopic surgery training. The first group comprised the doctors with left hemisphere dominant (right-handed); the second comprised the doctors with right hemisphere dominant (left-handed). Group 1 of doctors showed better and more effective practical skills in endoscopic surgery compare to group 2. According to the results of the educational experiment, it was established that the left-handed and right-handed doctors have different psychophysiological features and they comprehend the necessary practical skills in endoscopic surgery differently. For the effective and comprehensive development of practical skills in this discipline, more attention should be paid to persons with dominance of the right cerebral hemisphere.

Aim: to study the bioelectric activity of the brain in relation to changes in the tone of the autonomic nervous system as assessed by heart rate variability (HRV) and evoked skin  sympathetic potentials (ESSP) in patients with migraine and epilepsy.Materials and methods. We studied patients with epileptic foci in the left (group 1) or right (group 2) hemisphere with migraine attacks followed by epileptic seizures. We used EEG recording  and EEG mapping programs and also determined HRV and ESSP.Results. We found that the subjects in group 1 had the normothymic and parasympathetic types  of heart rate control, and the subjects in group 2 tended toward the sympathetic type of heart rate  regulation. The spectral correlation EEG data indicated that the epileptiform activity in group  1 was more generalized than in group 2, which pointed to a more pronounced epileptogenesis in  group 1. In this group, all pain scores were higher than those in group 2. In group 2 though, depression, anxiety, and emotional disorders were more pronounced than in group 1.Conclusion. The findings can be explained by a closer connection between the right hemisphere and the limbic system. This, in our view, underlies the more pronounced changes in the tone of the  autonomic nervous system as assessed by the HRV and ESSP in patients with the right hemisphere epileptic activity and migraine attacks.

AbstractThe use of an ileal segment is a standard method for urinary diversion after radical cystectomy. Unfortunately, utilization of this method can lead to numerous surgical and metabolic complications. This study aimed to assess the tissue-engineered artificial conduit for urinary diversion in a porcine model. Tissue-engineered tubular polypropylene mesh scaffolds were used for the right ureter incontinent urostomy model. Eighteen male pigs were divided into three equal groups: Group 1 (control ureterocutaneostomy), Group 2 (the right ureter-artificial conduit-skin anastomoses), and Group 3 (4 weeks before urostomy reconstruction, the artificial conduit was implanted between abdomen muscles). Follow-up was 6 months. Computed tomography, ultrasound examination, and pyelogram were used to confirm the patency of created diversions. Morphological and histological analyses were used to evaluate the tissue-engineered urinary diversion. All animals survived the experimental procedures and follow-up. The longest average patency was observed in the 3rd Group (15.8 weeks) compared to the 2nd Group (10 weeks) and the 1st Group (5.8 weeks). The implant’s remnants created a retroperitoneal post-inflammation tunnel confirmed by computed tomography and histological evaluation, which constitutes urostomy. The simultaneous urinary diversion using a tissue-engineered scaffold connected directly with the skin is inappropriate for clinical application.

Objective To investigate somatosensory deficits in the ipsilesional wrist and hand in chronic stroke patients and correlate these deficits with contralesional sensorimotor dysfunctions, functional testing, laterality and handedness.Methods Fifty subjects (twenty-two healthy volunteers and twenty-eight stroke patients) underwent evaluation with Semmes-Weinstein monofilaments, the sensory and motor Fugl-Meyer Assessment, the Nottingham Sensory Assessment in both wrists and hands and functional tests.Results Twenty-five patients had sensory changes in the wrist and hand contralateral to the stroke, and eighteen patients (64%) had sensory deficits in the ipsilesional wrist and hand. The most significant ipsilesional sensory loss was observed in the left-handed patients. We found that the patients with brain damage in the right hemisphere had better scores for ipsilesional tactile sensation.Conclusions A reduction in ipsilesional conscious proprioception, tactile or thermal sensation was found in stroke subjects. Right hemisphere damage and right-handed subjects had better scores in ipsilesional tactile sensation.

Aesthetic preferences for photographs with the main focal content either to the left or right of the photograph's center were examined in right- and left-handed subjects. Verbal responses or manual responses were required. In one experiment with 261 introductory psychology student-subjects, left-handers more often preferred photographs with the more important part on the left (“left-geared”) than did right-handers. Exp. 2, involving 84 right-handed student subjects, showed that left-geared photographs presented on the left side were preferred more often than left-geared photographs presented on the right side, and left-geared photographs presented on the left side were more often chosen when a left-handed manual response was required. Interactions between handedness, position of the stimulus, language hemisphere, and response mode make it extremely difficult to ascertain whether the right hemisphere is really more involved in aesthetic decisions.

ABSTRACT The role of the right cerebral hemisphere (RH) associated with semantic priming effects (SPEs) must be better understood, since the consequences of RH damage on SPE are not yet well established. Objective: The aim of this article was to investigate studies analyzing SPEs in patients affected by stroke in the RH through a systematic review, verifying whether there are deficits in SPEs, and whether performance varies depending on the type of semantic processing evaluated or stimulus in the task. Methods: A search was conducted on the LILACS, PUBMED and PSYCINFO databases. Results: Out of the initial 27 studies identified, 11 remained in the review. Difficulties in SPEs were shown in five studies. Performance does not seem to vary depending on the type of processing, but on the type of stimulus used. Conclusion: This ability should be evaluated in individuals that have suffered a stroke in the RH in order to provide treatments that will contribute to their recovery.

AIM: The current study aimed to provide a method for juvenile nasopharyngeal angiofibroma embolization using Glubran glue in patients with low stage tumor. This method not only has less blood loss and good visualization but also impose a low cost, where no pre-operative embolization complications were found for this procedure. METHODS: Between 2012 and 2014, 30 patients with angiofibroma undergoing endoscopic surgery. Age, sex, tumor stage, average blood loss, complications, length of hospitalization, and recurrence rate of the tumor were the main measured outcomes. Furthermore, 30 patients were divided into three groups with matched age, sex, and tumor staging. Group 1 received glue (Glubran), while Group 2 selected for study without glue and embolization and pre-operative embolization was considered for Group 3. RESULTS: Based on the amount bleeding, the mean blood hemorrhage in Groups 1, 2, and 3 was 510, 1655, and 800 ml, respectively, the difference of hemorrhage between Groups 1 and 2 was found to be statistically significant (p = 0.007). Blood loss in Group 1 was found to be less than Group 3, but the difference of hemorrhage between Group 1 and 3 was not statistically significant (p = 0.678). No blood transfusion and complication were recorded for individuals in Group 1. The recurrence was found in 1 patient (10%) in both groups of 2 and 3, and no patient (0%) in Group 1. CONCLUSIONS: The direct intraoperative embolization technique with glue was capable of providing a more complete and targeted embolization of the tumor. Some advantages can be mentioned for this technique, including decreased blood loss, less radiation exposure, lower rates of complications, and recurrence, as well as shorter hospitalization time, the ease of procedure with a spinal needle and low cost.

We previously reported decreased mean CBF between consecutive resting conditions, ascribed to habituation. Here we address the regional specificity of habituation over three consecutive flow studies. Regional CBF (rCBF) was measured in 55 adults (12 right-handed men, 12 right-handed women, 14 left-handed men, 17 left-handed women), with the 133Xe inhalation technique, during three conditions: Resting, verbal tasks (analogies), and spatial tasks (line orientation). Changes in rCBF attributable to the cognitive tasks were eliminated by correcting these values to a resting equivalent. There was a progressive decrease in mean rCBF over time, reflecting habituation. This effect differed by region, with specificity at frontal (prefrontal, inferior frontal, midfrontal, superior frontal) and inferior parietal regions. In the inferior parietal region, habituation was more marked in the left than the right hemisphere. Right-handers showed greater habituation than did left-handers. There was no sex difference in global habituation, but males showed greater left whereas females showed greater right hemispheric habituation. The results suggest that habituation to the experimental setting has measurable effects on rCBF, which are differently lateralized for men and women. These effects are superimposed on task activation and are most pronounced in regions that have been implicated in attentional processes. Thus, regional decrement in brain activity related to habituation seems to complement attentional effects, suggesting a neural network for habituation reciprocating that for attention.

Left handers and women show less anatomical brain asymmetry, larger corpus callosum and more bilateral representation of specific functions. Sensory and cognitive components of cortical auditory evoked potentials (AEF) have been shown to be asymmetric in right handed males and to be influenced by stimulus intensity. In this study the influence of sex, handedness and stimulus intensity upon AEP components is investigated under basal conditions of passive attention. 14 right handed males, 14 right handed females, 14 left handed males, and 14 left handed females were studied while lying awake and paying passive attention to auditory stimulation (series of 100 binaural clicks, duration 1 msec, rate 1/sec, at four intensities). Cz, C3 and C4 referenced to linked mastoids and right EOG were recorded. Analysis time was 400 msec, average evoked potentials were based on 100 clicks. Stimulus intensity and gender affect early sensory components (P1N1 and N1P2) at central leads, asymmetry is influenced only by handedness, right handers showing larger P1N1 amplitudes over the right hemisphere.

The present study of gender differences in hemispheric processing involved identification of tachistoscopically presented images of varying complexity. A computerized tachistoscopic program was administered to 24 men and 34 women. Time to identify contour and detailed pictures presented to the left or right cerebral hemisphere was recorded. Mean reaction time for contour pictures was significantly faster than for detailed pictures, and mean reaction time to the right hemisphere was significantly faster than that to the left hemisphere. The mean reaction time for men to identify pictures exposed to the left hemisphere was significantly slower than that for exposure to the right hemisphere for women. The mean reaction time for both men and women to identify contour pictures exposed to the right hemisphere was significantly faster than the mean time to identify detailed pictures presented to the left hemisphere. The interaction of gender, hemisphere, and complexity was also significant in that mean reaction times for men to identify detailed pictures presented to the left hemisphere were slower than the times for women to identify contour pictures presented to the right hemisphere. The results are discussed in relation to theories about hemispheres, gender, and differences in picture features.

Abstract Objective To determine the incidence of systemic to pulmonary collaterals (SPCs) in extremely low birth weight infants and to assess its clinical and hemodynamic significance beyond the neonatal period. Study Design Retrospective cohort study was conducted on 61 infants with echocardiogram performed at the time of discharge to determine the presence of SPC and to measure the right and left ventricular outputs and left atrium to aortic ratio. We compared two groups: small or no SPC (Group 1) to moderate or large SPC (Group 2) on demographics, clinical outcomes, and echocardiographic parameters. Results Sixty-one infants were included. The incidence of SPC was 57%; 21% of infants had moderate or large shunts and 31% had small SPC. Demographics, clinical outcomes, and echocardiographic parameters were not significantly different between small or no SPC and moderate to large SPC. Conclusion More than half of the infants had SPC. The size of the shunt did not affect the clinical outcomes nor the echocardiographic parameters measured. All infants had cardiac output above the normative mean.
