Fibromyalgia is a chronic musculoskeletal pain syndrome characterized by the presence of painful points on digito-palpation in anatomical specific areas, called trigger points, followed by psychological disorders like sleeping problems and alexithymia. Alexithymia is a dimension defined as the absence of words for emotions. The patient does not know how to process their own emotions and fail to externalize what feels. The investigation of possible relationship between Fibromyalgia and Alexithymia can increase knowledge about the clinical features resulting from both diseases, especially because alexithymia is poorly explored and has a significant impact in patient’s life quality. Objectives: To execute a literature review searching for a clinical association between Alexithymia and Fibromyalgia (FM), highlighting psychiatric and psychological aspects. Methods: Folowing PRISMA statement the research used Scielo, Pubmed, PsychLit and ISIweb of knowledge data base between 2009 and 2014 with the key-words: “alexithymia”, “fibromyalgia”, “psychosomatics”, “chronical pain”, “psycotherapy”, “psychoanalysis”. Results: Nine studies were selected for analysis which used instruments like the Toronto Alexithymia Scale, the psychoanalysis and cognitive comportamental therapy showing that the patient with Alexithymia and FM has a worse pain interpretation, deals badly with it and suffer the consequences in their physical and mental health due to that. Conclusion: This study suggested the important impact that alexithymia has in patients with FM, since the psychiatric and psychological factors lead to a bigger decrease in life quality of people who have already worse aspects in that, which warn for the importance of this emotional state, since it is under-diagnosed. Due to the fact that there were a small number of reviewed articles, more research about the relation between FM and Alexithymia must be done.

Introduction.Coccygodynia is a multidisciplinary disease which is diffi cult to treat. It seriously limits the ability to work and signifi cantly affects the quality of life of patients. The study of somatic dysfunctions in patients with coccygodynia and the analysis of the results of osteopathic treatment of such patients makes it possible to justify the necessity of osteopathic correction of coccygodynia.Goal of the study— to determine the structure of the leading somatic dysfunctions in patients with coccygodynia and to study the effectiveness of osteopathic treatment of this pathology.Materials and methods. The study involved 44 patients from 25 to 65 years old, randomly divided into two groups. The main group of 24 people (20 women and 4 men) received osteopathic treatment, in accordance with the identifi ed leading somatic dysfunctions. Patients of the control group (16 women and 4 men) were treated locally with soft manual techniques (the treatment area was limited by the pelvic region). In order to assess the results of the treatment, we examined the intensity of the pain syndrome and the psycho-emotional state of patients. The severity of the pain syndrome was assessed in accordance with the visual analogue scale (VAS). The psycho-emotional state (with physical and mental components) was assessed with the help of the SF-36 quality of life questionnaire.Results.Somatic dysfunctions typical for patients with coccygodynia have been identifi ed. Osteopathic treatment has proven to be more effective in comparison with local manual therapy of coccygodynia both in early periods and in 3 months after the end of the treatment course.Conclusion.Osteopathic treatment of post-traumatic coccygodynia is effective, and can be recommended for treatment of such patients.

Background. Being diagnosed with cancer is a dramatically stressful condition for a human. Radiation therapy (RT), which is an essential of combination antineoplastic treatment, can cause physical and emotional state changes. Realizing these changes is an additional stress factor that results in impaired life quality of a cancer patient as well refusal to undergo further treatment. At present, there is no distinct system of providing psychological aid for cancer patients in Ukraine. Purpose. To carry out analysis and generalization of research literature data in terms of providing psychological aid for cancer patients at RT stage and to present own findings of psychological study enrolling 55 female patients diagnosed with endometrial cancer at RT stage at SO «Grigoriev Institute for Medical Radiology and Oncology» within the period from 2015 to 2018. Materials and methods. The literature review involved available contributions obtained via literature search in domestic (Vernadsky National Library of Ukraine) and foreign databases (Scopus, Web of Science Core Collection, PubMed, BMC, ScienceDirect, Wiley Online Library, RNB). Fifty-five patients with endometrial cancer of stage I–II (T1b-c N0 M0 – T2a -b N0 M0) underwent clinical and laboratory examination. The diagnosis of adenocarcinoma was confirmed by means of a histologic study. All patients underwent panhysterectomy and postsurgery course of distant gamma-therapy on “РОКУС-АМ” device by split fractionation. Fifteen patients were provided with psychological correction at RT stage. In order to assess life quality, SF-36 (Health Status Survey) was used. An assessment of the severity of psychosomatic disorders in patients with endometrial cancer was made by means of EORTC QLQ-C30 survey. Results. The paper analyzes psychological aid provided to oncology patients at radiation therapy stage in Ukraine and overseas. It presents the peculiarities of patients’ psychological responses during irradiation and possible consequences of higher mental function impairment. The targets of psychological correction and factors influencing life quality of a patient have been considered as well as the effectiveness of psychotherapeutic interventions outcomes of different approaches has been provided. The study states that, currently, medical and psychological follow-up of cancer patients does not consist only in reassuring talk and drug correcting of their psychological state aimed to reduce distress, anxiety and depression level, but also in psychocorrection, psychoeducation, the access to psychotherapeutic groups, individual psychotherapy. The efficiency of applying own developed schemes of medical and psychological correction of emotional state of women with EC is presented, the use of which has made it possible to reduce the manifestations of sleep disorders, cognitive function and fatigue. Conclusions. Following a differentiated strategy when choosing methods of psychological aid for oncology patients and their families at radiation therapy stage via psychocorrection of emotional, cognitive and behavioral spheres, psychoeducation or psychotherapeutic interventions will make it possible to significantly improve their psychological status and quality of life.

The aim of the research is to study the results of minimally invasive surgery for the treatment of nonspecifi c infectious spine lesions, and to present personal experience. Material and methods. Three patients with purulent nonspecifi c spondylodiscitis of lumbar spine got minimally invasive surgical treatment. The essence of surgical intervention included X-ray-assisted placement of tubular retractors in the area of intervertebral disc aff ected by purulent process; debridement and irrigation with antiseptic solution of interbody space, and setting a drain tube. All patients were treated during the period from December 2016 to February 2019. They were examined before surgery and in 3 months aft er the discharge. Laboratory parameters (CBT, C-reactive protein, bacteriological analysis of intervertebral disc contents) and instrumental research methods (computer tomography, magnetic resonance imaging) were evaluated. ODI, VAS, SF-36 questionnaires helped to assess pain syndrome and life quality of patients. Results. In 100 % of studied cases infection was caused by Staphylococcus aureus. The result of treatment in all cases was assessed as good, because pain syndrome, neurological disorders and inflammatory process were arrested; there were no signs of disease progression. In all cases, life quality of patients was restored. In one case, additional surgical intervention was required (posterior instrumental fi xation of spine) because of instability signs. Conclusion. Minimally invasive surgical treatment of purulent single-level spondylodiscites can be recommended for practical use. The use of such approache allows to verify purulent infection agent, to damage soft tissues less getting proper sanitation of interbody space. It also allows to stop the infection and restore life quality of a patient

Introduction. The presence of somatic diseases during pregnancy leaves a serious imprint on the emotional state of a woman, significantly worsens the quality life indicators and affects the clinical characteristics of pregnancy. One of these diseases is chronic pyelonephritis. There are a fairly large number of recommendations for the treatment of this pathology. However the focus is mainly on drug therapy. At the same time the existing restrictions on the number of drugs using during pregnancy, the problem of polypragmasia, and the increase in the number of allergic complications of drugs using dictate the need to search alternative methods of treatment, primarily non-drug ones. Moreover, the problems of pregnant women life quality with chronic pyelonephritis during complex drug therapy are often not given due attention. The goal of research — to assess the impact of osteopathic correction on the psycho-emotional state and life quality of pregnant women with concomitant pathology of the urinary system.Materials and methods. A prospective controlled randomized study was conducted in the period 03.2016– 01.2018 on the basis of medical clinics of LLC «Mokhov Institute of osteopathy» and LLC «Vasileostrovskaya clinic of reproduction and genetics». There were observed work 48 pregnant women with chronic pyelonephritis aged 25–45 years, with a gestation period 13–27 weeks. During the processing of the clinical material, 8 patients were eliminated. As a result of the selection process, a group of 40 people was formed. All pregnant women with chronic kidney disease, depending on the used treatment method, were divided into two groups using a simple randomization method with a random number generator. There was formed the main group (20 people), and the control group (20 people). Patients of the main group received medication and osteopathic correction (3 procedures with an interval of 7–10 days). Patients in the control group received only traditional drug therapy. All pregnant women with chronic pyelonephritis had an osteopathic examination before and after treatment with forming an osteopathic conclusion, and the assessment of the pain syndrome severity by a visual analog scale (VAS), the general psycho-emotional state (the method of rapid assessment of health, activity and mood — HAM), and the level of life quality (Questionnaire Medical Outcomes Study — Short Form).Results. Pregnant women with chronic pyelonephritis were characterized by a weak pain syndrome, a psychoemotional state violations in the categories «well-being» and «activity», and a decrease in both the physical and mental components of life quality. After the complex treatment including the additional to traditional drug therapy osteopathic correction methods, the statistically significant decrease of the pain severity degree (p=0,001), increase of psychoemotional state rates in the category of «activity» (p=0,05) and the increase of physical and mental components of life quality (p=0,02) were observed in the main group compared with the control.Conclusion. Chronic pathology of the kidneys during pregnancy affects the emotional state of a woman, significantly worsens life quality indicators. The use of osteopathic correction as a part of the complex therapy of pregnant women with chronic pyelonephritis can improve the emotional state and life quality; reduce the pain severity, and so can be used to improve medical care for this population category.

Background:The subject of cancer is still the cause of fear and anxiety in the public opinion. The order of daily life of the patient and their family is disturbed by numerous medical consultations, treatment and the side effects of the therapy. In the situation, when the development of cancer goes beyond the possibilities of the support and care from the loved ones in household conditions and this care is no longer sufficient, there is an opportunity to provide care and therapy for the patient in the palliative medicine departments. In the studies of the life quality we take into account not only the clinical state of the patient, but also their subjective feelings. The quality of life is assessed in the dimension of the current health status. Aim of the study:The paper aims to show the physical, mental and spiritual state of the patients before admission to the department, and in the first week of the stay in the palliative medicine department. Material and Methods:The research was performed with the use of a questionnaire (EORTC) QLQ-C30. The study was conducted among the patients of the Palliative Medicine Department of the Medical Centre ‘Samaritan’ in Opole, in the period from November 2013 to January 2014. 35 patients participated in the study. The average age was 69±31. Results:On the day of admission to the department and while filling in the questionnaire, all the respondents felt pain ailments and had some sleeping problems. 89% (31) experienced shortness of breath. 94% (33) of the respondents assessed their level of life in the palliative medicine department as high. Conclusions:The patients have assessed their level of life in the palliative medicine department as high. The low assessment of the quality of life in household conditions during the week preceding the admittance to the palliative medicine department encourages the continuation of the research in household conditions, in order to improve the life quality of people with the diagnosed cancer remaining in home environment.

Abstract It is essential to enhance life quality in patients with premature ventricular complexes (PVCs) with the use of radiofrequency ablation (RFA). The aim of the study was to assess symptom burden and life quality in patients with a low PVC burden following RFA. 31 individuals with a low PVC burden in whom RFA was indicated were included in the study. At baseline and after a year following RFA, the Arrhythmia-Specific questionnaire in Tachycardia and Arrhythmia (ASTA) scale was used for appraisal. A 24-hour Holter electrocardiogram was used to detect recurrent PVCs 12 months after the RFA intervention. ASTA scores related to symptom burden, including the near-syncope score, health-related quality of life (HRQOL) scales pertaining to physical and mental health, and consequently the total HRQOL score, were all diminished a year after RFA (p<0.001). The ASTA score for syncope symptoms was also reduced (p<0.05). A fall in mean PVC burden was seen from 8.0% to 0.8% (p<0.001). The long-term clinical endpoint in individuals with a low PVC burden following RFA is reported. Symptom load, life quality and ultimate PVC burden were all enhanced. Additional studies incorporating longer follow-up and monitoring periods, respectively, would be beneficial.
