Evaluation of Neurotoxin and Filler Injection Training in Otolaryngology Residency

Introduction: Residency training courses are fundamental to an Otolaryngology (ENT) residency curriculum. Neurotoxin and filler injections have become common during ENT residency, and the outcomes of training courses have not been previously evaluated. We hypothesize that after participating in the course, resident knowledge, skill, and likelihood of using neurotoxin and filler injections in future practice will significantly improve. Materials and Methods: A prospective study was designed among ENT residents undergoing neurotoxin and filler injection educational training courses from April 2019 to November 2020. After the completion of the course and injections, residents completed a self-evaluation to assess their level of knowledge and skill level with neurotoxin and filler injections. Results: The mean number of neurotoxin and filler injections besides within the course was 1.67 and 0.33, whereas during the course was 3.39 ( P = .008) and 1.39 ( P = .0009), respectively. Resident knowledge, skill, and likelihood of using neurotoxin and filler injections in future practice all significantly improved ( P < .05). This study found that a biannual training course for neurotoxin and injectable fillers was an effective strategy at improving resident knowledge, skill, and likelihood of use in future practice.

SummaryTwenty-four dogs (27 limbs) were evaluated after surgery for correction of forelimb angular limb deformities. Partial ulnar ostectomies or definitive corrective osteotomies were performed depending upon the age of the dog. According to owner assessment nine of fourteen limbs were considered functionally good, or excellent, after partial ulnar ostectomies. Younger dogs appeared to have better functional results after dynamic correction with the mean age at surgery of dogs with good to excellent results being 6.5 months contrasted to the mean age at surgery of dogs with fair to poor results being 9.75 months. Ten of fourteen limbs were considered functionally good or excellent after definitive corrective osteotomy. One dog had definitive osteotomy after partial ulnar ostectomy in order to further correct a residual angular deformity. However, 58% of the limbs with radiographic follow-up had signs of degenerative joint disease (DJD). There were not significant differences between neither degree of angulation remaining after surgery and the functional result nor the degree of angulation remaining after surgery and the development of DJD. A prospective study is warranted to more objectively assess the efficacy of surgical correction of angular limb deformities in dogs.Twenty-four dogs were evaluated after surgery for correction of forelimb angular limb deformities. The results are described.

IntroductionTo evaluate the usefulness of point-of-care ultrasound (POCUS) performed by young military medicine residents after short training in the diagnosis of medical emergencies.MethodsA prospective study was performed in the emergency department of a French army teaching hospital. Two young military medicine residents received ultrasound training focused on gall bladder, kidneys and lower limb veins. After clinical examination, they assigned a ‘clinicaldiagnostic probability’ (CP) on a visual analogue scale from 0 (definitely not diagnosis) to 10 (definitive diagnosis). The same student performed ultrasound examination and assigned an ‘ultrasounddiagnostic probability’ (UP) in the same way. The absolute difference between CP and UP was calculated. This result corresponded to the Ultrasound Diagnostic Index (UDI), which was positive if UP was closer to the final diagnosis than CP (POCUS improved the diagnostic accuracy), and negative conversely (POCUS decreased the diagnostic accuracy).ResultsForty-eight patients were included and 48 ultrasound examinations were performed. The present pathologies were found in 14 patients (29%). The mean UDI value was +3 (0–5). UDI was positive in 35 exams (73%), zero in 12 exams (25%) and negative in only one exam (2%).ConclusionPOCUS performed after clinical examination increases the diagnostic accuracy of young military medicine residents.

OBJECT Peritumoral cysts are frequently associated with CNS hemangioblastomas and often underlie neurological morbidity and mortality. To determine their natural history and clinical impact, the authors prospectively analyzed hemangioblastoma-associated peritumoral cysts in patients with von Hippel-Lindau (VHL) disease. METHODS Patients with VHL disease who had 2 or more years of follow-up and who were enrolled in a prospective study at the National Institutes of Health were included. Serial prospectively acquired laboratory, genetic, imaging, and clinical data were analyzed. RESULTS One hundred thirty-two patients (of 225 in the VHL study with at least 2 years of follow-up) had peritumoral cysts that were followed for more than 2 years (total of 292 CNS peritumoral cysts). The mean age at study entrance was 37.4 ± 13.1 years ([mean ± SD], median 37.9, range 12.3–65.1 years). The mean follow-up was 7.0 ± 1.7 years (median 7.3, range 2.1–9.0 years). Over the study period, 121 of the 292 peritumoral cysts (41.4%) became symptomatic. Development of new cysts was associated with a larger number cysts at study enrollment (p = 0.002) and younger age (p < 0.0001). Cyst growth rate was associated with anatomical location (cerebellum cysts grew faster than spine and brainstem cysts; p = 0.0002 and p = 0.0008), younger age (< 35 years of age; p = 0.0006), and development of new neurological symptoms (p < 0.0001). Cyst size at symptom production depended on anatomical location (p < 0.0001; largest to smallest were found, successively, in the cerebellum, spinal cord, and brainstem). The most common location for peritumoral cysts was the cerebellum (184 cysts [63%]; p < 0.0001). CONCLUSIONS Peritumoral cysts frequently underlie symptom formation that requires surgical intervention in patients with VHL disease. Development of new cysts was associated with a larger number of cysts at study enrollment and younger age. Total peritumoral cyst burden was associated with germline partial deletion of the VHL gene.

Object Children with syndromic or complex craniosynostosis are evaluated for increased intracranial pressure (ICP) using funduscopy to detect papilledema. However, papilledema is a late sign of increased ICP. Because papilledema might be preceded by an increase in optic nerve sheath (ONS) diameter, the authors conducted a prospective study to establish the validity and applicability of measuring the ONS using ultrasonography. Methods From January 2007 to December 2009, 175 bilateral ultrasonography ONS measurements were performed in 128 patients with syndromic or complex craniosynostosis during the daytime. The measurements were correlated with ONS diameter assessed on CT and simultaneous funduscopy, when available. Furthermore, results were compared by using thresholds for ONS diameters on ultrasonography that are available in the literature. Results The mean ONS diameter on ultrasonography was 3.1 ± 0.5 mm. The CT measurement was significantly correlated with the ultrasonography measurement (r = 0.41, p < 0.001). The mean ONS diameter in 38 eyes with papilledema was 3.3 ± 0.5 mm, compared with 3.1 ± 0.5 mm in the eyes of patients without papilledema (p = 0.039). Relative to the age-related thresholds, the ONS diameter was too large in 11 eyes (3%), particularly in patients with Crouzon syndrome. Compared with funduscopy, ultrasonography sensitivity was 11%, specificity was 97%, and positive and negative predictive values were 40% and 86%, respectively. Conclusions Ultrasonography is a valid and easy way of quantifying the ONS. Although the ONS diameter is larger in children with papilledema, it cannot be used as a daytime screening tool instead of funduscopy. The ONS diameter is possibly a more real-time indicator of ICP.

Covid-19 (Coronavirus Disease 2019) caused by Severe Acute Respiratory SyndromeCoronavirus 2 (SARS-CoV2) was declared a pandemic on March 11, 2020.Various stepshave been taken to control the spread of the Covid-19 pandemic that is currentlyhappening in Indonesia. Despite all the efforts of the government and media to educatethe public, the increase in the number of daily cases in Indonesia has not shown adecline. This encourages researchers to assess the level of public knowledge andperceptions because factors have an influence on individual health behavior that cancontribute to pandemic control efforts. This research uses study method observationaldescriptive. The population in this study were 343 people who filled out a researchquestionnaire conducted by the IKM-IKK FK Unsri department. For 251 respondents whofilled out the online questionnaire completely, the mean age was 23.78 ± 7.46, 63.7%female, 63.7% college graduates, 77.7% Muslim, and the majority of occupations arecollege students and students (54.6%). In the assessment of knowledge, 137 people(35.7%) had a good level of knowledge with a mean valueknowledge 13.25 ± 3.2. Themajority of respondents have a good perception, namely 249 people (99.2%) with morethan half showing a positive perception of the risk of infection if not taking precautions,the importance of wearing masks and physical distancing, hand washing behavior, self-isolation, and consumption of nutritious food during the Covid-19 pandemic..

Background: The skill of intravenous cannulation must be practiced regularly to maintain a high level of competency. This is important to gain quick and efficient intravenous access in populations when required. The insertion of intravenous catheters into peripheral veins is probably the most commonly performed invasive medical procedure in hospitals. This procedure could be difficult sometimes requiring several attempts and causing distress to patients. The high success rates of nurses in intravenous cannulation have been due to the frequent performance of intravenous cannulation Objectives: 1. To assess the existing knowledge regarding intravenous cannulation among staff nurses working in selected hospital. 2. To observe the existing practice regarding intravenous cannulation among nurses. 3. To assess the effectiveness of lecture cum demonstration regarding Intravenous cannulation among staff nurses. 4. To observe the practice of staff nurses regarding intravenous cannulation after lecture cum demonstration. 5. To correlate the knowledge and practice of staff nurses regarding intravenous cannulation .Methodology: Pre experimental one group pre test post test design. Material: Structured knowledge questionnaire and Observational checklist adopted as per WHO guidelines.Sample size: 60. Result: Among the 60 samples, according to age, 39(65%)of subjects were of 21-30 years of age, 14(23.33%) were of 31-40 years, 7(11.67%) were of 41-50 years and no subjects were above 50 yearsAccording to the educational qualification, 38(58.33%) of the subjects were with the qualification of general nurse midwives, 12(20%) were B.sc nursing, 13(21.67%) were PB.B.sc Nursing.Majority of the subjects 45% have work experience of less than 2 years, 23.33% have 3-5 years, 20% have 6-8 years and 11.67% of them have work experience of more than 8 years In relation to area of work, 26.67% of the subjects each were from the Medicine ward and surgical ward, 21.66%were from Intensive care unit and 25% of them were from Casualty/Emergency ward. post test majority of the subjects gained Knowledge regarding intravenous cannulation. 34 (56.67%) subjects had good level of Knowledge score and 23(43.33%) subjects had average level of Knowledge. None of the samples have poor level of Knowledge. the post test the findings shows that 57(95%) subjects had satisfactory level of practice whereas only 3(5%) of subjects had unsatisfactory level of practice. The mean pretest knowledge score was 15.28 and post test knowledge score was 26.58. The mean practice score 7.61 and posttest practice score was 13.76. Student’s paired ‘t’ test is applied at 5% level of significance and The calculated‘t’ value for overall knowledge score of subjects was 35.51 and the calculated ‘t’ value for overall practice score of subjects was 23.44 that statistically interpreted that the lecture cum demonstration regarding intravenous cannulation was effective in increasing the Knowledge and practice of subject. There was significant association between selected demographic variable of year of experience with their pretest knowledge, whereas other demographic variables are not significantly associated.The correlation coefficient of post test knowledge and post test practice score was ‘r’ 0.21, which is indicates a positive correlation. Conclusion: The knowledge of the target population was significantly increased, and there was significant improvement in the level of practice after receiving lecture cum demonstration regarding intravenous cannulation. The improvement in knowledge and practice was found in all subjects irrespective of their demographic variable

Objective: To evaluate the use of ex vivo lung perfusion (EVLP) clinically to prepare donor lungs for transplantation. Methods: A prospective study involving EVLP for the reconditioning of extended-criteria donor lungs, the criteria for which include aspects such as a PaO2/FiO2 ratio < 300 mmHg. Between February of 2013 and February of 2014, the lungs of five donors were submitted to EVLP for up to 4 h each. During EVLP, respiratory mechanics were continuously evaluated. Once every hour during the procedure, samples of the perfusate were collected and the function of the lungs was evaluated. Results: The mean PaO2 of the recovered lungs was 262.9 ± 119.7 mmHg at baseline, compared with 357.0 ± 108.5 mmHg after 3 h of EVLP. The mean oxygenation capacity of the lungs improved slightly over the first 3 h of EVLP-246.1 ± 35.1, 257.9 ± 48.9, and 288.8 ± 120.5 mmHg after 1, 2, and 3 h, respectively-without significant differences among the time points (p = 0.508). The mean static compliance was 63.0 ± 18.7 mmHg, 75.6 ± 25.4 mmHg, and 70.4 ± 28.0 mmHg after 1, 2, and 3 h, respectively, with a significant improvement from hour 1 to hour 2 (p = 0.029) but not from hour 2 to hour 3 (p = 0.059). Pulmonary vascular resistance remained stable during EVLP, with no differences among time points (p = 0.284). Conclusions: Although the lungs evaluated remained under physiological conditions, the EVLP protocol did not effectively improve lung function, thus precluding transplantation.

Back ground: An experimental study was conducted to assess the effects of instructional teaching programme (ITP) on knowledge and practices of mothers regarding prevention of pneumonia in children, Pooncherry, Chengalpattu District, Tamil Nadu, India.Objectives of the study were to assess the effectiveness of ITP on prevention of pneumonia on level of knowledge and practices among mothers of under five children. Materials and Methods: Non probability purposive technique was adopted for this study. Samples who met inclusion criteria were participated in the study. Self-structured interview schedule for knowledge  and structured rating scale for practices were used to collect the data. Results: The study results shows that in the pre test 15.13 % of the mothers had adequate knowledge, 24.34 % of the mothers were had moderately adequate knowledge and 60.53 % of the mothers had inadequate knowledge and  and in post test 63.17% of mothers were had adequate knowledge, 28.94% of the mothers had moderately adequate knowledge and very few (8.55% ) mothers had inadequate knowledge. Pre test practices score shows that 63.82 % of the mothers had less desirable practice, 22.37% of the mothers had moderate desirable practices and 13.81% were had highly desirable practices whereas in the post test 63.82% of them had highly desirable practice, 23.03% of them had moderate desirable practices and 13.16% of them had less desirable practices. The mean pretest knowledge score was 5.45 ± 1.47 and the mean posttest knowledge score was 8.49 ± 0.71 and the t value was 15.17, it shows that statistically there was an improvement in the post test knowledge score. The mean pretest practice score was 22.38 ± 2.76, the mean posttest practice score was 43.64 ±1.97 and the t value was 13.35, it shows that statistically there was a significant improvement in the post test practice score. The instructional teaching programme on prevention of pneumonia was statistically effective in promoting the knowledge and desirable practices among mothers.

The research aims to study the teaching skills and evaluate the teaching skills of science teachers, and test significance of differences between the mean degree of practice according to the variables: gender, experience, educational qualification, and stage educational, and to achieve these objectives were applied a questionnaire consisting of (31) paragraph, to a sample Of (45) are teachers of science in the intermediate stage of the Ministry of Education and Mjardp. The researcher also followed the descriptive approach. The results indicated that the degree of science teachers and her teachers to teach constructivist was medium, and appeared statistically significant scientific qualification variable differences, for the benefit of graduate studies. The study recommended training courses for science teachers, to familiarize them with the skills of teaching constructivist, and trained to teach these skills to have procedures and guide administrators science with a list of skills (the subject of the study), during the supervision of the education in the schools of the intermediate stage and directing the attention of the authors and curriculum developers to take into account the good fundamentals in the article To provide learners with a leading to the development of scientific concepts in general.

Background: Childhood asthma is affected by events and conditions of the school environment. Teachers as de-facto caregivers of children with asthma have a major role to play in ensuring good asthma control in school. This study set out to determine the level of knowledge of school teacher about childhood asthma and factors influencing this knowledge.Methods: Four secondary schools (two private and two public) in Ilesa, South West Nigeria were selected by multistage sampling method. All the teachers in these schools were required to fill a self -administered questionnaire incorporating a validated 40-item asthma knowledge questions. Factors associated with the level of knowledge were determined appropriately.Results: A total of 132 teacher (M: F = 1:2) participated in the study with 85 (64.4%) from private school. The mean (SD) age of the teachers was 38.0 (9.1) years and median (IQR) years in teaching service was 7.0 (5.0 to 15.0) years. Majority (56.1%) of the teachers had university education while only 7 (5.3%) had a post graduate degree. The mean (SD) score of the 40 item questions was 21.5 (7.2) and majority (51.5%) had poor asthma knowledge (score < 22). Poorer knowledge was observed in questions related to the nature and management of childhood asthma than triggers and manifestations. No significant correlation was found between knowledge and age, teaching experience and qualifications (p > 0.05). However, teachers with previous training about childhood asthma had relatively good knowledge about the condition. (p <0.05)Conclusion: The level of knowledge about childhood asthma among school teachers in Ilesa is poor particularly as regards to nature and management of the disease. We advocate for training of school teachers about common childhood conditions including asthma to ensure optimal symptoms control in school.Keywords: Childhood asthma, Knowledge, School teachers
