In adults, greater social integration is associated with reduced risk of cardiovascular disease, including hypertension. Social integration earlier in life may be similarly associated with cardiovascular risk. Using a longitudinal sample of 267 Black and White men, we examined whether greater social integration with peers during childhood and adolescence, assessed by parent report, prospectively predicts lower blood pressure and body mass index two decades later in adulthood and whether these effects differ by race, given well-documented racial disparities in hypertension. Boys who were reported by their parents to be more socially integrated with peers evidenced lower blood pressure and body mass index in adulthood, and this effect was not accounted for by body mass index in childhood, childhood socioeconomic status, childhood hostility, childhood physical health, extraversion measured in adolescence, or concurrent adult self-reports of social integration. Results did not differ by race, but analyses were not powered to detect interactions of small effect size.

Background Breastfeeding in infancy is associated with lower cardiovascular disease risk in adulthood; however, the amount of breastfeeding required to achieve this benefit is unknown. Methods and Results In the CHILD (Canadian Healthy Infant Longitudinal Development) Cohort Study, we analyzed 2382 children with complete data on early life feeding and blood pressure. Infant feeding was documented from hospital records in the first few days of life and reported by mothers throughout infancy. Blood pressure was measured at 3 years of age. Analyses controlled for birth weight, gestational age, socioeconomic status, maternal body mass index, and other potential confounders. We found that nearly all children (2333/2382; 97.9%) were ever breastfed, of whom 98 (4.2%) only briefly received breast milk during their birth hospitalization (“early limited breastfeeding”). At 3 years of age, blood pressure was higher in children who were never breastfed (mean systolic/diastolic 103/60 mm Hg) compared with those who were ever breastfed (99/58 mm Hg), including those who received only early limited breastfeeding (99/57 mm Hg). These differences in systolic blood pressure persisted in adjusted models (ever breastfed: −3.47 mm Hg, 95% CI, −6.14 to −0.80; early limited breastfeeding: −4.24 mm Hg, 95% CI, −7.45 to −1.04). Among breastfed children, there was no significant dose‐response association according to the duration or exclusivity of breastfeeding. Associations were not mediated by child body mass index. Conclusions Although the benefits of sustained and exclusive breastfeeding are indisputable, this study indicates any breastfeeding, regardless of duration or exclusivity, is associated with lower blood pressure at 3 years of age. Further research examining the bioactive components of early breast milk, underlying mechanisms, and long‐term associations is warranted.

Background. High blood pressure in adults is directly correlated with increased risk of cardiovascular diseases. Hypertension in childhood and adolescence could be considered among the major causes of this problem in adults. This study aimed to investigate the factors associated with hypertension among the adolescents of Jahrom city in Iran and also standard percentiles of blood pressure were estimated for this group.Methods. In this community-based cross-sectional study 983 high school students from different areas of the city were included using a multistage random cluster sampling method in 2014. Blood pressure, weight, and height of each student measured using standard methods. Data were analyzed by statistical software SPSS 16.Results. In total, 498 male and 454 female students were included in this study. Average systolic blood pressure of students was 110.27 mmHg with a variation range of 80.6–151.3. Average diastolic blood pressure was 71.76 mmHg with the variation range of 49.3–105. Results of this study indicated that there was a significant relationship between gender, body mass index, and parental education level with systolic and diastolic blood pressure of the students (P<0.05).Conclusions. Body mass index was one of the most important changeable factors associated with blood pressure in adolescents. Paying attention to this factor in adolescence could be effective in prevention of cardiovascular diseases in adulthood.

AbstractPrevious studies have shown that the effect of adiposity on blood pressure (BP) intensifies as children become increasingly obese. Black children tend to have greater body mass index (BMI) and higher BP than age-matched white children. It is unclear whether the BP effects of BMI are race-specific among black and white children, and data on obese Hispanic children are sparse. We compared the BP effect of BMI in obese white, black, and Hispanic children.We examined the medical records of children enrolled in a pediatric obesity clinic. Height, weight, BP, and fasting insulin were assessed as part of routine clinical care. The concurrent effects of age and BMI on BP percentile values were examined using semiparametric regression, which allows the accommodation of nonlinear effects.The study included 873 children (338 male; 354 black, 447 white, 72 Hispanic; 11.7±3.5 years, BMI 36.2±8.5 kg/mAmong children referred for treatment of obesity, black children and Hispanic children are at a greater risk for having elevated BP when compared to white children of similar age and BMI.

Hipertensiadalah penyebab kematian utama di Indonesia. Kematian akibat hipertensi lebih banyak terjadi di perkotaan dibandingkan di desa. Tingginya kejadian hipertensi dipengaruhi oleh faktor yang dapat dikontrol (obesitas,berat badan lebih, konsumsi garam berlebih,aktivitas fisik rendah, perokok, dan konsumsi alkohol) dan faktor yang tidak dapat dikontrol (genetik, usia, dan jenis kelamin). Penelitian bertujuan memberikan gambarandistribusi tekanan darah dan indeks massa tubuh (IMT) pendudukperempuan di kota maupun di desa.Desain penelitian adalah deskriptif kuatitatif dengan pendekatan cross sectional. Data primer didapatkan melalui pengukuran tekanan darah, tinggi badan dan berat badan. Penelitian dilakukan di Desa Kutowinangun Kidul, Kecamatan Tingkir,Salatiga yang mewakili penduduk perempuan di perkotaan dan Desa Batur, Kecamatan Getasan, Kabupaten Semarangyang mewakili penduduk perempuan pedesaan. Adapun jumlah sampel masing-masing 66 respondendan 72responden. Hasil penelitian menunjukkan bahwa kejadian penyakit hipertensi, resiko obese dan obese lebih didominasi oleh respondendi perkotaan dibandingkan di pedesaan dengan rentang usia di atas 46 tahun. Respondendi desa dengan indeks massa tubuh normal cenderung mengalami pre-hipertensi (8,3%) dan hipertensi stadium I (6,9%) sementararesponden dengan resiko obese dan obese cenderung mengalami hipertensi stadium I (1,4%).Respondendi kota dengan indeks massa tubuh normal cenderung mengalami pre hipertensi (6,06%), hipertensi stadium I (4,5%) dan II (7,5%). Responden dengan resiko obese cenderung mengalami hipertensi stadium I (4,5%), dan responden dengan obese I dan II cenderung mengalami pre-hipertensi (4,5%).   Hypertension is the leading cause of death in Indonesia. Deaths due to hypertension are more common in urban areas than in villages. The high incidence of hypertension is influenced by factors that can be controlled (obesity, overweight, excessive salt consumption, low physical activity, smokers, and alcohol consumption) and factors that cannot be controlled (genetic, age, and sex). The study aimed to provide an overview of blood pressure distribution and body mass index (BMI) of female residents in cities and villages. The study design was descriptive quantitative with a cross sectional approach. Primary data was obtained through measurements of blood pressure, height and weight. The study was conducted in Kutowinangun Kidul Village, Tingkir Subdistrict, Salatiga representing women in urban areas and Batur Village, Getasan Subdistrict, Semarang Regency, representing rural women. The number of samples was 66 respondents and 72 respondents respectively. The results showed that the incidence of hypertension, the risk of obese and obese was more dominated by respondents in urban areas than in rural areas with ages above 46 years. Respondents in villages with normal body mass index tended to experience pre-hypertension (8.3%) and stage I hypertension (6.9%) while respondents with obese and obese risk tended to experience stage I hypertension (1.4%). Respondents in cities with normal body mass index tended to experience pre-hypertension (6.06%), stage I hypertension (4.5%) and II (7.5%). Respondents with obese risk tended to experience stage I hypertension (4.5%), and respondents with obese I and II tended to experience pre-hypertension (4.5%).

AbstractWe use a regression discontinuity design to estimate the causal effect of antiretroviral therapy (ART) eligibility according to national treatment guidelines of South Africa on two risk factors for cardiovascular disease, body mass index (BMI) and blood pressure. We combine survey data collected in 2010 in KwaZulu-Natal, South Africa, with clinical data on ART. We find that early ART eligibility significantly reduces systolic and diastolic blood pressure. We do not find any significant effects on BMI. The effect on blood pressure can be detected up to three years after becoming eligible for ART.
