Telephone-based consultation in an acute eye clinic service: A patient satisfaction survey

Objectives: The coronavirus pandemic has forced healthcare staff across all medical specialties to adapt new and different ways of working. A new approach has been set up in the Acute Referral Clinic (ARC) at Musgrove Park Hospital and a survey has been conducted to measure the impact of the new method on patient and healthcare professionals’ satisfaction with the new service. Methods: A telephone-based consultation was introduced in ARC at Musgrove Park Hospital in March 2020 and patients were instructed to fill out a questionnaire containing eight items using a Likert Scale 1 (‘very poor/disagree’) to 4 (‘very good/strongly agree’) plus two boxes for open positive and negative comments respectively. Likewise a questionnaire was designed in order to assess the healthcare professionals’ satisfaction using the new approach. Data collection took place over a two month period between the end of March 2020 and end of May 2020. The data underwent quality control and was analysed using descriptive statistics. Results: Patient responses illustrated high satisfaction scores with an overall rating of very good (89.4%). The healthcare professionals’ rating of the service was good (28.6% – ‘very good/strongly agree’, 57.1% – ‘good/agree’). The safety rating of the new approach was overall rated ‘very good’ with 90.4% and 71.4% of patients and healthcare professionals respectively. Conclusions: The telephone consultations introduced in the wake of COVID-19 are well accepted by both patients and doctors. There are some limitations of the approach, foremost being consultation time and clinic space but these do not outweigh the general benefit of this format amidst a pandemic setting.

Introduction: Planning is essential to healthcare institutions.  The first important factor that indicates the effectiveness and efficiency of an organization is planning and the realization of how the vision and mission that have been set up are implemented. However, organizational culture plays a key role in setting a plan for hospitals. It is upon this premises that this study seeks to find the impact of organizational culture on planning as professed by employees in the hospital and the significant difference between sex and age when planning was considered.    Method: This was quantitative research which was designed as descriptive-correlational. The self-constructed instrument was conveniently administered to 108 hospital employees within Laguna in the Philippines to measure their hospital organizational culture and planning. The statistical study treatment was based on SPSS version 23, where Mean and Standard Deviation and Pearson Correlation and t-test were used to analyze the data.    Result: The results revealed that hospital planning was very high, and they strongly agree with the influence of their organizational culture. Furthermore, the study resulted that there is a high correlation between organizational culture and planning. There was a significant difference in sex, however, there was no significant difference in age. The implication is that hospitals must make sure that their firms’ culture supports planning so that they can meet their objectives.  Discussion: The study recommends that hospitals must make sure that their cultural practices support planning. This is among the various ways to achieve hospitals success and make them effective and efficient.

The aim of the present paper is to evaluate the Malaysian Toxicological Database (MyToxData), a newly developed system for toxicological data management. MyToxData is a local system for collection of case reporting and toxicity statistics as well as its database of chemical products and toxicity information. An online questionnaire using JotForm was distributed among healthcare professionals in Malaysian hospitals. The participants were asked to perform tasks on MyToxData system before rating their satisfaction experience. The questionnaire was adopted from previous literature and comprised 12 items, each having a five-point scale that ranged from Strongly Disagree to Strongly Agree. The collected data were then analysed using the SPSS software package. Based on the results, the satisfaction testing showed a high satisfaction score for the login at 95.7% and case registration and toxicological/poisoning case notification at 65.2%. The poisoning substance identification component received a 78.2% agreement on the usefulness with a very high acceptance of 91.3% on the treatment and clinical information retrieval made available on the system. One sample t-test found that all constructs for the satisfaction testing of MyToxData, namely, accuracy, content, ease of use, format and timeliness, were significant. The higher acceptance by healthcare professionals on the satisfaction testing of MyToxData has identified the effectiveness of the development and subsequent usage of a dedicated system for toxicological data management.

Abstract. Background: Worldwide prevalence of peripheral artery disease (PAD) is increasing and peripheral vascular intervention (PVI) has become the primary invasive treatment. There is evidence that multidisciplinary team decision-making (MTD) has an impact on in-hospital outcomes. This study aims to depict practice patterns and time changes regarding MTD of different medical specialties. Methods: This is a retrospective cross-sectional study design. 20,748 invasive, percutaneous PVI of PAD conducted in the metropolitan area of Hamburg (Germany) were consecutively collected between January 2004 and December 2014. Results: MTD prior to PVI was associated with lower odds of early unsuccessful termination of the procedures (Odds Ratio 0.662, p < 0.001). The proportion of MTD decreased over the study period (30.9 % until 2009 vs. 16.6 % from 2010, p < 0.001) while rates of critical limb-threatening ischemia (34.5 % vs. 42.1 %), patients´ age (70 vs. 72 years), PVI below-the-knee (BTK) (13.2 % vs. 22.4 %), and rates of severe TASC C/D lesions BTK (43.2 % vs. 54.2 %) increased (all p < 0.001). Utilization of MTD was different between medical specialties with lowest frequency in procedures performed by internists when compared to other medical specialties (7.1 % vs. 25.7 %, p < 0.001). Conclusions: MTD prior to PVI is associated with technical success of the procedure. Nonetheless, rates of MTD prior to PVI are decreasing during the study period. Future studies should address the impact of multidisciplinary vascular teams on long-term outcomes.

The purpose of this study to determine the motives underlying adolescents to make early marriage and the impact caused in the household as a result of the marriage.

Chatbot has emerged as a significant resolution to the swiftly growing customer caredemands in recent times. Chatbot has emerged as one of the biggest technological disruption. Simply speaking, it is a software agent facilitating interaction between computers and humans in natural language. So basically, it is a simulated, intellectual dialogue agent functional in a range of consumer engagement circumstances. It is the easiest and simplest means enable interaction between the retailers and the customers. </p><p> • Purpose- Most of the research work done in this field is concerned with their technical aspects. The recent research on chatbot pay little attention to the impact it is creating on users’ experience. Through this work, author is making an effort to know the customer-oriented impact that the chatbot bear on the shoppers. The purpose of this study is to develop and empirically test a framework that identify the customer oriented attributes of chatbot and impact of these attributes on customers. </p><p> • Objectives- The study intends to bridge the gap between concepts and actual attributes and applications on the subject of Chatbot. The following research objectives can address the various aspects of Chatbot affecting the different characteristics of consumers shopping behaviors: a) Identify the various attributes of chatbot that bears an impression on consumer shopping behavior. b) Evaluate the impact of chatbot on consumer shopping behavior that leads to the development of chatbot usage and adoption among the customer. </p><p> • Design/Methodology/Approach – For the purpose of analysis, author has administered Factor analysis and Multiple regression using SPSS version 23 for identification of various attributes of Chatbot and knowing their impact on shoppers. A self-administered questionnaire from the review of literature is developed. Industry experts in the field of retailing and academician evaluate the questionnaire. Primary information from the respondents is gathered using this questionnaire. The questionnaire comprises of Likert scale on a scale of 1 to 5 where 1 stands for strongly disagree and 5 stands for strongly agree. Data is collected from 126 respondents, out of which 111 respondents were finally considered for study and analysis purpose. </p><p> • Findings – The empirical results show that the study identifies various attributes of chatbot like Trust, Usefulness, Satisfaction, Readiness to Use and Accessibility. It is also found that chatbot is really influencing the customers in providing them with shopping experience, which can be very helpful to the businesses for increasing the sales and creating repurchase intention among the customers. </p><p> • Originality/value – The recent research on chatbot pay little attention to the impact it is creating on customers who are actually interacting with it on regular basis. The research paper extends information for understanding and appreciating the customer oriented attributes of artificially intelligent Chatbot. In this regard, the author has developed a model framework and proposed the attributes identified. Through the work, author is also making an effort to test empirically the impact of the identified attributes on the shoppers.

Much eighteenth-century Dissenting educational activity was built on an older tradition of Puritan endeavour. In the middle of the seventeenth century, the godly had seen education as an important tool in spreading their ideas but, in the aftermath of the Restoration, had found themselves increasingly excluded from universities and schools. Consequently, Dissenters began to develop their own higher educational institutions (in the shape of Dissenting academies) and also began to set up their own schools. While the enforcement of some of the legal restrictions that made it difficult for Dissenting institutions diminished across the eighteenth century, the restrictions did not disappear entirely. While there has been considerable focus on Dissenting academies and their contribution to debates about doctrinal orthodoxy, the impact of Dissenting schools was also considerable.


The prevalence of cognitive impairment caused by neurodegenerative diseases and other neurologic disorders associated with aging is expected to rise dramatically between now and year 2050, when the population of Americans aged 65 or older will nearly double. Cognitive impairment also commonly occurs in other neurologic conditions, as well as in non-neurologic medical disorders (and their treatments), idiopathic psychiatric illnesses, and adult neurodevelopmental disorders. Cognitive impairment can thus infiltrate all aspects of healthcare, making it necessary for clinicians and clinical researchers to have an integrated knowledge of the spectrum of adult cognitive disorders. The Oxford Handbook of Adult Cognitive Disorders is meant to serve as an up-to-date, scholarly, and comprehensive volume covering most diseases, conditions, and injuries resulting in impairments in cognitive function in adults. Topics covered include normal cognitive and brain aging, the impact of medical disorders (e.g., cardiovascular, liver, pulmonary) and psychiatric illnesses (e.g., depression and bipolar disorder) on cognitive function, adult neurodevelopmental disorders (e.g., Down Syndrome, Attention Deficit/Hyperactivity Disorder), as well as the various neurological conditions (e.g., Alzheimer’s disease, chronic traumatic encephalopathy, concussion). The theory of the vibrations of the pianoforte string put forward by Kaufmann in a well-known paper has figured prominently in recent discussions on the acoustics of this instrument. It proceeds on lines radically different from those adopted by Helmholtz in his classical treatment of the subject. While recognising that the elasticity of the pianoforte hammer is not a negligible factor, Kaufmann set out to simplify the mathematical analysis by ignoring its effect altogether, and treating the hammer as a particle possessing only inertia without spring. The motion of the string following the impact of the hammer is found from the initial conditions and from the functional solutions of the equation of wave-propagation on the string. On this basis he gave a rigorous treatment of two cases: (1) a particle impinging on a stretched string of infinite length, and (2) a particle impinging on the centre of a finite string, neither of which cases is of much interest from an acoustical point of view. The case of practical importance treated by him is that in which a particle impinges on the string near one end. For this case, he gave only an approximate theory from which the duration of contact, the motion of the point struck, and the form of the vibration-curves for various points of the string could be found. There can be no doubt of the importance of Kaufmann’s work, and it naturally becomes necessary to extend and revise his theory in various directions. In several respects, the theory awaits fuller development, especially as regards the harmonic analysis of the modes of vibration set up by impact, and the detailed discussion of the influence of the elasticity of the hammer and of varying velocities of impact. Apart from these points, the question arises whether the approximate method used by Kaufmann is sufficiently accurate for practical purposes, and whether it may be regarded as applicable when, as in the pianoforte, the point struck is distant one-eighth or one-ninth of the length of the string from one end. Kaufmann’s treatment is practically based on the assumption that the part of the string between the end and the point struck remains straight as long as the hammer and string remain in contact. Primâ facie , it is clear that this assumption would introduce error when the part of the string under reference is an appreciable fraction of the whole. For the effect of the impact would obviously be to excite the vibrations of this portion of the string, which continue so long as the hammer is in contact, and would also influence the mode of vibration of the string as a whole when the hammer loses contact. A mathematical theory which is not subject to this error, and which is applicable for any position of the striking point, thus seems called for.

Objectives As a cross-section discipline within the hospital infrastructure, radiological departments might be able to provide important information regarding the impact of the COVID-19 pandemic on healthcare. The goal of this study was to quantify changes in medical care during the first wave of the pandemic using radiological examinations as a comprehensive surrogate marker and to determine potential future workload. Methods A retrospective analysis of all radiological examinations during the first wave of the pandemic was performed. The number of examinations was compared to time-matched control periods. Furthermore, an in-depth analysis of radiological examinations attributed to various medical specialties was conducted and postponed examinations were extrapolated to calculate additional workload in the near future. Results A total of 596,760 examinations were analyzed. Overall case volumes decreased by an average of 41 % during the shutdown compared to the control period. The most affected radiological modalities were sonography (–54 %), X-ray (–47 %) followed by MRI (–42 %). The most affected medical specialty was trauma and orthopedics (–60 % case volume) followed by general surgery (–49 %). Examination numbers increased during the post-shutdown period leading to a predicted additional workload of up to 22 %. Conclusion This study shows a marked decrease in radiological examinations in total and among several core medical specialties, indicating a significant reduction in medical care during the first COVID-19 shutdown. Abstract Purpose The purpose of the present cross-sectional study is to investigate the role of perceived COVID-19-related organizational demands and threats in predicting emotional exhaustion, and the role of organizational support in reducing the negative influence of perceived COVID-19 work-related stressors on burnout. Moreover, the present study aims to add to the understanding of the role of personal resources in the Job Demands-Resources model (JD-R) by examining whether personal resources—such as the professionals’ orientation towards patient engagement—may also strengthen the impact of job resources and mitigate the impact of job demands. Methods This cross-sectional study involved 532 healthcare professionals working during the COVID-19 pandemic in Italy. It adopted the Job-Demands-Resource Model to study the determinants of professional’s burnout. An integrative model describing how increasing job demands experienced by this specific population are related to burnout and in particular to emotional exhaustion symptoms was developed. Results The results of the logistic regression models provided strong support for the proposed model, as both Job Demands and Resources are significant predictors (OR = 2.359 and 0.563 respectively, with p < 0.001). Moreover, healthcare professionals’ orientation towards patient engagement appears as a significant moderator of this relationship, as it reduces Demands’ effect (OR = 1.188) and increases Resources’ effect (OR = 0.501). Conclusions These findings integrate previous findings on the JD-R Model and suggest the relevance of personal resources and of relational factors in affecting professionals’ experience of burnout.
