Exercise in the Treatment of Claudication: Assessment and Treatment of Functional Impairment

Claudication is an important cause of impaired exercise capacity, which limits a patient's ability to walk and thus to meet the personal, social and occupational demands of daily life. Given that improvement of the walking impairment is the primary goal of treating claudication, exercise performance and functional status of patients with claudication should be measured before and after any intervention. Assessment of functional status in both the laboratory and community settings is important so that the various treatments can be judged as to relative efficacy. Studies have shown that exercise training programs have a clinically important impact on functional capacity in persons with claudication. A meta-analysis showed that pain-free walking time increased 180% and maximal walking time increased 120% in claudicants who participated in an exercise program. Substantial improvements have been found in walking speeds and distances (65% and 44%, respectively), caloric expenditure (31%) and physical functioning (67%).

Abstract Background Physical activity and exercise are among the most important, simplest, and cheapest approaches to anxiety treatment, especially for the elderly. Their positive effects on improvement of mental disorders in the elderly have attracted a considerable level of attention. Therefore, the present study was conducted to determine the effect of sport on reducing anxiety in the elderly using meta-analysis. Methods In this study, national and international databases of SID, MagIran, IranMedex, IranDoc, Cochrane, Embase, ScienceDirect, Scopus, PubMed, and Web of Science were searched to find studies published electronically from 1999 to 2019. Heterogeneity between the collected studies was determined using the Cochran's test (Q) and I2. Due to presence of heterogeneity, the random effects model was used to estimate the standardized mean difference of sport test scores obtained from the measurement of anxiety reduction among the elderly, between the intervention group before and after the test. Results In this meta-analysis and systematic review, 19 papers finally met the inclusion criteria. The overall sample size of all collected studies for the meta-analysis was 841 s. Mean anxiety score before and after intervention were 38.7 ± 5.6 33.7 ± 3.4 respectively, denoting a decrease in anxiety score after intervention. Conclusion Results of this study indicates that Sport significantly reduces Anxiety in the Elderly. Therefore, a regular exercise program can be considered as a part of the elderly care program.

The objective of this study is to assess the impact of deceleration training method on improving dynamic balance skills of young soccer players in comparison with the conventional training methods. This study included 26 young soccer players (mean age: 13.54±1.48, height: 154.98±12.44, weight: 46.16±11.06, Body Mass Index 18.96±2.21 kg/m2, fat percentile 18.22%±3.22). Thirteen players participated in the deceleration training group (DTG) group and thirteen players participated in the conventional training group (CTG). The training programs were executed on three nonconsecutive days in a week for a total of eight weeks. Y-balance tests were conducted both before and after the eight week training programs. The differences between the averages were analyzed with Wilcoxon, comparison between the groups were analyzed with Mann-Whitney-U post hoc analyses. There was no difference in left-right balance variation (p > 0.05) performance measurements between DTG and CTG before the exercise program. However, after the 8 week training programs, the performance measurements between DTG and CTG were significantly different (p < 0.05) and the dynamic balance skill development of the DTG were more advanced. Based on these results, the deceleration training method seems to be a more effective method in developing dynamic balance skills than conventional training methods.

The present study aimed to investigate the impact of a 6-month multicomponent exercise program (MCEP) on physical function, cognition, and hemodynamic parameters of elderly normotensive (NTS) and hypertensive (HTS) osteoarthritis patients. A total of 99 elderly osteoarthritis patients (44 NTS and 55 HTS) were recruited and submitted to functional, cognitive, and hemodynamic evaluations before and after six months of a MCEP. The program of exercise was performed twice a week at moderate intensity. The physical exercises aggregated functional and walking exercises. Results indicate that 6 months of MCEP were able to improve one-leg stand and mobility (walking speeds) of osteoarthritis patients regardless of hypertension. On the other hand, cognitive and hemodynamic parameters were not altered after the MCEP. The findings of the present study demonstrate that 6 months of MCEP were able to improve the physical functioning (i.e., usual and maximal walking speed and balance) of osteoarthritis patients regardless of hypertensive condition.

The chronic adaptations of the cardiovascular system to exercise training in children and adolescents yield inconclusive results, in spite of previous meta-analytic evidence. The aim of this meta-analysis was to determine the effectiveness of exercise training on the VO2max of children and adolescents. The studies meta-analyzed met the following inclusion criteria: a) healthy individuals, b) participants between 10 and 18 years old, c) being trained in some type of supervised exercise program, d) VO2max reported in ml·kg-1·min-1, e) the study reported before and after mean and standard deviation, f) pre-test post-test research designs, and g) the study was published in a peer-reviewed journal up to February, 2013. Thirty-four studies and 96 effect sizes (ES) were obtained. Compared to controls, experimental groups showed a significant improvement in VO2max (p < 0.01) following exercise training. The increase in VO2max was maintained regardless of the sample size characteristics and the exercise protocol. Based on the findings of this study, in order for children and adolescents to improve their VO2max, exercise training should include at least two sessions per week of > 20 minutes duration per session, performed at a moderate intensity. In summary, planned exercise training improves VO2max in children and adolescents.Key words. VO2max, aerobic power, training, children, adolescents, meta-analysis.Resumen. Las adaptaciones crónicas del sistema cardiovascular al entrenamiento físico en niños y adolescentes arrojan resultados inconcluyentes, a pesar de previa evidencia meta analítica. El objetivo de este meta-análisis fue determinar la efectividad de la práctica de ejercicio en el VO2máx de los niños y adolescentes. Los estudios meta analizados cumplieron los siguientes criterios de inclusión: a) individuos sanos, b) participantes entre 10 y 18 años de edad, c) entrenados en algún tipo de programa de ejercicio supervisado, d) VO2máx reportado en ml•kg-1•min-1, e) se contara con desviación estándar antes y después del experimento, f) diseños de investigación con pre-test y post-test, y g) estudio publicado en una revista revisada por pares hasta febrero de 2013. Se obtuvieron treinta y cuatro estudios y 96 tamaños del efecto (ES). En comparación con los controles, los grupos experimentales mostraron una mejora significativa en el VO2máx (p < 0.01) después del entrenamiento de ejercicio físico. El aumento en el VO2máx se mantuvo independientemente de las características de tamaño de la muestra y el protocolo de ejercicio. Con base en los hallazgos de este estudio, a fin de que los niños y adolescentes puedan mejorar su VO2máx, el entrenamiento debe incluir por lo menos dos sesiones por semana > 20 min de duración por sesión, realizados a una intensidad moderada. This paper reports on a quasi-experimental study that aimed to identify changes in muscle function (i.e., mobility, maximal walking speed, lower limb muscle strength, balance, and transfer capacity), cognition (i.e., executive function) and hemodynamic parameters of community-dwelling Brazilian older adults during a six-month multicomponent exercise program (MCEP). A total of 436 community-dwelling older adults performed functional, cognitive and hemodynamic assessments before and after a six-month MCEP. The program of exercise was performed twice a week over 26 weeks at moderate intensity. Results indicate that balance, mobility (i.e., usual and maximal walking speeds) and transfer capacity (p < 0.05) were significantly improved after the MCEP. Moreover, all hemodynamic parameters (i.e., systolic, diastolic and mean arterial pressures), except for heart rate (p > 0.05), were significantly reduced after the intervention. The current findings indicate that a six-month MCEP may provide physical and hemodynamic benefits in community-dwelling older adults. Nevertheless, our findings need to be confirmed in larger samples and better designed studies.

Purpose Semi-occluded vocal tract (SOVT) exercises with tubes or straws have been widely used for a variety of voice disorders. Yet, the effects of longer periods of SOVT exercises (lasting for weeks) on the aging voice are not well understood. This study investigated the effects of a 6-week straw phonation in water (SPW) exercise program. Method Thirty-seven elderly subjects with self-perceived voice problems were assigned into two groups: (a) SPW exercises with six weekly sessions and home practice (experimental group) and (b) vocal hygiene education (control group). Before and after intervention (2 weeks after the completion of the exercise program), acoustic analysis, auditory–perceptual evaluation, and self-assessment of vocal impairment were conducted. Results Analysis of covariance revealed significant differences between the two groups in smoothed cepstral peak prominence measures, harmonics-to-noise ratio, the auditory–perceptual parameter of breathiness, and Voice Handicap Index-10 scores postintervention. No significant differences between the two groups were found for other measures. Conclusions Our results supported the positive effects of SOVT exercises for the aging voice, with a 6-week SPW exercise program being a clinical option. Future studies should involve long-term follow-up and additional outcome measures to better understand the efficacy of SOVT exercises, particularly SPW exercises, for the aging voice.

AbstractBackgroundTherapeutic management of pain in patients with complex regional pain syndrome (CRPS) is challenging. Repetitive transcranial magnetic stimulation (rTMS) has analgesic effects on several types of pain. However, its effect on CRPS has not been elucidated clearly. Therefore, we conducted a meta-analysis of the available clinical studies on rTMS treatment in patients with CRPS.Materials and methodsA comprehensive literature search was conducted using the PubMed, EMBASE, Cochrane Library, and SCOPUS databases. We included studies published up to February 09, 2020, that fulfilled our inclusion and exclusion criteria. Data regarding measurement of pain using the visual analog scale before and after rTMS treatment were collected to perform the meta-analysis. The meta-analysis was performed using Comprehensive Meta-analysis Version 2.ResultsA total of three studies (one randomized controlled trial and two prospective observational studies) involving 41 patients were included in this meta-analysis. No significant reduction in pain was observed immediately after one rTMS treatment session or immediately after the entire schedule of rTMS treatment sessions (5 or 10 sessions; P > 0.05). However, pain significantly reduced 1 week after the entire schedule of rTMS sessions (P < 0.001).ConclusionrTMS appears to have a functional analgesic effect in patients with CRPS.
