An investigation into ethical issues in occupational therapists in adult with physical disabilities: Using the qualitative approach

Background Occupational therapists may be encountered with a variety of ethical issues. The aim of this study was to explore ethical issues of Occupational therapist’s practice in adult physical dysfunction field. Methods Ten graduated Occupational therapists were selected by purposive sampling method. Data were gathered by semi-structured interview. Data were analyzed by content analysis approach. Results Data analysis ultimately leads to the emergence of three themes which reflects Ethical issues in Occupational Therapy. These themes include: unethical practice of Occupational therapists, factors influencing ethical practice and Strategies to improving ethical practice. Conclusion The findings show that occupational therapists have various ethical problems related to client’s rights in addition to their clinical practice, that according to the participants in this study, lack of awareness about professional ethics related to the occupational therapy and also lack of comprehensive monitoring rules in this field are the major causes of unethical practice.

Therapists have always encountered ethical problems at work. However, these are becoming more important as changes in health care systems take place and patients' rights and autonomy become dominant in practice. An introduction is given to ethical theories and processes, and these are related to clinical issues identified during a series of research studies with occupational therapists and physiotherapists.

Em 2008, foi criado, pelo Ministério da Saúde, o Núcleo de Apoio à Saúde da Família (NASF), com o intuito de apoiar as equipes da Estratégia de Saúde da Família (ESF) nas redes de saúde e ampliar a ação da Atenção Básica da Saúde. O NASF é composto por uma equipe multidisciplinar e o terapeuta ocupacional é um dos integrantes desta equipe. O objetivo deste trabalho é o de conhecer a atuação dos terapeutas ocupacionais nos NASFs do município de Maceió, Alagoas, e suas reflexões sobre o trabalho desempenhado. Trata-se de uma pesquisa qualitativa, sendo a coleta de dados realizada por meio de entrevista semiestruturada. Como resultados, foram elencadas três categorias temáticas: A atuação do terapeuta ocupacional no NASF: ações e atividades efetuadas; Os principais obstáculos encontrados pelo terapeuta ocupacional no NASF e As ferramentas tecnológicas utilizadas para a realização do trabalho no NASF. A Terapia Ocupacional ainda é uma profissão que busca seu reconhecimento no NASF. Visitas domiciliares, salas de espera e atividades em grupo foram identificadas como ações desempenhadas pelos profissionais, apesar das dificuldades como falta de recursos materiais e de uma estrutura física melhor. Este estudo serviu para enfatizar a importância do trabalho do terapeuta ocupacional no serviço, bem como as dificuldades encontradas por eles, citando assim a falta de reconhecimento/conhecimento da profissão, recursos e espaço. Apesar das dificuldades mencionadas, os mesmos sempre buscam realizar um atendimento humanizado, preocupando-se com os usuários, minimizando os riscos e promovendo a saúde. AbstractIn 2008, the Family Health Support Center (NASF) was created by the Ministry of Health to support the Family Health Strategy (ESF) teams in health networks and expand the action of Primary Care of health. The NASF is composed of a multidisciplinary team and the occupational therapist is one of the members of this team. The objective of this work is to know the performance of occupational therapists in the NASFs of the city of Maceió-AL, and their reflections on the work performed. This is qualitative research, and the data collection is done through a semi-structured interview with occupational therapists of the NASFs in the municipality of Maceió-AL. As a result, three thematic categories were listed: The work of the occupational therapist in the NASF: actions and activities carried out; the main obstacles encountered by the occupational therapist in NASF; at technological tools used to perform work in the NASF. Occupational Therapy is still a profession that seeks recognition in the NASF. Home visits, waiting rooms and group activities were identified as actions performed by professionals, despite difficulties such as lack of material resources and a better physical structure. This study contributed to emphasize the practice of the work of the occupational therapist in the service, as well as the difficulties encountered to know: citing the lack of recognition/knowledge of the profession, resources and space. Este estudio contribuido para enfatizar la práctica del terapeuta ocupacional en el servicio, así como las dificultades a saber:la falta de reconocimiento / conocimiento de la profesión, recursos y espacio. Continuing competence is essential to occupational therapists and occupational therapy assistants for fulfilling their roles and abilities across experience, context, and time. It requires an ongoing process to keep up with new developments related to the profession and specialty areas throughout one’s career (i.e., early, mid-, late, change, or reentry). To build capacity, occupational therapists and occupational therapy assistants must commit to a process of self-assessment, reflecting on, in, and toward action to advance the knowledge, professional reasoning, interpersonal skills, performance skills, and ethical practice necessary to perform current and future roles and responsibilities within the profession. The American Occupational Therapy Association’s Standards for Continuing Competence serve as a foundation for analyzing the occupation in regard to continuing competence. These standards can be viewed separately and collectively and can be combined with other standards documents to gain an overarching perspective.

This qualitative study was designed to explore ethical issues in public health nursing in the Canadian context, and to begin to identify strategies to support ethical practice. Twenty-two public health nurses, 11 in rural and 11 in urban settings, were asked to describe ethical problems they had experienced in the course of their work. These participants most often described situations that required a relational response rather than an active choice between options. Their goal was to optimize the good, while at the same time maintaining a supportive relationship. Analysis revealed five interrelated themes, each with several subthemes: relationships with health care professionals; systems issues; character of relationships; respect for persons; and putting self at risk. It was clear that all aspects of public health nursing have ethical components.

This article discusses the ethical issues facing rehabilitation counseling supervisors and administrators (RCS/As). A brief overview of supervision in rehabilitation counseling is provided in addition to: a) an examination of common ethical issues in rehabilitation counseling supervision, b) the availability of ethical guidance, and c) uses and limitations of the recently revised Code of Professional Ethics for Rehabilitation Counselors. Recommendations for facilitating and supporting ethical practice are also presented.

Objectives: Considering the significant role of physiotherapists in the process of rehabilitation, their ethical commitment is essential to successful interaction and care provision. However, investigations on the medical professional ethics ​​in Iran are limited. Generally, research in this regard is rare in Asia. Thus, such studies could improve the moral knowledge of the Iranian physiotherapy community. Accordingly, this study aimed to explore professional ethics issues in physiotherapy to provide a platform concerning the challenges of professional ethics in physiotherapy in Iran. Methods: This qualitative study was conducted in 2016-2017 using the content analysis method. The study samples were recruited through purposive sampling approach until data saturation (12 physiotherapists). The required data were gathered by an in-depth semi-structured interview. All of the interviews were transcribed and analyzed, inductively. Results: Physiotherapists in Ahvaz and Tehran cities, Iran experienced challenges in 6 different categories during daily practice. These aspects included the following: therapists’ self-interest-craving, observing patients’ rights, maintaining professional competence, the effect of workplace on ethical conduct, personal ethical outlook, and insufficient professional ethics education. This study implicated the existence of a trend of kick-backs in the physiotherapy community; a problematic trend in the medical community, i.e. addressed by physicians in several articles. Additionally, the lack of receiving ethical education leads to ethical judgments based on personal values rather than ethical codes. Anderson’s Foundations of Ethical Practice, Research, and Teaching in Psychology and Counseling (2011) they use the term practical wisdom or prudence as a way to make right decisions in real life situation. The authors lay the foundation for conceptually dealing with ethical problems for psychologists, counselors, students, and trainees. The book is in two parts. In the first six chapters, the authors focus on the foundations of ethical reasoning. The next part focuses on the ethical issues psychologists and counselors are confronted with in their roles.

Introduction Over the past decade, yoga has gained popularity in health care. The benefits of yoga can go beyond the physical to address the psychosocial and emotional. Such gains can be beneficial for people who have had a stroke. As a leading cause of disability, stroke can lead to challenges returning to daily activities. Occupational therapists may use yoga to promote engagement in daily activities after stroke. This study aimed to understand how and why occupational therapists integrate yoga into stroke rehabilitation. Methods This study involved occupational therapists practicing in the United States. Ten occupational therapists using yoga in practice participated in a semi-structured interview. Interview data were transcribed and analyzed using deductive and inductive coding. Themes emerged from the data related to the benefits of using yoga in stroke rehabilitation. Results According to this study, yoga is currently being used as a treatment technique and merged with occupational therapy. Yoga may promote client-centered recovery; bring “context” to therapy; and address physical, emotional, and psychosocial needs following stroke. Conclusion Individuals who have had a stroke may face challenges related to both physical and psychosocial engagement. The use of yoga in occupational therapy may be beneficial after stroke for both physical and psychosocial rehabilitation.

This first article outlines how student research projects have developed over the past 10 years, resulting in the subsequent need to expand the subject of ethical reasoning in the education of occupational therapists. The literature is examined in respect of providing learning opportunities on ethical matters for the student, particularly in the preparation of an investigative study, the formation and administration of ethical screening procedures with in a school of occupational therapy are described as a means of providing an experiential understanding of ethical issues for students. In a second article, the implications and outcome of this initiative are discussed.

This study aimed to establish the attitudes of occupational therapists towards chronic fatigue syndrome. Previously, no study had investigated the attitudes of health professionals towards this condition, which may be a factor in maintaining the uncertainty surrounding chronic fatigue syndrome. Twenty occupational therapists practising in either physical or mental health settings were recruited to this study. In order to achieve a depth of knowledge not gained by previous studies, the data were collected through a combination of quantitative and qualitative methodology by using a structured questionnaire and a semi-structured interview; the findings were analysed statistically and thematically. At variance with the literature, the results revealed a neutral to positive attitude towards chronic fatigue syndrome, with no significant difference in responses from participants practising in physical or mental health settings. A unique and valuable role was clearly identified for the profession of occupational therapy within the wider multidisciplinary framework. However, as 16 (80%) of the participants had met individuals with chronic fatigue syndrome, the lack of knowledge and need for further education and training were highlighted. Continuing professional development is fundamental in order to provide appropriate, sensitive and holistic services in the future for all individuals with chronic fatigue syndrome.
