Abstract Background The COVID-19 pandemic, as well as efforts to prevent its spread, have had a strong impact on the delivery of rehabilitative services in Germany. While several studies have addressed the impact of these developments on health service providers and COVID-19 patients, little is known about its impact on patients in need of rehabilitative treatment because of other conditions. This study aims to identify expectations, concerns and experiences of rehabilitation patients related to service delivery in this situation. Methods Using a qualitative study design, user posts from six German online forums between March and Mid-November 2020 were systematically searched with respect to experiences, concerns and expectations of health care users toward receiving rehabilitative treatment. We used qualitative content analysis with inductive coding as our methodological approach. Results Users fearing physical or psychological impairment were concerned about not receiving timely or effective treatment due to closed hospitals, reduced treatments and limited admissions. In contrast, patients more concerned about getting infected with COVID-19 worried about the effectiveness of protective measures and being denied postponement of treatment by the funding bodies. During their stay, some patients reported feeling isolated due to contact restrictions and did not feel their treatment was effective, while others reported being satisfied and praised hospitals for their efforts to ensure the safety of the patients. Many patients reported communication problems before and during their treatment, including concerns about the safety and effectiveness of their treatment, as well as financial concerns and worries about future treatments. Several users felt that their concerns were disregarded by the hospitals and the funding bodies, leaving them feeling distressed, insecure and dissatisfied. Conclusions While some users report only minor concerns related to the pandemic and its impact on rehabilitation, others report strong concerns relating not only to their own health and safety, but also to financial aspects and their ability to work. Many users feel ignored and disregarded, showing a strong need for more coordinated strategies and improved communication specifically with funding bodies like health insurance companies and the German pension funds.

Facilities are important aspects of rehabilitative treatment. To fully understand the impact of a facility on the effectiveness of treatment, the direct perspectives of service providers are critical. We examine four autoethnographic accounts from correctional service providers to determine the role of the facility in their provision of care and organize these into considerations of three central aspects of correctional architecture—space, layout, and setting (SLS)—which play an important role in the efficacy of rehabilitative services. We conclude by proposing how research may advance in this area, especially through the use of practitioner accounts.

Background Although the COVID-19 pandemic has significantly boosted the implementation of digital services worldwide, it has become increasingly important to understand how these solutions are integrated into professionals’ routine work. Professionals who are using the services are key influencers in the success of implementations. To ensure successful implementations, it is important to understand the multiprofessional perspective, especially because implementations are likely to increase even more. Objective The aim of this study is to examine health and social care professionals’ experiences of digital service implementations and to identify factors that support successful implementations and should be considered in the future to ensure that the services are integrated into professionals’ routine work. Methods A qualitative approach was used, in which 8 focus group interviews were conducted with 30 health and social care professionals from 4 different health centers in Finland. Data were analyzed using qualitative content analysis. The resulting categories were organized under the components of normalization process theory. Results Our results suggested 14 practices that should be considered when implementing new digital services into routine work. To get professionals to understand and make sense of the new service, (1) the communication related to the implementation should be comprehensive and continuous and (2) the implementation process should be consistent. (3) A justification for the service being implemented should also be given. The best way to engage the professionals with the service is (4) to give them opportunities to influence and (5) to make sure that they have a positive attitude toward the service. To enact the new service into professionals’ routine work, it is important that (6) the organization take a supportive approach by providing support from several easy and efficient sources. The professionals should also have (7) enough time to become familiar with the service, and they should have (8) enough know-how about the service. The training should be (9) targeted individually according to skills and work tasks, and (10) it should be diverse. The impact of the implementation on the professionals’ work should be evaluated. The service (11) should be easy to use, and (12) usage monitoring should happen. An opportunity (13) to give feedback on the service should also be offered. Moreover, (14) the service should support professionals’ work tasks. Conclusions We introduce 14 practices for organizations and service providers on how to ensure sustainable implementation of new digital services and the smooth integration into routine work. It is important to pay more attention to comprehensive and continuing communication. Organizations should conduct a competence assessment before training in order to ensure proper alignment. Follow-ups to the implementation process should be performed to guarantee sustainability of the service. Our findings from a forerunner country of digitalization can be useful for countries that are beginning their service digitalization or further developing their digital services.

The mixed methods study presented in this paper investigates the influence of planning in oral teaching methods.. This research article presented examines the impact of planned and unplanned conditions on three different variables in the context of oral teaching methods: fluency, vocabulary complexity, and accuracy. As a research tool two narrative tasks were used which required the participant construct linguistic forms based on visual information, requiring the use of the participant’s imagination. With the help of qualitative content analysis, the most common ‘pattern-finding’ moves were described from the collected data in detail. In order to compare data from unplanned and planned conditions, three analyses were conducted with the help of three software tools: D-tool, P-lex, and Coh-Metrix. The results showed that pre-task planning time had a positive effect on lexical sophistication and accuracy. However, fluency under unplanned conditions was greater than under planned conditions, suggesting that the planning process does not produce a strong impact on fluency. According to most of the participants, they paid close attention to the logical order of the pictures, organization of ideas, and the coherence of their utterance while using their individual planning time.

14 Background: Studies have described financial burden during and after cancer treatment, but little data are available on financial burden to patients and caregivers at end of life. We conducted a qualitative study with bereaved caregivers and discussed the impact of cancer-associated medical and nonmedical costs on family finances. Methods: From October-December 2017, we audio-recorded semi-structured phone interviews with 35 adults who were primary caregivers for a family member who died with cancer (July-December 2016). We asked about ability to meet costs of care. Drawing from previous studies, we defined substantial financial burden as incurring debt, selling assets or draining savings to meet health care costs. Two analysts coded transcripts using qualitative content analysis and summarized results as major themes. Results: Patients ranged from 30-88 years old, caregivers 34-85. Most caregivers were white (91%), female (63%) and spouses (89%). Ten patients (29%) were insured commercially, 24 (69%) had Medicare, and one (2%) had Medicaid. Ten caregivers (29%) reported substantial financial burden, irrespective of patients’ insurance coverage. Issues associated with substantial financial burden were: 1) insurance hurdles: “my husband’s dying and…I have to run interference with insurance companies, which is frustrating;” 2) limited financial resources: “It is hard when…you really don't have a lot of savings because…you never really made a lot of money” and 3) financial burden that continues after the patient’s death : “I went broke, basically, looking after my parents.” “We lost two incomes and we had to live on our savings.” Three other caregivers noted their family did not experience financial burden because they switched health insurance plans or had a supplemental insurance plan. Conclusions: In this pilot study of caregivers of advanced cancer patients, a substantial portion of commercially insured patients and Medicare enrollees with limited savings and income reported financial burden associated with cancer treatment and end-of-life care that persisted after the patient’s death. Future research is needed to determine best ways to mitigate financial burden among patients and their survivors.

Abstract Direct support professional (DSP) turnover significantly impacts both human service providers and people with intellectual and developmental disabilities (IDD). This study explored how DSP turnover impacts people with IDD's health and safety. We analyzed secondary data regarding DSP turnover and health and safety (i.e., emergency room visits, incidents of abuse and neglect, injuries, and behavioral events) from a random sample of 251 people with IDD. Findings revealed, regardless of their support needs, people who experienced DSP turnover had more emergency room visits, experienced more instances of abuse and neglect, and had more injuries than people who did not experience DSP turnover. Our findings suggest extended tenure of DSPs can help promote the health and safety of people with IDD.

The paper proposes a methodological approach to evaluating the impact of the transformation taking place in the economies of cities of regional significance (CRSs) on the socio-economic development of regions (oblasts). The analysis is conducted for cities of regional significance in the western part of Ukraine. The article emphasizes the asymmetry of regional socio-economic development as a result of the concentration of business activity and capital in the CRSs, a higher level of reception of growing direct foreign investment in the economies of CRSs compared to capital investment; a strong impact of structural changes in the economies of CRS on the Gross Regional Product (GRP). The authors found that the development of most CRSs was based on industrial production. Only half of the oblasts in Western Ukraine analyzed in the study showed evidence indicating that economies of their CRSs are undergoing a transformation towards service economy (which should correspond to the post-industrial development of the country).

Abstract As adolescents with intellectual disability (ID) transition to adulthood, there is a need to plan for effective community-based supports that address the post-school life. There is also a need to plan for the impact of factors (e.g., medical/behavioral support needs) on supports needed for community participation. Data from the Supports Intensity Scale—Adult Version (SIS—A) was used to examine relations between medical/behavior support needs and support needs assessed in the standardized portion of the SIS—A. Results suggested that the presence of medical/behavioral needs had a strong impact on supports needed to participate in the community activities, and that more intensive medical support needs were related to higher support needs in the Home Living, Community Living, and Health and Safety domains.

This study has addressed the impact of domestic investment and FDI net flow from the services sector on economic growth for a period of 10 years. The data are taken from CBK, in annual periods, using the technique of Pearson correlation and multiple OLS regression, conducted with the statistical program SPSS 21.0. FDI attraction by the services sector is found to be greater than by the other sectors. This becomes more evident by findings from econometric analysis, where the services sector has a strong impact on economic growth in the country. Meanwhile, the "crowding out" effect has happened, indicating that local investment (local firms) has a modest level of technical and managerial changes, since increasing effect is estimated to be negative. According to this study, the economy of Kosovo has grown with an average rate of 4% for 16 years and the biggest contributor is the services sector with 56% of GDP, which includes financial services, insurance companies, construction, telecommunications, real estate, trade etc. This pace still continues to be a model of economic growth in Kosovo and continues to be supported by certain economic policies, while other sectors lag behind. This result clearly shows that, although the level of FDI inflows in Kosovo is low, the sector specification shows positive trend of incoming flows in the services sector. This is due to dominance in market of the financial sector and that of insurance with foreign capital as the only financing source of the local businesses.

To the article the results of the theoretical and experimental researches are given on questions of estimates of the dynamic rate effect of raindrop impact on soil. The aim of this work was to analyze the current methods to determine the rate of artificial rain pressure on the soil for the assessment of splash erosion. There are the developed author’s method for calculation the pressure of artificial rain on the soil and the assessment of splash erosion. The study aims to the justification of evaluation methods and the obtaining of quantitative characteristics, prevention and elimination of accelerated (anthropogenic) erosion, the creation and the realization of the required erosion control measures. The paper considers the question of determining the pressure of artificial rain on the soil. At the moment of raindrops impact, there is the tension in the soil, which is called vertical effective pressure. It is noted that the impact of rain drops in the soil there are stresses called vertical effective pressure. The equation for calculation of vertical effective pressure is proposed in this study using the known spectrum of raindrops. Effective pressure was 1.4 Pa for the artificial rain by sprinkler machine «Fregat» and 5.9 Pa for long distance sprinkler DD-30. The article deals with a block diagram of the sequence for determining the effective pressure of rain drops on the soil. This diagram was created by the author’s method of calculation of the effective pressure of rain drops on the soil. The need for an integrated approach to the description of the artificial rain impact on the soil is noted. Various parameters characterizing drop erosion are considered. There are data about the mass of splashed soil in the irrigation of various irrigation machinery and installations. For example, the rate (mass) of splashed soil was 0.28…0.78 t/ha under irrigation sprinkler apparatus RACO 4260–55/701C in the conditions of the Ryazan region. The method allows examining the environmental impact of sprinkler techniques for analyzes of the pressure, caused by raindrops, on the soil. It can also be useful in determining the irrigation rate before the runoff for different types of sprinkler equipment and soil conditions.
