Abstract Background Food insecurity is a widespread public health concern in many communities of sub-Saharan Africa. This study involved the Hadza hunter-gatherers of Tanzania, the only ethnic group in the country that has traditionally subsisted on hunting and gathering. In recent years, however, these communities have adopted mixed foraging economies. Information on how this change affects household food security is rather limited. The aim of this study was to assess the status of food security and the factors influencing household food security in the Hadza hunter-gatherer communities. Methods A cross-sectional study of 200 households was conducted in Mkalama district, Tanzania. Sampled householders represented individuals whose livelihood is mainly dependent on foraging (n = 129), beekeeping (n = 30) and farming (n = 41). Food security was measured by assessing food availability (Months of Adequate Food Provisioning (MAHFP)), food access (Household Food Insecurity Access Scale (HFIAS)) and food utilization (Dietary Diversity Scores (DDS)). Results Mean MAHFP was lower (p = 0.000) in predominantly foraging households (8.4 ± 1.1) compared with those involved in beekeeping (8.7 ± 1.6) or farming (9.6 ± 1.9). Based on HFIAS indicator, the prevalence of food insecurity varied with the household’s main activity (83.0% foraging, 46.7% beekeeping and 26.8% farming). Further, regression analyses show that the farming households were more likely to be food secure than the foraging households (OR = 10.7, p = 0.01). Dietary diversity scores also varied significantly with household’s main activity. About 65% of households (86% foraging, 63.3% beekeeping and 2.4% farming) consumed diets below the critical value of ≤ 4 food groups 24 h prior to survey. Social demographic characteristics and livelihood options are strong predictors of household food security. Conclusion All indicators used to assess food security point to high level of food insecurity in households mainly subsisting on foraging compared with beekeeping and farming. The primary dependence on foraging is associated with a longer period of food shortage, high prevalence of food insecurity conditions and low consumption of food varieties. Livelihood diversification coupled with provision of agricultural support services is necessary for the development of a secure future of the Hadza communities.

Smallholder farmers and pastoralists produce the largest proportion of food consumed in sub-Saharan Africa. However, they remain among the food insecure populations. This paper explores the food (in)security among smallholder farmers and pastoralists using a sample of 175 households in three agro-food value chains of wheat, dairy, and beef in the north-west Mt. Kenya region. The study seeks to answer if a farmer’s participation in a particular agro-food value chain determines his/her food security situation. We use the Household Food Insecurity Access Scale (HFIAS) and two Poisson regression models, parsimonious and full, to assess the household food security status and determinants of food security among the smallholder farmers and pastoralists. The results show that 61% of the households were either mildly, moderately, or severely food insecure. Households in the beef value chain experienced relatively higher incidences of food insecurity compared to households in the wheat and dairy value chains. The HFIAS scores revealed a wide gap between households with minimum and maximum score. Household size, income and income-related variables (ability to save and borrow to meet family needs), transport assets, membership in farmers’ associations, and household energy were significant in determining household food security, while access to credit and to extension services was not. Strategies that focus on boosting smallholder farmers’ incomes, building strong and resilient farmers associations to improve inclusive and equitable value chains have the potential to get smallholder farmers out of recurrent food insecurity.

Iraqi refugees in Lebanon are vulnerable to food insecurity because of their limited rights and fragile livelihoods. The objective of the present study was to assess household food insecurity among Iraqi refugees living in Lebanon, almost 10 years after the invasion of Iraq. A representative survey of 800 UN High Commissioner for Refugees-registered refugee households in Lebanon was conducted using multi-stage cluster random sampling. We measured food insecurity using a modified US Department of Agriculture household food security module. We collected data on household demographic, socio-economic, health, housing and dietary diversity status and analysed these factors by food security status. Hb level was measured in a subset of children below 5 years of age (n 85). Weighted data were used in univariate and multivariate analyses. Among the Iraqi refugee households surveyed (n 630), 20·1 % (95 % CI 17·3, 23·2) were found to be food secure, 35·5 % (95 % CI 32·0, 39·2) moderately food insecure and 44·4 % (95 % CI 40·8, 48·1) severely food insecure. Severe food insecurity was associated with the respondent's good self-reported health (OR 0·3, 95 % CI 0·2, 0·5), length of stay as a refugee (OR 1·1, 95 % CI 1·0, 1·2), very poor housing quality (OR 3·3, 95 % CI 1·6, 6·5) and the number of children in the household (OR 1·2, 95 % CI 1·0, 1·4), and resulted in poor dietary diversity (P< 0·0001). Anaemia was found in 41 % (95 % CI 30·6, 51·9) of children below 5 years of age, but was not associated with food insecurity. High food insecurity, low diet quality and high prevalence of anaemia in Iraqi refugees living in Lebanon call for urgent programmes to address the food and health situation of this population with restricted rights.

Objective. Rural and urban differences affect food availability, accessibility, and sustainability; thus, it has a greatimpact on household food security and dietary diversity. The relationship between the human environment andother factors threatens different domains of food systems resulting in food security. The objective of this study isto determine significant differences between the prevalence of household food security and diet diversity betweenurban and rural communities in Occidental Mindoro. Specifically, to determine the pattern of usual food intake ofpreschool children between rural and urban communities Methods. This study utilized a comparative, cross-sectional, analytic study design in order to determine theprevalence of each variable of interest in the two study areas. Radimer-Cornell Tool was utilized to assess the foodsecurity status of the household while the FAO-Dietary Diversity Score Questionnaire was used to the diversityof diet among PSC. A total of 480 (rural: n=240; urban: n=240) preschool children were recruited to participatein this study. Ratio and proportion using the point and interval estimate were used to determine the prevalencein different areas, meanwhile, chi-square of homogeneity was used to determine significant difference in the twoareas under study. Results. Food insecurity in rural communities was found to be at 56.25% (95% CI: 49.97% to 62.53%) while theprevalence in urban communities was 45.83% (95% CI: 39.53% to 52.14%). There was a significant differencein the prevalence of household food insecurity between rural and urban communities (p=0.0224). Meanwhile,the prevalence of less dietary diversity among preschool children in rural communities was 37.08% (95%: 30.97%to 43.19%) and 26.25% (CI: 20.68% to 31.82%) for urban communities. There was a significant differencein prevalence of low dietary diversity score among preschool children between rural and urban communities(p=0.0107). Conclusion. There were significant differences in terms of household food insecurity and less diverse dietbetween two community settings. Higher prevalence in rural areas signifies that there is a need to prioritize thesevulnerable communities in terms of hunger mitigation and nutrition programs. A combination of milk-rice-meat-fishwas observed in the diet of preschool children for both communities however, higher prevalence of less dietarydiversity was detected among rural communities.

Abstract Background To identify individual and household characteristics associated with food security and dietary diversity in seven Haitian–Dominican bateyes. Methods A cross-sectional sample of 667 households were surveyed. Novel household food security scores were calculated from components of the Household Food Insecurity Assessment Scale, while the Food and Agricultural Organization’s Household Dietary Diversity Score was utilized to calculate individual dietary diversity scores. Multivariable analyses were performed using ordinal logistic regression models to estimate the association between these scores and the covariate variables. Secondary dietary diversity analyses were performed after removing non-nutritious food groups. Results Food security was significantly associated with being above the poverty line (OR 3.14, 95% CI 1.92 to 5.14), living in a rural batey (OR 1.44, 95% CI 1.02 to 2.03), receiving gifts and/or donations (OR 1.76, 95% CI 1.03 to 3.00) and having a salaried job (i.e., not being paid hourly; OR 1.67, 95% CI 1.05 to 2.64). Dietary diversity was significantly associated with living in a semi-urban batey (OR 1.70, 95% CI 1.26 to 2.30), living with a partner (OR 1.47, 95% CI 1.08 to 2.00), growing at least some of one’s own food (OR 1.62, 95% CI 1.17 to 2.23), and receiving gifts and/or donations (OR 1.72, 95% CI 1.08 to 2.73). Conclusions Food insecurity and low dietary diversity are highly prevalent in Haitian–Dominican bateyes. The inclusion of sweets and non-milk beverages in dietary diversity calculations appear to skew scores towards higher levels of diversity, despite limited nutritional gains.

AbstractObjectiveTo determine access to traditional and indigenous foods (TIF) and the association with household food security, dietary diversity and women’s BMI in low socio-economic households.DesignSequential explanatory mixed-methods design, including a random household cross-sectional survey on household food insecurity access (HFIA), household dietary diversity (HDD) and women’s BMI, followed by focus group discussions.SettingTwo rural and two urban areas of Botswana.SubjectsPersons responsible for food preparation or an adult in a household (n400); for BMI, non-pregnant women aged 18–49 years (n253).ResultsAlmost two-thirds of households experienced moderate or severe food insecurity (28·8 and 37·3 %, respectively), but more than half of women were overweight or obese (26·9 and 26·9 %, respectively). Median HDD score was 6 (interquartile range 5–7) out of a total of 12. A positive correlation was found between number of TIF accessed and HDD score (r=0·457;P<0·001) and a negative correlation between number of TIF accessed and HFIA score (r=−0·272;P<0·001). There was no correlation between number of TIF accessed and women’s BMI (r=−0·066;P=0·297). TIF were perceived as healthy but with declining consumption due to preference for modern foods.ConclusionsTIF may potentially have an important role in household food security and dietary diversity. There is need to explore potential benefits that may be associated with their optimal use on food security and nutrition outcomes.

AbstractObjectiveTo meet some of the UN’s seventeen Sustainable Development Goals by 2030, there is a need for more effective policy to reduce food insecurity in low-income and lower-middle-income countries (LMIC). Measuring progress towards these goals requires reliable indicators of food security in these countries. Routinely conducted household consumption and expenditure surveys (HCES) provide potentially valuable and nationally representative data sets for this purpose. The present study aimed to assess methods used to determine national food security status using proxy measures from HCES data in LMIC globally.DesignA scoping literature review was conducted using electronic databases. Of the 929 abstracts identified, a total of twenty articles were reviewed against strict inclusion and exclusion criteria and included for further analysis.ResultsFourteen LMIC globally were represented in the twenty articles. The simplest metric used to indicate food insecurity compared household food expenditure against a level of expenditure considered to be below the poverty line. Data on acquisition of food was commonly converted to available energy for the household using local food composition tables and expressed as a proportion of household total energy requirements. Dietary diversity was also assessed in some studies as well as experience of food insecurity.ConclusionsThe review demonstrated that routinely collected HCES data sets provide a useful resource for the measurement of household food security in often resource-limited LMIC. Standardisation of methods used to assess food security is needed to allow for more useful comparisons between countries, as well as to assess temporal trends.

Abstract Objectives There is limited research on the associations between maternal social support and household food security and child feeding practices in Madagascar. Our study examined the association between maternal social support, household food security, and feeding practices among mothers of children 6 - 24 months in the Vakinankaratra region, Madagascar. Methods This cross-sectional study included 344 mother-child dyads. The Medical Outcomes Study (MOS) Social Support Index was used for maternal social support scores. A mean score of &gt;4 was considered optimal social support with ≤2 as none or little support. Food insecurity scores were determined using the Household Food Insecurity Access Scale (HFIAS). Complementary feeding practices were assessed using the World Health Organization (WHO) IYCF guidelines. Logistic and linear regression models tested the association between social support, complementary feeding practices, and household food insecurity. The models were adjusted for household, maternal and child confounders. Statistical significance was set at P &lt; 0.05. Results Almost half of the mothers (49.1%) were supported by their spouses, although only 37.8% of mothers had optimal social support. Less than half of infants (36.34%) met minimum dietary diversity (≥4 of 7 food groups). Mothers with optimal social support had lower household food insecurity (b = −0.85, P = 0.016, R,2 = 0.10) in the adjusted models. Furthermore, children of mothers with higher social support were more likely to meet minimum dietary diversity scores (AOR = 2.36 [1.21–4.56], P = 0.021) and to achieve the minimum adequate diet (AOR = 2.32 [1.17–4.61], P = 0.018). Although maternal social support was significantly associated with child minimum meal frequency in the bivariate analysis, it became nonsignificant in the final adjusted model (AOR = 1.70 [0.93–3.11], P = 0.387). Conclusions Increased maternal social support was significantly associated with household food security, child dietary diversity, and diet adequacy. Integrating parental peer support in nutrition interventions may be effective in improving food security, and child feeding practices in the Vakinankaratra region of Madagascar. Food insecurity is a global public health challenge. Household food insecurity is the leading risk factor of malnutrition, claiming approximately 300,000 deaths each year. Whether directly or indirectly, due to inadequate food consumption and poor diet quality, it is also accountable for over half of all deaths among children in Sub-Saharan Africa. It is estimated that the prevalence of food-insecure individuals in the area will reach 17 million by the year 2021. Inadequate nutrition may result in low immunity, impaired physical and mental development and reduced productivity among children under five years and throughout the life course. This review article attempts to discuss the various household food insecurity determinants and their association with child malnutrition in a Sub-Saharan Africa context. The purposes of this article are to inform governments, policymakers and service providers regarding the importance of household food security and its determinants on child malnutrition, and to plan strategies to improve the household food security status in Sub-Saharan Africa.

Abstract Background Empowering women, land tilting to enhance the security of Land Tenure and Property Rights (LTPRs) in agriculture vis-a-vis food and nutrition security are crucial in the pursuit of the Sustainable Development Goals. The main goal of this paper is to examine the crucial roles of women’s empowerment and LTPRs as they affect household food security among smallholder farmers in Nigeria. Methodology Cross-sectional data were obtained from 1152 maize and rice farmers, selected by multistage random sampling across 192 communities, 16 States and the six geopolitical zones in Nigeria. The data were collected on households’ socio-economics, food security situations, empowerment and LTPRs on parcels cultivated during the 2016/17 farming season by interviewing the adult members of the farmers’ households. HFS was assessed using the United States Department of Agriculture’ HFS Survey Module and Food and Agriculture Organization guidelines for measuring Household Dietary Diversity Score. LTPRs were measured in terms of tenure type and title registration to farmlands. HFS modelling was within the framework of Poisson, Instrumental Variable Poisson (IVP) and Zero-inflated Poisson (ZIP) regression methods, with endogeneity concerns and choice of specification addressed within Hausman specification tests. Results The results of the study show that households that have a share of farmland on purchase and also participate in off-farm activities are likely to be certainly food-secure in all regards. Crop diversity, households that cultivate maize only, the share of farmland on purchase and access to extension contact significantly reduce the severity of food insecurity while an increase in farm size increases the severity of food insecurity. Similarly, IV Poisson and ZIP Count results show that increase in the farm size results in the severity of food insecurity. The evidence with respect to women’s empowerment reveals that gender parity and female achievement in group membership, income control, as well as workload; reduce the extent of food insecurity among the farming households in Nigeria. We, however, discover that the female achievement in the productive decision and credit increases the severity of food insecurity among the smallholder farmers. The analyses also reveal that education of the household head, female achievement in the asset; group membership and workload are the major factors that positively influenced household dietary diversity. Conclusion The study suggests the adoption of climate-smart practices to overcome the issue of marginal farmlands, promotion of crop and livelihood diversification among the smallholder farmers, effective extension services delivery, and improving women’s access to productive resources to enhance household food security status in Nigeria.

Abstract Background Food insecurity impacts nearly one-in-four Latinx households in the United States and has been exacerbated by the novel coronavirus or COVID-19 pandemic. Methods We examined the impact of COVID-19 on household and child food security in three preexisting, longitudinal, Latinx urban cohorts in the San Francisco Bay Area (N = 375 households, 1875 individuals). Households were initially recruited during pregnancy and postpartum at Zuckerberg San Francisco General Hospital (ZSFG) and UCSF Benioff prior to the COVID-19 pandemic. For this COVID-19 sub-study, participants responded to a 15-min telephonic interview. Participants answered 18 questions from the US Food Security Food Module (US HFSSM) and questions on types of food consumption, housing and employment status, and history of COVID-19 infection as per community or hospital-based testing. Food security and insecurity levels were compared with prior year metrics. Results We found low levels of household food security in Latinx families (by cohort: 29.2%; 34.2%; 60.0%) and child food security (56.9%, 54.1%, 78.0%) with differences between cohorts explained by self-reported levels of education and employment status. Food security levels were much lower than those reported previously in two cohorts where data had been recorded from prior years. Reported history of COVID-19 infection in households was 4.8% (95% Confidence Interval (CI); 1.5–14.3%); 7.2% (95%CI, 3.6–13.9%) and 3.5% (95%CI, 1.7–7.2%) by cohort and was associated with food insecurity in the two larger cohorts (p = 0.03; p = 0.01 respectively). Conclusions Latinx families in the Bay Area with children are experiencing a sharp rise in food insecurity levels during the COVID-19 epidemic. Food insecurity, similar to other indices of poverty, is associated with increased risk for COVID-19 infection. Comprehensive interventions are needed to address food insecurity in Latinx populations and further studies are needed to better assess independent associations between household food insecurity, poor nutritional health and risk of COVID-19 infection.
