Pigment epithelium-derived factor (PEDF) is an antiinflammatory protein that circulates at high levels in the metabolic syndrome. Metabolic studies of PEDF knockout (KO) mice were conducted to investigate the relationship between PEDF, inflammatory markers, and metabolic homeostasis. Male PEDF KO mice demonstrated a phenotype consisting of increased adiposity, glucose intolerance, and elevated serum levels of metabolites associated with the metabolic syndrome. Genome expression analysis revealed an increase in IL-1β signaling in the livers of PEDF KO mice that was accompanied by impaired IRS and Akt signaling. In human hepatocytes, PEDF blocked the effects of an IL-1β challenge by suppressing activation of the inflammatory mediator c-Jun N-terminal kinase while restoring Akt signaling. RNA interference of PEDF in human hepatocytes was permissive for c-Jun N-terminal kinase activation and decreased Akt signaling. A metabolomics profile identified elevated circulating levels of tricarboxyclic acid cycle intermediates including succinate, an inducer of IL-1β, in PEDF KO mice. Succinate-dependent IL-1β expression was blocked by PEDF in PEDF KO, but not wild-type hepatocytes. In vivo, PEDF restoration reduced hyperglycemia and improved hepatic insulin signaling in PEDF KO mice. These findings identify elevated PEDF as a homeostatic mechanism in the human metabolic syndrome.

ObjectiveAdipose tissue releases factors (adipokines) that influence local, peripheral as well as central processes. In the present study, we determined the relationship between plasma concentration of a recently identified adipokine, pigment epithelium-derived factor (SERPINF1), and human obesity, particularly specific adipose tissue depots, and other features of the metabolic syndrome.MethodsWe examined the plasma concentration of SERPINF1, anthropometric parameters, abdominal s.c. and visceral adipose tissue, lipid, glucose, insulin, and alanine aminotransferase level in a non-diabetic general Caucasian population (n=59).ResultsPlasma SERPINF1 level in males (6.2±2.1 μg/ml) was higher than in females (3.1±1.4 μg/ml; P<0.001). Plasma SERPINF1 was positively correlated with age and all features of metabolic syndrome. However, in multiple linear regression analysis with adjustment for age and gender, only visceral fat thickness (β=0.361, P=0.010) and body mass index (β=0.288, P=0.008) were significant independent determinants of plasma SERPINF1 level, together with gender (β=−0.424, P<0.001).ConclusionsWe conclude that the plasma SERPINF1 level is strongly associated with body adiposity, in particular with the visceral fat depot in the non-diabetic general population. This association may (partly) explain the relationship between SERPINF1 and metabolic syndrome in this population.

       Some studies indicated a relationship between increased serum levels of osteoprotegerin with arterial calcification and as a result, it leads to the risk of cardiovascular disease. In our study group we selected patients with osteoporosis, with similar age and body mass index for the assessment of the relationship between cardiovascular disease and osteoprotegerin serum level. We took into account the analysis of correlation and association between the presence of distinct patterns of atherosclerosis and associated diseases like high blood pressure,  diabetes mellitus, low HDL cholesterol, increased LDL cholesterol, increased triglycerides and was the case of presence of any type of dyslipidemia, in case of pre-existent treatment. Objective of study was the assessment of osteoprotegerin value as predictive marker for cardiovascular and metabolic risk in osteoporotic patients. Our results showed significant correlations of parathyroid hormone, osteocalcin and biochemical markers of bone with glucose metabolism and lipid were found in our research, maintaining crosstalk between calcium and biochemical markers of bone and cardiovascular risk. The serum level of Osteoprotegerin has been shown to have a large predictive value for the metabolic syndrome as a cardiovascular risk standard in patients with osteoporosis. The osteoprotegerin serum levels were increased in the patients with metabolic syndrome as a protective response facing the atherosclerotic lesions.

The serum level of uric acid (UA) appears to be associated with a variety of cardiometabolic risk factors; however, direct association with the metabolic syndrome (MetS) remains controversial. The aim of this study is to investigate the association between serum levels of UA and the components that define MetS, differentiated by gender. 262 patients were enrolled (132 women and 130 men); mean value of the age: 58.7�16 year. Hyperuricemia was considered when the level of serum UA �7mg/dL in men, and � 6mg/dL in women; MetS was defined according to the IDF criteria. The prevalence of MetS in the studied group was 35.11% and the prevalence of hyperuricemia was 16.79%. Men with hyperuricemia had the highest prevalence of abdominal obesity (87.5% vs. 66.32%, p [0.001) and hypertriglyceridemia (65.62% vs. 45.91%, p [ 0.001) versus men with normal level of serum UA. Women with hyperuricemia also had a significantly higher incidence of abdominal obesity (75% vs. 57.51%, p [0.001), hypertriglyceridemia (58.33% vs. 38.33%, p [0.001), decreased HDL (50% vs. 33.33%, p [0.001) and hyperglycemia (66.66% versus 50%, p [0.001) compared to those with normal levels of serum UA. The majority of men with hyperuricemia have more than 4 of the MetS components. Hyperuricemia had a higher prevalence in patients with MetS, it may be considered as a causal factor of MetS. Elevated levels of serum uric acid were significantly more associated with the increasing number of MetS components. Early detection and treatment of hyperuricemia is essential for preventing the metabolic syndrome and its complications.

Pigment epithelium-derived factor (PEDF) is a glycoprotein that belongs to the superfamily of serine protease inhibitors, serpins. It was first identified as a neuronal differentiating factor secreted by human retinal pigment epithelial cells, and then found to be the most potent inhibitor of pathological angiogenesis in mammalian eyes. Recently, PEDF has been shown not only to suppress oxidative stress and inflammatory reactions in vascular wall cells, T cells and macrophages, and adipocytes, but also to exert antithrombotic and anti-fibrotic properties, thereby protecting against the development and progression of various cardiometabolic diseases and related complications. Furthermore, accumulating evidence has suggested that circulating PEDF levels may be a biomarker of severity and prognosis of these devastating disorders. Number of subjects with visceral obesity and insulin resistance is increasing, and the metabolic syndrome and its related complications, such as diabetes, nonalcoholic fatty liver disease/non-alcoholic steatohepatits, and atherosclerotic cardiovascular disease are a growing health challenge. Therefore, in this study, we review the pathophysiological role of PEDF in obesity and metabolic disorders, cardiovascular disease, diabetic eye and kidney complications, liver diseases, and reproductive system disorders, and discuss the potential clinical utility of modulating the expression and actions of PEDF for preventing these cardiometabolic disorders. We also refer to the clinical value of PEDF as a biomarker in cardiometabolic complications.

Background: Increments in inflammatory indicators and low levels of physical activity are correlated to the expansion of the metabolic syndrome (MetS). Objective: The purpose of this study was to establish if exercise training ameliorates inflammatory status in MetS patients. Data sources: PubMed, CINAHL, and Medline, Google Scholar, and Scopus databases and reference lists of included studies were searched. Study selection: Twenty randomized controlled trials (RCTs) of exercise-training impact on inflammatory markers (tumor necrosis factor (TNF) α, C-reactive protein (CRP), interleukin (IL) 6, IL-8, IL-10, and IL-18) with concurrent control groups were included in this analysis. Results: Results demonstrated an overall significant decrease in serum levels of TNF-α (mean difference (MD): −1.21 pg/ml; 95% confidence interval (CI): −1.77, −0.66), CRP (MD: −0.52 mg/l; 95% CI: −0.79, −0.25), IL-8 (MD: −1.31 pg/ml; 95% CI: −2.57, −0.06), and a significant increase in IL-10 (MD: 0.48 pg/ml; 95% CI: 0.10, 0.86). But exercise training did not change the level of IL-6 (MD: −0.69 pg/ml; 95% CI: −1.53, 0.14) and IL-18 (MD: −53.01 pg/ml; 95% CI: −166.64, 60.62). Conclusion: Exercise training improves TNF-α, CRP, IL-8, and IL-10 levels in patients with MetS. For some variables, isolated aerobic exercise, and combined aerobic and resistance exercise appears to be optimal. Future research is needed to clarify the mechanisms underlying exercise training’s effect on this population’s inflammatory markers. More studies are required to confirm these findings.
