Background: Urinary tract infections (UTIs) are the most common of all bacterial infections and occur at any time in the life of an individual. ESBL producing bacteria particularly Escherichia.coli is one of the most common causes of UTIs both in community and healthcare associated settings. Emergence of multidrug resistance (MDR) is quite alarming and cause failure of empirical treatment of UTIs. As a result increase the morbidity and mortality rate in the developing countries like Bangladesh.Objective: The objective of this study was to find out the bacteria causing UTI from urine culture and detection of ESBL producing Esch.coli and K.pneumoniae with their anti- microbial susceptibility pattern.Materials and Methods: A total of 1750 urine samples were collected from patients with symptoms and suspected UTI. Clean catch mid-stream urine samples were collected from indoor and outdoor patients of Dhaka Medical College Hospital during January 2015 to July 2015.Urine specimens were cultured in 5% Blood agar and MacConkeys agar media. The isolated bacteria were identified by gram staining and biochemical tests. Antimicrobial susceptibility and detection of ESBL were done by disc diffusion method.Result: Out of 1750 urine samples, 403(23.03%) were positive by culture. Among the culture positive cases, 216 (53.59%) were female and 187 (46.41%) were male. The most common isolated bacteria were Esch.coli 295(73.20%) followed by Pseudomonas aeuroginosa 85(21.09%), K.pneumoniae10(2.48%), Proteus spp. 4(.99%), Acinetobacter spp. 5(1.2%), Coagulase negative Staphylococcus (CONS) 4(.99%). Among the isolated Esch.coli and K.pneumoniae, ESBL producing bacteria were 202 (68.47%) and 5 (50%) respectively. All the isolated bacteria showed low level susceptibility to all antibiotics that are used during the study period.Conclusion: Treatment of UTIs is difficult when caused by multidrug resistant bacteria. Analysis of culture and sensitivity data should be done periodically to identify ESBL producing bacteria for proper treatment of UTIs.J Dhaka Medical College, Vol. 25, No.1, April, 2016, Page 26-31

Isolation and identification of post operative hospital acquired infection was carried out from July 2008 to December 2008 in Holy Family Red Crescent Medical College Hospital (private hospital). The major pathogen of wound infection was E. coli. A total; of 120 samples were collected from the surrounding environment of post operative room like floor, bed sheets, instruments, dressing materials, catheter, nasogastric and endotracheal tube. E. coli (40%) was the predominant organism followed by S. aureus (24%). Background: Chest trauma is responsible for 50% of deaths due to trauma. This kind of death usually occurs immediately after the trauma has occurred. Various therapeutic options have been reported for management of chest injuries like clinical observation, thoracocentesis, tube thoracostomy and open thoracotomy. Objective: To observe the pattern and outcome of management in chest trauma Methods: This is an observational study carried out in Casualty department of Chittagong Medical College Hospital (CMCH), Chittagong, between April 2015 to March 2016. Our study was included all patients, both sexes, following chest injury at Casualty units of Chittagong Medical College Hospital. All the data were recorded through the preformed data collection sheet and analyzed. Result: The mean age was found 37.7±18.1 years with range from 12 to 80 years. Male female ratio was 11.8:1. The mean time elapsed after trauma was found 6.1±3.1 hours with range from 1 to 72 hours. Almost one third (35.7%) patients was affecting road traffic accident followed by 42(27.3%) assault, 35(22.7%) stab injury, 15(9.7%) fall and 7(4.5%) gun shot . More than three fourth (80.5%) patients were managed by tube thoracostomy followed by 28(18.2%) observation and 2(1.3%) ventilatory support. No thoracotomy was done in emergency department. 42(27.2%) patients was found open pneumothorax followed by 41(26.6%) rib fracture, 31(20.1%) haemopneumothorax, 14(9%) simple pneumothorax, 12(7.8%) haemothorax, 6(3.9%) chest wall injury, 5(3.2%) tension pneumothorax, and 3(1.9%) flail chest. About the side of tube 60(39.0%) patients were given tube on left side followed by 57(37.0%) patients on right side, 9(5.8%) patients on both (left & right) side and 28(18.2%) patients needed no tube. Regarding the complications, 13(30%) patients had persistent haemothorax followed by 12(29%)tubes were placed outside triangle of safety, 6(13.9%) tubes were kinked, 6(13.9%) patients developed port side infection, 2(4.5%)tube was placed too shallow, 2(4.5%) patients developed empyema thoracis and 2(4.5%) patients developed bronchopleural fistula. The mean ICT removal information was found 8.8±3.6 days with range from 4 to 18 days. Reinsertion of ICT was done in 6(4.7%) patients. More than two third (68.2%) patients were recovered well, 43(27.9%) patients developed complication and 6(3.9%)patients died. More than two third (66.9%) patients had length of hospital stay 11-20 days. Conclusion: Most of the patients were in 3rd decade and male predominant. Road traffic accident and tube thoracostomy were more common. Open pneumothorax, rib fracture and haemopneumothorax were commonest injuries. Nearly one third of the patients had developed complications. Re-insertion of ICT needed almost five percent and death almost four percent. Journal of Surgical Sciences (2018) Vol. 22 (2) : 110-117

Objective: This study aims to describe main perpetrator of violence area and prevention of violence against women. Study design: A cross sectional study was carried out in One-stop Crisis Centre (OCC) of Dhaka Medical College Hospital, Dhaka, Bangladesh during the period of June 2006 to May 2007. Methods: 284 victims reported in OCC during the study period were selected as cases for the study and quantitative information was collected from them using cross sectional for the present study. Results: Study shows that most of the violence is done by husband (46.48%) and prevalence is more in rural areas (80.98%) in comparison to urban area (19.02%). Physical assault is the most common type of violence (50.35%), followed by sexual assault (36.63%) and burn (13.02%) Housewives are tortured more (67%). Conclusion: Proper implementation of existing rules and community based interventions for raising awareness about the domestic violence against women are recommended. Background: Now a days, there is increased demand and progress in the nursing profession. Along with these stress among the nurses has also increased.Objective: Purpose of the study was to assess the extent of perceived occupational stress and its association with work-related and non-work related causes among nurses.Materials and method: This Cross-sectional study was carried out among 197 nurses of Shaheed Suhrawardy Medical College hospital (ShSMCH), Dhaka, Bangladesh, from the period of 1st July 2015 to 30th June 2016. Sampling method was simple random sampling. Data was collected by face to face interview with the help of semi-structured questionnaire and finally all data were analyzed using SPSS software version 21.Results: It was found that 69% nurses were between the age of 21 to 39 years, 94.9% were female, 68% were married, 79.2% were Muslim, 84.3% had diploma, family income of 91.4% were equal to more than 40,000 Tk. and 95.5% used to do clinical work. Only 5.1% found to experience high stress. Statistically significant difference was found between sex, occupational stress and religion, educational status, and type of work (p < 0.05). Few nurses suffered from high stress but high level of stress were more in male, non Muslim, post graduate nurse and those who were engaged in administrative work.Conclusion: Nurse Managers should take appropriate actions to decrease stress helping their nurses to work efficiently and effectively.Delta Med Col J.
