Core surgical field camps: a new deanery-based model for enhancing advanced skills in core surgical trainees through simulation

Hands-on wet lab simulation training is a vital part of modern surgical training. Since 2010, surgical ‘boot camps’ have been run by many UK deaneries to teach core surgical trainees basic entry level skills. Training in advanced skills often requires attendance at national fee-paying courses. In the Wessex Deanery, multiple, free of charge, core surgical ‘field camps’ were developed to provide more advanced level teaching in the particular specialty preference of each core surgical trainee. After the COVID-19 pandemic, national hands-on courses will be challenging to provide and deanery-based advanced skills training may be the way forward for craft-based specialties. The experiences over 2 years of delivering the Wessex core surgical field camps are shared, giving a guide and advice for other trainers on how to run a field camp.

Abstract Background The COVID-19 pandemic has resulted in dynamic changes to healthcare delivery. Surgery as a specialty has been significantly affected and with that the delivery of surgical training. Method This national, collaborative, cross sectional study comprising 13 surgical trainee associations distributed a pan surgical specialty survey on the COVID-19 impact on surgical training over a 4-week period (11th May - 8th June 2020). The survey was voluntary and open to medical students and surgical trainees of all specialties and training grades. All aspects of training were qualitatively assessed. This study was reported according to STROBE guidelines. Results 810 completed responses were analysed. (M401: F 390) with representation from all deaneries and training grades. 41% of respondents (n = 301) were redeployed with 74% (n = 223) redeployed > 4 weeks. Complete loss of training was reported in elective operating (69.5% n = 474), outpatient activity (67.3%, n = 457), Elective endoscopy (69.5% n = 246) with > 50% reduction in training time reported in emergency operating (48%, n = 326) and completion of work-based assessments (WBA) (46%, n = 309). 81% (n = 551) reported course cancellations and departmental and regional teaching programmes were cancelled without rescheduling in 58% and 60% of cases respectively. A perceived lack of Elective operative exposure and completions of WBA’s were the primary reported factor affecting potential training progression. Overall, > 50% of trainees (n = 377) felt they would not meet the competencies required for that training period. Conclusion This study has demonstrated a perceived negative impact on numerous aspects of surgical training affecting all training specialties and grades.

Considerable anecdotal evidence indicates that some of the most difficult concepts that both high school and undergraduate elementary-education students struggle with are those surrounding evolutionary principles, especially speciation. It’s no wonder that entry-level biology students are confused, when biologists have multiple definitions of “species.” We developed this speciation activity to provide clarity and allow students a hands-on experience with a speciation model.

Abstract Aim The COVID-19 pandemic has caused significant disruption in surgical training. We aim to explore the preferences of higher general surgical trainees for Annual Review of Competency Progression (ARCP) also compare the responses across different training grades. Method All higher general surgical trainee in a single deanery were invited to participate in an online voluntary anonymous survey. The respondents were divided in two groups: junior (ST3-ST5) and senior higher surgical trainees (ST6-ST8) and responses compared. Results Sixty-four of 88 trainees responded. Thirty-three (51.6%) were ST3–ST5, 24 (37.5%) were ST6–ST8 and 7 (10.9%) were out-of-training. More trainees in ST3–ST5 group preferred to defer the next rotation for 12 months (18.2% vs 0%, p = 0.034), repeat current sub-specialty (33.3% vs 4.2%, p = 0.009), or add 12 months to training and delay predicted CCT date by 12 months (18.2% vs 0%, p = 0.034). Most trainees in both groups preferred the option of prolonging training should be offered to all trainees with an option to decline extension if ARCP competencies met (66.7% vs 50.0%, p = 0.276). Conclusions The preference for ARCP and length of training was different between two training groups, hence the need of trainees should be considered by training committees when addressing the impact of COVID-19.

To provide a pathway to employment, petroleum industry employers are sponsoring a standard induction program, delivered through a national network of training centres, which will be recognised as a passport for workers to gain access to oil and gas facilities.The Australian upstream petroleum industry faces many staffing challenges including: difficulties recruiting staff for new developments, competition from overseas projects for construction contractors, and the imminent retirement of an aging workforce. This growing employment demand and limited labour supply has created a strong competitive recruitment market that is characterised by skills shortages. Consequently, the industry is having to recruit workers from non-traditional labour pools and engage contractors whose workforce has limited oil and gas experience. Many of these workers are not familiar with petroleum industry processes, safety procedures, or environmental hazards.APPEA is supporting the development of the Induction Program to raise awareness and commence acquisition of these petroleum industry competencies; to facilitate wider staff recruitment; to up-skill contractors’ workforces; and, reduce repeated induction training while ensuring safety standards are maintained. The Induction Program will establish a qualification underpinned by a competency- based approach that is recognised as the minimum standard of entry level training for all workers in upstream petroleum workplaces. The Induction Program will be delivered through a national network of purpose-built Induction Centres designed to provide practical hands-on training in a simulated workplace environment.

This chapter discusses common elective and emergency presentations to hepatobiliary surgery. Gallstone disease, being the commonest hepatobiliary condition encountered by general surgical trainees, is discussed in detail. A separate section on acute ascending cholangitis is included with a brief description of the steps involved in laparoscopic cholecystectomy. Acute pancreatitis is discussed in Pancreatic Surgery Chapter 3. An overview of the assessment and management of post-cholecystectomy complications, such as bile duct injury and vascular injuries, is provided with illustrations. Management of common malignant conditions of the liver, such as colorectal liver metastasis, hepatocellular carcinoma, and cholangiocarcinoma, is included with detailed discussion of pre-operative imaging. Liver resection surgery and liver transplant surgery, as well as non-surgical management, are discussed. Details of post-operative management of hepatobiliary patients are aimed at the junior surgical trainee working in a tertiary hepatobiliary unit to aid day-to-day management of post-operative patients on the wards, as well as subsequent follow-up.

Objective The aim of this study is to assess the relationship between eye-tracking measures and perceived workload in robotic surgical tasks. Background Robotic techniques provide improved dexterity, stereoscopic vision, and ergonomic control system over laparoscopic surgery, but the complexity of the interfaces and operations may pose new challenges to surgeons and compromise patient safety. Limited studies have objectively quantified workload and its impact on performance in robotic surgery. Although not yet implemented in robotic surgery, minimally intrusive and continuous eye-tracking metrics have been shown to be sensitive to changes in workload in other domains. Methods Eight surgical trainees participated in 15 robotic skills simulation sessions. In each session, participants performed up to 12 simulated exercises. Correlation and mixed-effects analyses were conducted to explore the relationships between eye-tracking metrics and perceived workload. Machine learning classifiers were used to determine the sensitivity of differentiating between low and high workload with eye-tracking features. Results Gaze entropy increased as perceived workload increased, with a correlation of .51. Pupil diameter and gaze entropy distinguished differences in workload between task difficulty levels, and both metrics increased as task level difficulty increased. The classification model using eye-tracking features achieved an accuracy of 84.7% in predicting workload levels. Conclusion Eye-tracking measures can detect perceived workload during robotic tasks. They can potentially be used to identify task contributors to high workload and provide measures for robotic surgery training. Application Workload assessment can be used for real-time monitoring of workload in robotic surgical training and provide assessments for performance and learning.

Objective: The American health care system's adoption of the patient-centered care (PCC) model has transformed how medical providers communicate with patients about prescription pain medication. Concomitantly, the nation's opioid epidemic has necessitated a proactive response from the medical profession, requiring providers who frequently dispense opioids for acute pain to exercise vigilance in monitoring and limiting outpatient prescriptions. This qualitative study explores how surgical trainees balance PCC directives, including shared decision making, exchanging information with patients, and relationship maintenance, with opioid prescribing vigilance.Design: Investigators conducted interviews with 17 surgical residents and fellows (trainees) who routinely prescribe opioids at an academic medical center.Results: A qualitative descriptive analysis produced four codes, which were reduced to themes depicting problematic intersections between PCC imperatives and opioid vigilance during post-operative opioid-prescribing communication: (a) sharing the decision-making process contended with exerting medical authority, (b) reciprocating information contended with negotiating opioid prescribing terms with patients, (c) maintaining symbiotic relationships contended with prescribing ethics, and (d) achieving patient satisfaction contended with safeguarding opioid medications.Conclusion: Surgical training programs must supply trainees with post-surgical prescribing guidelines and communication skills training. Training should emphasize how PCC directives may work in tandem with--not in opposition to--opioid vigilance.

Child care offers an ideal setting to promote more healthful eating through nutrition education. However, many child care providers lack knowledge about nutrition and receive infrequent nutrition training. Although web-based training shows promise as a way to address this, most current trainings focus on information provision rather than direct skills training that promotes hands-on, experiential learning. The present study describes the development of nutrition education videos using a participatory action research (PAR) approach that has been successful in generating research evidence to address community-based challenges. This approach involves four major phases: (1) external consumer influence, (2) consumer-empowered team, (3) the research process, and (4) provider-valued outcomes. A case report design is used to describe the development of six nutrition education videos for child care providers (e.g., teaching taste, texture, and color; teaching nutritional benefits; teaching portion size). One hundred and fourteen providers, parents, and public health professionals viewed the videos. Results showed that the vast majority found the videos to be helpful or very helpful for instructing adults to teach children about nutrition. The videos were made publically available on a website ( http://TeachKidsNutrition.org ), and as of 1 year of being posted, the website had more than 4,919 visitors across the United States and internationally. Lessons learned throughout this process are discussed. The development of these videos using PAR presents a promising approach to assure that child care providers receive hands-on, experiential learning in nutrition education. Furthermore, this approach may contribute to obesity prevention efforts among child care providers.
