Antimicrobial Therapy Against Staphylococcus aureus, Pseudomonas aeruginosa, and Pseudomonas cepacia

The need to find a broad-based nutritional and antimicrobial therapy gave impetus to this investigation. The crude extracts of Baphia nitida stem bark were used to check for antimicrobial effect on selected micro-organisms for possible nutritional and therapeutic application. The stem bark components were extracted with four solvent systems using maceration. A synthetic drug gentamycin - 80 mg/mL (2 mL) was dissolved in 20 mL of distilled water to obtained 8 mg/mL, was used as a control. The crude extract exhibited an antibacterial effect on gram-positive organisms (Staphylococcus aureus and Bacillus cereus), and gram-negative organism (Pseudomonas aeruginosa, and Escherichia coli) at 2 mg/mL. These showed competitiveness with the 8 mg/mL of the control drug. The normal hexane fraction had an effect on Staphylococcus aureus but showed no effect on other bacteria. Minimum inhibitory concentration (MIC) showed a dose-dependent decrease in the effect for the crude extract. Ethyl acetate and methanol fraction had an effect on Staphylococcus aureus, Bacillus cereus, Pseudomonas aeruginosa, and Escherichia coli at varying concentrations - 20, 10, and 5 mg/mL. the initial screening of the Baphia nitida stem bark revealed alkaloids in the methanol-methylene chloride crude extract and methanol fraction only. Other solvents showed the alkaloids, flavonoids, saponins, terpenoids, glycosides, hydrogen cyanides and steroids respectively. Therefore, the presence of the phytochemicals implicates its antimicrobial effect and the basis for its efficacy as antimicrobial therapy.

Background: The battle against micro-organisms, in their role as primary cause of the disease and infective complications of medical and surgical techniques, has not decreased in spite of modern antimicrobial therapy. Chronic suppurative otitis media (CSOM) is a disease with worldwide prevalence having potentially serious long term effects. The disease remains an important global public health problem leading to hearing impairment, and due to wide spread irrational use microbial resistance is very common to these antibiotics, thereby leading to treatment failure. Hence it is important to know the type of bacteria and their sensitivity pattern so that appropriate antibiotics may be given for treatment and prevention of complications.Methods: 428 patients of otitis media were enrolled from ENT OPD of Tertiary Care Hospital. Ear swab was taken from diagnosed cases of CSOM and culture and sensitivity were done.Results: The microbiology of the swab showed no growth in (25.4%) of samples. Staphylococcus aureus (26%) and Pseudomonas (25%) were the main organisms isolated. Staphylococcus aureus isolated was sensitive to vancomycin, clindamycin, cefixime, gentamicin and cefipime in descending order. Pseudomonas aeruginosa was sensitive to ceftazidime, imipenem, piperacillin, gentamicin, cefipime. In the present study Staph. aureus and Pseudomonas were the predominant bacteria, it is suggested to undertake a gram staining in all patients. If gram positive organisms are isolated it is suggested that presumptive treatment should be directed against Staphylococcus aureus and if gram negative then against Pseudomonas aeruginosa. Such a treatment is not only likely to be effective but will also go a long way in preventing emergence of drug resistance.Conclusions: The antimicrobial therapy should be based on locally determined microbiological isolates and local sensitivity patterns to a particular antimicrobial agent. The presumptive antimicrobial therapy should therefore be directed against these organisms.

A zygomycosis (mucormycosis) ritka, kifejezetten agresszív lefolyású opportunista infekció, amelyet a Zygomycota (Járomspórás gombák) törzs Zygomycetes osztályába tartozó gombák okoznak. Jellemzően immunhiány, neutropenia, vastúlterhelés, hematológiai betegség, illetve diabeteses ketoacidosis eseteiben fordul elő. Esetismertetés: A szerzők 18 éves, 9 éve inzulindependens diabetes mellitusban szenvedő, rossz compliance-szel rendelkező beteg kórtörténetét ismertetik. Súlyos állapotban, beszűkült tudattal, ketoacidosisban, orrmelléküreg-gyulladásra utaló egyoldali periorbitalis duzzanattal, bőrpírral került kórházba. Bár az endoszkópos sinusműtét során vett váladékból methicillinrezisztens Staphylococcus aureus és Pseudomonas aeruginosa tenyészett, a beteg anamnézise, klinikuma zygomycosis gyanúját keltette, emiatt amphotericin-B-kezelést indítottak. Szövettani, illetve mikrobiológiai vizsgálatok során Rhizopus oryzae okozta invazív infekció igazolódott. Megbeszélés: Az antifungális kezelés ismételt sebészeti beavatkozásokkal és adjuváns granulocytakolónia-stimuláló faktor alkalmazásával kiegészítve eredményesnek bizonyult. The study involved the selection of two isolates from Bacillus subtilis to investigate their inhibitory activity against some bacterial pathogens. B sub-bacteria were found to have a broad spectrum against test bacteria such as Staphylococcus aureus and Pseudomonas aeruginosa. They were about 23-30 mm and less against Klebsiella sp. The sensitivity of some antibodies was tested on the test samples. The results showed that the inhibitory ability of bacterial growth in the test samples using B. subtilis extract was more effective than the antibiotics used.

Objetivos: avaliar a atividade antimicrobiana em extrato aquoso, hidroalcoólico e alcoólico das folhas de espécies da família Lamiaceae frente a bactérias de interesse. Método: Foram escolhidas quatro espécies: Ocimum gratissimum, Plectranthus amboinicus, Mentha arvensis e Plectranthus barbatus. A partir das folhas foram confeccionados os extratos aquoso, hidroalcoólico e alcoólico nas concentrações 100mg/mL, 50mg/mL e 25mg/mL. Foram selecionadas as bactérias Streptococcus pyogenes, Enterococcus faecalis, Staphylococcus aureus e Pseudomonas aeruginosa para os ensaios de antibiose em Ágar Mueller-Hinton. Resultados: P. barbatus, em seu extrato hidroalcoólico mostrou ativo nas três concentrações para bactéria S. aureus, e ainda foi ativo para P. aeruginosa, demonstrando no extrato alcoólico atividade frente as bactérias. Para M. arvensis e P. amboinicus, seus extratos hidroalcoólico e alcoólico apresentaram atividade para S. aureus. Conclusão: Sugere-se que as espécies em questão apresentem boa atividade antimicrobiana, sendo necessária a realização de mais estudos para melhor entender esse mecanismo.
