BACKGROUND Dental appearance influences the facial aesthetics that includes smile harmony, tooth colour, shape, arrangement and visibility. METHODS The current study aims to identify and evaluate factors affecting patients' satisfaction towards the appearance of teeth & gums; and dental treatments that facilitate facial and dental aesthetics. 300 patients were surveyed with a questionnaire containing close-ended questions on their gender, age, education, tooth appearance, orthodontic and teeth whitening treatments, tooth & gum-related problems and satisfaction levels towards dental and facial aesthetics. Chi-square test and logistic regression analysis were performed on the collected data. RESULTS A majority (76 %) of the study population desired to undergo bleaching treatment for teeth whitening. 70 % of the respondents also wish for composites, while only 40 % desired orthodontic appliances. Satisfaction with dental appearance is significantly found to affect facial appearance. Also, similar to dental appearance, there is a statistically significant relation between the desire for tooth bleaching, orthodontic appliance, and composites and satisfaction towards facial appearance. Satisfaction with gingival appearance, tooth-colour, and satisfaction with smile appearance are found to affect the satisfaction towards facial appearance. CONCLUSIONS Tooth colour was observed to be a crucial aspect that affects the overall dental aesthetics and satisfaction levels of the participants. It was realized from this investigation that dental care, better hygiene and treatments for improving dental aesthetics are relevant for patients’ satisfaction regarding overall facial appearance and aesthetics. KEY WORDS Dental Aesthetics, Facial Aesthetics, Orthodontic Treatment, Tooth Whitening

The teeth are an integral part of facial aesthetics and are involved in a complex social, cultural and psychological interaction. Discolouration of tooth is a common aesthetic problem caused by either intrinsic or extrinsic factor. For people with severe discolourations, bleaching can be an important and valuable treatment. In-office bleaching is one of the most requested procedures in many dental offices, a great way to get a fast and immediate change in the colour of their teeth. The normal colour of such a tooth can be restored by decolourizing the stain with a powerful oxidizing agent such as 35% hydrogen peroxide which is directly placed on the labial surfaces of the tooth to be treated. The bleaching agent is commercially available in the form of a gel. These case reports reflect the remarkable change of tooth colour by in-office bleaching. Key Words : Bleaching agents; Fluorosis; Hydrogen peroxide.

BACKGROUND: In recent years’ medical management with misoprostol is an effective alternative to surgical evacuation. But there is a dearth of evidence to reveal the effectiveness of the different routes of misoprostol and satisfaction rate among the patients treated with these routes. AIM: This study was conducted to compare the effectiveness and patient’s satisfaction rate of vaginal versus oral misoprostol. METHODS: It was a prospective non-interventional study. One hundred women of having missed abortion confirmed by ultrasonography examination were enrolled in the trial. Fifty-eight subjects were administered 200 mcg of oral and 42 subjects received 200 mcg of vaginal misoprostol every four hours up to four doses. If complete expulsion did not occur 12 hours after the last dose, the surgical evacuation was done. RESULTS: There was no significant statistical difference between the effectiveness of treatment with vaginal (78.57%) and oral misoprostol (79.31%) (p = 0.928). The difference between Patients’ satisfaction at the time of discharge for the vaginal group (64.29%) and oral group (65.52%) was not statistically significant (P = 0.991). There was an increase in patients’ satisfaction for both groups at the follow-up session, but still, the difference was not significant (P = 0.897). CONCLUSION: This study confirms that there is no statistical difference between the effectiveness and patient satisfaction of oral and vaginal misoprostol in the treatment of missed abortion.

Background: Patient’s satisfaction is one of indicators measured in the hospital minimum service standards. In Installation of Inpatient (IRNA) of Haji Surabaya hospital, there are 17 indicators, but only nine are met (52.94%). One of them is patients’ satisfaction level amounted to 74.35% out of the standard (82%). The high number of complaints on nurses’ competence in providing services causes the unachieved patients’ satisfaction. There were 61 complaints from 2014 to 2017.Aim: This study aimed to analyze the influence of nurses’ responsiveness to patients’ satisfaction of in-patient installation (IRNA) at Haji Surabaya Hospital.Method: The questionnaires used Likert scale 1-5 for independent variables and dependent variables. The scoring scales for the independent variable or nurses’ responsiveness range from strongly disagree to strongly agree. Meanwhile, the scoring scale for the dependent variable ranges from very dissatisfied to very satisfied. Results: The findings indicated that there was a significant influence of responsiveness on patients’ satisfaction amounted to 0.003 (<α = 0.05). This indicated that the better assessment on the nurses’ responsiveness is, themore satisfied the patients are at in-patient installation (IRNA), Haji Surabaya Hospital. Conclusion: It can be concluded that more than 20% of responses was satisfied with the nurses’ responsiveness. The hospital needs to hold a human resource training especially a nurse-patient therapeutic communication training for maintaining the service quality at the hospital. Keywords: hospitalization, patient, responsiveness, satisfaction

Abstract Background Preoperative anxiety is a common problem with an impact on surgical outcome, anesthetic drug amount and patient’s satisfaction. An important component of preoperative anxiety is worries related to anesthesia. Suitable patients information has been shown to reduce preoperative anxiety level and this can be effectively achieved through a video. Objectives To assess the impact of an informative video about regional intra-venous anesthesia technique on patient’s preoperative anxiety levels before minor ambulatory hand surgery procedures. Study design retrospective, single-centre, case-control clinical trial. Methods To assess the impact of an educational video illustrating all the passages of intravenous regional anesthesia on preoperative anxiety level and overall patients’ satisfaction. Results Anxiety level measured after admission in the day hospital clinic did not differ between the two groups, however overall patients’ satisfaction levels were higher when patients were shown the video. Conclusions Informative videos does not seem to significantly reduce preoperative anxiety but have the potential to increase patients’ satisfaction in the ambulatory setting.

Abstract Background and Aims Quality of life of hemodialysis patients and adequacy of hemodialysis therapy in general, is defined by the number and duration of incidents during hemodialysis procedures. In this study we examined the effect of telemedical system for control and monitoring of hemodialysis procedures on patients’ condition and their quality of life. Method The system described in this work included: doctor/patient video call functionality initiated from both ends; functionality of hemodialysis procedure parameters and patient’s condition parameters monitoring and registration; functionality of alerting medical staff about registered incidents, functionality of visual control of hemodialysis procedure. The effect of control and monitoring system usage was studied on population of 2300 hemodialysis patients (at the start of the study) with median follow-up of 2 years. The primary end-point was doctor’s reaction time on patient’s complaint, medical staff reaction time on intradialysis hypertension incidents. Secondary end-points were: number of patients who left the clinic due to reasons besides lethality, patients’ satisfaction by hemodialysis therapy (according to survey), number of incidents of intradialysis and interdialysis hypertension. Results During the study we observed that as a result of system deployment average doctor’s reaction time on patient’s complaint (defined as the time from emergence of the complaint to start of patient/doctor communication) reduced from 8 to 1.5 minutes, average staff reaction time on intradialysis hypertension incidents (defined as time from registration of hypertension incident to start of blood pressure normalization actions) reduced from 5 to 2 minutes. Number of patients who left the clinic due to reasons besides lethality reduced from 2.5 per 100 patients before system deployment to 1.7 per 100 patients at the end of the study. Average value of patient’s satisfaction by dialysis therapy increased from 7.2 to 9.1 points on 10-point scale (according to survey conducted at the beginning and at the end of the study). By the end of the study, average number (across population) of hypertension incidents per month reduced from 8.3 to 6.2 and from 20.7 to 16.5 episodes for intradialysis and interdialysis hypertension correspondingly. Conclusion The use of telemedical tools of hemodialysis procedures control and monitoring has positive impact on patients’ satisfaction level by the dialysis procedure and on duration/frequency of incidents registered by these tools, which, in return may improve the quality of patient’s life.

Abstract Objectives To assess the perceived facial changes in class II division 1, convex profile patients treated with functional followed by fixed orthodontic appliances. Subjects and methods The study sample consisted of 36 pairs of pre- and post-treatment photographs (frontal and profile, at rest) of 12 patients treated with activator, 12 with twin-block, and 12 controls with normal profiles, treated without functional appliances. All photographs were presented in pairs to 10 orthodontists, 10 patients, 10 parents, and 10 laypersons. Visual analog scale (VAS) ratings of changes in facial appearance were assessed. Results The patient groups were similar in sex distributions, age, and treatment duration. The different rater groups showed strong to excellent agreement. There were no significant differences among treatment groups (F = 0.91; P = 0.526; Wilks lambda = 0.93), raters (F = 1.68; P = 0.054; Wilks lambda = 0.83), and when testing the combined effect of treatment and rater on the results (F = 0.72; P = 0.866; Wilks lambda = 0.85). The raters detected slightly more positive changes in the activator and twin-block groups, compared to the control group, regarding the lower face and the lips, but these findings did not reach significance. Furthermore, their magnitude hardly exceeded 1/20th of the total VAS length. Limitations Retrospective study design. Conclusions The perceived facial changes of convex profile patients treated with functional, followed by fixed orthodontic appliances, did not differ from those observed in normal profile patients, when full-face frontal and profile photos were simultaneously assessed. Consequently, professionals should be skeptical regarding the improvement of a patient’s facial appearance when this treatment option is used.

Nickel and chromium ions released from fixed orthodontic appliances may act as allergens. This study aimed to systematically review the effect of fixed orthodontic treatment on salivary levels of these ions by doing a meta-analysis on cross-sectional and cohort studies. The Web of Science, Scopus, Cochrane Library, and PubMed databases were searched for articles on salivary profile of nickel or chromium in patients under fixed orthodontic treatment published from January 1983 to October 2017. A random-effect meta-analysis was done using Review Manager 5.3 to calculate mean difference (MD) and 95% confidence interval (CI), and the quality of questionnaire was evaluated by the Newcastle–Ottawa scale. Fourteen studies were included and analyzed in this meta-analysis. Salivary nickel level was higher in periods of 10 min or less (MD = −11.5 µg/L, 95% CI = −16.92 to −6.07; P < 0.0001) and one day (MD = −1.38 µg/L, 95% CI = −1.97 to −0.80; P < 0.00001) after initiation of treatment compared to baseline (before the insertion of appliance). Salivary chromium level was higher in periods of one day (MD = −6.25 µg/L, 95% CI = −12.00 to −0.49; P = 0.03) and one week (MD = −2.07 µg/L, 95% CI = −3.88 to −0.26; P = 0.03) after the initiation of treatment compared to baseline. Corrosion of fixed orthodontic appliances leads to elevated salivary nickel and chromium concentrations early after initiation of orthodontic treatment. Randomized clinical trials controlling for factors affecting the saliva composition are recommended on a higher number of patients and among different ethnicities.
