Background: While tPA may be safe in the elderly, increasing age appears to augment risk of post-tPA symptomatic intracranial hemorrhage (sICH). Age-related white matter changes (ARWMC) are associated with increased sICH. Patients evaluated for acute ischemic stroke (AIS) via telestroke (TS) may not have access to MRI to allow incorporation of microbleeds in tPA decisions. We assessed if increased CT-based ARWMC was associated with increased sICH in elderly patients. Methods: Patients 80 years and older who received tPA for AIS at spoke hospitals were selected from our TS network registry from 9/2015 to 12/2018. TS spoke CT scans from patient presentation were reviewed by three of the authors for periventricular white matter (PWMC) and deep white matter (DWMC) changes. Total ARWMC score, based on the Fazekas score, was collected. Total ARWMC score was either mild (0-2), moderate (3-4), or severe (5-6). PWMC and DWMC were either mild (0-1) or moderate-severe (2-3). Logistic regression adjusted for age, sex, race, ethnicity, NIHSS and premorbid mRS was used to analyze relationship of ARWMC scores with sICH and patient-outcomes. Results: Of 241 patients, median age overall was 86 years (IQR 83-90), and 66% were female. The overall median ARWMC score was 3 (IQR 2-5). Regression analysis showed that more severe ARWMC scores did not lead to higher frequency of post-tPA ICH (moderate OR 0.57, CI 0.19-1.71; severe OR 1.32, CI 0.48-3.65) including sICH (moderate OR 0.78, CI 0.21-2.94; severe OR 2.09, CI 0.62-7.02). Similarly, severe PWMC and DWMC were not associated with increased risk of post-tPA ICH (PWMC OR 1.31, CI 0.51-3.38; DWMC OR 1.25, CI 0.52-3.01), including sICH (PWMC OR 1.61, CI 0.51-5.08; DWMC OR 1.81, CI 0.65-5.01). In our cohort, older patients had no difference in hemorrhage (ICH OR 0.93 CI 0.85-1.00: sICH OR 0.95 CI 0.86-1.04), and patients with less severe stroke were more likely to have hemorrhage (ICH OR 1.06 CI 1.02-1.10; sICH OR 1.08 CI 1.03-1.13). IRR among the CT scan readers was moderate (k=0.504). Conclusions: ARWMC scores were not associated with post-tPA ICH in the elderly. Our analysis lends support for the use of tPA despite severity of white matter disease. ARWMC should not be used to assist in tPA decision-making in elderly patients via telestroke.

In this study, the Assessment of Motor and Process Skills (AMPS), an observational assessment of functional ability, was compared with other tests of function and cognition, namely the Functional Independence Measure (RM), the CAMCOG (the cognitive component of the CAMDEX [Cambridge Examination for Mental Status in the Elderly]) and the Mini-Mental State Examination (MMSE). The subjects were people with diagnosed dementia or mild memory impairment. The results were as expected, revealing significant relationships between AMPS process ability and the CAMCOG, the MMSE and the FIM social/cognition scale, and between AMPS motor ability and the RM physical scale. This supported the validity of the AMPS as an evaluation of the interaction between cognitive impairments and disability in complex activities of daily living.

Objective To verify the occurrence of maltreatment of the elderly and its characteristics (location, type, reason, involvement of alcohol/drugs, profile and family relationship of victims and perpetrators) from the police records of a specialized police station over a five year period. Method A cross-sectional, descriptive and documentary analytical study was performed. The police reports of a medium-sized municipality in the northwest of São Paulo were analyzed from 2008 to 2012. The sociodemographic characteristics of the perpetrators and victims and the data relating to the aggression investigated were: violence; location; reason; the presence of alcohol and drugs during the assault and victim-offender relationship. Data was tabulated using descriptive statistical analysis. Results Of a total of 572 cases, most of the perpetrators were men (69.2%), Caucasian (56.5%), aged 31-40 years (14.20%) with no information regarding occupation provided (50.70%); most of the victims were women (93.0%), Caucasian (71.5%), aged 60-65 years (46.30%), married (34.10%) and unemployed/retired (59.98 %). Emotional abuse was prevalent (57.0%), occurred in the home of the elderly person (81.3%) and the children of the elderly were the main perpetrators (25.3%). The motives were related to arguments in most cases (53.1%). Conclusion Women who were Caucasian, married, aged 60-65, unemployed/retired were the main victims, with the perpetrators most frequently their own children, also Caucasian, single and aged 31-40 years. Psychological violence was the most prevalent, primarily triggered by disagreements among those involved, and it occurred most frequently in the residence of the victim. Most of the attackers were not under the influence of alcohol/drugs.

Background: Health practitioners around the world are now dealing withincreasing health problems, including oral disease. One of them is tooth loss.Losing teeth can cause problems or disturbances in the main functions of teeth(mastication, aesthetics and phonetics). These conditions can be overcome byusing denture. If the loss of untreated teeth doesnt restored using denture, it will cause the function of the missing tooth can not be restored, which will result in disruption of nutritional status and quality of life of the elderly. This study aimed to determine the relationship of tooth loss with nutritional status in elderly society in the Penatahan village Penebel Tabanan. Methods: The research was an observational analytic study with cross sectionalresearch design. Total sampling used with total sample 109 people. The datawere collected by filling the dental chart, measuring IMT and interview using theOHIP-14 questionnaire. Results: The results showed 65.1% respondents had loss <6 teeth, 42.2%respondents had obesity, 75.2% respondents observed poor quality of life. Dataanalyzed using kolmogorov smirnov obtained p = 0,952 for relationship betweentooth loss with nuturional status and p = 0.676 for relationship between useof denture with nutritional status. Chi square used to test relationship betweentooth loss with quality of life (p= 0,735) and use of denture with quality of lifeshowed p = 0,139.Conclusions: The conclusion is there is no relationship between tooth lossand use of denture with nutritional status and quality of life in elderly society inPenatahan Village Penebel Tabanan

AbstractA known prevalence of concurrent cervical and lumbar spinal stenosis was shown to be 5–25%, but there is a lack of evidence regarding direct relationships in canal dimension and canal-body ratio between cervical and lumbar spine. Total 247 patients (mean age: 61 years, male: 135) with cervical and lumbar computed tomography scans were retrospectively reviewed. Midsagittal vertebral body and canal diameters in reconstructed images were measured at all cervical and lumbar vertebrae, and canal-body ratios were calculated. The canal diameter and ratio were also compared according to the gender and age, and correlation analysis was performed for each value. There were significant correlations between cervical (C3–C7) and lumbar (L1–L5) canal dimension (p < 0.001). C5 canal diameter was most significantly correlated with L4 canal diameter (r = 0.435, p < 0.001). Cervical canal-body ratios (C3–C7) were also correlated with those of lumbar spine (L1–L5) (p < 0.001). The canal-body ratio of C3 was most highly correlated with L3 (r = 0.477, p < 0.001). Meanwhile, mean canal-body ratios of C3 and L3 were significantly smaller in male patients than female (p = 0.038 and p < 0.001) and patient’s age was inversely correlated with C5 canal diameter (r = − 0.223, p < 0.001) and C3 canal-body ratio (r = − 0.224, p < 0.001). Spinal canal dimension and canal-body ratio have moderate degrees of correlations between cervical and lumbar spine and the elderly male patients show the tendency of small canal diameter and canal-body ratio. This relationship of cervical and lumbar spine can be an important evidence to explain to the patients.

Thailand is becoming an elderly society like many countries in the world. The number of elderly people is increasing continuously every year. In order to enable the elderly to live with good quality of life in the rapidly changing society, knowledge and information related to their health and living factors are considered to be necessary for them. Therefore, this study was carried out in order to develop a model of knowledge provision for promoting quality of life of the elderly in rural areas of the country. The samples were drawn from every region of the country which included 480 elderly people, 480 elderly caretakers, and 160 people representing the community leaders, community committee members and staff of local government agencies. Both quantitative and qualitative methods were employed for data collection. The study found that there were five areas of knowledge for promoting quality of life of the elderly: physical health, mental health, social relationship, economic, and learning. The model of knowledge provision to the elderly synthesized from the study could enable the elderly to gain necessary knowledge deemed useful for promoting their quality of life. The elderly, the elderly care caretakers and related people were found to be satisfied with the model.
