“Just don’t be creepy”: A phenomenological study of the experiences of men in massage therapy

AbstractBackgroundSocietal expectations around traditional binary gender roles result in some professions being considered “men’s work” or “women’s work”. Massage therapy (MT) is one such profession that, despite being predominantly female, is joined by an increasing number of men with a desire to help others.MethodsThis descriptive phenomenological study asked male massage therapists in Ontario, Canada, about their experience of gender in their professional lives. Fourteen men shared their experiences of practice, which included discriminatory hiring and patient preferences for female practitioners. These issues resulted in difficulty establishing a clinical practice. To create a successful practice, men described the need for professionalism, clear communication, and a comfortable treatment environment.Results and conclusionsResearchers should explore the impact of discrimination on men in MT, patient preferences based on the therapist’s gender, and the role of education in perpetuating societal heterosexual norms. Finally, as with any shift in culture, all levels of organization must take action to remove discrimination and bias within the profession of MT.

Background: The physician associate/assistant (PA) role was introduced into the Irish healthcare system in 2015 when four PAs from North America were recruited to work in Ireland as part of a two-year pilot program. In parallel, the Royal College Surgeons in Ireland (RCSI) began its own PA program in January of 2016 and has graduated 25 PAs. To date, there is little published on the few Irish-trained PAs. This study seeks to share initial experiences with the PA profession in Ireland.Methods: This descriptive phenomenological study explores the newly implemented PA role in Ireland through semi-structured, one-to-one interviews with five PAs and their supervising consultants in Dublin, Ireland. Questions centered on the PA scope of clinical practice, satisfaction with the role, acceptance by the medical team, and integration into the Irish healthcare system.Results: Six themes were identified from the interviews: continuity of patient care, flexible scope of practice, lack of role clarity, complementary to the medical team, regulation and prescribing challenges, and the emerging role of the PA.Conclusions: The development of the PA role is in its infancy in Ireland, but this study suggests that they fill an important role by providing continuity of care to both patients and the medical team. Currently, there are significant challenges imposed by a lack of regulation of the PA profession and the inability to prescribe. However, PAs in Ireland appear to enjoy their work and have great potential to expand throughout Irish healthcare and are valued by their team members.

While some higher education institutions attempt to help support and retain Latino students until graduation, these same institutions often neglect the experiences of Latino faculty members. The main challenges Latino faculties encounter in higher education include cultural taxation, discrimination, and feelings of isolation. Research is limited on the experiences and role of mentorship for Latino male faculty. This phenomenological study examines how Latino male faculty members in higher education perceive mentoring both as a mentee and as a mentor. This study also examines the impact of mentoring on professional development and the path of Latino male faculty. Findings illustrate that family values, mentoring in higher education, and desire to pay forward the mentoring experience are factors that impact Latino male faculty perception of mentorship.

This chapter assesses theory and evidence on the efficacy of medical malpractice liability and limitations to it in improving healthcare outcomes, and identifies unresolved issues that merit further attention from scholars. First, it explores the theoretical and legal background on medical malpractice. It then turns to the available evidence by focusing on three basic areas of study: the impact of malpractice limitations on payouts and litigation, the effect of malpractice limitations on overall healthcare costs, and the effect of malpractice on two major cost drivers in the healthcare system: cardiac and obstetrics practice. It argues that limitations on liability did not and likely cannot significantly reduce healthcare costs. Finally, the chapter discusses new and important trends in the literature regarding reforms to standards of care and the role of clinical practice guidelines and communication and disclosure programs.

Objective: Medical podcasts are becoming increasingly available; however, it is unclear how these new resources are being used by trainees or whether they influence clinical practice. This study explores the preferences and experiences of otolaryngology residents with otolaryngology-specific podcasts, and the impact of these podcasts on resident education and clinical practice. Methods: An 18-question survey was distributed anonymously to a representative junior (up to post-graduate year 3) and senior (post-graduate year 4 or greater) otolaryngology residents at most programs across the US. Along with demographic information, the survey was designed to explore the preferences of educational materials, podcast listening habits and motivations, and influence of podcasts on medical practice. Descriptive statistics and student t-tests were used to analyze the results. Results: The survey was distributed to 198 current otolaryngology residents representing 94% of eligible residency programs and was completed by 73 residents (37% response rate). Nearly 3-quarters of respondents reported previous use of otolaryngology podcasts, among which 83% listen at least monthly. Over half of residents changed their overall clinical (53%) and consult (51%) practice based on podcast use. Residents rank-ordered listening to podcasts last among traditional options for asynchronous learning, including reading textbooks and watching online videos. Conclusions: While other asynchronous learning tools remain popular, most residents responding to this survey use podcasts and report that podcasts influence their clinical practice. This study reveals how podcasts are currently used as a supplement to formal otolaryngology education. Results from the survey may inform how medical podcasts could be implemented into resident education in the future.

Self-transcendence has become and remains an important research theme. Little is known about the role of self-transcendence in cultivating meaningful work and its impact on the wellbeing of middle managers in the face of adversity, such as the COVID-19 pandemic. The aim of this qualitative hermeneutic phenomenological study was to explore the impact of the COVID-19 pandemic on the meaning middle managers attach to their work by investigating the role of self-transcendence in cultivating meaning and wellbeing in a cohort of seven South African middle managers employed in cross-boundary service industry settings. Data were collected through unstructured narratives. Findings confirm that self-transcendence serves as a coping mechanism during adversity and that it facilitates the re-negotiation of meaning, resulting in three potential shifts: the shift from a blame orientation to a work orientation, the shift from reflection to reflexivity and the shift from self-consciousness to other-consciousness. The findings also highlight how self-transcendence enables the exploration of the adaptive benefits of anxiety. The findings contribute new insights into the construct of self-transcendence and extend research on existential positive psychology. It is suggested that organizations invest in reflexive practices as a tool to promote deep learning and connectivity by exploring dialectical processes through reflexive work.

The worth of women’s entrepreneurship is accepted globally, but there is less focus on it in developing countries, and societal expectations mean women often lack the confidence to start their own business. The core purpose of this research is to investigate the influence of personality traits on women’s inclination toward entrepreneurship. The personality traits are measured through the dimensions of openness, neuroticism, extraversion, conscientiousness, and agreeableness. Further, the study introduced the mediator of self-leadership on personality traits and entrepreneurial intentions of women. The study is quantitative in nature and used a questionnaire survey to collect the data by convenience sampling technique. The data was collected in the context of Pakistan, and Smart PLS was chosen for further analysis. The findings revealed the significance of the relationship between personality traits and entrepreneurial intention. Furthermore, the study also highlighted the significance of self-leadership as a mediator and proposed significant relationships. The study suggested that personality issues should be considered and used from a business perspective, and self-leadership is important for women. The study provides room for policymakers and institutes to inform educational policies to motivate women entrepreneurs for the future.

Abstract BackgroundDecision impact studies have become increasingly prevalent in oncology in recent years, particularly in breast cancer prognostic research. Such studies, which aim to evaluate the impact of a test on clinical decision-making, appear to be a new form of knowledge with the potential to impact clinical practice and regulatory decision-making in genomic medicine. Yet their origins, intended purpose and usage have not yet been explored. The objectives of this review are to identify and characterize decision impact studies in genomic medicine in cancer care. This review is comprised of two parts. First, we will conduct a scoping review to catalogue the characteristics of decision impact studies. The scoping review will be followed by a bibliometric analysis to understand the role of actors and institutions in the production and dissemination of this new knowledge, by identifying influential articles, authors, global research trends and collaboration networks. MethodsWe will conduct a scoping review and a bibliometric analysis of the scoping review results. The search will include four databases, Medline, Embase, Scopus and Web of Science, using a comprehensive search strategy developed through a preliminary review of the literature. Arksey & O’Malley’s scoping review methodology, with updates by Levac et al. will be used, and the review will be reported following the PRISMA-ScR checklist. The FT Model will be used to collect and analyze data on clinical utility of decision impact studies. Our bibliometric analysis, using Bibliometrix software, will elucidate the evolution of these studies and provide data on the trends, influences and networks emerging in the field.DiscussionThis review will be a first step in understanding the evolution and uses of these studies and their potential influence on the integration of emerging genomic technologies into clinical practice. By exploring their origin and evolution across space and time, this study will equip future research to investigate the role of these studies in decision-making for regulatory processes, including market access and public and private coverage decision-making. AbstractA recurring theme from the literature is that the definition of reflection is nebulous and/or complex. Many authors have suggested that more research needs to be conducted to better understand an individual’s perception of reflection and reflective practice, and how these concepts affect their clinical practice as well as their personal growth and development. This paper offers the findings of a qualitative study of radiation therapists in Canada. The aim of the study was to explore radiation therapist’s understanding of the concept of reflection, and to understand how they incorporated it into their daily practice. Secondary objectives were to examine some of the perceived barriers to its use, and the possible challenges of implementing reflective writing. Two focus groups were initially conducted, and a follow-up questionnaire was developed using the themes generated from the focus groups. The questionnaire was distributed to radiation therapists at two large cancer centres in Toronto, Canada. Most participants indicated that it is an integral part of their practice and professional lives, and that they use a variety of different methods for engaging in reflection. It is not without its barriers, but many of these can be overcome by providing time, coaching and a supportive work environment. Respondents were divided as to whether they would benefit from being taught reflection; however, small group teaching would be the favoured method. Further study is suggested to determine whether there are any improvements to patient care and in particular patient outcomes.

ObjectiveMany treatment decisions are preference-sensitive and call for shared decision-making, notably when benefits are limited or uncertain, and harms impact quality of life. We explored if clinical practice guidelines (CPGs) acknowledge preference-sensitive decisions in how they motivate and phrase their recommendations.DesignWe performed a qualitative analysis of the content of CPGs and verified the results in semistructured interviews with CPG panel members.SettingDutch oncology CPGs issued in 2010 or later, concerning primary treatment with curative intent.Participants14 CPG panel members.Main outcomesFor treatment recommendations from six CPG modules, two researchers extracted the following: strength of recommendation in terms of the Grading of Recommendations Assessment, Development and Evaluation and its consistency with the CPG text; completeness of presentation of benefits and harms; incorporation of patient preferences; statements on the panel’s benefits–harm trade-off underlying recommendation; and advice on patient involvement in decision-making.ResultsWe identified 32 recommendations, 18 were acknowledged preference-sensitive decisions. Three of 14 strong recommendations should have been weak based on the module text. The reporting of benefits and harms, and their probabilities, was sufficiently complete and clear to inform the strength of the recommendation in one of the six modules only. Numerical probabilities were seldom presented. None of the modules presented information on patient preferences. CPG panel’s preferences were not made explicit, but appeared to have impacted 15 of 32 recommendations. Advice to involve patients and their preferences in decision-making was given for 20 recommendations (14 weak). Interviewees confirmed these findings. Explanations for lack of information were, for example, that clinicians know the information and that CPGs must be short. Explanations for trade-offs made were cultural-historical preferences, compliance with daily care, presumed role of CPGs and lack of time.ConclusionsThe motivation and phrasing of CPG recommendations do not stimulate choice awareness and a neutral presentation of options, thus hindering shared decision-making.
