Abstract: Introduction: Numerous beliefs have arisen in relation to the emigration of doctors. First of all, in most cases emigration has been attributed to, almost exclusively, low salaries. There are a number of non-financial issues that could possibly be more easily addressed. Method: In order to get closer to these issues, we sent out a 37-item questionnaire to doctors, who originally graduated from the University of Debrecen. Altogether 82 of them sent back the questionnaire. Results: The mean age at the time of emigration was 32 years. The most popular destination was the United States, however, many colleagues left for the US to do research, as well as clinical practice. Among the European countries, the most popular ones were Sweden, Germany, United Kingdom, Switzerland and Norway. The five most popular clinical specialties were general practice, internal medicine, anesthesiology/intensive care, laboratory/pathology and surgery. With respect to the planned duration of working abroad, at the time of emigration one-fourth of our colleagues planned very short (<2 years) stay. When we asked them now, 38% plans much longer, even indefinite stay. The most common reasons of emigration were also assessed. The most common one was the lack of career track, followed by low salary, quality of life, family issues and adventurousness. Interestingly, burnout and “gratuities” were at the end of the list. A mean 1.3 million HUF per month would make our colleagues satisfied but they think that a mean 486.000 HUF would be realistic and would keep most doctors within the country. As far as university education is concerned, our colleagues agreed that the basic medical knowledge of Hungarian doctors is excellent, but their practical skills are much worse. Conclusions: Although returning to Hungary would be unrealistic by most colleagues, low salaries are definitely not the most important or exclusive reason for leaving the country. This chapter discusses how the growth of precarious work and the polarization of the US labor market have produced major problems for the employment experiences of young workers. A prominent indicator of young workers’ difficulties in the labor market has been the sharp increase in their unemployment rates since the Great Recession. Another, equally if not more severe, problem faced by young workers today is the relatively low quality of the jobs that they were able to get. Other problems include the exclusion of young workers from the labor market and from education and training opportunities; the inability to find jobs that utilize their education, training, and skills; and the inability to obtain jobs that provide them with an opportunity to get a foothold in a career that would lead to progressively better jobs and thus be able to construct career narratives.

Studies on vitamin D receptor (VDR) and its association with multiple disorders are expanding. This bibliometric study aims to find and summarize VDR-related publications, and compare them across various countries, organizations, and journals to demonstrate trends in VDR research. VOSviewer and Excel 2019 were used to classify and summarize Web of Science articles from 1900 to mid-2021. Total records of 8762 articles were analyzed, and maps of co-citations bibliometric keywords co-occurrence were designed. In conclusion, relative research interest and published papers related to VDR were growing in the past 30 years. The United States of America dominates the research regarding VDR. The highest quality of VDR research was achieved by the University of California System, University of Wisconsin System, and Harvard University. J Steroid Biochem Mol Biol, PLoS One, and J Biol Chem are the leading three productive journals on VDR. Various aspects of vitamin D deficiency associated disorders and genetic studies regarding VDR, including single nucleotide polymorphism, gene variants, epigenome, long non-coding ribonucleic acid (lncRNA), and small nucleolar RNA host gene 6 are potentially the recent research hotspot in this field. Moreover, coronavirus disease, polycystic ovary syndrome, non-alcoholic fatty liver disease, gut microbiota, gestational diabetes, systemic sclerosis, and chemoresistance are the trending medical conditions associated with VDR.

The clinical practice of oncology has become increasingly complex. An explosion of medical knowledge, increased demands on provider time, and involved patients have changed the way many oncologists practice. What was an acceptable practice model in the past may now be relatively inefficient. This review covers three areas that address these changes. The American Society of Clinical Oncology (ASCO) National Oncology Census defines who the U.S. oncology community is, and their perceptions of how practice patterns may be changing. The National Cancer Institute (NCI)-ASCO Teams in Cancer Care Project explores how best to employ team science to improve the efficiency and quality of cancer care in the United States. Finally, how physician assistants (PAs) and nurse practitioners (NPs) might be best integrated into team-based care in oncology and the barriers to integration are reviewed.

The relevance of linking the concept of sustainable development and the security discourse reveals the possibility of believing that education is a prerequisite for ensuring that “sustainable development” goals become a reality. The university has a twofold task: first, to produce knowledge that meets the demands of our time, i.e. technical knowledge, and second, to form human capital, to train specialists capable of the practical application of instrumental knowledge. The initial orientation of the concept of “sustainable development” towards a global perspective: the representation of reality in an economic paradigm, i.e., totally determined by the “logic of capital”, “monocausal economic logic”, determines the criteria by which the quality of human capital, its price, and efficiency of production of a standardized product are evaluated, the production of which is undertaken by the university-corporation that has replaced the classical “university of reason”, whose ontic foundations - the “Hegelian science”, the romantic “education of humanity” - are no longer valid in what is called modernity. The article demonstrates how modernity, constituted concerning a certain self-representation of the New European subject and presented in the liberal economic paradigm, predetermines both the goal-setting in determined by its representation of the development and the content and methods of the reform of the university. It is concluded that “sustainable development”, “security” and “university-corporation” are essentially connected with the representation of reality in the liberal version of the economic paradigm.

ABSTRACT State medical boards have long recognized the importance of evaluating the ongoing knowledge and competence of licensed physicians under a variety of circumstances. Before granting or renewing a license, it may be necessary for state boards to evaluate physicians as part of a disciplinary process or following a period of inactivity for either disciplinary or non-disciplinary reasons. The Post-Licensure Assessment System (PLAS), a joint program of the Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners (NBME), has played a role in assisting state boards with evaluating physicians' basic medical knowledge in all of these circumstances by providing the Special Purpose Examination (SPEX). While SPEX has been administered since 1988, there has not been a nationally published study summarizing the characteristics of physicians taking the exam and their examination pass rates. To address this, we examined physicians who took SPEX between 2003 and 2011, and the outcomes of their exams. Our research demonstrates that the majority of examinees take SPEX for non-disciplinary reasons, with those who take SPEX for disciplinary reasons having lower pass rates. Future research should focus on evaluating the ultimate outcomes for physicians taking SPEX, including the ability to attain and retain a license to practice medicine.

AbstractThis article focuses on the work and efforts put forth by the University of Houston’s Recovering the US Hispanic Literary Heritage program (Recovery) to create the first digital humanities center for US Latina/o Research: #usLdh. Recovery is a program to locate, preserve, and make available the written legacy of Latinas/os in the United States since colonial times until 1960. Through 27 years of successful work Recovery has not only been able to inscribe the excluded history of Latinas/os, but also has created an inclusive and vast digital repository that facilitates scholarship in this area of studies. This article focuses on the importance of recovery work in the writing, teaching, and understanding of history and considers how local personal archives have helped to fill in the gaps of mainstream history. We will detail the goals and challenges of this mission, as well as the importance of educating the community in digital methods that preserve and disseminate minority voices.

This chapter reviews the member organizations of the United States Intelligence Community, the strategic environment that has informed intelligence and accountability in the United States, including scandals as a key driver of innovation, and the current and future threat environment as seen by the United States. The chapter examines the US intelligence accountability architecture: the House of Representative Permanent Select Committee on Intelligence, the Senate Select Committee on Intelligence, the Foreign Intelligence Surveillance Court, the Inspectors General, the Government Accountability Office, the Privacy and Civil Liberties Oversight Office, and the President’s Intelligence Advisory Board. The sheer number and complexity of accountability bodies in the US gives rise to inefficiencies, ineffectiveness, and duplication. The accountability system is replete with gaps and vulnerabilities: partisanship, collective-action problems, resource allocation, and inconsistent quality of review in congressional accountability; GAO’s limited authority to review the USIC and sensitive operations; the adequacy of the FISA court in adequately protecting the rights of Americans; and Presidential discretion in appointing and removing IGs. These issues have implications not just for the United States, but for allies, partners as well as regional and global stability.

Landscape architecture programs in the United States are assessed based on the quality of the professional education received by their students. Research is becoming an increasingly important part of the profession as evidence-based landscape architecture grows, and it is critical that university faculty provide information that can be used in professional practice to resolve important environmental and social issues. In many universities, individual landscape architecture faculty are encouraged to conduct research and their performance is evaluated based largely on the quantity and quality of their scholarly output. This paper used publicly-available information to conduct a citation analysis for individual faculty and professionally accredited landscape architecture programs across the US. There was a wide range in the contribution level with some programs and some individuals who were very productive, while many others contributed very little. This might point to an attempt by programs to maintain a balance between scholarly contributions and the education of professional landscape architects. As research becomes an increasing important part of the profession, the productive programs and individuals identified in this study might provide models for others to emulate.
