Cystic changes in the mammary gland structure in different age periods

The article is devoted to problems of diagnosis of cystic disease of breast at different ages including children and teens. Analyzed various options for classification of cystic changes. The main method of detecting liquid formation is breast ultrasound, then, as a final verification is possible after thorough laboratory studies education (cytological, biochemical and histological method). Therapeutic and diagnostic puncture cysts do not affect the continued operation of the glandular tissue and can be used at any age. Active sonirovanie cysts in children due to the high probability of occurrence of inflammation on the available background cystic cavity. In the adult diagnostic and treatment interventions have a diagnostic character and for reducing local pain symptoms associated with overexertion cysts. Analysis of the results carried out on the basis of examination and treatment of 22 381 patients aged 8 to 72 years, who applied to receive a specialized mammography department of the Federal State Autonomous Institution «National Health Research and Practical Center for Children» of the Ministry of Health of the Russian Federation for the period from 2008 to 2015. Проанализирован международный опыт реформирования проверок соблюдения требований пожарной безопасности и внедрения риск-ориентированного подхода. Разработана модель отнесения объектов защиты к категориям риска в зависимости от вероятного причинения вреда, который рассчитывается исходя из количества погибших и травмированных при пожарах людей. Сформулированы критерии отнесения объектов защиты к категориям риска. Выполнен расчет категорий риска для групп объектов, однородных по группам экономической деятельности и классам функциональной пожарной опасности. Проведено сравнение с существующей классификацией объектов защиты по категориям риска. The international experience of reforming of fire safety compliance checks and implementing a risk-based approach is considered. There are presented methodological approaches to calculating the risk of causing harm (damage) in buildings (structures) as a result of fire for the purpose of assignment of buildings and structures according to risk categories as well as justification of the frequency of scheduled inspections at these facilities. There is calculated the probability of fire occurrence for a group of objects of protection that are homogeneous by type of economic activity and functional fire hazard classes in order to assign objects of protection to certain risk categories. The social damage expressed in the death and injury of people as a result of fire is also calculated in order to assign objects of protection to certain risk categories. Classification of objects of protection according to the risk categories is performed using the indicator of the severity of potential negative consequences of fires. This indicator characterizes the degree of excess of the expected risk of negative consequences of fires for the corresponding group of objects of protection in relation to the value of the permissible risk of negative consequences of fire. The permissible risk of negative consequences of fires is calculated on the basis of statistical data, taking into account the value of the individual fire risk of exposure of critical values of fire hazards on person in buildings and structures. The criteria for assigning groups of objects of protection to the appropriate risk categories are formulated on the basis of formation of distribution of numerical values of the severity of potential negative consequences of fires. There are carried out the assessment of the severity of potential negative consequences of fires for objects of protection that are homogeneous by type of economic activity and functional fire hazard classes, and also the risk categories of the corresponding groups of objects are determined. The proposed classification of objects of protection according to risk categories is compared with the existing classification. The obtained results of calculations showed that scheduled inspections of objects of protection by the Federal state supervision bodies, depending on the assigned risk category and with corresponding frequency, have significant role in improving the level of fire safety of objects. The decrease in the intensity of scheduled inspections, at the same time, may lead to a corresponding decrease in the level of fire protection of objects.

ObjectiveTo assess the dose-response of daridorexant, a new dual orexin receptor antagonist, on wake after sleep onset (WASO).MethodsElderly (≥65 years) participants (n = 58) with insomnia were randomly allocated (Latin square design) to receive 5 treatments (5, 10, 25, and 50 mg daridorexant and placebo) during 5 treatment periods, each consisting of 2 treatment nights followed by a 5- to 12-day washout period. Main efficacy endpoints were the absolute change from baseline in WASO (primary) and latency to persistent sleep (LPS; secondary) to days 1 and 2 (mean of 2 treatment nights assessed by polysomnography) in each period. Safety and tolerability were also assessed.ResultsOf 58 participants included, 67% were female, and the median age was 69 years (range 65–85 years). WASO and LPS were dose-dependently reduced from baseline to days 1 and 2 after daridorexant administration (multiple comparison procedure modeling, p < 0.0001 and p = 0.004, respectively); reductions were statistically significant for doses ≥10 mg compared with placebo (WASO: −32.0, −45.1, −61.4 minutes; LPS: −44.9, −43.8, −45.4 minutes for 10, 25, and 50 mg, respectively, p ≤ 0.025). Treatment-emergent adverse events were similar for daridorexant and placebo; the most frequent were fatigue, nasopharyngitis, gait disturbance, and headache (≤7% in any group).ConclusionsDaridorexant was well tolerated. Dose-dependent improvements in WASO and LPS were statistically significant (dose range 10–50 mg) in elderly people with insomnia disorder.ClinicalTrials.gov identifier:NCT02841709.Classification of evidenceThis study provides Class I evidence that, for elderly people with insomnia, daridorexant reduced WASO.

Writing this journal aims to analyze the credit agreement of the bank between the bank as a creditor with the Civil Servants as the debtor who uses a photocopy of the Decree of the Appointment of Civil Servants who are legalized as collateral. This journal is prepared using empirical juridical research method and the approach used is case approach and concept approach. Based on the results of this study it is known that the Bank piahk dare to receive a copy of the Decree of the Appointment of Civil Servants who are legalized as collateral in the credit agreement using the juridical basis of credit analysis 5c which is the basic principle of bank credit analysis based on the Law of the Republic of Indonesia Number 10 of 1998 on Amendment to Act Number 7 of 1992 Concerning Banking. Photocopy of a legalized appointment letter of civil servants belonging to movable and intangible objects that can be used as collateral based on the classification of legal guarantees because it has economic value in the form of slip of gajih civil servants. Trust to the debtor is the foundation of the bank providing credit funds. Penulisan jurnal ini bertujuan untuk menganalisis perjanjian kredit bank antara pihak bank selaku kreditor dengan pihak Pegawai Negeri Sipil selaku debitor yang menggunakan fotokopi Surat Keputusan Pengangkatan Pegawai Negeri Sipil yang dilegalisasi sebagai agunan. Jurnal ini disusun dengan menggunakan metode penelitian yuridis empiris dan pendekatan yang digunakan adalah pendekatan kasus dan pendekatan konsep. Berdasarkan hasil penelitian ini diketahui bahwa piahk Bank berani menerima fotokopi Surat Keputusan Pengangkatan Pegawai Negeri Sipil yang dilegalisasi sebagai agunan dalam perjanjian kredit dengan menggunakan dasar yuridis yaitu analisis kredit 5c yang merupakan prinsip dasar analisis kredit bank berdasarkan Undang-Undang Republik Indonesia Nomor 10 tahun 1998 tentang Perubahan Atas Undang-Undang Nomor 7 tahun 1992 Tentang Perbankan. Fotokopi Surat Keputusan Pengangkatan Pegawai Negeri Sipil yang dilegalisasi termasuk ke dalam benda bergerak dan tidak berwujud yang dapat dijadikan agunan berdasarkan klasifikasi hukum jaminan karena memiliki nilai ekonomis berupa petikan slip gajih pegawai negeri sipil. Kepercayaan kepada debitor merupakan dasar pihak bank memberikan dana kredit.

This study uses cluster analysis to develop a classification of sexually aggressive juveniles, based on demographic information, offense details, psychological and behavioral issues, and participation in treatment. Data were collected from two treatment programs (n = 95) that serve this population. Four distinct types were found: sexually aggressive (those who were otherwise nondelinquent), revenge (those who were mostly nondelinquent but specifically targeted step- and half-siblings), disturbed revenge (those who offended against half- and stepsiblings, and demonstrated more significant behavioral and psychological disturbances) , and disturbed aggressive (overall antisocial individuals who displayed problems with psychological issues and violent behaviors). The results suggest that sexually aggressive juveniles are a heterogeneous population that, while sharing some similarities with their adult counterparts, are different in many ways, and early treatment interventions may help reduce the risk of adult offending.
