The objective: depression of frequency of unsuccessful attempts of auxiliary genesial technologies on the basis of studying of new aspects of pathogenesis of female sterility, and also improvement and introduction of algorithm of prognostic and treatment-and-prophylactic actions. Patients and methods. Аt 1 stage (forecasting of results of ART) 97 married couples (1 group) which passed sterility treatment were surveyed. At the 2nd stage (prophylaxis of unsuccessful attempts of ART) 108 patients of genesial age who had in the anamnesis of 2 and more attempts of ART were surveyed. Results. Unsuccessful attempts ART arise at various reasons of sterility: a tubal and peritoneal factor – 48.5%; endocrine disturbances – 44.3%; a man’s factor – 29.9%; an external genital endometriosis of the I–II st – 21,7%, and also a combination of above-mentioned reasons of sterility – 60.8%. At female factors of sterility depression of an ovarialny reserve and unavailability of functional state an endometrium which take place are the main reasons of unsuccessful attempts of auxiliary genesial technologies as is independent, and in a combination. Conclusion. For rising of efficiency of ART it is necessary to use, first of all, effective forecasting taking into account functional, laboratory and morphological methods of researches. Further, it is necessary to develop the individual program of specific preparation taking into account all preliminary results. The objective: development of algorithm of forecasting of unsuccessful attempts of auxiliary genesial technologies on the basis of the mathematical analysis of the main reasons and risk factors. Patients and methods. 97 married couples which passed sterility treatment on the basis of clinic of auxiliary genesial technologies were surveyed. The complex of the conducted researches included clinical, endocrinologic, immunohistochemical, ehografical and statistical methods. Results. For forecasting of unsuccessful attempts of auxiliary genesial technologies it is necessary to use the constructed mathematical model which includes 4 most significant changes (as significance value depression): optical density of an expression of LIF in superficial and glandular epithelium the endometrium, received in the period of assumed «implantation windows» in cycle before carrying out an extracorporal fertilization, the maintenance of VEGF in cervical slime in day of transvaginal puncture, S/D and IR of spiral arteries in day of introduction of triger of an ovulation. Conclusion. Use of offered algorithm allows to increase forecasting effectiveness of unsuccessful attempts of auxiliary genesial technologies. Under high-intensity loads, the athlete's bodies take place a number of biochemical reactions and physiological processes that can lead to hyperbilirubinemia. The factors that can initiate the onset of this phenomenon include the syndrome of micro-damage muscle, violation of the integrity of erythrocyte membranes, decreased blood pH, malnutrition and increase oxygen demand of the body. Degree of expression of manifestations of physiological bilirubinemia depends on the level of adaptation of the athlete to the physical activities offered. Hyperbilirubinemia in athletes can be one of the components of the deterioration of the functional state, forming the symptoms of endogenous intoxication. The relevance of this problem in sport lies in the relatively low detection rate of hyperbilirubinemia due to the lack of regular screening studies. However, in drawing up a plan of nutritional- metabolic support for training and competitive activity and recovery measures, must not only the individual reaction of the athlete body to physical activity, but also the severity of shifts in the indicators of bilirubin metabolism and their ratio. The article describes the reasons for the increase in bilirubin levels, which can be caused by both the effect of physical activity and by the presence of pathological processes in athletes. The factors influencing the blood serum’s bilirubin content are also highlighted, which include the state of erythrocyte cell membranes and the rate of hemoglobin destruction, the functional state of the liver, the specifics of physical loads and the use of ergogenic pharmacological agents by athletes. Particular accent has been placed on the illumination of hereditary hyperbilirubinemias, which may have been detected at the stage of selection of athletes. The most common phenomenon is Gilbert's syndrome, which occurs in 2-5% of cases in the general population, is characterized in the clinic by a benign flow and is manifested by episodes of jaundice and an increase in total bilirubin content to moderate values due to indirect. The frequency of detection of hyperbilirubinemias in the population of athletes is 4.68%, among which Gilbert's disease accounts for almost half (48.7%). Conclusion. The work highlighted the pathogenesis and diagnostic algorithm of Gilbert's disease, and also emphasized that its drug prevention and correction in athletes to maintain functional and physical fitness should be carried out taking into account anti-doping rules, which requires upon diagnosis timely receipt of a therapeutic exclusion

The objective: rising of efficiency of treatment of sterility at married couples with the burdened genetic anamnesis on the basis of scientific justification and introduction of advanced algorithm of prognostic and treatment-and-prophylactic actions. Materials and methods. For the purpose of detection of risk factors of development of sterility and carrying out the correlation analysis were allocated the main group – 100 married couples with sterility in the anamnesis and control group of patients who had labors (n=100) in the anamnesis. The complex of the conducted researches included clinical, laboratory, tool and statistical methods of research. Results. Rising of efficiency of treatment of sterility at married couples with the burdened genetic anamnesis requires carrying out medicogenetic consultation with simultaneous planned preimplantation genetic research in programs of auxiliary genesial technologies. At a choice of this or that method of auxiliary genesial technologies at women with the burdened genetic anamnesis it is necessary to use a technique at which one spermatozoon is entered into an oocyte cytoplasma by means of equipment of a micromanipulation. In the program of training of doctors the obstetrician-gynecologists and urologists of an out-patient link it is necessary to loop thematic improvement concerning sterility treatment at married couples with use of auxiliary genesial technologies. Conclusion. Results of the conducted researches testify to need of rising of efficiency of auxiliary genesial technologies at married couples with the burdened genetic anamnesis. Use by our algorithm of prognostic and treatment-and-prophylactic actions allows to increase efficiency of auxiliary genesial technologies at married couples with the burdened genetic anamnesis for 26.7%. ObjectiveTo determine the stage of training at which neurology residents should achieve individual elements of the Accreditation Council for Graduate Medical Education neurology Milestones and to examine the relationship between perceived importance of Milestones and the stage by which they should be achieved.MethodsA modified Delphi technique was used to establish consensus postgraduate year (PGY) expectations for neurology Milestone competencies across 3 geographically and administratively distinct Mayo Clinic neurology residency programs. Timing expectations were examined for relationships to perceived importance of the individual Milestones and effects of participant characteristics.ResultsPGY expectations for neurology Milestone elements ranged from PGY 1.3 to PGY 4.1. Extent of rater educational seniority had no effect on PGY competency expectations. There was a moderate inverse relationship between perceived importance of the Milestone element and the PGY by which it should be achieved (rs = −0.74, p < 0.0001).Conclusions and relevanceExpectations for neurology Milestone competency acquisition can be measured and may help inform individual program design, educational expectations, and future Milestone design.

The first and preliminary results of the photometry of Large Yield Radiometer (LYRA) and Sun Watcher using Active Pixel system detector and Image Processing (SWAP) onboard PROBA2 are presented in this paper. To study the day-to-day variations of LYRA irradiance, we have compared the LYRA irradiance values (observed Sun as a star) measured in Aluminum filter channel (171 Å–500 Å) with spatially resolved full-disk integrated intensity values measured with SWAP (174 Å) and Ca II K 1 Å index values (ground-based observations from NSO/Sac Peak) for the period from 01 April 2010 to 15 Mar 2011. We found that there is a good correlation between these parameters. This indicates that the spatial resolution of SWAP complements the high temporal resolution of LYRA. Hence SWAP can be considered as an additional radiometric channel. Also the K emission index is the integrated intensity (or flux) over a 1 Å band centered on the K line and is proportional to the total emission from the chromosphere; this comparison clearly explains that the LYRA irradiance variations are due to the various magnetic features, which are contributing significantly. In addition to this we have made an attempt to segregate coronal features from full-disk SWAP images. This will help to understand and determine the actual contribution of the individual coronal feature to LYRA irradiance variations.

Introduction: Many factors impact the performance of graduating residents on certification exams. It is thought that most factors are related to the individual candidate’s ability, motivation, and work ethic. Less understood, however, is whether a training program has any impact on the preparation and performance of its graduates on certification exams. We present 20 years of results of a national preparatory exam that all graduating residents complete about three months before the Royal College of Physicians and Surgeons of Canada (RCPSC) qualifying urology exam. This exam, known colloquially as QUEST, aims to simulate the RCPSC exam with written and oral components. We aimed to analyze the impact of a training program on the performance of its residents. Methods: A retrospective review of exam results from 1997–2016 was conducted. During that time, 495 candidates from all 12 Canadian urology training programs undertook the exam. The performance of graduating residents from each individual program was grouped together for any given year. The different programs were anonymized, as the aim of this study is to assess the impact of a training program and not to rate the different programs. Statistical analysis using one-way ANOVA was conducted. Results: All training programs fall within one standard deviation of the mean for the written component, the oral component, and the overall score. The residents of four training programs had statistically better scores than the overall mean of the written component. The residents of three out of these four training programs also had statistically better scores than the overall mean of the oral component and the overall results of the exam. Conclusions: Most Canadian training programs prepare their residents adequately for this simulated certification exam in urology. However, there are some training programs that consistently prepare graduating residents to outperform their peers.

Resumen Cuando se empezaron a conocer los testimonios de los supervivientes de los campos de exterminio nazis, la humanidad se consternó. El sufrimiento provocado y la aplicación sistemática y consciente de la técnica a la destrucción de la persona, fueron descubrimientos que pusieron en alerta al hombre sobre la naturaleza del hombre mismo.   Tanto fue el horror que se alcanzó a entrever a través de las narraciones que el  tiempo  se  congeló.  El  reclamo  silencioso  de  las  auténticas  ví­ctimas, quienes murieron, se suspendió en el aire de la memoria hasta que los responsables reconocieran sus culpas. El olvido no podí­a abrazar tan profundos crí­menes.   Por este motivo no puede realizarse el fin de la historia. Los sufrimientos del hombre provocados hasta este grado por el mismo hombre fuerzan un pendiente que ya no puede borrarse. El grito de dolor recuerda a las generaciones futuras la necesidad de una reparación, del perdón, del reconocimiento.   Palabras Clave: Testimonio, memoria, campos de exterminio, fenomenologí­a, hermenéutica, sufrimiento, herencia.   Abstract When testimony from the survivors from Nazi extermination fields were first known, the human race filled with dismay. The suffering provoked and the systematic conscious application of the technique of destruction of the individual, were discoveries that alerted the individual on the nature of the individual itself.   Such a horror was seen through the narrations that time froze.     The silent demand from the authentic victims, who died, was suspended on the air of memory until the responsible recognized their  guilt. Obscurity could not hold such deep crimes.   For this reason the end of history cannot be made. The suffering of the individual provoked up to this point by the individual itself, force an unresolved point that cannot be erased.   The scream of pain reminds the future generations the need to repair, forgive and recognize it.   Key words: Testimony, memory, extermination fields, phenomenology, hermeneutics, suffering, inheritance.

Buscamos discutir a singularidade do trabalho de campo, envolvendo a relação entre o pesquisador e as pessoas ouvidas, como condição de uma ciência que lida essencialmente com o indivíduo. Abstract In this article we reflect upon the production and uses of sources and methodological tools which guide the actions of the historian who deals with the field conventionally referred to as Oral History. We aim at discussing the singularity of field work, bringing into play the relationship between the researcher and people who are listened to – a vital condition for a science that essentially deals with the individual.
